Activities of Daily Living, Physical Activity, Physical Fitness and Quality of Life in Children with Congenital Heart Disease: A Case-Control Study

Berfin Kişin et al. Sema Savci Buse Ozcan Kahraman Aylin Tanriverdi … Hazer Erçan Bozyer Halise Zeynep Genç Mustafa Kir

Abstract

Background

Despite reports of reduced physical fitness in children with congenital heart disease (CHD), no specific performance evaluations for activities of daily living have been conducted.

Objectives

The aim was to compare the activities of daily living, quality of life, posture, physical fitness and physical activity levels of children with CHD with healthy controls (HC).

Methods

The study included 30 children aged 6-14 diagnosed with moderate or severe CHD and 30 age-sex-matched HC. The sociodemographic and clinical data of the participants were recorded. All participants went through several tests, namely the TGlittre-P test for activities of daily living, the 6-minute walk test (6MWT) for functional capacity, the Fitnessgram test battery for physical fitness, the hand dynamometer for measuring grip strength, the pedometer for measuring physical activity, and both the child and parents reported the Pediatric Quality of Life Inventory (PedsQL) for evaluating the quality of life, in addition to posture analyses. Values of p < 0.05 were considered statistically significant.

Results

Individuals with CHD had a longer TGlittre-P test completion time and a shorter 6MWT distance than HC (TGlittre-P: CHD 3.45 [3.24-4.02]min vs. HC 3.10 [2.57-3.23]min, 6MWT: CHD 514.00 [412.50-566.00]m vs. HC 591.50 [533.00-631.00]m). For the CHD group, sit-ups, push-ups, trunk lift, and sit-and-reach test scores within the Fitnessgram battery, grip strength, posture, and quality of life scores were lower than those for the HC group. Physical activity levels were similar in the groups.

Conclusions

The performance of activities of daily living, functional capacity, physical fitness, posture, and quality of life of children with moderate and severe CHD were affected compared to healthy peers.

My Approach to Echocardiographic Evaluation after Pediatric Heart Transplantation to Control Rejection and/or Graft Vascular Disease

Adailson Wagner da Silva Siqueira et al. Mirela Frederico de Almeida

Abstract

Pediatric heart transplantation is the definitive therapeutic option for patients with heart failure refractory to optimized clinical treatment, due either to cardiomyopathies or congenital heart disease. Morbidity and mortality remain concerning factors during evolution, and primary dysfunction, rejection, and graft vascular disease (GVD) are the main causes of death in the first 5 years after transplantation. As a surveillance method, transthoracic echocardiography has significant benefits in assisting diagnosis when there is clinical suspicion of rejection or GVD. The objective of this article is to present, in a clear and objective manner, which echocardiographic data assist pediatric cardiologists in follow-up of transplanted patients. The use of echocardiography, either through conventional tools or advanced methods, carefully focusing on different early and late post-cardiac transplantation phases, with information from current and comparative examination of evolution, mainly regarding tissue Doppler and longitudinal strain, can assist pediatric cardiologists in the decision to anticipate endomyocardial biopsy and coronary angiography (gold standard) for early diagnosis and immediate intervention, allowing greater graft durability.

Family Functioning and Congenital Heart Disease

Klébia Magalhães Pereira Castello Branco et al. Maria Verônica Câmara dos Santos

In the last decades, we have been watching a remarkable increase in survival of children with congenital heart disease (CHD), due to advances in surgical and medical management, leading to a significant growth in the number of adolescents and young adults with the disease. In developed countries, the number of adults with CHD surpasses the number of children with CHD.

Unfortunately, prolonged survival has been achieved at a high cost, as an expressive number of patients continue to suffer from significant medical, psychological and social issues. The knowledge that CHD brings an individual burden is not a new concept. In 1964, Glaser concluded that “Despite benefits derived from these remarkable therapeutic gains, children with CHD face many difficulties in their efforts toward social and emotional adjustment.”

Noblestitch Failure After Percutaneous Patent Foramen Ovale Closure in a Case Of Platypnea-Orthodeoxia Syndrome: Is this Device Suitable for All Patients?

Ana Rita Moura et al. Mariana Silva Alberto Rodrigues João Carlos Silva … José Ribeiro Daniel Caeiro Jorge Casanova Ricardo Fontes-Carvalho

Introduction

Patent foramen ovale (PFO) is a common congenital cardiac lesion corresponding to a normal fetal interatrial communication that typically closes after birth with the fusion of the atrial septum primum to the septum secundum . When the foramen ovale persists, it is usually asymptomatic and benign, with no need for treatment. In rare cases, it can be implicated in the pathogenesis of some medical conditions, such as platypnea-orthodeoxia syndrome (POS). In this context, PFO closure may be indicated.

Several occluder devices, traditionally based on a double-disc design, have shown that percutaneous PFO closure is possible. Despite the proven efficacy, their use is not technically feasible for some patients. In addition, they have the potential to cause some complications. NobleStitch EL (NS) is an alternative percutaneous PFO closure technique available in Europe, European Commission (EC) marked for cardiovascular suturing and PFO closure, and in the US, with Food and Drug Administration (FDA) clearance for vascular and cardiovascular suturing. It consists of a “deviceless” system with two dedicated suture delivery catheters. , To date, the experience with the NS device is limited, and we still lack information about its failure determinants, which could help select patients. –

Increased Wasted Myocardial Work as an Indicator of Significant Coronary Lesion

Márcio Mendes Pereira et al. Amanda Castro Barroso Marco Túlio Hercos Juliano Márcio Mesquita Barbosa … Gustavo Travassos Gama Rodrigo de Jesus Louzeiro Melo José Bonifácio Barbosa Carlos Alberto Vieira Gama José Xavier de Melo Filho Auridesio Cutrim de Araujo

Abstract

Introduction

The assessment of global myocardial work indices under baseline conditions may be useful for the clinical stratification of patients with suspected coronary obstruction.

Objective

To correlate the value of global myocardial work indices and the presence of significant obstructive coronary lesions.

Method

Cross-sectional study, with patients referred for elective coronary angiography. An echocardiogram was performed to obtain measurements to calculate the value of myocardial work and evaluated the presence or presence of significant obstructive coronary lesions at coronary angiography.

Results

The sample consisted of 30 patients, with a mean age of 64.2±12.8 years, the majority being male (63.3%), of which 68.4% had significant obstructive coronary lesions. The global myocardial work indices was 1,876mmHg%±253.8 in the group with significant obstructive coronary lesions and 2,054.2mmHg%±417.3 in those without significant lesions (p=0.089). Global constructive myocardial work in patients without significant obstructive coronary lesions was higher (2,329.3mmHg%±462.9) than in those with significant obstructive coronary lesions (2,109.5mmHg%±332.3; p=0.064). Global wasted myocardial work was higher in patients with significant obstructive coronary lesions (103.7mmHg%±47.1 versus 68.3mmHg%±33.8; p=0.038). The cutoff point of 115 mmHg% was the one with the best area under the curve (0.625), with a sensitivity of 83.3%.

Conclusion

The increase in global wasted myocardial work correlated with the presence of significant obstructive coronary lesions in our sample.

João Luiz Langer Manica et al. Juliana Rodrigues Neves Raul Arrieta Pedro Abujamra … Raul Ivo Rossi Filho Luiz Carlos Giuliano Germana Coimbra Pablo Tomé Teixeirense João Henrique Aramayo Rossi Rodrigo Nieckel da Costa Salvador André Bavaresco Cristóvão Carlos Pedra

Abstract

Background

The presence of patent ductus arteriosus can be as high as 50% in preterm babies. Hemodynamically significant patent ductus arteriosus is a common cause of delayed weaning of respiratory support and an important risk factor of necrotizing enterocolitis, intraventricular hemorrhage, and bronchopulmonary dysplasia in this population.

Objective

The aim of this study is to describe an initial experience of percutaneous closure of the ductus arteriosus in preterm infants weighing less than 2 kg.

Methods

This was a prospective study, comprised of 14 consecutive patients submitted to percutaneous closure of ductus arteriosus between March 2020 and February 2021 in 6 institutions in Brazil.

Results

Mean gestational age was 28.45±3.14 weeks, mean age at the procedure was 38.85±17.35 days and mean weight was 1.41 ±0.41 kg; 79% of the patients were under mechanical ventilation, and 79% had been submitted, on average, to a 1.5 cycle of non-steroidal anti-inflammatory drugs. Most patients were weaned off of mechanical ventilation in a mean of 12.6 ±7.24 days after the procedure. Success rate was 100%. No procedure-related mortality was observed.

Conclusion

This study concluded that percutaneous closure of ductus arteriosus in premature babies below 2 kg has satisfactory results and a low complication rate in this study sample.

The First Stage Norwood Operation, in Brazil – The Bar Was Raised

Walter Villela de Andrade Vicente

Outcomes of the first stage Norwood operation in Brazil have been scarcely reported. Countrywide dismal results, contrasting to those from the developed world, possibly explain this finding.

Contrariwise to this scenario, in this issue of The Braz J Cardiol, da Silva´s group presents their single-center experience comprising the largest series of the first-stage Norwood operation with adjuvant postoperative extracorporeal membrane oxygenation (ECMO) support in our country. Their 30-day 91.3% survival, an outstanding surgical result, is on par with those from just a few international elite institutions where ECMO is also available.

Tetralogy of Fallot Associated with Aberrant Right Subclavian Artery. Clinical Implications

Maciej Michałowski et al. Pawel Tyczynski Magdalena Lipczynska Anna Wójcik … Piotr Hoffman Adam Witkowski Ilona Michałowska

Abstract

Since the first description of Tetralogy of Fallot (ToF) in 1671 by Niels Stensen and in 1888 by Étienne-Louis Arthur Fallot, numerous papers have reported on this anomaly, along with its variants and concomitant cardiovascular anomalies. Aberrant right subclavian artery (ARSA) is the most common anomaly of the aortic arch. Different from the left aberrant subclavian artery, occurrence of ARSA in ToF-patients has only casuistically been reported so far. The present study reports on two ToF-patients with ARSA. It is important to note that knowledge of the coexistence of both anomalies has highly practical points during surgical or endovascular corrections of congenital heart defects (including ToF).

Systemic Immune-Inflammation Index Predicts Major Cardiovascular Adverse Events in Patients with ST-Segment Elevated Myocardial Infarction

Faysal Saylik et al. Tayyar Akbulut

Abstract

Background

The systemic immune-inflammation index (SII) has been reported as a new prognostic marker in tumors and cardiovascular diseases

Objective

To investigate the association of SII with adverse cardiovascular events in patients with ST-segment elevated myocardial infarction (STEMI).

Methods

A retrospective observational study was conducted on 843 patients with STEMI. Patients were divided into two groups based on the median value of SII. Major adverse cardiovascular events were compared between SII groups. Cox regression analysis was used for detecting independent predictors of cardiovascular adverse events. The improvement of discrimination ability by adding SII to the traditional risk factors such as age, hypertension, diabetes mellitus, and male gender for major adverse events was calculated by c-statistics, integrated discrimination improvement, and net reclassification improvement. A two-sided p-value <0.05 was considered significant.

Results

High SII group was older than the low SII group (61.2±11.2, 59.2±7.9, respectively, p=0.002). The high SII group had higher rates of cardiac death, nonfatal myocardial infarction, nonfatal stroke, hospitalization for heart failure, revascularization, and composite major adverse cardiovascular events than the low SII group. SII was an independent predictor of all events mentioned above. Adding SII to traditional risk factors improved their discrimination ability for cardiovascular events. SII was superior to the neutrophil-to-lymphocyte and platelet-to- lymphocyte ratios for predicting cardiovascular adverse events.

Conclusion

SII was an independent predictor of major adverse events in patients with STEMI and may be used to improve the prediction of adverse events, especially when combined with traditional risk factors.

Impact of Preoperative Functional Capacity on Postoperative Outcomes in Congenital Heart Surgery: An Observational and Prospective Study

Angela Sachiko Inoue et al. Antônio Augusto Barbosa Lopes Ana Cristina Sayuri Tanaka Maria Ignêz Zanetti Feltrim … Filomena R.B.G. Galas Juliano Pinheiro Almeida Ludhmila Abrahão Hajjar Emilia Nozawa

Abstract

Background

Despite advances in surgical technique and postoperative care in congenital heart disease, cardiovascular morbidity is still high.

Objective

To evaluate the association between preoperative cardiovascular fitness of children and adolescents, measured by the 6-minute walk test (6MWT) and Heart Rate Variability (HRV), and the occurrence of cardiogenic, septic shock and death in the postoperative period.

Methods

Prospective, observational clinic study including 81 patients aged from 8 to 18 years. In the preoperative period, the 6MWT (distance walked and SpO2) and HRV were performed. The adjusted risk score for surgeries for congenital heart disease (RACHS-1) was applied to predict the surgical risk factor for mortality. The occurrence of at least one of the listed complications was considered as a combined event. P values < 0.05 were considered as significant.

Results

Of the patients, 59% were male, with mean age of 12 years; 33% were cyanotic; and 72% had undergone previous cardiac surgery. Cardiogenic shock was the most common complication, and 31% had a combined event. Prior to surgery, type of current heart disease, RACHS-1, SpO2at rest, during the 6MWT and recovery were selected for the multivariate analysis. The SpO2at recovery by the 6MWT remained as an independent risk factor (OR 0.93, 95%CI [0.88 – 0.99], p=0.02) for the increasing occurrence of combined events.

Conclusion

SpO2after the application of the 6MWT in the preoperative period was an independent predictor of prognosis in children and adolescents undergoing surgical correction; the walked distance and the HRV did not present this association.

A Three-Leaflets Mitral Valve Associated to Three Papillary Muscles in a Patient with Hypertrophic Cardiomyopathy

Antonio Tito Paladino et al. Luiz Antônio Vasconcelos Leonardo Mello Guimarães de Toledo Natalia Massoni … Mariana Oliveira Rezende Natasha Soares Simões dos Santos David Costa de Souza Le Bihan Rodrigo Bellio de Mattos Barretto Jorge Eduardo Assef

Abstract

Mitral valve malformations are considered rare even in their association with other anomalies, except for mitral valve prolapse, which is found in approximately 10% of the population. Mitral valve abnormalities may affect any portion of the valve or subvalvular apparatus (annulus, cusp, tendinous cords or papillary muscles).1,2 Mitral valve anatomy and function can be assessed by echocardiography, resonance or cardiac tomography, and is essential for potential therapeutic programming.2

Can we Trust in Routine Echocardiography to Assess the Right Ventricle and Pulmonary Insufficiency? A Comparative Study with Cardiac Magnetic Resonance

Manuela Baima Cabral et al. Marcelo Felipe Kozak Jorge Yussef Afiune

Abstract

Background

Cardiac magnetic resonance (CMR) is the method of choice for assessing right ventricular (RV) dimensions and function, and pulmonary insufficiency (PI).

Objectives

To assess the accuracy of two-dimensional echocardiography (2D ECHO) in estimating RV function and dimensions, and the degree of PI, and compare the 2D ECHO and CMR findings.

Methods

We compared ECHO and CMR reports of patients whose indication for CMR had been to assess RV and PI. A p-value < 0.05 was considered statistically significant.

Results

We included 51 congenital heart disease patients, with a median age of 9.3 years (7-13.3 years). There was poor agreement between 2D ECHO and CMR for classification of the RV dimension (Kappa 0.19; 95% CI 0.05 to 0.33, p 0.004) and function (Kappa 0.16; 95% CI -0.01 to +0.34; p 0.034). The RV was undersized by 2D ECHO in 43% of the cases, and RV function was overestimated by ECHO in 29% of the cases. The degree of agreement between the methods in the classification of PI was not significant (Kappa 0.014; 95% CI -0.03 to +0.06, p 0.27). 2D ECHO tended to overestimate the degree of PI.

Conclusions

The 2D ECHO showed a low agreement with CMR regarding the RV dimensions and function, and degree of PI. In general, ECHO underestimated the dimensions of the RV and overestimated the function of the RV and the degree of PI as compared with CMR.

The Congenital Heart Disease Diagnosis List for the eleventh iteration of the International Classification of Diseases (ICD11): Translation to Brazilian Portuguese

Vera Demarchi Aiello et al. Sandra da Silva Mattos

Background

The International Society for Nomenclature of Pediatric and Congenital Heart Disease (ISNPCHD) is an organization composed by specialists in Pediatric Cardiology, Cardiac Surgery and Morphology. It was created aiming at unifying the terminology of congenital and acquired pediatric heart disease across the world. The unified lexicon allows for inter-institutional and international diagnostic comparisons and exchange of information on the outcomes of children and adults with congenital heart defects.

Initially gathering annually, as the “The International Nomenclature Working Group for the Mapping and Coding of Nomenclatures for Pediatric and Congenital Heart Disease”, the members decided to create, in 2005, the International Society (ISNPCHD), extending the membership to incorporate professionals from underrepresented areas of the world.

Palliative Endovascular Intervention in Infants with Tetralogy of Fallot: A Case Series

Kerli Dreier Kupas et al. Isabela Oldoni Juliano Mendes Souza

Abstract

Background

Endovascular stent placement in the right ventricular outflow tract (RVOT) has been an alternative to Blalock-Taussig (BT) surgery in the treatment of Tetralogy of Fallot (TOF) in symptomatic infants with low birth weight and complex anatomy.

Objective

To evaluate endovascular stent placement in the RVOT as a primary treatment for infants with TOF who are not candidates for BT surgery, and evaluate medium-term outcomes until the stent is removed during corrective surgery.

Methods

Six infants with TOF were treated with RVOT stenting from October 2015 to April 2018. Hemodynamic parameters were compared between the pre- and post-stenting periods.

Results

At the time of stenting, participants had a median age and weight of 146.5 days and 4.9 kg, respectively. Peak systolic gradient decreased from 63.5 mm Hg to 50.5 mm Hg, while the diameter of the left and right pulmonary arteries increased from 3.5 mm to 4.9 mm and 4.3 mm, respectively. The Nakata index increased from 96.5 mm to 108.3 mm; weight increased from 4.9 kg to 5.5 kg; and oxygen saturation, from 83.5% to 93%. There was one case of stent migration and two deaths, one caused by stent embolization and the other unrelated to study procedures.

Conclusions

RVOT stenting is a promising alternative for the palliative treatment of TOF in infants with low birth weight and complex anatomy.

17-Year-Old Man with Pulmonary Atresia and Intact Ventricular Septum Submitted to Fontan Operation, and with Persistent Coronary-Cavitary Fistula

Edmar Atik

Introduction

Pulmonary atresia with intact ventricular septum is a congenital anomaly with total interruption of blood flow between the right ventricle and the pulmonary trunk, and in general it is not accompanied by associated defects, except for interatrial septal defect, present in 20%, with predominance of patent foramen ovale. Valve atresia results from the lack of embryological development of this fibrous structure, which can be located at the valve level (fibrous membrane) but also at the infundibular level (blind fundus). As a consequence, myocardial hypertrophy and hypoplasia of the right ventricle, hypoplasia of the ring and tricuspid valve, mild tricuspid insufficiency and pulmonary flow dependent on the ductus arteriosus appear. In view of greater hypertension in right ventricle, direct connections with the coronary circulation are formed through sinusoids, with flow towards the aorta. In this situation, it is said that the coronary circulation is dependent on the right ventricle, and when these connections are present in great magnitude, they predispose to myocardial infarction, arrhythmias and right ventricular volume overload, due to retrograde flow from the aorta.

In cases where valve atresia develops later in the fetus, the right ventricular cavity may be well formed with its three portions, the entrance route, the trabecular and the exit route and, as a consequence, there is marked tricuspid insufficiency, even with Ebstein-type alteration of the redundant and myxomatous valve, ventricular wall thinning, ventricular dysfunction and right heart failure, the latter superimposed on hypoxia. In general, it is not accompanied by other associated defects and the pulmonary arteries are of adequate size. In these cases, there is no connection between the right ventricle and the coronary arteries by sinusoids.

Persistent Aneurysm of the Right Coronary Artery, Even after Correction of a Fistula with the Right Ventricle

Edmar Atik

Introduction

Among the coronary-cavitary fistulas, the most commonly found type corresponds to that involving the right coronary artery and the right cardiac cavities, with corresponding volume overload due to arteriovenous deviation. The clinical manifestations correspond to heart failure, angina with myocardial infarction and arrhythmias. A continuous murmur easily suggests the diagnosis of a congenital defect, which is consolidated by the imaging examinations. Other fistulas should always be suspected in the differential diagnosis, such as the patent ductus arteriosus, aortopulmonary window, systemic-pulmonary collaterals, and fistulas of the sinus of Valsalva with the right ventricle cavity. Cardiac surgery and / or percutaneous interventions comprise the therapeutic basis for fistula resolution. Little is said, however, about the evolution after the procedures, given that the previous coronary dilation persists and may constitute another evolution problem in the longer term.

This aspect comprises the main reason for this assessment.

Three-Dimensional-Printed Heart Prototype for Application in Pediatric Cardiology: An Initial Experiment

Maíra Levorato Basso et al. Alessandra Möbius Gebran Julia Dullius Oliveira Katrin Möbius Gebran … Letícia Carlota Bonatto Maria Cecília Knoll Farah

Introduction

Three-dimensional (3D) prototyping technology is developing rapidly in medicine, and recent studies have shown its important applicability in pediatric cardiology.

Clinicians and surgeons use two-dimensional technologies, such as echocardiography, computed tomography (CT) angiography, and magnetic resonance imaging, to characterize structures and understand complex diseases. Because these images are often projected and extended on a flat screen, they may not represent the actual size of the structures, depth perception, or the proximity between them.

Safety, Efficacy, and Dose Protocol of Metoprolol for Heart Rate Reduction in Pediatric Outpatients Undergoing Cardiac CT Angiography

Mariana de Oliveira Nunes et al. Dawn R. Witt Susan A. Casey Larissa I. Stanberry … David J. Caye Bradford J.Chu B. Jana Lindberg John R. Lesser B. Kelly Han

Abstract

Background

Image quality and radiation dose are optimized with a slow, steady heart rate (HR) when imaging the coronary arteries during cardiac computed tomography angiography (CCTA). The safety, efficacy, and protocol for HR reduction with beta blocker medication is not well described in a pediatric patient population.

Objective

Provide a safe and efficient metoprolol dose protocol to be used in pediatric outpatients undergoing CCTA.

Methods

We conducted a retrospective review of all pediatric outpatients who received metoprolol during CCTA. Demographic and clinical characteristics were summarized and the average reduction in HR was estimated using a multivariate linear regression model. Images were evaluated on a 1-4 scale (1= optimal).

Results

Seventy-eight pediatric outpatients underwent a CCTA scan with the use of metoprolol. The median age was 13 years, median weight of 46 kg, and 36 (46%) were male. The median doses of metoprolol were 1.5 (IQR 1.1, 1.8) mg/kg and 0.4 (IQR 0.2, 0.7) mg/kg for oral and intravenous administrations, respectively. Procedural dose-length product was 57 (IQR 30, 119) mGy*cm. The average reduction in HR was 19 (IQR 12, 26) beats per minute, or 23%. No complications or adverse events were reported.

Conclusion

Use of metoprolol in a pediatric outpatient setting for HR reduction prior to CCTA is safe and effective. A metoprolol dose protocol can be reproduced when a slower HR is needed, ensuring faster acquisition times, clear images, and associated reduction in radiation exposure in this population.
