Secondary hypertension to rare adrenal gland tumor
Case Report

Secondary hypertension to rare adrenal gland tumor

Joana Silva et al. Maria do Carmo Cachulo António Leitão-Marques

The ganglioneuroma is a tumor of the sympathetic nervous system and may be associated with hypersecretion of vasoactive substances responsible for various symptoms and signs such as hypertension. The authors report a case of ganglioneuroma and a literature review, focusing on the most important aspects of diagnosis and therapy. The patient had recurrent symptomatic hypertensive crises, having performed an imaging study that found a nodule in the right adrenal gland. As neuroblastic tumors are radiologically undistinguishable, the patient underwent excision, confirming the diagnosis by pathology analysis. This case demonstrates the clinical variability of ganglioneuroma, the usefulness of imaging and the importance of histological study.

Imaging assessment of visceral adipose tissue area and its correlations with metabolic alterations

Anna Karla Carneiro Roriz et al. Adriana Lima Mello Juliana Fontes Guimarães Fabiana Cajuhy dos Santos … Jairza Maria Barreto Medeiros Lílian Ramos Sampaio

BACKGROUND: Androgenic obesity is associated with a higher risk of metabolic disorders, thus favoring the occurrence of cardiovascular diseases and other morbidities. OBJECTIVE: To verify the influence of the visceral adipose tissue (VAT) area, measured by computed tomography (CT), on the metabolic alterations in adult and elderly individuals. METHODS: CT results and lipoprotein levels, total cholesterol and fractions, triglycerides, glycemia and uric acid levels, were obtained from 194 individuals stratified by sex, age group and body mass and analyzed using the tests of correlation and means. RESULTS: The elderly individuals presented higher VAT area, glycemia, uric acid and total cholesterol levels. The most important correlations were observed between VAT area, triglycerides (TG) and VLDL-c (r > 0.5; p < 0.01), in both age groups. The mean VAT area was always higher when TG and glycemia levels were altered, in both age groups. CONCLUSION: Most tests showed a strong correlation with VAT area, which was considered as risk for metabolic alterations. In elderly individuals, the risk VAT area seems to be higher than that of adult individuals.

Long-term analysis in acute coronary syndrome: are there any differences in morbidity and mortality?

Adolfo Alexandre Farah de Aguiar et al. Ricardo Mourilhe-Rocha Roberto Esporcatte Liana Correa Amorim … Bernardo Rangel Tura Denilson Campos de Albuquerque

BACKGROUND: Heart failure (HF) is extremely important as a predictor of morbidity and mortality in patients with acute coronary syndrome (ACS). OBJECTIVE: To evaluate the predictors of morbidity and mortality in ACS in the long term. METHODS: A cohort study of 403 consecutive patients with complaints of chest pain. Demographic, clinical, laboratory and therapy-related data were described and the patients were evaluated during hospitalization and for up to eight years after being discharged, for the presence or absence or cardiovascular events and deaths. RESULTS: There were 403 patients complaining of chest pain, 65.8% of whom had been diagnosed as having ACS without ST elevation, 27.8% had ACS with ST elevation and 6.5% without ACS. Among such patients, the 377 patients with ACS were evaluated (37.9% of whom were females), and the mean age was 62.2 ± 11.6 years. The presence of HF before or during hospitalization influenced mortality. Among the prognostic factors, emphasis should be placed on the initial creatinine level, with the cutoff point being set at 1.4 mg/dl (accuracy = 62.1%, HR = 3.27; p < 0.001). We noted a worse prognosis for each additional ten years of age (HR = 1.37, p < 0.001) and for each increment of 10 bpm heart rate (HR = 1.22 p < 0.001). As for the therapies used before and after 2002, there was an increase of beta-blockers, angiotensin-converting enzyme inhibitors (ACEIs), statins and antiplatelet agents, having an impact on mortality. CONCLUSION: HF upon admission, creatinine, age and HR were independent predictors of mortality. It was observed that HF patients treated before 2002 had a worse survival when compared with that seen after 2002 and the change in therapy was responsible for it.

Endoscopic left sympathetic blockade in the treatment for dilated cardiomyopathy

Paulo M. Pêgo-Fernandes et al. Luiz Felipe P. Moreira Germano Emílio C. Souza Fernando Bacal … Edimar Alcides Bocchi Noedir Antônio G. Stolf Fábio Biscegli Jatene

BACKGROUND: The level of sympathetic nervous activity is a major determinant of prognosis in patients with heart failure. OBJECTIVE: The purpose of this investigation was to perform a proof-of-principle trial of therapeutic endoscopic left thoracic sympathetic blockade in heart failure patients to assess safety and immediate effects. METHODS: Fifteen patients with dilated cardiomyopathy and left ventricular ejection fraction (LVEF) 65 bpm, despite either adequate betablocker use or intolerant to it, were enrolled. Ten patients underwent left infra-stellate ganglion plus T3-T4 interspinal space clipping through videothoracoscopy, while the other five patients were randomized to a control group. RESULTS: None of the treated patients had any procedure-related adverse cardiovascular events at the perioperative period. Two patients from the surgical group died due to pulmonary thromboembolism or myocardial infarction within 6 months of the initial follow-up, while three patients from the control group had heart failure progression and died or developed cardiogenic shock during the same period. Treated patients presented improvement in quality of life, level of physical activity and LVEF (from 25 ± 9% to 32 ± 8%, p=0.024) at 6 months of follow-up, whereas these parameters did not change in control patients. CONCLUSION: Endoscopic left thoracic sympathetic blockade is feasible and appears to be safe in severe heart failure patients. This initial study suggests that this procedure might be an effective alternative approach to sympathetic blockade in the treatment of dilated cardiomyopathies.

Quality of life in patients with severe and stable coronary atherosclerotic disease

Gisele Moriel et al. Meliza Goi Roscani Luiz Shiguero Matsubara Ana Teresa de A. Ramos Cerqueira Beatriz Bojikian Matsubara

BACKGROUND: There are few data on the factors decreasing quality of life (QoL) in patients with coronary artery disease (CAD) before a percutaneous coronary intervention (PCI). OBJECTIVE: To associate clinical variables with QoL scores in patients with stable CAD before the PCI and with unfavorable outcomes, 12 months after the procedure. METHODS: The present is a longitudinal study of 78 patients (43 men and 35 women), before an elective PCI. The associations between the QoL scores (SF-36 questionnaire) and age, sex, weight, body mass index, diabetes mellitus (DM), arterial hypertension, dyslipidemia, current smoking, previous cardiovascular event or PCI, glycemia control and blood pressure (BP) were analyzed by multivariate logistic regression. We also analyzed the associations between the clinical features and the unfavorable outcomes (death due to any cause, heart failure or nonfatal infarction). The level of significance was set at p < 0.05. RESULTS: The medians of the QoL scores were < 70 percentage in all domains. Female sex, age 25 kg/m², DM and high BP were associated with a higher degree of impairment of at least one QoL score. Female sex (OR: 7.19; 95%CI: 1.55 – 33.36; p = 0.012), previous cardiovascular event (OR: 3.97; 95%CI: 1.01 – 15.66; p = 0.049) and PCI failure (OR: 10.60; 95%CI: 1.83 – 61.46; p = 0.008) were associated with increased risk of combined outcome. CONCLUSION: In the presence of CAD, women and patients with comorbidities present a higher degree of QoL impairment. The unfavorable outcomes 12 months after the PCI are associated with the female sex, previous event or procedure failure.

Is there an association between non-steroidal anti-inflammatory drugs and contrast nephropathy?

Luciano Passamani Diogo et al. David Saitovitch Michelle Biehl Laura Fuchs Bahlis … Maria Claudia Guterres Cinthia Fonseca O'Keeffe Gustavo Franco Carvalhal Paulo Ricardo Avancini Caramori

BACKGROUND: The association between the use of nonsteroidal anti-inflammatory drugs (NSAIDs) and acute or chronic renal failure is well documented, but evidence of such association between NSAIDs and Contrast-Induced Nephropathies (CIN) is not found in the indexed literature. OBJECTIVE: To evaluate the possible association between NSAIDs and CIN. METHODS: In a cohort study, through clinical interviews of patients that underwent cardiac catheterization, we analyzed the use of NSAIDs and its association with the development of CIN, through alterations in serum creatinine or glomerular filtration rate in 48 or 72 hours. RESULTS: From July 2005 to July 2006, 236 patients were enrolled in the study, of which 29 were later excluded. The incidence of CIN was 10.37% (20 of 207) and 42% of the patients were using NSAIDs until the moment of the evaluation. There was no association between the use of NSAIDs and the development of CIN with OR of 1.293 95% CI (0.46-4.2). The study detected known risk factors for the development of CIN, such as diabetes with OR of 2.77 95%CI (1.05-7.47) and chronic renal failure with OR 3.48 95%CI (1.1-11.07). A protective action of saline solution hydrationis also suggested, with OR of 0.166 95%CI (0.03-0.92). CONCLUSION: Based on the data obtained, we conclude that there was no association between CIN and previous use of NSAIDs, at least with an OR higher then 2.85, which our sample detected.

Angiographic evaluation of plaque volume and cardiovascular events after coronary Stent Implantation

Erlon Oliveira de Abreu-Silva et al. Alexandre Schaan de Quadros Marco Tulio Zanettini Carlos Antonio Gottschall

BACKGROUND: Plaque volume (PV) is related to major cardiovascular events (MACE) after coronary stent implantation. OBJECTIVE: To evaluate the association between PV before the procedure evaluated by angiography and clinical outcomes. METHODS: This is a prospective cohort study of patients undergoing coronary stent implantation in a referral center. PV before the stent implantation was evaluated by the formula described by Giugliano (Am J Cardiol 2005, 95:173): VP = À X (lesion length) X [(vessel diameter/2)² – (minimum luminal diameter/2)²]. The MACE were registered at clinical follow-up of one year and multiple linear regression analysis was performed to identify predictors of events. RESULTS: The sample consisted of 824 patients, mean age 60 ± 11 years, 70.0% were male. Diabetes mellitus was present in 21.0% and triple vessel involvement in 12.0%. Average reference diameter was 3.3 ± 3.2 mm, average lesion length was 10.2 ± 4.7 mm and mean residual stenosis was 1.0% ± 12.0%. Patients with MACE had higher PV than those without events (92.84 ± 42.85 vs 85 ± 46.85, p = 0.03). Other variables associated with MACE in the univariate analysis were triple vessel involvement, myocardial infarction, vessel diameter and length of the lesion treated. PV maintained a significant association with MACE after adjusting for the variables described and diabetes mellitus. CONCLUSION: The volume of atheromatous plaque before stenting was higher in patients with MACE on clinical follow-up in one year, regardless of other predictors of events.

Electrocardiographic profile and muscle glycogen content of rats treated with nandrolone

Carlos Alberto da Silva et al. Adriano Cesar Rocco Pardi Talita Moral Gonçalves Sergio Henrique Borin

BACKGROUND: We considered both the indiscriminate use of steroids by top athletes and by physically active individuals. OBJECTIVE: To evaluate the effects of nandrolone decanoate on the electrocardiographic profile, glycogen content and total-protein profile of skeletal and cardiac muscles, as well as the plasma albumin concentrations. METHODS: The drug was administered subcutaneously, at a concentration of 5 mg/kg, twice a week for three weeks, to animals in the treated group. Once a week, the rats were anesthetized with sodium pentobarbital (50 mg/kg, ip) and they underwent an electrocardiogram (ECG). After the trial period, samples of the cardiac muscle (left ventricle – LV), soleus muscle (S), white gastrocnemius muscle (WG), red gastrocnemius muscle (RG), pectoral muscle (P), intercostal muscle (IC) and diaphragm muscle (D) were promptly collected and analyzed. An analysis of variance (ANOVA) and then a Tukey test (p>0.05) were carried out to assess the data (mean ± sem). RESULTS: There were changes in the following parameters of rats in the treated group: QRS interval, QTc interval and heart rate, characterized by an increase in these parameters, with the peak being reached in the period between the pre-treatment week and the first week. There was an increase of 127% in glycogen reserves in the LV. In relation to the total-protein amount, the significant difference was found in S, RG and D. As for the hematocrit and biochemical profile, it was possible to notice an increase in the percentage of erythrocytes. CONCLUSION: The study shows that major cardiac changes are triggered at an early stage, which indicates a hierarchy in the sequence of changes that compromise the homeostasis of the body.

Predictive value of resting heart rate for cardiovascular and all-cause mortality

Jaqueline Eilert Fagundes et al. Iran Castro

BACKGROUND: Resting heart rate (which ranges from 60 to 80 bpm) is one of the simplest cardiovascular parameters, and has been considered as a predictor of cardiovascular and all-cause mortality. OBJECTIVE: To determine the predictive value of resting heart rate (RHR) before exercise stress testing (ET) for cardiovascular (CV) and all-cause mortality. METHODS: This was a case-control study using data from the database of the Exercise Testing Laboratory of a cardiac hospital and the death records of the Health Department of a city located in the South of Brazil from January 1995 to June 2007. A total of 7,055 patients were studied; 1,645 (23.3%) in the case group (deceased) and 5,410 (76.7%) in the control group (alive). The cut-off value of RHR for mortality was derived from the ROC curve, and a multivariate analysis was performed for the selected variables. The study’s outcome measures were cardiovascular and all-cause mortality. RESULTS: Six hundred and seventy-four patients died of cardiovascular diseases (9.5%); the cut-off value was RHR > 78 bpm). After adjusting for selected variables, the odds ratio (OR) of RHR > 78 bpm was 3.5 (95% CI 2.9 to 4.2) for CV mortality and 3.6 (95% CI 3.2 to 4.0) for all-cause mortality. CONCLUSION: Resting heart rate > 78 is an independent predictor of cardiovascular and all-cause mortality.

Acute effects of levosimendan and dobutamine on QRS duration in patients with heart failure

Osman Can Yontar et al. Mehmet Birhan Yilmaz Kenan Yalta Alim Erdem Izzet Tandogan

BACKGROUND: Levosimendan is a novel inotropic agent that enhances cardiac contractility without increasing cellular calcium intake, so that it is not supposed to cause intracellular calcium overload and related arrhythmias. In patients with heart failure, prolonged QRS duration is associated with increased risk of mortality and sudden cardiac death. Structural changes in the left ventricle may lead to asynchronous contraction, causing conduction delay and a prolonged QRS on the surface electrocardiogram. OBJECTIVE: We aimed to compare the acute effects of levosimendan and dobutamine on QRS duration in patients with severe heart failure and sinus rhythm. METHODS: Sixty consecutive patients with ischemic heart failure were enrolled for the study and randomized into two groups for levosimendan (n=37) or dobutamine (n=23) infusions. 67.2 % were male; mean age was 66.4±9.2 years for all patients. Baseline QRS durations in levosimendan and dobutamine groups were, 120.44 ± 23.82 ms vs 116.59 ± 13.80 ms respectively. Baseline ejection fractions were both depressed (23.15 ± 8.3% vs 24.56 ± 7.5%). RESULTS: In the levosimendan group, QRS duration shortened from baseline value to 116.47 ± 24.56 msec (p=0.006), whereas dobutamine group showed no significant change (p=0.605). Both drugs caused an increase in ejection fraction, but only the levosimendan group showed significance (27.95 ± 8.9% p=0.003 vs 26.67 ± 7.6%, p=0.315). CONCLUSION: We suggest that the administration of levosimendan, not dobutamine, shortens QRS duration on the surface ECG, possibly by means of providing collective contraction in the left ventricle muscle fibers. The molecular basis of this effect remains to be clarified.

Warfarin or Aspirin in embolism prevention in patients with mitral valvulopathy and atrial fibrillation

Paulo de Lara Lavitola et al. Roney Orismar Sampaio Walter Amorim de Oliveira Berta Napchan Bôer … Flavio Tarasoutchi Guilherme Sobreira Spina Max Grinberg

BACKGROUND: Atrial fibrillation (AF) associated to rheumatic mitral valve disease (RMVD) increases the incidence of thromboembolism (TE), with warfarin being the standard therapy, in spite of difficulties in treatment adherence and therapeutic control. OBJECTIVE: To compare the effectiveness of Aspirin vs Warfarin in TE prevention in patients with AF and RMVD. METHODS: A total of 229 patients (pts) with AF and RMVD were followed in a prospective and randomized study. The first group consisted of 110 pts receiving Aspirin – 200 mg/day (Group Aspirin – GA) and the second group consisted of 119 pts receiving Warfarin at individually-adjusted doses (Group Warfarin – GW). RESULTS: There were 15 embolic events in GA and 24 in GW (p = 0.187), of which 21 presented INR < 2.0. Thus, after excluding patients with inadequate INR, there was a higher number of embolic events in GA than in GW (15 vs 3) (p < 0.0061). The GW showed lower treatment adherence (p = 0.001). Neither group presented episodes of major bleeding. Small bleeding episodes were more frequent in the GW (p < 0.01). Increased serum levels of cholesterol and triglycerides constituted a risk factor for a higher number of thromboembolic events in the studied population, with no difference between the groups. CONCLUSION: In patients presenting RMVD with AF for less than a year and no previous embolism, Aspirin is little effective in preventing TE. Patients with lower-risk mitral valvulopathy (mitral regurgitation and mitral biological prosthesis), especially in cases presenting contraindication to or low adherence to Warfarin, Aspirin use can present some benefit in TE prevention.

B-type natriuretic peptide is predictive of postoperative events in orthopedic surgery

Humberto Villacorta Junior et al. Isabela Simões de Castro Marise Godinho Camila Mattos … Rogério Visconti Maurício Saud Mônica Novais Cláudia Murad Fernanda Nogueira

BACKGROND: Clinical assessment is not always sufficient to predict postoperative (PO) cardiac complications. B-type natriuretic peptide (BNP) has an important prognostic value in patients with heart failure. Its value as a predictor of events in orthopedic surgeries has not yet been tested. OBJECTIVE: To assess the value of BNP in predicting cardiac complications in the PO period of orthopedic surgeries. METHODS: A total of 208 patients undergoing surgical treatment of femur fracture and hip or knee arthroplasty were prospectively evaluated. Of these, 149 (71.6%) were women and the mean age was 72.6 ± 8.8 years. In the preoperative period, the patients underwent conventional clinical assessment and their surgical risk was estimated according to the American Society of Anesthesiologists’ (ASA) classification. BNP was determined in the preoperative period, and its ability to predict PO cardiac events (death; acute myocardial infarction; unstable angina; atrial fibrillation; ventricular tachycardia; or heart failure) was analyzed using multivariate logistic regression analysis. RESULTS: Seventeen patients (8.0%) experienced cardiac events. Median BNP was significantly higher in these patients in comparison to those without cardiac events (93 [interquartile range 73-424] vs 26.6 [13.2-53.1], p = 0.0001). BNP was the main independent predictor of events (p = 0.01). The ASA classification was not an independent predictor. Analysis of the ROC curve demonstrated that for a cut-off point of 60pg/mL, BNP showed sensitivity of 76.0% and specificity of 79.0% in the prediction of events, with an area under the curve of 83.0%. CONCLUSION: BNP is an independent predictor of PO cardiac events in orthopedic surgeries.

Relationship between depression, BNP levels and ventricular impairment in heart failure

BACKGROUND: Depression is a common comorbidity in heart failure (HF); however, the mechanisms related to a poorer outcome of depressed patients with HF remain unclear. OBJECTIVE: To evaluate the role of severe depression in the outcome of patients with decompensated HF. METHODS: A total of 43 patients with advanced HF, EF < 40.0%, and hospitalized for cardiac compensation were consecutively studied. After history taking and physical examination, the patients underwent laboratory tests including BNP determination. After the diagnosis of depression was made, the Hamilton-D scale was applied. Severe depression was defined by a score equal to or greater than 18. The clinical and laboratory variables according to the presence or absence of severe depression were analyzed using logistic regression. The ROC curve defined the cut-off point for BNP. RESULTS: Severe or very severe depression was identified in 24 (55.8%) patients. Severely depressed patients did not differ from non-depressed patients as regards age, gender and renal function, but showed less cardiac impairment (EF 23.4 ± 7.2% vs 19.5 ± 5.2%; p = 0.046) and higher BNP levels (2,582.8 ± 1,596.6 pg/ml vs 1,206.6 ± 587.0 pg/ml; p < 0.001). However, patients with BNP levels higher than 1,100 pg/ml had a 12.0-fold higher chance (odds ratio [95% CI] = 2.61 – 55.26) of developing severe depression. CONCLUSION: Patients with severe depression showed a higher degree of neurohormonal stimulation despite their lower degree of ventricular dysfunction. The pathophysiological changes related to depression, leading to increased neurohormonal stimulation and cytokines, probably contributed to this more intense clinical manifestation even in the presence of less cardiac damage.
