A New Risk Predictor in Acute Myocardial Infarction. Is There Still Room for One More?

Dalton Bertolim Précoma

Cardiology is one of the specialties that traditionally uses scientific evidence in daily practice, both in risk stratification and diagnosis, therapy and prognosis. One of the most discussed topics is atherosclerosis and inflammation, arousing great interest in the continuous knowledge acquired over the last two centuries. Several authors stand out in this historical context, for example, Rudolf Virchow in the 19th century who described the association of atherosclerosis with inflammation; Marchand in 1904, who suggested the relationship between atherosclerosis and the process of clogging of the arteries and in 1908, Ignatowski who observed the relationship between dietary cholesterol and atherosclerosis. Over the last 100 years, many articles have elucidated the pathophysiological sequence that we know today. Understanding atherosclerotic plaque formation and evolution through complex molecular mechanisms and innate and adaptive immunity, which culminate in acute myocardial infarction (AMI), is well established.– In 1974, Friedman GD et al., described the role of leukocyte count in the prognosis of AMI, and subsequently, other studies highlighted the importance of these cells in the deterioration and recovery of infarcted myocardium.– In the same line of investigation, Coste MER et al. investigated cytokines in patients with ST-segment elevation myocardial infarction (STEMI) and the relationship with ventricular function. They observed a balance of pro-inflammatory and anti-inflammatory cytokines, except for IL-6, suggesting a residual inflammatory risk.

In addition to these aspects related to atherosclerotic plaque and inflammatory activity in AMI, another process related to these so-called immuno-inflammatory cells, such as platelets, leukocytes, neutrophils, and lymphocytes, initially gained prominence in the field of oncology when described as a reliable prognostic marker in the progression of various malignant tumors, by the so-called “systemic immuno-inflammation index” (IIIS). The systematic review and meta-analysis by Zhong et al. emphasize the importance of this index in predicting survival since high rates were associated with a worse prognosis in solid tumors. In addition to neoplasms, other factors alter IIIS, such as age, obesity, type 2 diabetes, emotional stress, exogenous steroids, endogenous sex hormones, hematological disorders, stroke, pulmonary embolism, and trauma.

Evaluating the Severity of Coronary Artery Disease in Patients Treated with Chemotherapy: The Further Need for Cardio-Oncology

Matthew E. Harinstein

Cardiovascular toxicity related to cancer therapies has been recognized for years. The number and types of toxicities have increased rapidly due to several factors including new and improved therapies and treatment regimens which have resulted in patients living longer. This is the basis for the burgeoning field of cardio-oncology to help identify cardiotoxicities and aim to minimize adverse outcomes.

Cardiomyopathies related to anthracyclines, which are typically irreversible, and trastuzumab, typically reversible, as well as more recently recognized cardiotoxicities including myocarditis related to immune checkpoint inhibitors have made the evaluation of concomitant cardiac disease critical in the care of patients being treated for cancer. , Coronary artery disease (CAD) is also a consequence of cancer therapies and adverse coronary events such as myocardial infarction and thrombosis can complicate treatment and result in poor outcomes. Thus, further understanding of the adverse effects of specific therapies is crucial to assessing patients’ clinical statuses and making decisions on treatment strategies in order to maximize overall outcomes, both oncologic and cardiac. CAD and has been associated with radiation therapy. , The risk and anatomic severity of CAD related to radiotherapy treatment has been described. , In a study of 152 thoracic cancer survivors who underwent radiotherapy, the investigators observed that the study patients had higher SYNTAX scores and were at a higher risk of developing anatomically severe CAD, independent of chemotherapy. While although CAD is known to be present in patients being treated with chemotherapy, independent of radiotherapy, the association between anatomic severity of CAD and chemotherapy is less well known.

Evolved Inferior Wall Myocardial Infarction with Left Ventricular Pseudoaneurysm: A Diagnostic Dilemma

Sónia Gomes Coelho et al. Clara F. Jorge Pedro B. Carlos Anne Delgado Leopoldina Vicente

Introduction

The left ventricular (LV) pseudoaneurysm (PA) is a rare mechanical complication of acute myocardial infarction (AMI). It results from myocardial rupture, in which the hemorrhagic process is contained by the adherent pericardium. It occurs most commonly in the inferior and posterior ventricular wall, since the rupture of the anterior ventricular wall usually leads to cardiac tamponade and immediate death, while the inferior-posterior face of the heart rests on the diaphragm, facilitating the ventricular cavity containment by the pericardium. – Imaging methods are crucial to establish the diagnosis. Transthoracic (TTE) and transesophageal echocardiography (TEE) allow the definitive diagnosis in 26% and 75% of cases, respectively. , Cardiac magnetic resonance (CMR) imaging is useful in the differential diagnosis of LV PA and aneurysm, with a reported sensitivity of 100%. The presence of late pericardial enhancement in the CMR is highly suggestive of LV PA, which may represent the effect of the passage of blood into the pericardial space at the time of myocardial rupture, with subsequent pericardial inflammation and fibrosis. , ,

Abdominal Pain: an Uncommon Presentation of Myocardial Rupture

Daniel Seabra et al. Ana Neto Inês Oliveira Rui Pontes dos Santos … João Azevedo Paula Pinto

An 84-years-old woman with hypertension and dyslipidemia was admitted in the emergency room with acute abdominal pain; patient complained of chest pain three weeks before hospital admission. On physical examination, she was hypotensive, tachycardic (with arrhythmic pulse), tachypneic and with diffuse abdominal pain. Electrocardiogram showed atrial fibrillation with rapid ventricular response, complete left bundle branch block and inferior Q waves. Abdominal computed tomography (CT) revealed a thrombus in the superior mesenteric artery (, white asterisk). The patient showed a clinical course with congestive heart failure and low cardiac output. Transthoracic echocardiogram (-) showed mild left ventricular dilatation with a mild dysfunction and a pseudoaneurysm of the basal half of the posterior and inferior walls with left-to-right shunt, confirmed by color Doppler imaging (-). Cardiac CT () revealed contained myocardial rupture, located at the basal segments of the inferior and posterior septal walls, extending to the free wall of the right ventricle, forming a pseudo-cavity, which communicates with the true cavity of the right ventricle (). Despite vasopressor and inotropic support and proposal for cardiac surgery, the patient had an unfavorable course.

Clinical Management of Patients with First-Episode Atrial Fibrillation Detected in the Acute Phase of Myocardial Infarction

Mauricio Scanavacca et al. Tan Chen Wu

Atrial fibrillation (AF) episodes have been traditionally observed in up to 20% of the patients who suffer an acute myocardial infarction (AMI); 5-10% of them, as a “first episode”, during hospital admission. The etiology of AF, in this phase of AMI, includes several factors, such as elevated atrial pressure resulting from acute ventricular dysfunction, associated atrial ischemia (more common in inferior wall AMI), secondary inflammatory reaction, changes in autonomic nervous system behavior and in the neurohumoral pattern related to the pathophysiology of AMI, especially in those with ventricular dysfunction.–

A recent study published in the Arquivos Brasileiros de Cardiologia by Congo et al. suggest that, in the last years, the incidence of AF in the acute phase of AMI may be decreasing, due to the greater access of patients to early reperfusion and to better clinical treatment, associated with the more frequent use of angiotensin converting enzyme inhibitors (ACE inhibitors), beta blockers, statins and antiplatelet therapy. In addition, the study corroborates previous observations that new onset AF is associated to poorer clinical evolution and raises awareness of the clinical management of these patients, in particular in relation to the non-use of oral anticoagulants at hospital discharge.

Partial Papillary Muscle Rupture after Myocardial Infarction and Early Severe Obstructive Bioprosthetic Valve Thrombosis: an Unusual Combination

Inês Silveira et al. Marta Oliveira Catarina Gomes Sofia Cabral … André Luz Severo Torres

Introduction

Mechanical complications after myocardial infarction (MI) have become uncommon since the introduction of primary angioplasty. They can lead to a rapid clinical deterioration and a fatal outcome, with patient’s survival being dependent on their prompt recognition and intervention. We describe a case of two rare mechanical complications: a partial papillary muscle rupture after MI, followed by an early severe obstructive thrombosis of the implanted bioprosthetic valve.

Obesity and its Association with Food Consumption, Diabetes Mellitus, and Acute Myocardial Infarction in the Elderly

Erika Aparecida da Silveira et al. Liana Lima Vieira Thiago Veiga Jardim Jacqueline Danesio de Souza

Abstract

Background:

Obesity affects a large part of elderly individuals worldwide and is considered a risk predictor for the development of chronic diseases such as cardiac diseases, the leading causes of death in the elderly population.

Objective:

To investigate the prevalence of obesity and associated factors, with emphasis on the occurrence of other diseases and on food consumption in elderly individuals treated at the Brazilian Unified Health System (Sistema Único de Saúde, SUS).

Methods:

Cross-sectional sampling study performed in the city of Goiânia (Brazil) including elderly individuals (≥ 60 years) receiving primary care. During home visits, we performed anthropometric measurements and applied a structured, standardized, and pre-tested questionnaire assessing socioeconomic, demographic and lifestyle conditions, occurrence of diseases, and food consumption. We performed multiple Poisson regression analysis using a hierarchical model and adopting a significance level of 5%.

Results:

We evaluated 418 elderly patients with a mean age of 70.7 ± 7 years. Their body mass indices had a mean value of 27.0 kg/m2 and were higher in women than in men (27.4 kg/m2 versus 26.1 kg/m2, respectively, p = 0.017). Obesity had a prevalence of 49.0%, a risk 1.87 times higher between the ages of 60-69 years and 70-79 years, and a rate 1.4 times higher among individuals with more than four morbidities. On multivariate analysis, the factors associated with obesity were age 60-69 and 70-79 years, inadequate consumption of whole-wheat grains and adequate consumption of fruit, musculoskeletal diseases, diabetes mellitus, and acute myocardial infarction.

Conclusions:

Obesity had a high prevalence in the evaluated elderly population and was associated with food consumption, musculoskeletal disease, diabetes mellitus, and acute myocardial infarction.

Giant and Calcified Post-Infarction True Left Ventricular Aneurysm: What to Do?

Alexandre Hideo Kajita et al. Marcos Danillo Peixoto Oliveira Fernando Reis Menezes Marcelo Franken … Luciano Moreira Baraciolli José Carlos Nicolau

Introduction

A calcified true left ventricular aneurysm (LVA) is a serious complication in the chronic phase post an acute myocardial infarction (MI); however, it is seldom observed in modern clinical practice¹. LVA may be asymptomatic; however, it can be the cause of refractory heart failure, sustained ventricular arrhythmias, and arterial embolism. The indications for left calcified ventricular aneurysmectomy remain controversial.– We report a case of a giant, true LVA that was reasonably managed clinically in an inoperable patient.

ST Elevation Myocardial Infarction Early After Heart Transplantation

Sérgio Lourenço Madeira et al. Luís Filipe Raposo Márcio Madeira Marta Marques … Maria José Rebocho José Pedro Neves

Introduction

Heart transplantation is the last option to improve survival in end-stage heart failure. Unfortunately, the increased need for organs far outweighs the availability of donors. To overcome this situation and to increase the number of available hearts for transplantation, donor selection criteria were made less stringent – . Nowadays, the one-year survival rate after transplantation is close to 90% – , with 50% of the patients surviving for more than 10 years. The main factor limiting survival is allograft vasculopathy (AV), which occurs in up to 10% and 50% of the patients at 1-year and 5-year after transplantation, respectively.

ST elevation myocardial infarction (STEMI) is rare in these patients and is even more uncommon shortly after transplantation. Despite being an uncommon manifestation of AV, STEMI is usually related to AV , .
