Accuracy of Transthoracic Echocardiogram as a Screening Method in the Clinical Practice of Pulmonary Hypertension Investigation

Camila Farnese Rezende et al. Eliane Viana Mancuzo Maria do Carmo Pereira Nunes Ricardo Amorim Corrêa

Abstract

Background

The transthoracic echocardiogram (TTE) plays a screening role in the diagnostic algorithm of pulmonary hypertension (PH). Studies have shown a significant disagreement between TTE measurements of the systolic pulmonary artery pressure (sPAP) and right atrial pressure (RAP) and those obtained by right heart catheterization (RHC).

Objective

To compare TTE measurements of sPAP and RAP with those obtained by RHC in patients being investigated for PH.

Methods

Patients referred to a PH reference center with a high or intermediate TTE probability of PH upon admission were submitted to RHC. The agreement between sPAP and RAP from both procedures was assessed through the Bland-Altman test. Differences of up to 10 mmHg for sPAP and 5 mmHg for RAP were considered within the variability of the test. Receiver Operating Characteristic (ROC) curve was constructed to determine the most accurate sPAP and Tricuspid regurgitation maximal velocity (TRV)values associated with the diagnosis of PH by RHC. The adopted level of statistical significance was 5%.

Results

Ninety-five patients were included. The Bland-Altman analysis showed a bias of 8.03 mmHg (95% CI:-34.9-50.9) for sPAP and -3.30 mmHg (95% CI:-15.9-9.3) for RAP. AUC for sPAP and TRV measured by TTE for discrimination of probable PH were 0.936 (95% CI: 0.836-1.0) and 0.919 (95% CI: 0.837-1.0), respectively. However, only 33.4% of the echocardiographic estimate of sPAP and 55.1% of RAP were accurate, as compared to the measurements obtained by RHC.

Conclusion

TTE has a high discriminatory power as a screening diagnostic method for PH despite presenting disagreements between sPAP and RAP absolute values when compared to RHC measurements.

Biologic Left Ventricular Assist: A New Strategy for Patients with Advanced Heart Failure with Pulmonary Hypertension

Fábio Antônio Gaiotto et al. Samuel Padovani Steffen

Pulmonary arterial hypertension due to increased vascular resistance is a clinical condition that directly impacts the prognosis of patients with heart failure (HF), and it is considered one of the main contraindications to orthotopic heart transplantation. Although hemodynamic values vary in the literature, there is a consensus that pulmonary artery systolic pressure above 50 mmHg, transpulmonary gradient above 15 mmHg, and pulmonary vascular resistance above 3 Wood units, when unresponsive to vasodilators, contraindicate orthotopic transplantation. Therapeutic strategies to decrease or reverse this condition are a challenge in clinical practice, which, if resolved, may allow definitive treatment of HF with orthotopic transplantation.

The use of long-term continuous-flow left circulatory assistance devices as a bridge to candidacy is a reality in large transplant centers in developed countries. Several studies have demonstrated that device implantation not only leads to the expected eligibility for orthotopic transplantation in some patients; it also improves the quality of life of patients for whom it ends up remaining as the target therapy. In a study recently carried out by Ruan et al, it was demonstrated that, approximately 6 months after device implantation, there was a reduction and stabilization of pulmonary pressure. The reversal of pulmonary hypertension through left assistance devices is possible, given that there is an important decrease in filling pressures by means of the continuous emptying of the left ventricle.

Soluble Guanylate Cyclase Stimulators (Riociguat) in Pulmonary Hypertension: Data from Real-Life Clinical Practice in a 3-Year Follow-Up

Fernanda Brum Spilimbergo et al. Taís Silveira Assmann Marcelo Bellon Laís Machado Hoscheidt … Cássia Ferreira Braz Caurio Márcia Puchalski Bruno Hochhegger Gabriela Roncato Gisela Martina Bohns Meyer

Abstract

Background

Pulmonary hypertension (PH) is a rare and complex disease with poor prognosis, which requires lifelong treatment.

Objective

To describe 3-year follow-up real-life data on treatment with soluble guanylate cyclase stimulators (Riociguat) of patients with PH, measuring current risk assessment parameters.

Methods

This study retrospectively collected clinical and epidemiological data of patients with PH of group 1 (pulmonary arterial hypertension) and group 4 (chronic thromboembolic PH). Non-invasive and invasive parameters corresponding to the risk assessment were analyzed at baseline and follow-up. Statistical analyses were performed using the SPSS 18.0 software, and p-values < 0.050 were considered statistically significant.

Results

In total, 41 patients receiving riociguat were included in the study. Of them, 31 had already completed 3 years of treatment and were selected for the following analysis. At baseline, 70.7% of patients were in WHO functional class III or IV. After 3 years of treatment, the WHO functional class significantly improved in all patients. In addition, the median of the 6-minute walk test (6MWT) significantly increased from 394 ± 91 m at baseline to 458 ± 100 m after 3 years of follow-up (p= 0.014). The three-year survival rate was 96.7%.

Conclusion

In our real-life cohort, most patients with PH treated with riociguat showed stable or improved risk parameters, especially in the 6MWT, at 3 years of follow-up.

Correlation between Degrees of Pulmonary Hypertension and Findings in Complementary Tests

João Arthur Pazello et al. Ana Cristina Camarozano

Abstract

Introduction

Pulmonary hypertension is potentially fatal and courses with important day-to-day limitation. While the treatment is capable of slowing the disease’s progression, increase in life expectancy is directly linked to early diagnosis and treatment.

Objectives

To analyze the relation between alterations detectable on routine exams performed on pulmonary hypertension patients (echocardiography, electrocardiography and computerized tomography) and increases on pulmonary artery systolic pressure.

Methods

We analyzed the recordings of patients presented with pulmonary hypertension, and separated them in two groups based on their echocardiography-estimated pulmonary artery systolic pressure. Group 1 was composed of patients with pulmonary artery systolic pressure between 35 mmHg and 65 mmHg, and Group 2 of patients with pulmonary artery systolic pressure > 65 mmHg. We analyzed the prevalence of alterations suggestive of pulmonary hypertension on echocardiography, electrocardiography and computerized tomography. A descriptive analysis of the findings was conducted, followed by comparative analysis between the groups.

Results

We analyzed 101 patients (43 from Group 1 and 58 from Group 2). Most were women (82). Higher pulmonary artery systolic pressure values were correlated with right-heart dilation on echocardiography (p < 0.001). Electrocardiography findings revealed that right-heart hypertrophy and ventricular strain pattern were more common on Group 2 (p < 0.05). Computerized tomography analysis showed Group 2 patients had superior pulmonary artery diameters (p < 0.05). There was correlation between high pulmonary artery systolic pressure and clinical progression of the disease measured via the NYHA score (p < 0.05).

Conclusion

It is possible to make use of clinical examination and a simple electrocardiography to stratify the severity of a pulmonary artery patient. Computerized tomography and echocardiography should be used to confirm these findings. (Arq Bras Cardiol: Imagem cardiovasc. 2018;31(3):175-182)

Echocardiographic Evaluation of Pulmonary Hypertension in Children

Karen Saori Shiraishi Sawamura et al. Alessandro Cavalcanti Lianza Gabriela Nunes Leal Samira Saady Morhy

Abstract

Hypertensive Pulmonary Disease (HPD) can be defined as a set of pathophysiological pulmonary disorders that result in severe, progressive disease with high morbidity and mortality. Transthoracic echocardiogram (TTE) is an easily accessible and essential imaging method for the evaluation of this disease, especially in children, where there are limitations to frequent and routine right-heart catheterization. In this review, we address the main echocardiographic techniques for the diagnosis and hemodynamic evaluation of pulmonary hypertension in the pediatric population. Early diagnosis and appropriate staging in the follow-up of clinical interventions are fundamental for the assertive choice of therapeutic approach and, consequently, improvement of clinical outcomes.

Long-Term Clinical and Hemodynamic Outcomes after Heart Transplantation in Patients Pre-Treated with Sildenafil

Sofia Lázaro Mendes et al. Nadia Moreira Manuel Batista Ana Rita Ferreira … Ana Vera Marinho David Prieto Rui Baptista Susana Costa Fatima Franco Mariano Pego Manuel de Jesus Antunes

Abstract

Background

Elevated pulmonary vascular resistance remains a major problem for heart transplant (HT) candidate selection.

Objective

This study sought at assess the effect of pre-HT sildenafil administration in patients with fixed pulmonary hypertension.

Methods

This retrospective, single-center study included 300 consecutive, HT candidates treated between 2003 and 2013, in which 95 patients had fixed PH, and of these, 30 patients were treated with sildenafil and eventually received a transplant, forming Group A. Group B included 205 patients without PH who underwent HT. Pulmonary hemodynamics were evaluated before HT, as well as 1 week after and 1 year after HT. Survival was compared between the groups. In this study, a p value < 0.05 was considered statistically significant.

Results

After treatment with sildenafil but before HT, PVR (-39%) and sPAP (-10%) decreased significantly. sPAP decreased after HT in both groups, but it remained significantly higher in group A vs. group B (40.3 ± 8.0 mmHg vs 36.5 ± 11.5 mmHg, p=0.022). One year after HT, sPAP was 32.4 ± 6.3 mmHg in group A vs 30.5 ± 8.2 mmHg in group B (p=0.274). The survival rate after HT at 30 days (97% in group A versus 96% in group B), at 6 months (87% versus 93%) and at one year (80% vs 91%) were not statistically significant (Log-rank p=0.063). After this first year, the attrition rate was similar among both groups (conditional survival after 1 year, Log-rank p=0.321).

Conclusion

In patients with severe PH pre-treated with sildenafil, early post-operative hemodynamics and prognosis are numerically worse than in patients without PH, but after 1 year, the medium to long-term mortality proved to be similar. (Arq Bras Cardiol. 2021; 116(2):219-226)

Systematic Staged Percutaneous Balloon Pulmonary Angioplasty in Severe Inoperable Chronic Thromboembolic Pulmonary Hypertension

Kazuhiro Dan et al. Akira Shionoda Hiromi Matsubara

Introduction

The treatment for chronic thromboembolic pulmonary hypertension (CTEPH) is limited to pulmonary endarterectomy (PEA), drug therapy, and percutaneous balloon pulmonary angioplasty (BPA). The gold-standard treatment is PEA, and patients with CTEPH and proximal lesions are generally good surgical candidates. Perioperative complications and persistent pulmonary hypertension due to incomplete endarterectomy or secondary vasculopathy are typical problems after the procedure. A meta-analysis has shown that the efficacy of drug therapy for severe CTEPH is limited, and many patients do not achieve sufficiently reduced pulmonary artery (PA) pressure, even if exercise tolerance is marginally improved. Percutaneous BPA was first reported in 2001, but its safety was not proven at that time. As recently as one decade ago, pulmonary hypertension was first associated with poor prognosis. Fortunately, treatments for pulmonary hypertension have dramatically improved ever since, particularly for patients with CTEPH. BPA is still a challenging strategy, limited to a specific operator and facility, but its results have been improving.

Mitral Valve Repair in Young Rheumatic Patients

Pablo Maria Alberto Pomerantzeff

Mitral valve repair surgery presents excellent immediate and late results in young patients with valve disease resulting from chronic rheumatic heart disease such as those reported in the article by Cruz et al. in this issue.

There is agreement in the literature on the lower morbidity and mortality of patients submitted to mitral repair surgery in relation to valve replacement, but there is no uniformity of results of mitral valve reconstruction in patients with lesions resulting from rheumatic fever, perhaps, due to the recurrence of new rheumatic episodes in the evolution of these patients.

Pulmonary Hypertension in General Cardiology Practice

Daniela Calderaro et al. José Leonidas Alves Junior Caio Júlio César dos Santos Fernandes Rogério Souza

Abstract

The finding of pulmonary hypertension (PH) by echocardiography is common and of concern. However, echocardiography is just a suggestive and non-diagnostic assessment of PH. When direct involvement of pulmonary circulation is suspected, invasive hemodynamic monitoring is recommended to establish the diagnosis. This assessent provides, in addition to the diagnostic confirmation, the correct identification of the vascular territory predominantly involved (arterial pulmonary or postcapillary). Treatment with specific medication for PH (phosphodiesterase type 5 inhibitors, endothelin receptor antagonists and prostacyclin analogues) has been proven effective in patients with pulmonary arterial hypertension, but its use in patients with PH due to left heart disease can even be damaging. In this review, we discuss the diagnosis criteria, how etiological investigation should be carried out, the clinical classification and, finally, the therapeutic recommendations for PH.

Left Ventricular Assist Device as a Bridge to Candidacy in End-stage Chagas Cardiomyopathy

Fernando Antibas Atik et al. Claudio Ribeiro Cunha Renato Bueno Chaves Marcelo Botelho Ulhoa Vitor Salvatore Barzilai

Introduction

Chronic Chagas cardiomyopathy manifests late after Trypanosoma cruzi infection, and it is still one of the major causes of end-stage heart failure in Latin America. Published experiences, with heart transplantation for chronic Chagas disease have shown the feasibility and efficacy of this therapy, being the disease reactivation not a major concern when appropriately diagnosed and timely treated.

Nevertheless, the need for some sort of mechanical circulatory support on the waiting list has progressively increased, according to the International Society for Heart and Lung Transplantation Registry annual report. Since the experience with mechanical circulatory support in Latin America is very limited, and biventricular systolic failure is common in Chagas cardiomyopathy, many unanswered questions regarding modalities of support need to be elucidated.

Right Ventricular Dysfunction in Lupus Patients With Pulmonary Hypertension

Silvio Henrique Barberato

The importance of the right ventricle in cardiovascular physiology has been underestimated for decades. Previously considered a mere conduit, the right ventricle is currently known to play a major role in maintaining global cardiac function intact. In parallel, right ventricular (RV) systolic function has been shown to be an essential determinant of clinical outcomes in several scenarios, and should thus be considered in the individualized management of patients. The need to diagnose RV dysfunction is evident. Because of its wide availability, echocardiography is the most frequently used imaging test in clinical practice to assess RV size and function. That assessment can be hindered by the complex RV anatomy; thus, important international societies of cardiovascular imaging have recommended the routine and systematic addition of several echocardiographic measurements and techniques., That approach includes conventional parameters, such as RV basal diameter (normal ≤ 41 mm) and tricuspid annular plane systolic excursion (TAPSE – normal ≥ 17 mm), as well as advanced parameters, such as the s wave of the RV free wall on tissue Doppler (normal ≥ 9.5 cm/s), ejection fraction on 3D echocardiography (normal ≥ 45%) and longitudinal strain of the RV free wall (normal ≥ -20%).

In this scenario, strain (systolic shortening percentage) and strain rate (shortening rate), calculated by speckle tracking on two-dimensional echocardiography (2D speckle tracking or 2D-STE), emerge as alternatives in the RV systolic function analysis. The longitudinal strain of the RV free wall, excluding the ventricular septum, showed prognostic value in patients with signs and symptoms of cardiopulmonary disease, such as heart failure, myocardial infarction, pulmonary hypertension, congenital heart diseases, RV arrhythmogenic cardiomyopathy and amyloidosis. Right ventricular longitudinal strain is a parameter less dependent on the angle, with less intra- and interobserver variability, that can apparently detect early RV dysfunction. Its drawbacks include the high dependence on image quality and the variability of the software of the equipment available in the market. Recently an international consensus has been reached to standardize the use of 2D-STE to obtain RV strain. The specific use of right-ventricle-focused apical 4-chamber view is recommended for correct strain measurement. Extreme care should be taken to define the region of interest (ROI) of the endocardial border (suggested ROI: 5 mm), because of the RV shape and thin walls. The pericardium should be excluded from the analysis, because of the risk of strain underestimation.

Early Assessment of Right Ventricular Function in Systemic Lupus Erythematosus Patients using Strain and Strain Rate Imaging

Runlan Luo et al. Hongyan Cui Dongmei Huang Lihua Sun … Shengda Song Mengyao Sun Guangsen Li

Abstract

Background:

Right ventricular function is a crucial factor of the prognosis of systemic lupus erythematosus (SLE).

Objectives:

To evaluate the right ventricular function in SLE patients with different degrees of pulmonary hypertension (PH) by strain and strain rate imaging.

Methods:

A total of 102 SLE patients and 30 healthy volunteers were studied between October 2015 and May 2016. Patients were divided into three groups according to pulmonary artery systolic pressure (PASP) estimated by echocardiography: group control (A); PASP ≤ 30 mmHg (group B, n = 37); PASP 30-50 mmHg (mild PH; group C, n = 34); and PASP ≥ 50 mmHg (moderate-to-severe PH; group D, n = 31). Longitudinal peak systolic strain (ε) and strain rate (SR), including systolic strain rate (SRs), early diastolic strain rate (SRe) and late diastolic strain rate (SRa) were measured in the basal, middle and apical segments of the right ventricular free wall in participants by two-dimensional speckle tracking echocardiography (2D-STE) from the apical four-chamber view. A p < 0.05 was set for statistical significance.

Results:

The parameters of ε, SRs, SRe, and SRa were significantly decreased in groups C and D compared with groups A and B. The ε of each segments was significantly lower in group D than in group C, while there were no differences in SRs, SRe and SRa between groups C and D.

Conclusions:

Strain and strain rate imaging could early detect the right ventricular dysfunction in SLE patients with PH, and provide important value for clinical therapy and prognosis of these patients. (Arq Bras Cardiol. 2018; [online].ahead print, PP.0-0)

Use of Thrombolysis in PE Said to be of no High-Risk, which Presents Acute Cor Pulmonale

Nicolle Lopes Guenther et al. Nágela Simão Vinhosa Nunes Valdênia Pereira de Souza Ronaldo Vegni e Souza

Introduction

PE is a condition that is often difficult to recognize due to its varied clinical presentation, ranging from unclear symptoms to severe conditions that quickly leading the patient to shock and death. It is the third most common cardiovascular disease worldwide, of high mortality and morbidity, with an estimated incidence of 100-200 per 100,000 inhabitants, according to the Guidelines of the European Society of Cardiology.

There is a variety of related risk factors, including major orthopedic surgeries, traumas, prolonged immobilization, malignant neoplasia, spastic and flaccid paralysis, hormonal contraception, congestive heart failure, thrombophilia, obesity and pregnancy., However, PE can occur even in the absence of known risk factors.

Fractal Dimension in Quantifying Experimental-Pulmonary-Hypertension-Induced Cardiac Dysfunction in Rats

Francis Lopes Pacagnelli et al. Ana Karênina Dias de Almeida Sabela Thaoan Bruno Mariano Guilherme Akio Tamura Ozaki … Robson Chacon Castoldi Edna Maria do Carmo Robson Francisco Carvalho Loreta Casquel Tomasi Katashi Okoshi Luiz Carlos Marques Vanderlei

Abstract

Background:

Right-sided heart failure has high morbidity and mortality, and may be caused by pulmonary arterial hypertension. Fractal dimension is a differentiated and innovative method used in histological evaluations that allows the characterization of irregular and complex structures and the quantification of structural tissue changes.

Objective:

To assess the use of fractal dimension in cardiomyocytes of rats with monocrotaline-induced pulmonary arterial hypertension, in addition to providing histological and functional analysis.

Methods:

Male Wistar rats were divided into 2 groups: control (C; n = 8) and monocrotaline-induced pulmonary arterial hypertension (M; n = 8). Five weeks after pulmonary arterial hypertension induction with monocrotaline, echocardiography was performed and the animals were euthanized. The heart was dissected, the ventricles weighed to assess anatomical parameters, and histological slides were prepared and stained with hematoxylin/eosin for fractal dimension analysis, performed using box-counting method. Data normality was tested (Shapiro-Wilk test), and the groups were compared with non-paired Student t test or Mann Whitney test (p < 0.05).

Results:

Higher fractal dimension values were observed in group M as compared to group C (1.39 ± 0.05 vs. 1.37 ± 0.04; p < 0.05). Echocardiography showed lower pulmonary artery flow velocity, pulmonary acceleration time and ejection time values in group M, suggesting function worsening in those animals.

Conclusion:

The changes observed confirm pulmonary-arterial-hypertension-induced cardiac dysfunction, and point to fractal dimension as an effective method to evaluate cardiac morphological changes induced by ventricular dysfunction.

Case 1/2016 – 56-Year-Old Male with Atrial Septal Defect, Pulmonary Arterial Hypertension, Hospitalized Due to Eisenmenger Syndrome

Carolina Santana et al. Antonio Augusto B. Lopes Antonio Fernando Lins de Paiva Luiz Alberto Benvenuti

Postmortem examination

The heart was enlarged and globoid, weighing 832 g. On gross examination, the RV was prominent. The opening of the cavities evidenced eccentric hypertrophy of the four chambers, mainly the right ones, which showed marked dilation. A large ASD was identified in the fossa ovalis (ostium secundum type), measuring 3.5 x 3.0 cm (). The ventricular septum was intact, no other heart malformation being identified. The microscopic examination revealed hypertrophy of the cardiomyocytes and myocardial interstitial fibrosis in both ventricles, with no evidence of ischemic lesions (infarction). The pulmonary trunk was dilated (diameter of 4.0 cm) and showed complicated atherosclerosis, with calcified areas. The right pulmonary artery showed calcified atherosclerosis, organized partial thrombosis, and markedly dilation of its parenchymatous branches. The process was similar in the left lung, but the thrombosis was more extensive, affecting numerous arterial branches, in an occlusive way, including the pulmonary hilum (). The microscopic examination of the lungs confirmed the extensive organizing arterial thrombosis detected on gross examination, in addition to revealing: organized and rechanneled arteriolar thrombosis (); recent focal thrombosis of a small artery and small hemorrhagic infarction in the lower lobe of the right lung (); areas of alveolar hemorrhage; anthracosis; microscopic evidence of chronic bronchitis; and diffuse pulmonary emphysema. In addition, a small in situ adenocarcinoma was found in the right lung, measuring 9 mm. Moderate aorta atherosclerosis was identified, with calcified plaques. Neither pneumothorax nor bronchopneumonia were evidenced.

Óscar Fabregat-Andrés et al. Jordi Estornell-Erill Francisco Ridocci-Soriano José Leandro Pérez-Boscá … Pilar García-González Rafael Payá-Serrano Salvador Morell Julio Cortijo

Abstract

Background:

Pulmonary hypertension is associated with poor prognosis in heart failure. However, non-invasive diagnosis is still challenging in clinical practice.

Objective:

We sought to assess the prognostic utility of non-invasive estimation of pulmonary vascular resistances (PVR) by cardiovascular magnetic resonance to predict adverse cardiovascular outcomes in heart failure with reduced ejection fraction (HFrEF).

Methods:

Prospective registry of patients with left ventricular ejection fraction (LVEF) < 40% and recently admitted for decompensated heart failure during three years. PVRwere calculated based on right ventricular ejection fraction and average velocity of the pulmonary artery estimated during cardiac magnetic resonance. Readmission for heart failure and all-cause mortality were considered as adverse events at follow-up.

Results:

105 patients (average LVEF 26.0 ±7.7%, ischemic etiology 43%) were included. Patients with adverse events at long-term follow-up had higher values of PVR (6.93 ± 1.9 vs. 4.6 ± 1.7estimated Wood Units (eWu), p < 0.001). In multivariate Cox regression analysis, PVR ≥ 5 eWu(cutoff value according to ROC curve) was independently associated with increased risk of adverse events at 9 months follow-up (HR2.98; 95% CI 1.12-7.88; p < 0.03).

Conclusions:

In patients with HFrEF, the presence of PVR ≥ 5.0 Wu is associated with significantly worse clinical outcome at follow-up. Non-invasive estimation of PVR by cardiac magnetic resonance might be useful for risk stratification in HFrEF, irrespective of etiology, presence of late gadolinium enhancement or LVEF.

Association Between Hemodynamic Profile, Physical Capacity and Quality of Life in Pulmonary Hypertension

Diego de Faria Magalhães Torres et al. Walter Araujo Zin Agnaldo José Lopes Patrícia dos Santos Vigário … Marcelo Iorio Garcia Daniel Waetge5 Marcelo Luiz da Silva Bandeira Luiz Gustavo Pignataro Bessa Fernando Silva Guimarães

Background:

No studies have described and evaluated the association between hemodynamics, physical limitations and quality of life in patients with pulmonary hypertension (PH) without concomitant cardiovascular or respiratory disease.

Objective:

To describe the hemodynamic profile, quality of life and physical capacity of patients with PH from groups I and IV and to study the association between these outcomes.

Methods:

Cross-sectional study of patients with PH from clinical groups I and IV and functional classes II and III undergoing the following assessments: hemodynamics, exercise tolerance and quality of life.

Results:

This study assessed 20 patients with a mean age of 46.8 ± 14.3 years. They had pulmonary capillary wedge pressure of 10.5 ± 3.7 mm Hg, 6-minute walk distance test (6MWDT) of 463 ± 78 m, oxygen consumption at peak exercise of 12.9 ± 4.3 mLO2.kg-1.min-1 and scores of quality of life domains < 60%. There were associations between cardiac index (CI) and ventilatory equivalent for CO2 (r=-0.59, p <0.01), IC and ventilatory equivalent for oxygen (r=-0.49, p<0.05), right atrial pressure (RAP) and 'general health perception' domain (r=-0.61, p<0.01), RAP and 6MWTD (r=-0.49, p<0.05), pulmonary vascular resistance (PVR) and 'physical functioning' domain (r=-0.56, p<0.01), PVR and 6MWTD (r=-0.49, p<0.05) and PVR index and physical capacity (r=-0.51, p<0.01).

Conclusion:

Patients with PH from groups I and IV and functional classes II and III exhibit a reduction in physical capacity and in the physical and mental components of quality of life. The hemodynamic variables CI, diastolic pulmonary arterial pressure, RAP, PVR and PVR index are associated with exercise tolerance and quality of life domains.

Atrial Septal Defect in Adults: Does Repair Always Mean Cure?

Antônio Augusto Lopes et al. Sonia M. F. Mesquita

Atrial septal defect (ASD) is the most prevalent congenital cardiac anomaly in adults, accounting for ~35% of all congenital heart defects. Late presentation is due to the insidious development of right ventricular remodeling, with enlargement of right cardiac chambers. Patients may seek medical attention with symptoms related to right or global myocardial dysfunction, arrhythmias or thromboembolic events. Until recently, pulmonary vasculopathy was assumed to be a rare complication in ASD patients. However, with progressive knowledge and interest in treating pulmonary arterial hypertension (PAH), numbers are changing. Of 1877 adults with congenital heart disease followed up for five years in the European Union, 896 had ASD, with PAH prevalences of 12% and 34% for repaired and unrepaired defects respectively. Fifteen patients had the full picture of the Eisenmenger syndrome.

The fortunate side of the scenario is that the widespread use of echocardiography has allowed a big number of patients to be diagnosed early. Furthermore, transcatheter closure of secundum defects offers a less invasive alternative of repair to patients who fulfill anatomical and size criteria, and may be an appropriate therapeutic option for subjects with paradoxical embolism. Also minimally invasive video-assisted surgical techniques have allowed successful closure of the communications, and represent a new and promising therapeutic approach to patients with ASD. The dark side of the story is: with late presentation, treatments may allow patients to become free from the communications, but not from advanced right ventricular remodeling and failure, severe arrhythmias and pulmonary vasculopathy.

Pulmonary arterial hypertension: use of delayed contrast-enhanced cardiovascular magnetic resonance in risk assessment

Luiz Gustavo Pignataro Bessa et al. Flávia Pegado Junqueira Marcelo Luiz da Silva Bandeira Marcelo Iorio Garcia … Sérgio Salles Xavier Guilherme Lavall Diego Torres Daniel Waetge

BACKGROUND: Pulmonary arterial hypertension is a severe and progressive disease. Its early diagnosis is the greatest clinical challenge. OBJECTIVE: To evaluate the presence and extension of the delayed myocardial contrast-enhanced cardiovascular magnetic resonance, as well as to verify if the percentage of the myocardial fibrosis mass is a severity predictor. METHODS: Cross-sectional study with 30 patients with pulmonary arterial hypertension of groups I and IV, subjected to clinical, functional and hemodynamic evaluation, and to cardiac magnetic resonance. RESULTS: The mean age of patients was 52 years old, with female predominance (77%). Among the patients, 53% had right ventricular failure at diagnosis, and 90% were in functional class II/III. The mean of the 6-minute walk test was 395m. In hemodynamic study with right catheterism, the mean average pulmonary arterial pressure was 53.3mmHg, of the cardiac index of 2.1L/ min.m², and median right atrial pressure was 13.5 mmHg. Delayed myocardial contrast-enhanced cardiovascular magnetic resonance was found in 28 patients. The mean fibrosis mass was 9.9 g and the median percentage of fibrosis mass was 6.17%. The presence of functional class IV, right ventricular failure at diagnosis, 6-minute walk test 15mmHg, with cardiac index < 2.0L/ min.m², there was a relevant association with the increased percentage of myocardial fibrosis. CONCLUSION: The percentage of the myocardial fibrosis mass indicates a non-invasive marker with promising perspectives in identifying patients with high risk factors for pulmonary hypertension.
