Reliability of symptoms suggestive of angina in patients with chronic obstructive pulmonary disease

Yilmaz Gunes et al. Mustafa Tuncer Unal Guntekin Hasan Ali Gumrukcuoglu … Serkan Akdag Bulent Ozbay Bunyamin Sertogullarindan

BACKGROUND: Due to overlapping symptoms and inadequate exercise capacity, noninvasive diagnosis of coronary artery disease (CAD) may be under- or overestimated in patients with chronic obstructive pulmonary disease (COPD). OBJECTIVE: To assess outcomes of coronary angiography in COPD patients depending on baseline clinical characteristics. METHODS: Medical records of 157 patients with COPD and 157 patients without COPD matched for baseline clinical characteristics who had undergone diagnostic coronary angiography for the first time were reviewed retrospectively. RESULTS: The frequency of significant CAD was significantly lower in COPD patients than in the control group (52.8% vs. 80.2%, p<0.001). Frequencies of CAD risk factors (older age, hypertension, diabetes, smoking history) were significantly more frequent among COPD patients having significant CAD. Among patients reporting stable angina pectoris, significant CAD was detected in 32.7% of COPD patients and 71.0% of non-COPD patients (p<0.001). However, among the patients with a diagnosis of unstable angina pectoris, significant CAD was detected in 87.5% of COPD patients and 90.2% of non-COPD patients (p=0.755). CONCLUSION: Diagnosis of CAD in COPD patients by symptomatology may be difficult. However, clinical diagnosis of CAD in the setting of unstable angina is accurate in most of the COPD patients. Therefore, further noninvasive diagnostic methods or careful follow up may be more appropriate for COPD patients reporting stable angina pectoris.

Evaluation of atherosclerosis in renal transplanted patients by non-invasive methods

Cláudio Domênico Sahione Schettino et al. Aristarco Gonçalves Siqueira Filho Romeu Cortes Domingues Iugiro Kuroki … Flávia Cristina Carvalho de Deus Renato Torres Gonçalves Hélio Magarino Torres Emerson Gasparetto Lúcio Cardoso Pacheco Eduardo Rocha

BACKGROUND: Endothelial dysfunction can be considered an early atherogenic event. OBJECTIVE: To assess atherosclerosis in renal transplanted patients through the coronary calcium score, carotid duplex scan and brachial reactivity through ultrasonography. METHODS: We assessed 30 renal transplanted male patients with stable renal function, with a mean age of 41.3 years. RESULTS: The detection of the atherosclerotic load in this population was very significant when the brachial reactivity technique was used (86.7%); it was less frequent when based on the presence of carotid plaque (33.3%) or the coronary calcium score (20%). The carotid plaque was considered when the thickness was > 12 mm. The coronary calcium score was abnormal when > 80 according to the Agatston scale, being observed in a low percentage of patients (21.7%), possibly due to the fact that the tomography is not the ideal method to detect atherosclerosis in renal patients, as it does not differentiates intimal calcifications of the medial layer. The adequate clinical control, the low age range and the factors related to the time of pre-transplant dialysis or the anti-inflammatory effect of the post-transplant drugs can delay the onset of the calcifications. CONCLUSION: The evaluation of the atherosclerotic load through the carotid duplex scan (33,3%) and the coronary calcium score (20%) was not frequent; there was no correlation with the high rate of endothelial dysfunction detection observed with the brachial reactivity assessment (86.7%).

Oxidative stress and endothelial dysfunction in chronic kidney disease

Valeria Costa-Hong et al. Luiz Aparecido Bortolotto Vanda Jorgetti Fernanda Consolim-Colombo … Eduardo M Krieger Jose Jayme Galvão de Lima

BACKGROUND: Chronic kidney disease (CKD) is characterized by the high prevalence of atherosclerosis. Considering that endothelial dysfunction and oxidative stress are promoters of atherosclerosis, it is of interest to verify whether the two conditions are associated in CKD patients still free of clinical cardiovascular disease (CVD). OBJECTIVE: To evaluate the association between oxidative stress and endothelial function in end-stage CKD patients without clinically evident CVD. METHODS: We studied 22 nondiabetic, nonsmoker CKD patients without clinical CVD treated by maintenance hemodialysis and 22 healthy controls. Endothelium- dependent and independent vascular reactivity and oxidative stress, as determined by the plasma levels of thiobarbituric acid-reactive substances – TBARS, were evaluated in all subjects. RESULTS: Endothelium-dependent (6.0 ± 4.25 vs. 11.3 ± 4.46 %, p< 0.001) and endothelium-independent (11.9 ± 7.68 vs. 19.1 % ± 6.43 %, p< 0.001) vascular reactivity were reduced, while TBARS (2.63 ± 0.51 vs. 1.49 ± 0.42 nmols/mL) was increased in CKD patients when compared to controls. TBARS levels were significantly related to endothelium-dependent vascular reactivity (r= – 0.56, p< 0.001) and to systolic blood pressure (r= – 0.48, p= 0.002). CONCLUSION: Oxidative stress is increased in CKD patients free of CVD and is associated with endothelial dysfunction in patients and controls. The results suggest that oxidative stress and endothelial dysfunction may be involved in the increased susceptibility of CKD patients to CVD and cardiovascular complications.

Relationship between electrocardiogram with diabetes mellitus and metabolic syndrome in Japanese-Brazilians

Luigi Brollo et al. Maria Teresa Nogueira Bombig Cleber do Lago Mazzaro Yoná Afonso Francisco … Francisco Antonio Helfenstein Fonseca Antonio Carlos Camargo Carvalho Helena Harima Amélia Hirai Rui Povoa

BACKGROUND: When the Japanese immigrated to the Americas, they were subjected to Westernization, with a great change in lifestyle, specially in dietary habits, and this may explain the increase in the incidence of diabetes mellitus (DM), metabolic syndrome (MS) and cardiovascular disease among them. OBJECTIVE: To study the presence of myocardial necrosis and left ventricular hypertrophy (LVH) in a population of Japanese-Brazilians, using the ECG and its relationship with DM and MS. METHODS: This was a cross-sectional study which evaluated 1,042 Japanese-Brazilians aged 30 or over, 202 of them born in Japan (Issei) and 840 of them born in Brazil (Nissei), from the second phase of the Japanese-Brazilian Diabetes Study Group initiated in 2000. MS was defined according to the NCEP-ATP III criteria modified for the Japanese. DM and MS were associated with the presence of myocardial necrosis (according to the Minnesota criteria) and LVH (according the Perugia score on the ECG). The statistic chi square method was used to reject the null hypothesis.? RESULTS: Of the 1,042 participants, 35.3% had DM (38.6% of the Issei and 34.5% of the Nissei); 51.8% had MS (59.4% of the Issei and 50.0% of the Nissei). The presence of an inactive zone in the diabetic Issei group was not statistically significant when compared to the non-diabetic group, but among the diabetic Nissei group an inactive zone was present in 7.5% of them. There was a statistically significant correlation between MS and LVH in the Issei and Nissei groups. CONCLUSION: Metabolic disorders presented a high prevalence in Japanese-Brazilians with significant correlations with necrosis and hypertrophy on the ECG.

Three cases of hypertension and Renal Arteriovenous fistula with a de novo fistula

Natalia Correa Vieira Melo et al. Juliano Sacramento Mundim Elerson Carlos Costalonga Antonio Marmo Lucon … Jose Luiz Santello Jose Nery Praxedes

The Renal Arteriovenous Fistula (RAVF) is a rare and potentially reversible cause of hypertension and kidney and/or heart failure. The treatment of RAVF aims at preserving the most of the renal parenchyma and, concomitantly, eradicating the symptoms and hemodynamic effects caused by the RAVF. The present study reports three cases of RAVF, including one case of a de novo idiopathic RAVF, which presented with hypertension and kidney and/or heart failure and describes the therapeutic measures used to treat these patients as well as the outcomes.

Cardiac troponin T for risk stratification in decompensated chronic heart failure

Carlos Henrique Del Carlo et al. Antonio Carlos Pereira-Barretto Célia Maria Cassaro-Strunz Maria do Rosário Dias de Oliveira Latorre … Múcio Tavares de Oliveira Junior José Antonio Franchini Ramires

BACKGROUND: The cardiac troponins are highly sensitive and specific markers of myocardial injury. They have been detected in heart failure (HF) and are associated with a bad prognosis. OBJECTIVE: To evaluate the association of cardiac troponin T (cTnT) and its ranges with prognosis in decompensated HF. METHODS: A total of 70 patients with chronic HF worsening that needed hospitalization were studied. Cox model was used to evaluate the variables at admission capable of predicting the combined outcome that consisted of death or re-hospitalization due to HF worsening during a 1-year follow-up. RESULTS: During the follow-up, there were 44 deaths, 36 re-hospitalizations due to HF and 56 combined outcomes. At the multivariate analysis, the predictors of clinical events were the cTnT (cTnT >0.100 ng/mL; hazard ratio [HR] 3.95 95% confidence interval [CI]: 1.64-9.49, p = 0.002), left ventricular end diastolic diameter (LVDD >70 mm; HR 1.92, 95%CI: 1.06-3.47, p = 0.031) and serum sodium (Na <135 mEq/L; HR 1.79, 95%CI: 1.02-3.15, p = 0.044). To evaluate the association between the cTnT increase and the prognosis in decompensated HF, the patients were stratified in three groups: low-cTnT (cTnT 0.020 and 0.100 ng/ml, n = 12).The probabilities of survival and event-free survival were 54.2%, 31.5%, 16.7% (p = 0.020) and 36.4%, 11.5%, 8.3% (p = 0.005), respectively. CONCLUSION: The increase in cTnT is associated with a bad prognosis in decompensated HF and the degree of this increase can help the risk stratification.

Paulo Ricardo Nazário Viecili et al. Daiana Cristine Bündchen Cleusa Maria Richter Thiago Dipp … Daciano Bastos Lamberti Angela Maria Reis Pereira Luciana de Castro Barbosa Angélica Cunha Rubin Evanilda Goulart Barbosa Tiago Facchini Panigas

BACKGROUND: The effect of exercise on blood pressure (BP) is already known; however, the dose-response curve of the hypotensive effect of exercise in hypertensive individuals is yet to be clarified. OBJECTIVE: To evaluate the dose-response curve of the number of sessions that are necessary to cause a hypotensive effect in hypertensive individuals. METHODS: 88 individuals, aged 58 ± 11 years, divided in Experimental group (EG), with 48 that participated in a physical exercise program (PEP), which consisted of 40 minutes of aerobic exercises performed 3x/week, for 3 months, at 70% of the VO2max, and muscular exercises at 40% of the maximal voluntary contraction (MVC) and Control Group (CG) with 40 individuals that did not participate in the PEP. The systolic (SAP) and diastolic (DAP) arterial pressures were measured before each of the 36 sessions in the EG and assessed by ambulatory blood pressure monitoring (ABPM) in the CG. Differences in BP, the variation rate (D%) and the maximum hypotensive effect (MHE%) were observed between sessions. The data were expressed as means ± SD; the t test and correlation were used, with p<0.05 being considered significant. RESULTS: There was no difference regarding BP values in the CG. The EG showed an important decrease of 15 mmHg in SAP and 7 mmHg in DAP, with a large part of this effect occurring as early as the first session and the majority up to the 5th session. There was a strong inverse correlation (R:-0.66) with the number of sessions. CONCLUSION: An important hypotensive effect was observed from the 1st session on and it was observed that the dose-response curve can be abrupt and decrescent, instead of flat.

Large randomized controlled trials in perioperative cardiovascular medicine: a proposal for the design, conduct and efficient management

Otavio Berwanger et al. Erica Aranha Suzumura PJ Devereaux

The number of patients undergoing noncardiac surgery is growing worldwide. To optimally assist patients during the perioperative period, we must improve our knowledge of how to prevent major perioperative cardiovascular events around the time of noncardiac surgery. To achieve this goal there is a need for large randomized controlled trials that can provide reliable and conclusive results in this field. This narrative review describes a proposal for the design, conduct and management of large controlled trials in perioperative cardiovascular medicine.

Mustafa Tuncer et al. Yilmaz Gunes Unal Guntekin Hasan Ali Gumrukcuoglu … Beyhan Eryonucu Niyazi Guler Imdat Dilek Cengiz Demir

BACKGROUND: Heart rate variability (HRV) is associated with increased cardiac risk factor in several conditions. The iron status of an individual may play an important role in cardiovascular health. OBJECTIVE: To evaluate heart rate variability in patients with iron deficiency anemia. METHODS: Twenty-three patients with iron deficiency anemia (mean hemoglobin (Hb) 8.6±2.2 g/dl) and 10 healthy people (mean Hb 13.9±1.2 g/dl) were assessed with 24-hour ambulatory Holter recordings during in hospital course having limited physical activity. RESULTS: Although mean heart rate was significantly higher in patients with anemia, there was no significant difference regarding HRV parameters compared to the healthy group. CONCLUSION: There was no significant difference in HRV parameters between patients with iron deficiency anemia with limited physical activity and healthy ambulatory people.

Effect of exercise training on aging-induced changes in rat papillary muscle

Laura Beatriz Mesiano Maifrino et al. Rubens Correa Araújo Cristiane Castro Faccini Edson Aparecido Liberti … Eliane Florêncio Gama Antonio Augusto Coppi Maciel Ribeiro Romeu Rodrigues de Souza

BACKGROUND: The effects of aging on papillary muscle have been widely demonstrated, but no data on the effects of exercise on the age-related changes are available. OBJECTIVE: To analyze the effects of aging on the morphological and quantitative properties of papillary muscle and investigate whether a long-term moderate exercise program would exert a protective effect against the effects of aging. METHODS: We used electron microscopy to study the density of myocytes, capillaries and connective tissue and the cross-sectional area of myocytes of the papillary muscle of the left ventricle of 6- and 13-month-old untrained and exercised Wistar rats. RESULTS: As expected, the volume density of myocytes declined significantly (p<0.05) with aging. The length density of myocardial capillaries also declined with aging, but not significantly. The interstitial volume fraction of the papillary muscle tissue increased significantly (P<0.05) with age. The number of myocyte profiles showed a reduction of 20% that was accompanied by myocyte hypertrophy in the aged rats (P<0.05). Animals submitted to a 60-minute daily session,, 5 days/wk at 1.8 km.h-1 of moderate running on a treadmill for 28 weeks showed a reversion of all the observed aging effects on papillary muscle. CONCLUSION: The present study supports the concept that long-term exercise training restrains the aging-related deleterious changes in the papillary muscle.

Abdominal obesity and cardiovascular risk: performance of anthropometric indexes in women

Rogério Tosta de Almeida et al. Maura Maria Guimarães de Almeida Tânia Maria Araújo

BACKGROUND: Anthropometric indicators of abdominal obesity (AOB) estimate the amount of visceral fat tissue which, in turn, is associated with a higher risk of development of cardiovascular diseases. In the past decades, there has been an increase in the frequency of AOB in the brazilian female population, and this represents a major public health problem. OBJECTIVE: To evaluate the performance of different cut-off points of the conicity index (C-Index), waist-hip ratio (WHR), waist circumference (WC), and waist-to-height ratio (WHeR) in discriminating high coronary risk (HCR) in women. METHODS: Cross-sectional study conducted in Feira de Santana, State of Bahia, Brazil, with 270 female employees of a public University, with ages between 30 and 69 years. The analysis of sensitivity and specificity using the ROC curves allowed the identification and comparison of the best cut-off points to discriminate HCR, as calculated with base on the Framingham Risk Score. RESULTS: The cut-off points found were: WC (86 cm), WHR (0.87), C-Index (1.25) and WHeR (0.55), and the areas under the ROC curve were 0.70 (95%CI = 0.63-0.77), 0.74 (95%CI = 0.67-0.81), 0.76 (95%CI = 0.70-0.83) and 0.74 (95%CI = 0.67-0.81), respectively. The anthropometric indicators of AOB analyzed showed satisfactory and similar performances in discriminating HCR. However, the C-Index was the indicator that presented the highest discriminatory power. CONCLUSION: We expect that these findings will contribute to a better quantification of AOB in the brazilian female population, providing information so that health professionals can take preventive measures regarding this multifactorial clinical condition, thus preventing the development of cardiovascular diseases.

Interrupted aortic arch type B in A patient with cat eye syndrome

Sintia Iole Nogueira Belangero et al. Fernanda Teixeira da Silva Bellucco Mirlene C. S. P. Cernach April M. Hacker … Beverly S. Emanuel Maria Isabel Melaragno

We report a patient with cat eye syndrome and interrupted aortic arch type B, a typical finding in the 22q11.2 deletion syndrome. Chromosomal analysis and fluorescent in situ hybridization (FISH) showed a supernumerary bisatellited isodicentric marker chromosome derived from chromosome 22. The segment from 22pter to 22q11.2 in the supernumerary chromosome found in our patient does not overlap with the region deleted in patients with the 22q11.2 deletion syndrome. However, the finding of an interrupted aortic arch type B is unusual in CES, although it is a frequent heart defect in the 22q11 deletion syndrome.

Ventricular function following coronary artery bypass grafting: comparison between Gated SPECT and cardiac magnetic resonance imaging

Cláudio Tinoco Mesquita et al. Maria Carolina Pinheiro Pessoa Paulo Pontes Vasconcelos Amarino Carvalho Oliveira Júnior … Hans Fernando Rocha Dohmann Adair Gomes dos Reis Lea Mirian Barbosa da Fonseca

BACKGROUND: The assessment of left ventricular function may be impaired by the abnormal interventricular septal motion frequently found after coronary artery bypass grafting (CABG). Studies on the validation of gated SPECT as a tool for the assessment of left ventricular function in this patient group are scarce. OBJECTIVE: We investigated the agreement and correlation between left ventricular ejection fraction (LVEF), end-diastolic volume (EDV), and end-systolic volume (ESV) as obtained using electrocardiogram-gated myocardial perfusion scintigraphy (gated SPECT) and cardiac magnetic resonance imaging in 20 patients undergoing coronary artery bypass grafting. METHODS: Correlation was measured using Spearman’s correlation coefficient (ρ). Agreement was assessed using Bland-Altman analysis. RESULTS: A good correlation was found between gated SPECT and cardiac magnetic resonance imaging in patients after CABG with regard to left ventricular ejection fraction (ρ = 0.85; p =0.0001), moderate correlation for end-diastolic volume (ρ = 0.51; p = 0.02), and non-significant correlation for end-diastolic volume (ρ = 0.13; p = 0.5). Agreement ranges for LVEF, ESV and EDV were: -20% to 12%; -38 to 54 ml and; -96 to 100 ml, respectively. CONCLUSION: A reliable correlation was found for left ventricular ejection fraction as obtained by gated SPECT and magnetic resonance imaging in patients undergoing CABG. For ventricular volumes, however, the correlation is not adequate.

Origin of the sinoatrial and atrioventricular nodal arteries in South Indians: an angiographic study

Lakshmi Ramanathan et al. Prakash Shetty Soubhagya R. Nayak Ashwin Krishnamurthy … Ganesh K. Chettiar Annamalai Chockalingam

BACKGROUND: To study the arterial supply of the conducting system and its correlation with the dominance of the coronary arteries in the South Indian population. OBJECTIVE: To determine angiographically the origins of the sinoatrial nodal artery (SAna) and atrioventricular nodal artery (AVna) in Indians. METHODS: The study included 300 consecutive patients (114 females, 186 males; mean age, 55 years) living in the southern coastal region of India, who underwent coronary angiography either for the symptoms of chest pain, angina pectoris or positive Treadmill Test. The angiograms contained both coronary arteries (right and left) in the right and left anterior oblique position. The origin of SAna and AVna from the coronary arteries was observed and correlated with the arterial dominance. RESULTS: The SA (sinoatrial) node was supplied by the right coronary artery (RCA) in 53% of the cases, by the circumflex (Cx) branch of left coronary artery (LCA) in 42.66%, and by both coronary arteries in 4.33% of cases. The AV (atrioventricular) node was also more often supplied by the RCA (72.33% of cases) than by the Cx branch of the LCA (27.66%), and surprisingly in none of the cases was this node supplied by both coronary arteries. CONCLUSION: The results of the present study may help cardiac surgeons, particularly in surgeries related to certain valvular disorders, due to the proximity of the nodal branches to the valve complex.

ACE inhibitors and plasma B-type natriuretic peptide levels in elderly patients with heart failure

Felicio Savioli Neto et al. Hélio M. Magalhães Michel Batlouni Leopoldo S. Piegas

BACKGROUND: Clinical trials have demonstrated the benefits of ACE inhibitors (ACEI) in the neurohormonal activity and in the functional capacity of patients with heart failure (HF), and also that these effects are dose dependent. However, since elderly individuals have been systematically excluded from the majority of these studies, the validation and incorporation of these results in the geriatric population has been questioned. OBJECTIVE: To evaluate the effects of different doses of quinapril, an ACEI with a > 24-hour biological half-life, on plasma BNP levels, on the distance walked in the 6-minute walk test (6MWT) and on the incidence of adverse reactions in elderly individuals with systolic HF. METHODS: A total of 30 patients (76.1 ± 5.3 years; 15 women), in NYHA functional class II-III HF, with left ventricular EF < 40% (33.5 ± 4.5%), on diuretics (30), digoxin (24) and nitrates (13) were included. The patients were assessed at baseline and every two months, with escalating doses of quinapril of 10, 20, 30 and 40 mg. RESULTS: After eight months, BNP levels were 67.4% lower and the distance walked in the 6MWT was 64.9% longer in relation to baseline. Arterial hypotension with symptoms of low cerebral blood flow and/or renal dysfunction was not observed, so that the maximum quinapril dose could be used in all patients. CONCLUSION: The results demonstrated the benefits of ACEI on the neurohormonal profile and functional capacity of elderly individuals with systolic HF, as well as the positive relationship between dose and effect of these drugs.
