Diabetes mellitus classification

Diabetes mellitus classification

Jorge de Faria Maraschin et al. Nádia Murussi Vanessa Witter Sandra Pinho Silveiro

The right classification for diabetes mellitus (DM) allows a more adequate treatment and comprises four categories: type 1 DM, type 2 DM, other types, and gestational diabetes. In some cases, there might be a superposition of situations, especially with regard to the DM that initiates in the young adult or is initially presented with diabetic ketoacidosis intermediately to type 1 and 2 DM. Thus, additions to the classic classification system have been proposed as assessing the presence of autoimmunity (antibody) and b cell function (C-peptide) to precisely define the subtypes. The aim of this literature review was to analyze these diagnostic indexes’ performance in the DM classification and to describe subtypes with details. The antibodies against pancreas confirm autoimmunity, and the antibody against insulin is more accurate before 5 years old, while the anti-glutamic acid decarboxylase is more accurate after 20 years old, a test which remains positive for a longer period. The measurement of C-peptide evaluates the pancreatic insulin reserve, and the most largely used methods of stimulation are the measurement after meals or after intravenous glucagon. C-peptide values < 1.5 ng/ml define a patient with absent pancreatic function and, above this value, patients with preserved function. When the presence of antibodies (A+) directed to the pancreas is combined to its insulin secretion capability (β+), it is possible to subdivide DM’s classification in type 1A (A+β-) and 1B (A+β-); and type 2A (A+β+) and 2B (A-β+), which allows a more precise classification and treatment besides opening horizons for the understanding of DM pathogenesis.

Experimental atherosclerosis in rabbits

Waleska C. Dornas et al. Tânia T. de Oliveira Luis E. Franklin Augusto Tanus J. Nagem

Many researches have been conducted in experimental models in order to study the development of atherosclerosis from hyperlipidemia-inducing diets. Since rabbits are very sensitive to cholesterol-rich diets and accumulate large amounts of cholesterol in their plasma, their use as experimental models to evaluate the development of atherosclerosis is highly relevant and brings information on factors that contribute to the progression and regression of this condition that can be applied to humans. As such, this review includes studies on the atherogenic function of cholesterol based on rabbits as the experimental model, since they have become the most largely used experimental model of atherosclerosis.

Two-dimensional strain in Takotsubo cardiomyopathy

Carlos Bellini G. Gomes et al. Gustavo J. Veras

This report presents the late follow-up of a case of Takotsubo cardiomyopathy with good clinical outcome and improved left ventricular global systolic function. However, there was persistence of significant regional longitudinal systolic dysfunction evaluated using a new echocardiographic technique (speckle tracking), with corresponding measures of strain (S) and strain rate (SR). We emphasize the importance of this new method to monitoring this cardiomyopathy, since it identifies patients with persistent systolic dysfunction who will possibly benefit from maintenance of clinical therapy

Evaluation of the metabolism of high energy phosphates in patients with Chagas' disease

Ana Maria Betim Paes Leme et al. Vera Maria Cury Salemi José Rodrigues Parga Bárbara Maria Ianni … Charles Mady Robert G. Weiss Roberto Kalil-Filho

BACKGROUND: Abnormalities in myocardial metabolism have been observed in patients with heart failure of different etiologies. Magnetic resonance spectroscopy (MRS) with phosphorus-31 is a noninvasive technique that allows detection of myocardial metabolic changes. OBJECTIVE: To determine the resting metabolism of high-energy phosphates in patients with Chagas’ disease (CD) by MRS with phosphorus-31. METHODS: We studied 39 patients with CD, 23 with preserved ventricular function (PF Group) and 16 with ventricular dysfunction (VD Group), assessed by Doppler echocardiography. MRS of the anterosseptal region was performed in 39 patients and 8 normal subjects (C Group) through a Phillips 1.5 Tesla device, obtaining the phosphocreatine/beta-adenosine triphosphate myocardial ratio (PCr/β-ATP). RESULTS: The levels of cardiac PCr/β-ATP were reduced in VD Group in relation to PF Group, and the latter presented reduced levels compared to C Group (VD Group: 0.89 ± 0.31 vs PF Group: 1.47 ± 0.34 vs C Group: 1.88 ± 0.08, p < 0.001). A correlation was found between left ventricular ejection fraction and PCr/β-ATP in 39 patients (r = 0.64, p < 0.001). Patients under functional class I (n = 22) presented PCr/β-ATP of 1.45 ± 0.35, and those in functional classes II and III (n = 17), PCr/β-ATP of 0.94 ± 0.36 (p < 0.001). CONCLUSION: The 31-phosphorus MRS was able to detect non-invasively changes in the rest energy metabolism of patients with Chagas' disease, with and without systolic dysfunction. These changes were related to the severity of heart impairment.

Validation of Missouri aneroid sphygmomanometer to measure blood pressure in patients with cancer

Karine Azevedo São Leão Ferreira et al. Ana Claúdia dos Santos Thais Cardoso Arthur Daniela Aparecida A. dos Santos … Daniela Pereira Elizângela Oliveira Freitas Fernanda Medeiros Fukuda Wânia Regina Mollo Baia

BACKGROUND: Auscultatory mercury sphygmomanometers to measure blood pressure (BP) have been banned from health services because of risk of pollution and environmental accidents with mercury. Aneroid appliances could be an alternative. OBJECTIVE: To validate the MissouriTM aneroid device for blood pressure measurement in cancer patients according to the protocol of the European Society of Hypertension (ESH). METHODS: 33 patients hospitalized or under outpatient care at the Cancer Institute of the State of São Paulo, FMUSP, were evaluated. Three trained and blinded observers performed nine sequential blood pressure measurements interspersed with the mercury sphygmomanometers. The differences between the values of systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the test device with the mercury sphygmomanometer were classified according to the ESH protocol. RESULTS: The MissouriTM equipment underwent all three phases required by the ESH Protocol for SBP and DBP, and it was approved in all of the phases. The average difference between the test device and the mercury sphygmomanometer was 0.62 (SD = 4.53) and 0.06 (SD = 6.57) mmHg for SBP and DBP, respectively. No association was found between the differences in BP measurements with sex, age, body mass index and arm circumference and length. CONCLUSION: The results revealed that the aneroid MissouriTM device meets ESH accuracy recommendations for the measurement of SBP and DBP, and it can be used to replace the mercury sphygmomanometer.

Correlation between quality of life and functional capacity in cardiac failure

Ivan Daniel Bezerra Nogueira et al. Denise Maria Servantes Patrícia Angélica de Miranda Silva Nogueira Amália Pelcerman … Xiomara Miranda Salvetti Fernando Salles Dirceu Rodrigues Almeida Marco Túlio de Mello Orlando Campos Filho Japy Angelini Oliveira Filho

BACKGROUND: Patients with cardiac failure (CF) present progressive incapacity and decreased quality of life, both related to dyspnea and fatigue. Thus, there is the increasing interest in measring the quality of life (QL), by generic instrument, such as the 36-item Short-Form Health Survey (SF-36), by specific instrument, such as Minnesota Living with Heart Failure (MLHFQ). OBJECTIVE: This study has the objective to correlate the QL surveys, SF-36 and MLHFQ, with the functional capacity of patients with CF, expressed by the cardiopulmonary test and the TC6M. METHODS: Using the SF-36 and MLHFQ surveys for QL evaluation, for the evaluation of the functional capacity, it was used the cardiopulmonary test, being executed using a tredmill with Weber protocol, as well sa the distance covered in the walk test of six minutes (TC6M). RESULTS: Forty-six patients were selected with CF diagnosis (22 men, average age of 52 years old), classes II and III of New York Heart Association. It was observed that the mild correlation between the physical and emotional domains of SF-36 and VE/VCO2.peak (r=-0.3; p<0.05) and the distance covered in TC6M (r=0.4; p<0.05), respectively. It was also observed the mild to moderate correlations of MLHFQ total score with VO2.peak (r=-0.5; p<0.05), the aerobic threshold (r=-0.4; p<0.05) and the distance covered in TC6M (r=-0.5; p<0.05). CONCLUSION: The data suggest that the application of both evaluation instruments of QL, generic (SF-36) and specific (MLHFQ) in patients with CF, showed mild and moderate correlation with the variable of the cardiopulmonary test with the variables of the cardiopulmonary test and the distance covered in TC6M.

Therapeutic inertia and control of high blood pressure in primary health care units

Clóvis Hoepfner et al. Selma Cristina Franco

BACKGROUND: The importance of adequate management and control of high blood pressure (HBP). OBJECTIVE: To estimate the prevalence of hypertension control and therapeutic inertia among adults treated at primary health care units (PHCU) in the city of Joinville, as well as the associated factors. METHODS: A cross-sectional study, which included cluster sampling and analysis of medical records, with the evaluation of 415 patients with high blood pressure. We evaluated the blood pressure (BP), increments of therapy, risk factors and comorbidities. RESULTS: There was prevalence of females and nursing appointments. The age ranged between 28 and 90 years (mean of 61.5 years). There was a reduction in the mean BP (155.8 ± 20.8/95.7 ± 10.6 mmHg to 140.3 ± 22/84.1 ± 12.4 mmHg) between the first and last record and the final normal BP in 36.6% of patients, with similar results for men and women. Over the past 12 months, the BP was high on 1,295 occasions, and there was a therapeutic increment on only 156 occasions (12.0%). 1.85 drugs were used per patient, mainly diuretics and ACE inhibitors. There was high prevalence of obesity (40%), diabetes (41%), high LDL (46%) and left ventricular hypertrophy (25.5%). CONCLUSION: The high therapeutic inertia, low control of high blood pressure and the large number of comorbidities suggest the need for continuing education programs for health professionals and other measures to improve the disease control in primary health care units.

Blood pressure reduction in hyper-reactive individuals after aerobic exercise

Cleusa Maria Richter et al. Tiago Facchini Panigas Daiana Cristine Bündchen Thiago Dipp … Kalyse Claudino Belli Paulo Ricardo Nazário Viecili

BACKGROUND: Some normotensive sedentary individuals have an exaggerated increase in blood pressure (BP) during physical activity, which is a behavior that is called blood pressure hyper-reactivity. OBJECTIVE: To investigate the effect of an aerobic exercise program (AEP) on blood pressure in individuals with blood pressure hyper-reactivity (BPH). METHODS: Ten male volunteers with BPH, aged 45 ± 10 years, referred to as the experimental group (EG), took part in an AEP on a treadmill, 3 times a week for two months. They were compared to 14 men with BPH, aged 48 ± 8 years, referred to as the control group (CG), who remained sedentary. The subjects were evaluated before and after the AEP by stress test for comparison purposes. We evaluated the initial, peak and test-end heart rates (HR), systolic blood pressure (SBP) and diastolic blood pressure (DBP). RESULTS: There a significant decrease in the initial SBP (-5%; p=0.01), initial DBP (-4.6%; p=0.01), peak SBP (-12.4%; p=0.001), peak DBP (-14.7%; p=0.03), final SBP (-4.6%, p=0.03) in the EG. The CG continued with its hyper-reactive behavior, which evolved to more exaggerated levels when the results before and after the study were compared (p<0.04). In the HR, there was an increase only in the final HR, of 11.3 bpm, after training (p=0.02). The test-peak VO2 increased by 4.4 ml.kg-1 .min-1 in the EG (p=0.01) and remained similar in the CG. CONCLUSION: The AEP normalized the behavior of the blood pressure hyper-reactivity in sedentary men. (Arq Bras Cardiol. 2010; [online]. ahead print, PP.0-0)

Dietary habits and risk factors for atherosclerosis in students from Bento Gonçalves (state of Rio Grande do Sul)

Hosana Maria Speranza Cimadon et al. Renata Geremia Lucia Campos Pellanda

BACKGROUND: Atherosclerotic cardiovascular disease begins its process in early childhood and is influenced throughout life by genetic factors and environmental exposure to potentially modifiable risk factors. OBJECTIVE: To investigate the prevalence of risk factors for atherosclerosis with emphasis on dietary habits in a predominantly Italian colonization town. METHODS: Population-based cross sectional study, involving 590 primary school students aged between 9 and 18 years, with a cluster sample. The following were collected: identification data, family history and personal history, and information regarding students’ eating habits. Dietary habits considered inappropriate included: consumption of fast food, sugary snacks, sugar-sweetened beverages and animal fats four or more times a week, and fruits, green vegetables, and leguminous vegetables less than four times a week. RESULTS: The prevalence of overweight among students was 24.6% (n = 145), high blood pressure, 11.1% (n = 65); passive smoking, 35.4% (n = 208); sedentary lifestyle, 52.3% (n = 306), family history of 1st degree disease: hypertension, 21.4%, obesity 36.5%. Food items eaten four or more times a week: fast food, 70.3% (n = 411); sugary snacks, 42.7% (n = 252), sugar-sweetened beverages, 71% (n = 419), and animal fats, 24.4% (n = 143). Food items eaten less than four times a week: fruits, 36.8% (n = 215), green vegetables, 49.5% (n = 292) and leguminous vegetables, 63.7% (n = 374). CONCLUSION: Interventions are needed to promote changes in students’ eating habits: higher level of consumption of fruits, green vegetables and leguminous vegetables, and increased level of physical activity.

Obesity and cardiovascular risk factors in adolescents attending public schools

Fernanda Cobayashi et al. Fernanda Luisa Ceragioli Oliveira Maria Arlete Meil Schimith Escrivão Silveira Daniela José Augusto de Aguiar Carrazedo Taddei

BACKGROUND: Overweight in adolescence is a risk factor for the development of cardiovascular diseases in adulthood. OBJECTIVE: To study the association of cardiovascular risk factors (CRF) in overweight and normal-weight adolescents of both genders aged from 14 to 19 years, attending public schools. METHODS: Case-control study including 163 overweight and 151 normal-weight adolescents. Multiple logistic regression analysis was used to evaluate the associations between overweight and CRF (lipid profile, blood pressure and baseline insulin level). A set of CRF was defined for each individual, ranging from 0 (no risk factors) to 6 (all risk factors present). RESULTS: Overweight adolescents (body mass index > 85th percentile) presented a higher frequency of CRF in comparison to the normal- weight group. The CRF associated with overweight were HDLc 150 mg/dl (OR = 3.04; CI: 1.01-9.13), abnormal baseline insulin levels > 15 µU/ml (OR = 8.65; CI: 4.03-18.56) and abnormal blood pressure (OR = 3.69; CI: 1.76-7.72). Among overweight adolescents, 22.09% had more than three risk factors, whereas this percentage dropped to 6.12% among normal-weight adolescents. CONCLUSION: Overweight adolescents presented risk factors for cardiovascular diseases. The need for programs and policies targeted at the diagnosis and treatment of this condition is pointed out in order to reduce the risks of morbidity and mortality in adulthood.

Direct ophthalmoscopy versus detection of hypertensive retinopathy: a comparative study

José Albuquerque de Figueiredo Neto et al. Guilherme Lima Palácio Allison Nobrega dos Santos Pamela Suelen Serra Chaves … Germana Viana Gomes Tassiane Soares Cabral

BACKGROUND: Detection of hypertensive retinopathy (HR) with direct ophthalmoscopy is part of the assessment of hypertensive patients. Its use has been questioned because of its subjectivity and high interobserver variability. OBJECTIVE: To determine the prevalence of HR in hypertensive patients under outpatient monitoring, the correlation between diagnosis and ophthalmoscopy and angiography, and to correlate it with other target organ damages. METHODS: An observational, analytical and cross-sectional evaluation of 99 patients. Direct ophthalmoscopy and angiography performed by two investigators independently. Classification of RH, as described by Keith, Wagener and Barker. RESULTS: The prevalence of HR of any grade was higher than 90.0% by both methods. On ophthalmoscopy, we observed grade I abnormalities in 51.0%, grade II in 43.0%, with one patient with grade III HR. On angiography, we observed grade I abnormalities in 42.0% and grade II in 52.0%. We detected three patients with grade III HR, two of which were not detected by ophthalmoscopy. Observers’ agreement for the presence and severity of HR was poor with direct ophthalmoscopy and good with angiography. Renal dysfunction, ECG abnormalities (ventricular hypertrophy, pathological Q wave, repolarization abnormalities), and history of stroke were observed in 70.0%, 27.0% and 10.0% of patients, respectively. There was no relationship between the severity of HR and other target organ damages. CONCLUSION: We observed a high prevalence of HR using both methods. Observers’ agreement for the diagnosis and determination of the severity of HR was better with angiography. In our sample, there was no association of the severity of HR with other target organ damages.

Effect of diet and indoor cycling on body composition and serum lipid

Valéria Sales do Valle et al. Danielli Braga de Mello Marcos de Sá Rego Fortes Estélio Henrique Martin Dantas Marco Antonio de Mattos

BACKGROUND: Indoor cycling is an aerobic exercise that employs large muscle groups of the lower limbs, lacking osteoarticular impact and high energy expenditure, which makes it interesting to generate a non-pharmacological strategy. OBJECTIVE: To assess body composition and lipid profile in overweight women after twelve weeks of low-calorie diet and indoor cycling training. METHODS: We randomly assigned 40 women (23.90 ± 3.10 years), divided into four groups: control (C), indoor cycling (CI), indoor cycling combined with low-calorie diet (CD) and low-calorie diet (D). The variables were: height and body mass, BMI, fat percentage, lean body mass, triglycerides, cholesterol and lipoproteins (HDL, LDL, VLDL). The indoor cycling training consisted of three weekly sessions of 45 minutes each and an energy restriction of about 1,200 kcal. The study lasted 12 weeks. We used descriptive statistics (mean and standard deviation) and inferential statistics (Student’s t test). The level of significance was p < 0.05. RESULTS: The groups CI, CD and D significantly reduced the mean anthropometric variables after 12 weeks of intervention (body mass, fat percentage and body mass index), and serum levels of total cholesterol and triglycerides. HDL cholesterol increased significantly for groups CI and CD. CONCLUSION: According to the results, the indoor cycling and the low-calorie diet helped fight overweight and control serum lipids.

Effects of the corticosteroids in the lesions by radiofrequency on rats' thigh in different age groups

Mieko Okada et al. Sérgio de Araújo Marcello Franco Angelo de Paola Guilherme Fenelon

BACKGROUND: The corticosteroids limit the late growth of the lesions by radiofrequency (RF) on the rats’ infants’ thighs, but the effects on the pubescent and adult rats are unknown. OBJECTIVE: Evaluate the effects of the corticosteroids in the healing of the lesions by RF on the rats’ thighs muscles in different age groups. METHODS: Ablation was performed on the thigh muscle of 30 rats (1 lesion per animal): infants (30 days old, weight 73 g, n = 10), pubescents (60 days old, weight 230 g, n = 10) and 10 adults (90 days old, 310 g, n = 10), subdivided in control and treated groups, that received Hydrocortisone (10 mg/kg IM post-RF) and Betametasone (3.5 mg/kg IM, twice a week, for 29 days). The rats were sacrificed 60 days after the ablation for hystopathological and planimetric analysis with specified software (ImageJ®). RESULTS: In the infant, pubescent and adult groups, the weight gain in the follow up did not differ between the control and the treated ones. In the control group, the lesions of the infants and pubescents were superior (p = 0.01) to the adults’. The treatment reduced the size of the lesion in the infants (5.58+0.61 mm² vs 4.02+0.23 mm²; p < 0.01) and pubescents (5.20+0.47 mm² vs 4.16+0.48 mm²; p < 0.01), but not in the adults (4.44+0.50 mm² vs 4.79+0.53 mm², p = NS). Infant and pubescent treated groups presented lower collagen deposition and less fibrotic bands invading the healthy tissue from the central fibrosis area, and forming lesions with remarkably more reduced dimensions than their controls. There were no differences in the adult groups. CONCLUSION: The corticosteroids seem to reduce the late growth of lesion, in addition to attenuate the fibrotic proliferation in the infant and pubescent rats.

Evaluation of the musculoskeletal system of patients in cardiopulmonary and metabolic rehabilitation programs

Tales de Carvalho et al. Angelise Mozerle Anderson Zampier Ulbrich Alexsandro Luiz de Andrade … Emmanuele Elise Campos de Moraes Lourenço Sampaio de Mara

BACKGROUND: Participants of cardiopulmonary and metabolic rehabilitation (CPMR) programs may present with musculoskeletal changes that may affect treatment compliance and effectiveness. OBJECTIVE: To develop an instrument for evaluation of the musculoskeletal system and identification of problems, especially those related to exercise, so that patients can be cleared to exercise with no restrictions, cleared with restrictions, or not cleared before approval from a specialist. METHODS: Construction and validation (according to Cronbach’s alpha) of a musculoskeletal system assessment inventory (MSSAI), for subsequent administration to participants in CPMR programs. RESULTS: A total of 103 individuals participating in CPMR programs were evaluated by means of the MSSAI, whose internal validity and reliability proved to be satisfactory. Of these, 33 were men (32%) and 70 were women (68%), with age ranging from 36 to 84 years; 47 (45.6%) had already been diagnosed with musculoskeletal system disorders; 39 (37.9%) had already received specific treatment for the musculoskeletal system; 33 (32%) used to take medications to relieve symptoms related to the musculoskeletal system; and 10 (9.7%) had a medical restriction for performing some type of exercise. We should point out that 48 individuals (46.6%) reported pain in the musculoskeletal system; in 14 (13.6%) of them, the pain worsened by exercise, and this should have prevented them from participating in exercise programs before receiving approval from a specialist. CONCLUSION: The MSSAI, whose internal validity and reliability proved satisfactory, showed that there was some restriction to exercise practice for almost half of the individuals participating in CPMR programs, and that some of them should not have been cleared without approval from a specialist.

Prevalence of angina pectoris in Pelotas, south of Brazil

Leonardo Alves et al. Juraci A. Cesar Bernardo L. Horta

BACKGROUND: Ischemic heart disease is the leading cause of death in the world and angina is its cardinal manifestation. OBJECTIVE: To determine the prevalence of angina and possible angina and its distribution by main demographic and socioeconomic characteristics among adults 40 years of age or older. METHODS: This is a population-based, cross-sectional study featuring residents of the urban area in Pelotas, a city located in the south of Brazil, from October through December 2007. A two-stage cluster sampling – census tracts and households – was used. The prevalence of angina and possible angina was defined according to the Rose questionnaire. These conditions were assessed by demographic and socioeconomic characteristics: age, sex, skin color, economic status, and schooling. Data were collected using standardized questionnaires in interviews with the individuals in their homes. The non-respondent rate was 6.8%. RESULTS: The prevalence of angina among the 1,680 individuals taking part in the study was 8.2% (95% CI: 6.7 – 9.6), while that of possible angina came to 12.3% (95% CI: 10.6 – 14.0). The prevalence of angina and possible angina was higher among women, black/brown-skinned individuals, low economic class individuals and subjects with the low schooling. The prevalence of angina was higher among older individuals, while possible angina was found not to be associated with age. CONCLUSION: The prevalence of angina and possible angina was found to be high, affecting approximately 20% of the population in Pelotas.

Balloon atrial septostomy guided by echocardiography in a neonatal intensive care unit

Lilian M Lopes et al. Christiane Kawano Salvador André Bavaresco Cristóvão Célia Toshie Nagamatsu … Luciana da Fonseca Beatriz Helena Sanchez Furlanetto Gláucio Furlanetto José Pedro da Silva José Armando Mangione

BACKGROUND: The bedside two-dimensional echocardiography (2-D ECHO) has been successfully used to guide the balloon atrial septostomy, speeding up the procedure and preventing the risks of transportation to the hemodynamics laboratory. OBJECTIVE: To assess the results of the bedside balloon atrial septostomy in cyanotic neonates. METHODS: Between January/1997 and July/2008, 102 atrioseptostomies by balloon catheter guided by echocardiography were carried out and saturation levels, defect diameter, oxygen saturation, clinical-laboratory response and complications related to the procedure were evaluated. RESULTS: Of 102 cases of bedside Rashkind septostomy, 98 met the inclusion criteria, with 90 neonates in Group A (Rashkind procedure in the preoperative phase) and 8 in Group B (procedure at the postoperative phase). There was a predominance of the male sex (75%), mean age was 8.3 ± 9.3 days and the mean weight was 3,100 ± 1,100 g; the transposition of the great arteries was the most frequent congenital heart disease (n = 74). When comparing the levels of saturation pre and post-procedure (65.9 ± 19.5% and 86 ± 9.7%) and the diameter of the interatrial septal defect pre and post-procedure (2.3 ± 1.0 mm and 5.5 ± 1.3 mm) there was a statistically significant difference (p 0.05). CONCLUSION: The balloon atrial septostomy guided by the echocardiography reduces the risks by allowing the effective palliative procedure to be carried out promptly in cyanotic neonates, in addition to presenting lower hospital costs.

Blood pressure, excess weight and level of physical activity in students of a public university

Maria do Carmo de Carvalho e Martins et al. Irapuá Ferreira Ricarte Cláudio Henrique Lima Rocha Rodrigo Batista Maia … Vitor Brito da Silva André Bastos Veras Manoel Dias de Souza Filho

BACKGROUND: High blood pressure, excess weight and sedentary lifestyle are important risk factors for cardiovascular diseases, and they are closely associated. OBJECTIVE: To evaluate the nutritional status, level of physical activity and blood pressure levels of students of Universidade Federal do Piauí, Teresina, Brazil. METHODS: Cross-sectional study with a sample of 605 students (46.1% males and 53.9% females), with a mean age of 21.7 ± 3.7 years. The nutritional status was classified according to body mass index (BMI), and central adiposity according to waist circumference (WC). The level of physical activity was evaluated using the short version of the International Physical Activity Questionnaire (IPAQ). Elevated blood pressure was defined as systolic blood pressure > 140 mmHg and/or diastolic blood pressure > 90 mmHg. RESULTS: The prevalence of elevated blood pressure was 9.7%, and was higher among men. Excess weight (BMI > 25 kg/m²) was found in 18.2% of the students, with overweight and obesity rates of 15.2% and 3%, respectively. Abdominal obesity was found in 2.4% of the students regardless of gender, and sedentary lifestyle in 52%. The mean blood pressure increased with increasing BMI and WC. No association was found between the levels of physical activity and blood pressure. CONCLUSION: An association of increased body weight and waist circumference with higher blood pressure levels was observed among the participants. Instruments for an early assessment of the cardiovascular risk and preventive advice should be established for these young individuals.

Levosimendan in acute decompensated heart failure: systematic review and meta-analysis

Rodrigo Antonini Ribeiro et al. Luis Eduardo Paim Rohde Carisi Anne Polanczyk

BACKGROUND: Congestive heart failure (CHF) is a rather prevalent condition with a high mortality rate. Levosimendan is one among the new drugs that have been tested for its management. OBJECTIVE: To undertake a systematic review and meta-analysis of the morbidity and mortality reduction associated with levosimendan in the treatment of CHF. METHODS: A bibliographic search was conducted in the Medline database for all randomized controlled trials (RCTs) that assessed the use of levosimendan in CHF. The outcomes were death from all causes, length of hospital stay, and hospital readmission for CHF. All RCTs with outcomes of interest were included. Methodological quality criteria, such as blinding and confidentiality of the list of allocation, were evaluated in sensitivity analysis. The main calculation was done with random effects. RESULTS: Of the 179 articles identified, 48 were RCTs, 19 of them with outcomes of interest. In the comparison with placebo (7 trials, 1,652 patients), the relative risk (RR) for overall death was 0.87 (95% confidence interval [CI]: 0.65 to 1.18). In comparison with dobutamine (10 trials, 2,067 patients), the RR was 0.87 (95% CI: 0.75-1.02). Three studies had data on length of stay, in which levosimendan showed a decrease of 2.27 and 2.30 days compared to placebo and dobutamine, respectively (p < 0.05 for both). No article presented data on readmission alone. CONCLUSION: The evidence available so far has shown no benefit in terms of mortality in association with the use of levosimendan, which only showed a small benefit in the time of hospitalization.

AGT*M235T polymorphism in acute ischemic cardiac dysfunction: the gisca project

Claudia Guerra Murad Saud et al. Amália Faria dos Reis Arlisa Monteiro de Castro Dias Rosemery Nunes Cardoso … Ana Cristina Klem Vargas Carneiro Leandro Pereira de Souza Ana Beatriz Monteiro Fonseca Georgina Severo Ribeiro Carlos Augusto Cardozo de Faria

BACKGROUND: AGT*M235T polymorphism has been associated with high serum angiotensinogen (AGT) levels, systemic hypertension and cardiac dysfunction (CD). OBJECTIVE: To test the hypothesis of AGT*M235T polymorphism being associated with the risk of developing cardiac dysfunction (heart failure or asymptomatic left ventricular systolic dysfunction) after acute coronary syndrome (ACS) during hospitalization. METHODS: A total of 363 patients (mean age of 62 ± 12 years), of whom 233 (64%) were men and 130 (36%) were women, all from the same cohort and hospitalized for ACS, were studied. Clinical and genetic data from the 117 (32.2%) patients who developed cardiac dysfunction (case group) were compared to those of the 246 (67.8%) who did not develop this condition (control group). The AGT*M235T polymorphism was determined by sequence analysis and was in Hardy-Weinberg equilibrium. RESULTS: There was a significant difference in the distribution of genotypes among women, with a predominance of the *235MM genotype in the control group (p = 0.001) and of the *235T allele in the case group. In the logistic regression models, the diagnosis of anterior wall myocardial infarction at admission was related to an increased risk of CD in both genders, whereas unstable angina at admission.; absence of the *235T allele; blood glucose <100 mg/dl; use of betablocker; serum creatinine level 60 and < 90 bpm; and current cigarette smoking were related to a lower risk of CD. CONCLUSION: This study suggests that the absence of the AGT *235T allele contributes to a reduced risk of cardiac dysfunction after acute coronary syndrome.
