Effects of resistance training on blood pressure in the elderly

Andréia Cristiane Carrenho Queiroz et al. Hélcio Kanegusuku Cláudia Lúcia de Moraes Forjaz

The aging process drastically reduces muscle mass, strength and power, decreasing the capacity to perform the activities of daily living. The practice of resistance exercises can reverse this picture, helping to maintain the muscular mass and improving muscular strength and resistance. However, the aging process causes cardiovascular alterations, which can result in increased blood pressure levels at rest and it is important to analyze the effects of resistance exercises on the blood pressure of elderly individuals. The objective of this study is to evaluate the current scientific knowledge on the responses of the blood pressure to resistance exercises and their mechanisms in elderly individuals. In order to do that, we carried out a literature review related to the theme, in both Portuguese and English. Based on the studies found, the current corpus, although scarce and controversial, suggests that, chronically, resistance exercises can have a hypotensive effect on elderly individuals. However, this effect occurs mainly in normotensive elderly individuals submitted to low-intensity training. The mechanisms involved in the hypotensive response still need to be clarified. Although the resistance training is recommended for elderly individuals and there is some evidence that it can have a chronic hypotensive effect, there is still lack of scientific data and much debate on the subject, which demonstrates that this field is still open to investigation.

Point of view

The cardiologist: a chained Prometheus

Robespierre Queiroz da Costa Ribeiro

This critical review discusses the deviations from the Flexnerian hegemonic model of medical practice, carried out under the point of view of paleomedicine and relevant evidence from research. If we, on the one hand, have the required and sufficient knowledge on the burden of diseases that afflict mankind and its determinant factors, on the other hand the attention turns to strategies that do not have an effective impact on such diseases, consuming a larger portion of resources directed to healthcare: ischemic cardiovascular diseases, followed by type 2 diabetes mellitus. Their risk factors are well-known and have been shown to be controllable through health promotion actions, which constitute a process technology that is more cost-effective than biotechnology. However, we choose to apply large part of the financial resources on the care of the affected individual to the detriment of health promotion in the general population, who goes without this benefit, which characterizes an injustice in terms of healthcare actions.

Anthropometric indicators of insulin resistance
Clinical Update

Anthropometric indicators of insulin resistance

Ana Carolina Vasques et al. Lina Rosado Gilberto Rosado Rita de Cassia Ribeiro … Sylvia Franceschini Bruno Geloneze

Some studies have analyzed the efficacy of anthropometric indicators in predicting insulin resistance (IR), for they are more economic and accessible. In this study, the objective was to discuss the measures and anthropometric indices that have been associated with IR. A bibliographic review was done, based on Scielo, Science Direct and Pubmed. Among these studies, waist and sagittal abdominal diameter presented better predictive capacity for IR, with more consistent results. The waist-to-thigh, waist-to-size, neck-to-thigh ratios, the conicity and the sagittal index have showed positive results; nevertheless, more studies are necessary to consolidate them as predictors to IR. The obtained results, with the use of body mass index and of the waist-to-hip ratio, were inconsistent. In the Brazilian population, the realization of studies evaluating the performance of these indicators in predicting IR is suggested, since the results of the studies conducted in other populations are not always applicable to ours, due to ethnical differences resulting from the great miscegenation in the country.

The impact of continuous positive airway pressure (CPAP) on the cardiac rehabilitation of patients with congestive heart failure: case report

Murillo Frazão de Lima e Costa et al. Matheus Pires de Barros José Hariston Morais Lima

Congestive heart failure is a pathology that limits the patient’s physical function. This study analyzed one patient who was submitted to a cardiac rehabilitation program associated to Continuous Positive Airway Pressure (CPAP), by assessing the results of the six-minute walk test (6MWT) and a questionnaire on the quality of life and performing an echocardiographic assessment before the study and after six weeks. The distance walked by the patient increased from 152.5 m to 520.44 m at the 6MWT. The Minnesota questionnaire score decreased from 62 to 18. Ejection fraction increased from 33% to 36%. Therefore, the management chosen for this case improved the patient’s physical performance and quality of life.

Use of 6-minute walk test in pulmonary hypertension management

Elize Fumagalli et al. Maria ângela de Oliveira Ribeiro Mariana Simões Ferreira Camila Isabel da Silva Santos

The 6-minute walk test (T6) is used to assess cardiopulmonary diseases due to its prognosis capacity, easy performance and reproducibility. Pulmonary hypertension (PH) is defined and classified as a hemodynamic consequence leading to increased pulmonary arterial pressure and may result in right ventricular failure and consequent death. This case reports the prescription of T6 by the cardiology team at a university hospital as a means of physical control of a patient with HP, after the introduction and initiation of therapy.

Real-world use of drug-eluting stents: the importance of registries

Expedito E. Ribeiro et al. Henrique Barbosa Ribeiro

over the last decades the efficacy and safety of bare metal (BMS) and drug eluting stents (DES) have been demonstrated in many different clinical scenarios, leading to their use in more than 75% of the procedures worldwide. Compared to BMS, DES have shown lower rates of angiographic restenosis and target-vessel revascularization. This benefit was initially demonstrated in trials that excluded patients with more complex lesions, such as those with larger or smaller vessels, chronic total occlusions, bifurcation lesions, stent restenosis, long lesions and left main coronary artery disease. This real-world population has been recently evaluated in many registries and meta-analyses that are reviewed herein.

Effectiveness and safety of drug-eluting stents in a cardiology clinic in Curitiba, PR, Brazil

Chiu Yun Yu Braga et al. Luiz Antonio Rivetti Francisco de Paula Stella

BACKGROUND: The effectiveness and safety of drug-eluting stents (DES) have still been questioned. OBJECTIVE: The objective of this study was to evaluate the effectiveness and safety of these stents, as well as the incidence of target lesion revascularization (TLR), in addition to identifying possible variables influencing the need for TLR. METHODS: A total of 203 patients from Hospital Costantini who were clinically followed up for one to 3 years were selected. RESULTS: The sample characteristics were as follows: 470 lesions; 171 (84.24%) male patients; 54 (26.6%) had diabetes; 131 (64.35%) had hypertension; 127 (62.56%), dyslipidemia; 40 (19.70%) were smokers; and 79 (38.92%) had a family history of coronary artery disease. Also: 49 (24.14%) patients presented with stable angina; 58 (28.57%), unstable angina; and 6 (2.96%), myocardial infarction. Eighty five (41.87%) patients were asymptomatic, and 146 (71.92%), had multivessel disease. As for the characteristics of the lesions, 77.45% were B2/C (AHA/ACC). Taxus was implanted in 73.62% of the patients. Stents with diameter > 2.5 mm were used in 381 (81.96%) patients. The stent length was < 30 mm in 67.87% of the lesions, with a mean of 2.3 stents per patient. After follow-up, 19 patients (9.3%) underwent TLR. Four patients died (1.97%), two of them of MI (0.98%), one of stroke (0.49%), and one of abdominal aneurysm (0.49%). Also, one patient died of late thrombosis (0.49%), and one of reinfarction (0.49%). In the statistical analysis carried out, only the bifurcation lesions variable reached values close to the statistical significance level, with p < 0.06. CONCLUSION: In conclusion, drug-eluting stents have good effectiveness and safety profiles; the incidence of TLR was 9.3%, and we did not identify a variable correlated with the need for TLR.

Geometric indexes of heart rate variability in obese and eutrophic children

Luiz Carlos Marques Vanderlei et al. Carlos Marcelo Pastre Ismael Forte Freitas Júnior Moacir Fernandes de Godoy

BACKGROUND: Obesity causes changes in cardiac autonomic modulation. OBJECTIVE: To investigate the autonomic modulation of eutrophic and obese children by means of indexes of heart rate variability (HRV) obtained through geometric methods. METHODS: We analyzed data from 133 children aged 8 to 13, divided into two groups: obese (n = 61) and eutrophic (n = 72) according to body mass index for age and sex. For the analysis of HRV, heart rate was recorded beat-to-beat. The RR intervals were transformed into geometric figures, and from them, we calculated the triangular index (RRtri), triangular interpolation of RR intervals (TINN), the indexes SD1, SD2 and SD1/SD2 ratio, obtained from the Poincaré plot. Visual analysis of the plot was also performed. Student’s t test was performed for unpaired data and Mann-Whitney’s test, with significance level of 5.0%, for data analysis. RESULTS: In obese children, RRtri indexes were proven to be reduced (0.0730 vs 0.1084 [median]), TINN (171.80 ± 55.08 vs 218.26 ± 51.12), SD1 (19.93 ± 9.10 vs 24.10 ± 8.03) and SD2 (51.63 ± 16.53 vs 69.78 ± 17.19). The SD1/SD2 ratio showed no significant differences (0.3781 ± 0.12 vs 0.3467 ± 0.08). Visual analysis of the plot, in obese children, revealed a smaller dispersion of RR intervals both beat-to-beat, and in the long term, indicating lower HRV. CONCLUSION: Obese children presented changes in the autonomic nervous system characterized by decreases in parasympathetic activity and overall variability.

Normotensive individuals with exaggerated exercise blood pressure response have increased cardiac vagal tone

Plínio Santos Ramos et al. Claudio Gil S. Araújo

BACKGROUND: Exaggerated systolic blood pressure (SBP) levels during a maximal cardiopulmonary exercise test (CPET) are classically considered as inappropriate and associated with a higher risk for the development of cardiovascular diseases. It is known that the autonomic nervous system modulates the BP during exercise. However, the behavior of the cardiac vagal tone (CVT) has not been fully established in healthy individuals with an exaggerated BP response to CPET. OBJECTIVE: To analyze the behavior of the CVT in healthy adult males presenting an exaggerated BP response to CPET. METHODS: Of the 2,505 cases evaluated between 2002-2009, 154 cases were thoroughly identified, consisting of healthy male normotensive subjects aged 20-50 years. The evaluation included clinical assessment, anthropometric measurements, 4-second exercise test (cardiac vagal tone) and cardiopulmonary exercise test (CPET) performed in a cycle-ergometer, with BP measurements being taken every minute through auscultation. Based on the maximum SBP value obtained at the CPET, the sample was divided in tertiles, comparing CVT, maximum workload and VO2 max. RESULTS: The CVT results differed between individuals in the lower tertile and upper tertile for the SBP response to the CPET, respectively: 1.57 ± 0.03 and 1.65 ± 0.04 (mean ± standard error of mean) (p = 0.014). The two tertiles also differed regarding the VO2 max (40.7 ± 1.3 vs 46.4 ± 1.3 ml/kg-1.min-1; p = 0.013) and the maximum workload (206 ± 6.3 vs 275 ± 8.7 watts; p < 0.001). CONCLUSIOn: An increased BP response during the CPET in healthy adult males is accompanied by indicators of good clinical prognosis, including higher levels of aerobic fitness and cardiac vagal tone.

Lifestyle and cardiovascular health in school adolescents from São Paulo

Inês Lancarotte et al. Moacyr Roberto Nobre Rachel Zanetta Marcio Polydoro

BACKGROUND: Cardiovascular disease is the leading cause of mortality worldwide. There is evidence demonstrating the association of this disease with cardiovascular risk factors related to lifestyle, incorporated in adolescence. OBJECTIVE: To identify, in adolescents, the prevalence of overweight and lifestyle factors associated with risk for developing cardiovascular diseases, and the factors that influence them. METHODS: It was conducted an observational study of individual cross-sectional data with adolescents enrolled in public and private schools in São Paulo, covering grades 5 to 8 of elementary school. The information was obtained through the application of an anonymous questionnaire and weight and height measurements. RESULTS: 2,125 adolescents with mean age of 12.9 years were studied. From the total sample: 14.4% to 32.1% did not practice sports or competitions; 56.0% to 73.6% remained more than two hours watching TV, playing video game or working on the computer; approximately 80% consumed fruits and vegetables improperly; 34.9% to 45.3% reported high salt consumption; and 60.9% to 74.4% reported consumption of soft drinks. Prevalence of overweight ranged from 18.7% to 41.6%. CONCLUSION: In school adolescents, this study found high prevalence of risk factors associated with the development of cardiovascular disease in adults. Further studies are required to better understand how these risk factors are related and thus enable the implementation of preventive measures in adolescence in order to prevent cardiovascular disease in adults.

Fat content of dairy products, eggs, margarines and oils: implications for atherosclerosis

Carlos Scherr et al. Jorge Pinto Ribeiro

BACKGROUND: For appropriate advising on a meal plan aimed at the prevention of ischemic heart disease, it is necessary to know the chemical composition of foods. OBJECTIVE: To analyze the composition of fats, fatty acids and cholesterol in some Brazilian specimens of edible oils, butters, margarines, dairy products and eggs, using the limits of a diet to prevent coronary artery disease. METHODS: We analyzed the composition of edible oils, butters, margarines, dairy products and eggs. The findings were used as recommended by the American Heart Association for a 1,800 calorie diet. RESULTS: Comparing the edible oils, the canola oil was found to be the best.Among the milks, the skimmed milk is most advisable one, but no advantages were found over the semi-skimmed milk. For the eggs, no differences were found in the types of milk found in the market. For cheeses, the “minas” cheese had the lowest content of cholesterol and saturated fat. Comparing margarine and butter, the former was found to be better when trans fats do not exceed the recommended levels. CONCLUSION: The composition of foods used in Brazil indicates that diets designed to enable primary and secondary prevention of ischemic heart disease can prioritize the use of semi-skimmed milk, sunflower oil, margarines with low content of trans fats and “minas” cheese.

Morbimortality in octogenarian patients submitted to coronary artery bypass graft surgery

Fernando Pivatto Júnior et al. Renato A. K. Kalil Altamiro R. Costa Edemar M. C. Pereira … Eduardo Z. Santos Felipe H. Valle Luciano P. Bender Marcelo Trombka Thaís B. Modkovski Ivo A. Nesralla

BACKGROUND: Given the progressive increase in longevity and the need of an increasingly elderly population to undergo myocardial revascularization surgery (MRS), it becomes necessary to know its risks and benefits. OBJECTIVE: To evaluate the in-hospital morbimortality of patients aged 80 and older submitted to MRS and identify its predictor variables. METHODS: A total of 140 consecutive cases were studied between January 2002 and December 2007. The patients’ mean age was 82.5 ± 2.2 years (range: 80-89) and 55.7% were males. In the sample,72.9% had arterial hypertension, 26.4% had diabetes, 65.7% presented severe lesion in three or more vessels and 28.6% presented a severe lesion in the left coronary trunk. An associated surgery was present in 35.7% of the cases, with aortic valve in 26.4% and mitral valve in 5.6%. RESULTS: The mortality rate was 14.3% (isolated MRS 10.0% vs 22.0% with associated procedure; p = 0.091) and the morbidity was 37.9% (isolated MRS 34.4% vs 44.0% with associated procedure; p = 0.35). The most frequent complications were low cardiac output (27.9%), renal dysfunction (10.0%) and prolonged ventilatory support (9.6%). At the bivariate analysis, the most important mortality predictors were sepsis (RR 10.2; 95%CI: 6.10-17.7), previous MRS (RR 8.06; 95%CI: 5.16-12.6), postoperative low cardiac output (RR 7.77; 95%CI: 3.03-19.9) and postoperative renal dysfunction (RR 7.36; 95%CI: 3.71-14.6). The morbidity predictors were extracorporeal circulation time > 120 min. (RR: 2.34; 95%CI: 1.62-3.38) and time of ischemia > 90 min. (RR: 2.29 95%CI: 1.56-3.37). CONCLUSION: The MRS in octogenarians is associated with a higher morbimortality when compared to younger patients, which, however, does not prevent the procedure if the indication is justified by clinical condition.

Replacement of carvedilol for propranolol in patients with heart failure

Fabiana Marques et al. Renato Barroso Pereira de Castro Fernando Nobre Antonio Osvaldo Pintya … Lourenço Gallo Júnior Benedito Carlos Maciel Marcus Vinícius Simões

BACKGROUND: Large clinical trials using the betablockers carvedilol, metoprolol, bisoprolol and nebivolol have demonstrated improvement of survival and symptoms in patients with heart failure. Despite the lack of scientific evidence, it is plausible that their beneficial effects are extensible to other betablockers. OBJECTIVE: To evaluate the impact of the replacement of carvedilol for propranolol on left ventricular function, functional capacity, quality of life, pressure levels, and cardiac autonomic control in patients with heart failure. METHODS: Twenty nine patients receiving optimized drug therapy including maximum tolerated doses of carvedilol were divided into two groups: replacement of carvedilol for propranolol (n = 15) and continued carvedilol (n = 14). At baseline and 6 months later, clinical and laboratorial assessments were carried out with radionuclide ventriculography, echocardiography, Minnesota questionnaire, walk test, APBM and Holter monitoring. RESULTS: The clinical and demographic characteristics were similar in the two groups at baseline. Individualized propranolol dose adjustment ensured a similar degree of beta-blockade, as assessed by resting heart rate and chronotropic reserve. The mean propranolol dose used was 109 ± 43 mg/day. Only one patient presented with intolerance to propranolol, thus carvedilol was reintroduced. One death was recorded in group propranolol. Ejection fraction significantly increased in the propranolol group. No significant change was observed in the other cardiovascular variables after betablocker replacement. CONCLUSION: Our results indicate that replacement of carvedilol for propranolol in patients with heart failure is not associated with deterioration of the ejection fraction, functional capacity, quality of life, and other cardiovascular variables related to autonomic and blood pressure control., PP.0-0).

Effect of aerobic exercise on plasma renin in overweight patients with hypertension

Bruno Martinelli et al. Silvia Regina Barrile Eduardo Aguilar Arca Roberto Jorge da Silva Franco Luis Cuadrado Martin

BACKGROUND: The activity of the renin-angiotensin-aldosterone system (RAAS) is directly related to overweight and sedentary lifestyles, both of which are associated with hypertension. Aerobic exercise helps control blood pressure (BP) by acting on mechanisms of blood pressure regulation, such as plasma renin activity (PRA). OBJECTIVE: To assess the effect of aerobic exercise on plasma renin activity in overweight hypertensive patients. METHODS: Blood pressure, serum biochemistry and anthropometric parameters of twenty subjects were measured before and after a 16-week exercise training program, three days a week, at 60-80% of maximal heart rate. Data were expressed as mean ± standard deviation or median and interquartile range and analyzed using the student’s t-test, Mann-Whitney test, and ANOVA (p < 0.05). RESULTS: Mean age of the study population was 57 ± 7.0, and mean body mass index was 30 ± 3.5 kg/m². The aerobic training decreased body fat (35 ± 7.8 to 30 ± 5.6 %), heart rate (HR) (80 ± 10.4 to 77 ± 8.5 bpm), and pulse pressure (PP) (50 ± 11.8 to 46 ± 10.0 mmHg) (p < 0.05), but not plasma renin activity, which ranged from 0.8 (0.45-2.0) to 1.45 (0.8-2.15) ηg/ml/h (p = 0.055). The group that achieved a reduction in waist circumference (WC) (n = 8) experienced also a reduction in systolic BP and PP (p < 0.05). In the group without reduction in WC, none of the BP variables changed. Plasma renin activity (PRA) was not associated with any variable studied. Aerobic training was associated with a decrease in PP in the study group as a whole and with a decrease in systolic BP in the subgroup who experienced a decrease in waist circumference. CONCLUSION: The aerobic exercise training failed to reduce plasma renin activity in overweight hypertensive patients.

Comparative study of the Pro-Atherosclerotic Profile of students of Medicine and Physical Education

Marcelo de Aquino Resende et al. Roberta Barreto Vasconcelos Resende Rodrigo da Silveira Tavares Carlos Roberto Rodrigues Santos José Augusto Soares Barreto-Filho

BACKGROUND: Recent studies have demonstrated a strong association between physical activity, low level of cardiorespiratory fitness and the presence of cardiovascular risk factors. OBJECTIVE: Compare the level of physical activity, level of cardiorespiratory fitness and cardiovascular risk in students of medicine and physical education. METHODS: At the first phase, the International Physical Activity Questionnaire (IPAQ) was used to quantify the physical activity level of 126 students from the 7th and 8th semesters of the physical education and medicine courses. At the second phase, 40 students were randomly selected (20 from each course) to undergo cardiovascular risk assessment and cardiorespiratory fitness. The following data were assessed 1) arterial pressure; 2) body mass index (BMI); 3) percentage of fat (electrical bioimpedance); 4) abdominal circumference (AC); 5) laboratory biochemical assessment; and 6) cardiorespiratory fitness (Kline’s Test). RESULTS: The comparison of students of medicine and physical education, respectively, showed a higher frequency of individuals presenting: low level of physical activity (55% vs 15.0%; p = 0.008); pre-hypertension measured by SAP (80% vs 25.0%; p = 0.000) and by DAP (45% vs 5.0%; p = 0.003); overweight (50% vs 10.0%; p = 0.006); increased abdominal circumference (25% vs 0.0%; p = 0.017); increased total cholesterol (165 ± 28 vs 142 ± 28 mg/dl; p = 0.015); increased LDL-c (99 ± 27 vs 81 ± 23 mg/dl; p = 0.026); increased glycemia (81 ± 8.0 vs 75 ± 7.0 mg/dl; p = 0.013); lower cardiorespiratory fitness (48 ± 8.0 vs 56 ± 7.0 ml/kg/min; p = 0.001). CONCLUSION: Students of medicine presented lower levels of physical activity practice, lower level of cardiorespiratory fitness and higher frequency of cardiovascular risk factors, when compared to Physical Education students.

Effects of the continuous positive airway pressure on the airways of patients with chronic heart failure

João Carlos Moreno de Azevedo et al. Edison Ramos Migowisk de Carvalho Luis Augusto Feijó Fátima Palha de Oliveira … Sara Lúcia Silveira de Menezes Henrique Murad

BACKGROUND: Heart failure can present with asymptomatic dysfunction at decompensation, with limitations and decrease in the productive capacity. The Continuous Positive Airway Pressure (CPAP) is a non-pharmacological means to decrease afterload. OBJECTIVE: To analyze the effects of CPAP (10 cmH2O), for 30 days in patients with chronic heart failure. METHODS: We assessed 10 patients with heart failure (6 males, 4 females) of several etiologies, with a mean age of 54 ± 14 years, with a BMI of 21 ± 0.04 kg/m². The therapy was applied for 60 min., 5 times a week for 30 days, during the daytime. The echocardiogram and the ergospirometry were analyzed, before and 30 days after the therapy. RESULTS: There was a 19.59% increase in the left ventricular ejection fraction (LVEF): 23.9 ± 8.91 vs 27.65 ± 9.56%; p = 0.045. At the ergospirometry, the exercise time (ET) showed a significant increase from 547 ± 151.319 vs 700 ± 293.990 sec., p = 0.02; oxygen consumption (VO2) was 9.59 ± 6.1 vs 4.51 ± 2.67 ml.kg-1.min.-1, p = 0.01, whereas the carbon dioxide production (VCO2) at rest (9.85 ± 4.38 vs 6.44 ± 2.88 ml.kg-1.min.-1, p = 0.03) decreased. CONCLUSION: The CPAP resulted in an increase in the LVEF and ET, decreased the oxygen consumption and the carbon dioxide production at rest.

Scintigraphy for the detection of myocardial damage in the indeterminate form of Chagas disease

Ivana Moura Abuhid et al. Enio Roberto Pietra Pedroso Nilton Alves de Rezende

BACKGROUND: Non-invasive cardiological methods have been used for the identification of myocardial damage in Chagas disease. OBJECTIVE: To verify whether the rest/stress myocardial perfusion scintigraphy is able to identify early myocardial damage in the indeterminate form of Chagas disease. METHODS: Eighteen patients with the indeterminate form of Chagas Disease and the same number of normal controls, paired by sex and age, underwent rest/stress myocardial scintigraphy using sestamibi-99mTc, aiming at detecting early cardiac damage. RESULTS: The results did not show perfusion or ventricular function defects in patients at the indeterminate phase of Chagas disease and in the normal controls, except for a patient who presented signs of ventricular dysfunction in the myocardial perfusion scintigraphy with electrocardiographic gating. CONCLUSION: The results of this study, considering the small sample size, showed that the rest/stress myocardial scintigraphy using sestamibi-99mTc is not an effective method to detect early myocardial alterations in the indeterminate form of Chagas disease.

Antibodies against OxLDL and acute coronary syndrome

Ana Maria Brito Medeiros et al. Carlos Alberto von Mühlen Magnus Ake Gidlund Rodrigo Bodanese … Maria G. V. Gottlieb Luiz C. Bodanese

BACKGROUND: The oxidation of low-density lipoprotein (oxLDL) induces the formation of immunogenic epitopes in molecules. The presence of autoantibodies against oxLDL has been demonstrated in the serum of patients with coronary artery disease (CAD). However, the role of these autoantibodies in the pathophysiology of acute coronary syndromes (ACS) and their clinical significance remain undefined. OBJECTIVE: To evaluate the association between antibodies against oxLDL and ACS. METHODS: Titers of IgG autoantibodies against oxLDL by copper (anti-oxLDL) and anti-D synthetic peptide derived from apolipoprotein B (antipeptD) were determined by Enzyme-linked immunosorbent assay (ELISA) in 90 patients, in the first 12 hours of ACS (cases) and in 90 patients with chronic CAD (controls). RESULTS: The results showed that the titers of anti-oxLDL were significantly higher (p = 0.017) in cases (0.40 ± 0.22) than in controls (0.33 ± 0.23). On the other hand, the titers of antipeptD were significantly lower (p < 0.01) in cases (0.28 ± 0.23) than in controls (0.45 ± 0.30). The difference in the titers of both antibodies between the two groups was independent of age, sex, hypertension, diabetes mellitus, dyslipidemia, body mass index, smoking, lipid profile, statin use and family history of CAD. CONCLUSION: The results showed that the titers of anti-oxLDL were significantly higher in patients with acute coronary syndrome as compared to patients with coronary artery disease and may be associated with atherosclerotic plaque instability.

Analysis of the cost-effectiveness of thrombolysis with alteplase in stroke

Denizar Vianna Araújo et al. Vanessa Teich Roberta Benitez Freitas Passos Sheila Cristina Ouriques Martins

BACKGROUND: The cerebrovascular accident (CVA) or stroke is the main cause of death in Brazil and little information is available on the cost of treatment. OBJECTIVE: To carry out a cost-effectiveness analysis of thrombolysis in stroke, up to three hours after symptom onset, comparing the treatment with alteplase versus the conservative treatment, under the perspective of the Brazilian Public Health System (SUS). METHODS: A decision analysis model was developed to compare the two types of treatment. Cycles were considered, during which the patients would go through five stages of disability post-stroke, based on the modified Rankin scale. The probability to present intracranial hemorrhage in the first year was obtained from the NINDS trial. For the subsequent years, one-year cycles were considered to calculate patients’ mortality. The outcome was expressed as quality-adjusted life years (QALY). Both direct and indirect costs were considered in the analysis. Costs and outcomes were discounted at 5% a year. RESULTS: In the first year, the QALY gained was 0.06 for both sexes, with an incremental cost of R$ 2,558 for men and R$ 2,312 for women. The incremental cost-effectiveness ratio (ICER) in one year was R$ 40,539 / QALY (USD 28,956) for men and R$ 36,640 / QALY (USD 26,171) for women. After the second year, the treatment with alteplase reduced the cost of treatment (Purchasing Power Parity index: US$ 1 = R$ 1.4). CONCLUSION: The thrombolytic therapy with alteplase within the first three hours following a stroke is cost-effective in the Brazilian Public Health System scenario.

Implementation of a telecardiology system in the state of Minas Gerais: the Minas Telecardio Project

Antonio Luiz P. Ribeiro et al. Maria Beatriz Alkmim Clareci Silva Cardoso Gláucio Galeno R. Carvalho … Waleska Teixeira Caiaffa Monica Viegas Andrade Daniel Ferreira da Cunha Andre Pires Antunes Adélson Geraldo de A. Resende Elmiro Santos Resende

BACKGROUND: Although cardiovascular diseases are the main cause of morbimortality in Brazil, the access of small-town populations to electrocardiography and cardiology assessment is limited. The use of telecardiology to assist the access of distant towns to electrocardiography and a second opinion in cardiology is promising; however, it has not been formally assessed. OBJECTIVE: To assess the feasibility of implementing a low-cost public telecardiology system in small Brazilian towns. METHODS: A total of 82 towns in the state of Minas Gerais, with a population 70% coverage by the Family Health Program (Programa Saude da Familia – PSF), local government compliance and internet access, were selected. Each town was supplied with digital electrocardiography (ECG) device and a team was trained. The implementation was coordinated by HC/UFMG, together with four university hospitals in the state of Minas Gerais (UFU, UFTM, UFJF and UNIMONTES). The ECG assessments were carried out in the towns and sent through the Internet for prompt analysis by an on-duty telecardiology team. Online and offline discussions on the medical cases were carried out through the Internet, as well as refreshment courses. RESULTS: During the implementation period, a total of 253 health professionals were trained. From July 2006 to November 2008, the project assisted 42,664 patients, with a total of 62,865 ECG assessments being performed. A total of 2,148 emergency cases were treated, as well as 420 teleconsultations. The intermediate evaluation showed good acceptance of the implemented technology and a 70% decrease in patient referrals to other reference centers. CONCLUSION: The use of the customary resources in informatics to assist the access of small-town populations to electrocardiography and specialized cardiology assessment is feasible.

Amplification of the genes that codify endothelin-1 and its receptors in rheumatic mitral valves

Edmilson Bastos de Moura et al. Mariana Ribeiro Gomes Ricardo Barros Corso Cristiano Nicolleti Faber … Fabiana Pirani Carneiro Yolanda Galindo Pacheco

BACKGROUND: Cardiopathies are high prevalence conditions. Among them, rheumatic carditis is of high relevance in developing countries. Left cardiac chamber changes are associated to endothelial dysfunction and ET-1 levels increase. Pulmonary circulation is then affected, and not seldom leading to pulmonary hypertension (PH). However, the presence of ET-1 and its receptors in the mitral valve itself – promoting pulmonary vascular changes, with increased rheumatic valvular deformation – has not been discussed in the literature. OBJECTIVE: To determine the expression of endothelin gene and its receptors in rheumatic mitral valves through techniques of molecular genetics. METHODS: Twenty-seven patients submitted to mitral valve replacement had their valvular tissue examined to determine the presence of ET-1 genes and their A and B receptors. Histological and molecular analysis of the valves was performed (divided into M1, M2 and M3 fragments), with patients’ clinical and epidemiological data collected. Patients were divided into 3 groups (mitral valvopathy, mitroaortic valvopathy, and reoperation patients). RESULTS: The study showed endothelin-1 gene expression in 40.7% specimens and A receptor in all samples; receptor gene B had lower expression (22.2%). CONCLUSION: All patients showed A receptor gene expression. No statistically significant difference was observed in regard to condition severity, expressed according to functional class, and subgroups (mitral valvopathy, mitroaortic valvopathy, and reoperation patients).
