Abstract

Background

The spatial distribution of patients according to clinical characteristics, using geoprocessing techniques, enables the identification of disease clusters within a studied population. This approach supports the development of targeted interventions directed toward specific risk groups.

Objectives

To investigate geographical clusters of diseases of interest among patients attending a public tertiary hospital and to characterize their spatial distribution across the metropolitan area of São Paulo.

Methods

Based on the results of a previous study, two disease groups were defined: one representing the main diseases identified at the first hospital visit and another representing the main underlying causes of death. A detailed description is provided in the . Disease density was calculated by dividing the frequency of cases within each disease group by the population of each city/district and multiplying the result by 100,000 to enhance data visualization. Spatial data were subsequently analyzed and mapped to illustrate the spatial patterns and relationships identified in the study.

Results

Osasco and Taboão da Serra showed the highest density of underlying causes of death related to diseases of the circulatory system. Additionally, 12 cities/districts located at the borders of the study area showed no recorded neoplastic diseases.

Conclusions

Vargem Grande Paulista showed the highest case density for neoplasms as underlying causes of death and comorbidities. Notably, Embu das Artes, a city recognized for its extensive green areas, showed the highest density of diseases of the respiratory system.

Hyperkalemia in Heart Failure Hospitalizations: An Underreported Contributor to Poor Outcomes and Higher Costs in Brazil

Roberto Pecoits-Filho et al. Pedro Tulio Rocha Maristela Carvalho da Costa Pedro Schwartzmann … Lídia Ana Zytynski Moura Mauricio Longato Nicolas Segre Tamisa Rego Cinthia Montenegro Thyago Proença de Moraes

Abstract

Hyperkalemia is associated with poor prognosis in heart failure (HF), but its reporting in administrative data is likely suboptimal.

The aim was to evaluate the frequency and impact of hyperkalemia ICD coding during HF hospitalizations in Brazil.

We analyzed 3,551,738 HF hospitalizations from the DATASUS database (2008–2024). ICD codes were used to identify hyperkalemia. Logistic and Cox regression analyses assessed associations between comorbidities, outcomes, and resource utilization.

Only 491 hospitalizations (0.014%) were coded for hyperkalemia. These patients were older (mean 70.3 vs. 66.6 years) and had higher rates of chronic kidney disease, diabetes, and cardiovascular disease. Hyperkalemia was associated with increased in-hospital mortality (25% vs. 10%), ICU admissions (17% vs. 11%), dialysis (11% vs. 1%), and longer ICU stays (9.45 vs. 5.36 days). Average costs were 59% higher (2,402 vs. 1,512 BRL).

Hyperkalemia is severely underreported in Brazilian HF hospitalizations, despite being a clear marker of clinical severity and higher resource use. Improved awareness and coding may support better outcomes and planning.

The Congenital Heart Disease Diagnosis List for the eleventh iteration of the International Classification of Diseases (ICD11): Translation to Brazilian Portuguese

Vera Demarchi Aiello et al. Sandra da Silva Mattos

Background

The International Society for Nomenclature of Pediatric and Congenital Heart Disease (ISNPCHD) is an organization composed by specialists in Pediatric Cardiology, Cardiac Surgery and Morphology. It was created aiming at unifying the terminology of congenital and acquired pediatric heart disease across the world. The unified lexicon allows for inter-institutional and international diagnostic comparisons and exchange of information on the outcomes of children and adults with congenital heart defects.

Initially gathering annually, as the “The International Nomenclature Working Group for the Mapping and Coding of Nomenclatures for Pediatric and Congenital Heart Disease”, the members decided to create, in 2005, the International Society (ISNPCHD), extending the membership to incorporate professionals from underrepresented areas of the world.

Mortality due to heart failure: extended analysis and temporal trend in three states of Brazil

Eduardo Nagib Gaui et al. Carlos Henrique Klein Gláucia Maria Moraes de Oliveira

BACKGROUND: Heart failure (HF) is a chronic disease with high prevalence and mortality rates. The mortality due to HF in Brazil has been studied more frequently using hospitalization data. OBJECTIVE: To evaluate the mortality rates due to HF by sex and age range, in three states of Brazil, Rio de Janeiro, São Paulo and Rio Grande do Sul, from 1999 to 2005. METHODS: The data were obtained from death certificates assessed in the three states. The mortality due to HF was assessed in the restricted (underlying cause of death), comprehensive (mentioned in any line of the death certificate) and extended (all codes with the presence of HF) forms. RESULTS: The specific rates of mortality presented a clear decrease trend in the age groups, except in the group aged 80 years or older. The rates increased with age and were clearly higher among men up to 80 years of age. The rates of mortality due to HF were 3-fold higher in the comprehensive than in the restricted form of analysis. The extended form of analysis also added 20% of deaths in which HF was present. CONCLUSION: The results of this study demonstrated a decrease trend in the mortality rates due to HF when considering the three states – around 43% of Brazil – from 1999 to 2005. The methodology of multiple causes of death, in addition to the underlying ones, allows us to present a more comprehensive dimension of the importance of HF as cause of death. The adequate selection of the codes of the International Classification of Diseases (ICD), which comprehend the totality of the HF phenomenon, remains a challenge for further studies.
