Serial Echocardiographic Assessment of Right Ventricular Function and Its Association With Functional Capacity, Dyspnea, and Pulmonary Diffusion in Post-COVID-19 Survivors

Marcelo Rogelin et al. Adriana Ferraz Martins Graça Aranha Fernanda Rodrigues Fonseca Rosemeri Maurici da Silva Guilherme Loureiro Fialho

Abstract

Background

Survivors of post-COVID-19 may experience persistent functional impairment and late cardiopulmonary sequelae. The right ventricle (RV) is particularly susceptible to injury, and RV free wall longitudinal strain (RVFWLS), measured by 2D speckle-tracking echocardiography (2D-STE), can detect subclinical RV systolic dysfunction.

Objective

To evaluate longitudinal changes in RV systolic function, assessed by RVFWLS, tricuspid annular plane systolic excursion (TAPSE), fractional area change (FAC), and tricuspid annular systolic velocity (S′), and to investigate their associations with functional capacity (30-second sit-to-stand test [30STS]), functional status (Post-COVID-19 Functional Status Scale [PCFS]), dyspnea (modified Medical Research Council [mMRC] scale), and carbon monoxide transfer coefficient (KCO) in post-COVID-19 survivors.

Methods

This observational analysis included participants from a prospective post-COVID-19 cohort evaluated at two follow-up visits (AV1 and AV2), approximately 4 and 13 months after the acute infection. Adults with confirmed SARS-CoV-2 infection and analyzable transthoracic echocardiography using 2D-STE were included. Statistical significance was defined as a two-sided p < 0.05.

Results

A total of 49 patients were included; 55.1% were women, the mean age was 50.7 ± 10.3 years, 63.3% had obesity, 77.6% required intensive care unit admission, and 67.3% underwent invasive mechanical ventilation. Performance on the 30STS improved (10.1 ± 3.3 vs 11.6 ± 3.1 repetitions; p = 0.004), and KCO increased (4.1 ± 0.7 vs 4.3 ± 0.8 mL/min/mmHg/L; p = 0.002), whereas mMRC scores (p = 0.43) and median PCFS scores (p = 0.11) remained unchanged. TAPSE decreased modestly but remained within the normal reference range (2.2 ± 0.3 vs 2.1 ± 0.2 cm; p = 0.03), while RVFWLS, FAC, and S′ showed no significant changes over time. Cross-sectional correlations between RV systolic indices and clinical outcomes were weak and nonsignificant at both AV1 and AV2 (|r| 0.05). Longitudinally, changes in RVFWLS were positively correlated with changes in KCO (r = 0.33; p = 0.05), whereas changes in FAC were inversely correlated with changes in PCFS scores (r = −0.35; p = 0.02).

Conclusios

In this cohort of post-COVID-19 survivors, functional capacity and pulmonary diffusing capacity improved over time, whereas RVFWLS remained persistently reduced without significant longitudinal change. These exploratory findings suggest that RV systolic indices should not be interpreted in isolation as markers of functional limitation during the late post-COVID-19 period and support the use of an integrated, multiparametric approach to patient assessment.

Myocardial Work During Isometric Exercise: From Physiology to Clinical Practice

Rodrigo Bellio de Mattos Barretto et al. Carlos Eduardo Suaide Silva

Assessment of left ventricular function by echocardiography has advanced remarkably in its ability to characterize aspects that extend beyond traditional measures of volume and ejection fraction (EF). Among these advances, the evaluation of contractility through two-dimensional strain analysis has gained particular prominence. Technological developments now allow this measure to be adjusted for afterload through myocardial work (MW), providing greater potential to identify subtle changes in ventricular function.

Isometric exercise represents a strategy to increase afterload through handgrip, proving particularly useful in echocardiography because it enables the identification of functional changes in different clinical settings, including coronary artery disease, assessment of diastolic function, hypertension, athletes, and even atrial functional mitral regurgitation.–

Echocardiographic Reference Values: Why Brazil Needs to Speak With its Own Voice

Imara Correia de Queiroz Barbosa et al. Israel Nilton de Almeida Feitosa

Echocardiographic measurements are cornerstones of diagnosis, risk stratification, and therapeutic decision-making across a wide range of cardiovascular conditions. Despite this, clinical practice in Brazil still relies largely on reference ranges derived from European and North American populations. In a country of continental dimensions, marked by substantial ethnic, anthropometric, and socioeconomic heterogeneity, such extrapolation raises legitimate concerns regarding diagnostic accuracy and equity in cardiovascular assessment.

In this issue of the Arquivos Brasileiros de Cardiologia, Rodrigues et al. present a timely and methodologically sound study establishing contemporary reference values for echocardiographic parameters in healthy Brazilian adults. The inclusion of 496 volunteers recruited from 20 echocardiography laboratories across the five major geographic regions of the country provides unprecedented national representativeness and addresses a longstanding gap in Brazilian cardiology.

Apical Sparing and Granular Sparkling: Not Everything that Glitters Is Cardiac Amyloidosis

Ana Paula Morais Barros et al. Cristhian Espinoza Romero Juliano Novaes Cardoso Fábio Fernandes Viviane Tiemi Hotta

Introduction

With recent advances in the diagnosis and treatment of cardiac amyloidosis (CA), the role of complementary cardiac imaging methods for the early recognition of this disease has been widely discussed. In this context, myocardial hyperrefringence and preservation of left ventricular apex contractility (apical sparing; AS), evidenced by transthoracic echocardiography (TTE) with myocardial strain analysis (STE), are findings that frequently raise suspicion of CA.

However, despite this association, the heterogeneity of this finding in CA, depending on the population studied, in addition to its presentation in other conditions, leads to questions regarding the real accuracy of AS for the diagnosis of this disease. Through the report of three clinical cases of patients with different clinical conditions – hypertrophic cardiomyopathy (HCM), chronic kidney disease (CKD), and use of anabolic steroids – in which myocardial hyperrefringence and AS were described on echocardiography, a brief discussion is proposed regarding the pathophysiology of these findings and their limitations for the diagnosis of CA.

Left and Right Ventricular Strain in an Adult Brazilian Population from ELSA-Brasil Study: Reference Values and their Determinants

Eduardo Gatti Pianca et al. Murilo Foppa Giulia Bevilacqua Schmitz Wilson Cañon-Montañez … Bruce Bartholow Duncan Angela Barreto Santiago Santos

Abstract

Background

Impaired left ventricular (LV) and right ventricular functions are important predictors of cardiovascular risk. Global longitudinal strain (GLS) provides superior sensitivity for assessing systolic function compared to traditional parameters, enhancing diagnostic accuracy in various cardiac conditions. However, GLS reference values in diverse populations are lacking.

Objectives

To establish reference values for LVGLS and right ventricular free wall longitudinal strain (RVFWLS) in a Brazilian multiethnic population without cardiovascular risk factors or disease. We also explore how clinical and echocardiographic factors influence GLS distribution, addressing a gap in global guidelines that often rely on data from homogeneous or geographically distant populations.

Methods

We included 1,048 participants from the ELSA-Brasil cohort who underwent echocardiography with GLS analysis. A healthy subsample (n = 527) was defined by excluding individuals with cardiovascular or renal disease, hypertension, or diabetes to establish GLS reference ranges. The prevalence of abnormal GLS was assessed, and factors associated with reduced LVGLS and RVFWLS were identified. Statistical significance was defined as p < 0.05.

Results

In the healthy subsample (mean age 50.2 years, 59% female), mean LVGLS was 19.0% (95% confidence interval: 14.3 to 23.8) and RVFWLS was 28.3% (95% confidence interval: 22.3 to 34.3). Females exhibited higher LVGLS and RVFWLS values than males, with no significant age-related differences. Abnormal LVGLS and RVFWLS were observed in 3.8% and 1.6% of participants, respectively. Lower LVGLS was associated with obesity, hypertension, and diabetes; reduced RVFWLS correlated with higher body mass index and LV mass.

Conclusions

We propose reference values for LVGLS and RVFWLS in a large Brazilian cohort, highlighting associations with cardiovascular comorbidities and ventricular structure.

Prevalence of Subclinical Left Ventricular Dysfunction Evaluated by Global Longitudinal Strain in Female Carriers of Duchenne Muscular Dystrophy

Eduarda Vieira Rodrigues et al. Antonio Pedro Lima Costa Pereira Wilson Braz Correa Filho Maria Angelica de Faria Domingues de Lima … Marcio Aloysio Freitas Siqueira Junior Ana Paula Cassetta dos Santos Nucera Fabio de Souza

Abstract

Duchenne muscular dystrophy (DMD) is a muscular disease genetically linked to the DMD gene, which encodes dystrophin, a protein that is crucial for the integrity of skeletal muscle and myocytes. This condition follows an X-linked inheritance pattern. Female carriers are typically asymptomatic; however, they may develop cardiomyopathy, making screening for structural heart disease advisable. We conducted a cross-sectional study to assess the prevalence of subclinical left ventricular (LV) dysfunction, defined by reduced global longitudinal strain (GLS), in asymptomatic female DMD carriers. Participants underwent 2-dimensional speckle tracking echocardiography following a standardized protocol, with GLS values ≤ 18% classified as reduced. Continuous variables were described as mean ± standard deviation and categorical variables as percentages. The association between left ventricular ejection fraction (LVEF) and GLS was assessed using Pearson’s correlation coefficient. The significance level adopted was 5%. Using a convenience sample, 17 participants (mean age 39 ± 5 years) were enrolled. No significant changes were observed in LV diameters, and the mean LVEF was 62% ± 5%. Reduced GLS was identified in 3 participants, indicating a prevalence of 17.6% for subclinical LV dysfunction. A weak correlation was found between GLS and LVEF (r = 0.366). Our findings indicate that approximately 1 in 6 female DMD carriers had subclinical LV dysfunction. These results highlight the need for genetic counseling for mothers of patients with DMD, as well as early screening with sensitive methods to detect heart disease in female DMD carriers.

Echocardiographic Parameters for Evaluating the Right Ventricle in Patients with Chronic Obstructive Pulmonary Disease: Literature Review

Irving Gabriel Araújo Bispo et al. Vitor Morais Brambila Gabriela Câmara Chagas Maria Luisa Ribeiro Gonçalves Pereira Maria Angélica Miqueri de Melo

Abstract

Chronic obstructive pulmonary disease (COPD) is a lung disease that often has an indolent course and systemic repercussions. Its main cause is constant and prolonged exposure to cigarette smoke or other gases. Such particles lead to reduced airflow and consequent lung hyperinflation with varying degrees of airway obstruction, also causing skeletal muscle dysfunction, respiratory failure, and decreased peripheral blood flow. COPD patients with concomitant cardiovascular disease (CVD) have a worse prognosis than simply the prognosis of each disease alone. However, it is difficult to establish what the risk is for a given group of patients due to the complex network of etiological and pathophysiological interactions underlying both diseases. Quantitative and qualitative echocardiographic assessment of right ventricular function is becoming of increasing interest in cardiac diseases that affect the right ventricle (RV), such as congenital heart disease and pulmonary hypertension (PH). However, it is still challenging due to the complex anatomy of the RV. The objective is to revisit the echocardiographic parameters that functionally assess the RV in patients with COPD, even if they do not present clinical signs of ventricular dysfunction.

The Prognostic Impact of Myocardial Deformation Measures in Patients with COVID-19

João Giffoni Silveira Neto et al. Marcello de Brito Campos Luiz Mello Marcela de Castro Giffoni … Natanael Arnor da Silva Barros Goveia Antonio Claudio Lucas da Nóbrega Renata R. T. Castro

Abstract

Background:

Cardiovascular involvement in COVID-19 has been extensively studied. Echocardiography is an important technique in identifying cardiac involvement, serving as an excellent tool for prognostic variables.

Objectives:

Analyze the prognostic impact of speckle tracking in analyzing the left ventricle in patients with COVID-19 admitted to intensive care units (ICUs).

Methods:

Patients diagnosed with COVID-19 admitted to the ICU underwent an echocardiographic examination within the first 48 hours of hospitalization and were divided into two groups according to hospital outcome (discharge or death). Conventional echocardiographic data, as well as global longitudinal strain of the left ventricle (LVGLS) and right ventricle (RVGLS) of both groups, were compared using Student’s t test (for continuous variables) or the chi-square test (for categorical variables), considering p < 0.05 as statistically significant.

Results:

The average age was 56 ± 14 years, and the proportion of men and women was similar. The mortality rate was 64%, more frequent in elderly patients and in patients with a higher number of comorbidities. LVGLS revealed lower values in patients who progressed to death, with a cutoff point below −18.1% (sensitivity = 90.4%, specificity = 96.6%) for this outcome, and, after multivariate statistical analysis, it was found to be the only statistically significant echocardiographic variable.

Conclusion:

LVGLS is an important tool in the prognostic analysis of critically patients with COVID-19, offering a new window of possibilities for evaluating these patients.

Abstract

Introduction:

Left ventricular dysfunction as an independent predictor of mortality in Chagas disease. Global longitudinal strain (GLS) is an emerging echocardiographic modality with possible incremental value in early detection of cardiac involvement.

Objective:

Compare left ventricular function with multiple echocardiographic parameters between patients with the indeterminate and chronic forms of Chagas, and perform a literature review.

Methods:

In this observational study, 11 patients with a confirmed diagnosis of Chagas disease were evaluated. Individuals were distributed as follows: five with the indeterminate and six with the chronic forms. Conventional echocardiographic evaluation was followed by GLS measurement.

Results:

The mean left ventricle ejection fraction left ventricular ejection fraction (LVEF) was 33 ± 2.88, and the mean GLS of the left ventricle was −10.76 ± 2.20 in the chronic form as compared to 57.80 ± 6.49 and −17.34 ± 3.82, respectively in the indeterminate group. Contractility was reduced in segments of the inferior, inferoseptal and anteroseptal walls in both groups. These segmental alterations in the indeterminate group may be attributed to early myocardial damage from inflammation and fibrosis, even before significant left ventricular dysfunction is evident.

Conclusion:

Analysis of longitudinal strain in the indeterminate form has potential incremental value to the conventional analysis of ejection fraction when compared to patients with chronic Chagas cardiomyopathy, especially with individual segmental analysis, but the impact and clinical significance of these techniques are still unknown.

My Approach to Echocardiography for the Diagnosis of Amyloidosis

Liria Lima Maria da Silva et al. Andrea de Andrade Vilela Antonio Tito Paladino Filho Jorge Eduardo Assef

Abstract

Cardiac amyloidosis (CA) is a multisystemic, infiltrative disease characterized by the deposition of insoluble proteins, known as amyloid fibrils, in the interstitial space of different tissues, such as the heart, nervous system, gastrointestinal tract, and eyes. In the heart, it is characterized by a progressive myocardial thickening, evolving into a restrictive pattern. CA is being diagnosed with increasing frequency, probably due to the greater dissemination of related information and advances in diagnostic methods and therapies that modify its course. Echocardiography is the first-line examination for diagnostic screening, monitoring, and differentiation from phenocopies.

New Incremental Model for Predicting Mortality in Pre-Capillary Pulmonary Hypertension

Andressa Alves de Carvalho et al. Wanessa Alves de Carvalho Eliauria Rosa Martins Agostinho Hermes de Medeiros Neto … Fernando Bacal Marcelo Dantas Tavares de Melo

Abstract

Background:

In pulmonary hypertension (PH), the identification of easily obtainable prognostic markers associated with right ventricular (RV) dysfunction and survival is needed.

Objective:

To evaluate the association of red cell distribution width (RDW) with clinical, echocardiographic parameters and survival in patients with pre-capillary PH, with the development of a mortality prediction model.

Methods:

Observational, longitudinal, and prospective study conducted from May 2019 to December 2022. Thirty-four patients with pre-capillary PH underwent two-dimensional echocardiography and complete blood count. A cutoff point of 14.5% was considered to define RDW as altered (≥14.5%) or normal (<14.5%). P values <0.05 were considered significant.

Results:

The median RDW was 14.4%. There was a significant difference in peripheral arterial oxygen saturation (SpO2) (p=0.028), RV strain (p=0.047), and pericardial effusion (p=0.002) between the normal and elevated RDW groups. During a median follow-up of 15 months, 20.6% died. Patients with increased RDW had a shorter overall survival (44.7%, log-rank p=0.019), which was a predictor of mortality in univariate Cox regression (HR 8.55, p=0.048). The addition of RV strain <16% and SpO2 ≤93% to the model including RDW alone showed incremental value in predicting mortality (χ2=8.2, p=0.049; χ2=12.4, p=0.041), with increased area under the receiver operating characteristic curve (0.729 vs. 0.837 vs. 0.909) and decreased probability of survival (44.7% vs. 35.6% vs. 25%, log-rank p=0.019).

Conclusions:

RDW provides information on the severity of pre-capillary PH by correlating with echocardiographic parameters of RV dysfunction and mortality, which is best predicted by a model including RDW, RV strain and SpO2.

A Critical Review of Echocardiographic Findings for Diagnosing Cardiac Amyloidosis

Silvio Henrique Barberato et al. Adenalva Lima de Souza Beck Viviane Tiemi Hotta Daniela do Carmo Rassi

Abstract

This article provides a critical review of the diagnostic value of several echocardiographic findings in cardiac amyloidosis (CA). The importance of early and accurate detection of CA is emphasized, considering its challenging diagnosis and the need for a high index of suspicion by clinicians. Echocardiography is often the first choice for imaging assessment of cardiac structure and function when CA is suspected. The article encompasses several conventional echocardiographic features and speckle-tracking echocardiography-derived deformation parameters. Some of these indexes are grouped together to form scores, which can improve the accuracy of diagnosing CA. However, particularly in earlier stages, echocardiography has low specificity to distinguish amyloid from other hypertrophic phenotypes, highlighting the need for correlation with clinical red flags, laboratory tests, and additional cardiac imaging modalities.

Left Atrial Strain: Clinical Applications and Prognostic Implications

Fernanda de Azevedo Figueiredo et al. Admilson Lemos da Costa Filho Flávio de Azevedo Figueiredo Luz Marina Tacuri Chavez … Marcia Fabrícia Almeida Teixeira William Silva Barbosa Pedro Henrique Bronzatto Priscila Rabelo Cintra Maria Carmo Pereira Nunes

Abstract

Left atrial (LA) strain, obtained using two-dimensional speckle tracking echocardiography (2DSTE), has emerged as an accurate method for evaluating LA function. Recently, it has gained prominence in clinical practice due to its significant prognostic value in various cardiovascular diseases, standing out due to its greater sensitivity compared to traditional methods of volumetric analysis and Doppler parameters.

This review article addresses the complex function of the LA and its close interdependence with left ventricular (LV) function, highlighting its relevance in cardiac performance. The LA performs three distinct functions, acting as a reservoir during ventricular systole, as a conduit in early diastole, and as a contractile chamber during the atrial contraction phase.

This review analyzes the normal patterns of LA strain and its application in various clinical conditions, such as atrial fibrillation (AF), heart failure, coronary artery disease, obesity, diabetes mellitus, hypertrophic cardiomyopathy, and cardiac amyloidosis. The crucial role of atrial strain in rheumatic mitral stenosis (MS) is highlighted due to its capacity to predict clinical outcomes.

The Indiscriminate Use Of Androgenic Anabolic Steroids: The Contribution of Cardiovascular Imaging

Irving Gabriel Araöşjo Bispo et al. Isabella Montanher Zago

Abstract

Anabolic androgenic steroids (AAS) are natural or synthetic compounds similar to the male hormone testosterone. The literature also describes cases of AAS abuse, such as in bodybuilding, cosmetics, recreational use, and aesthetic enhancement. It was discovered that AAS alone directly induce myocardial injury, with the main pathological finding in autopsied hearts being left ventricle (LV) hypertrophy in frequent association with myocyte hypertrophy, increased collagen deposition in the matrix, increased activity of the cardiac angiotensin, and myocardial fibrosis.

Patients who used illicit AAS present decreased LV systolic function. In athletes who self-administer AAS, LV hypertrophy and elevated sympathetic modulation were observed, as well as elevated blood pressure (BP). High doses of AAS can cause the user to have ventricular arrhythmias and sudden death. Furthermore, AAS users demonstrated greater coronary artery plaque volume than non-users. The objective of this review is to revisit the main effects of the use of AAS on cardiac changes through cardiovascular imaging as well as to establish a difference in relation to athletic heart syndrome.

Assessment of Myocardial Fibrosis by Myocardial Deformation Analysis via Echocardiography

Luiz Mário Baptista Martinelli et al. Fábio Fernandes Juliano Novaes Cardoso Viviane Tiemi Hotta

Abstract

Myocardial fibrosis (MF) is a pathological condition common to several primary and secondary heart diseases. Diffuse MF is a common feature in nearly all heart diseases, playing a crucial role in the progression of heart failure (HF). Several studies have indicated that the existence of MF serves as a prognostic factor for adverse outcomes across a range of heart conditions, underscoring the significance of this factor in clinical settings. Consequently, diagnostic approaches for MF, whether in subclinical phases or within established heart diseases, emerge as valuable instruments for early detection, risk stratification, and/or ongoing monitoring of disease developments. Despite being the most accurate non-invasive test for MF research, cardiovascular magnetic resonance (CMR) imaging is a test that is not widely available and is expensive. An alternative method with increased accessibility and cost-effectiveness for assessing heart diseases and researching MF involves echocardiography with the Speckle Tracking (STE) technique to evaluate myocardial deformation (“strain”). Studies show that diminished strain values (both global and segmental) in various heart conditions are associated with the presence and extent of MF, as determined by CMR or histological analysis. MF research using echocardiographic myocardial deformation analysis with STE has gained more space in clinical and research routines as it is an easy-to-perform and low-cost test. The results found by this technique can have a relevant diagnostic, therapeutic, and prognostic impact on clinical practice.

Use of Artificial Intelligence to Assess Cardiac Function by Echocardiography: Systematic Review of the State of the Art

Henrique Alexsander Ferreira Neves et al. Bruna Sadae Yuasa Thamires Hadassa Leite Pereira Costa Isabela Ertes Santos … Yannirê Milagros Roman Benavides Marco Stephan Lofrano-Alves

Abstract

Background:

Echocardiography is part of the routine investigation of most heart diseases. However, its operator-dependent nature and the quality of the image obtained can cause significant variability in measurements and impair their interpretation. The method has advanced on its path of standardization and incorporation of new technologies in an attempt to reduce its deficiencies. Recently, part of this advancement was due to the incorporation of artificial intelligence (AI).

Objectives:

The objective of this work was to investigate the use of AI in the assessment of cardiac function through echocardiography, discussing its potential to improve the method.

Methods:

We conducted a systematic review in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), searching for articles within the period of 2000 to 2023.

Results:

A total of 31 articles were included, classified into four categories of studies that used AI: 1) for image analysis and echocardiographic parameters (N = 14); 2) in the image acquisition process (N = 11); 3) for integration with clinical data (N = 3); and 4) for application to the exam flow (N = 3).

Conclusion:

AI provides automation and standardization of images and respective measurements that result in more accurate diagnoses, overcoming the limitations encountered by the conventional method. However, the use of AI is still an evolving reality within echocardiography.

Incidental Finding of Arrhythmogenic Right Ventricular Cardiomyopathy in a 72-Year-Old Man Admitted With Acute Coronary Syndrome

Rafael Modesto Fernandes et al. Luciana Cunha Weber Vitor Queiroz de Castro Souza Mariana Baptista Guedes … Diogo Freitas Cardoso de Azevedo Marcia Maria Nova Rabelo

Introduction

Arrhythmogenic right ventricular cardiomyopathy (ARVC) – also known as arrhythmogenic right ventricular (RV) dysplasia – is a genetic disorder characterized by progressive loss of RV cardiomyocytes, which are replaced with fibrofatty tissue. This replacement may delay intraventricular conduction and contribute to ventricular arrhythmias through a fibrosis-related macro-reentry mechanism. Thus, ARVC is one of the main causes of arrhythmic cardiac arrest in young people and athletes.

Although initially designated as dysplasia, ARVC is not a congenital defect in the myocardium development. It is caused by mutations in the genes that encode desmosomal proteins, which are responsible for cell-to-cell adhesion. This discovery led to the disease being recognized as cardiomyopathy and included in the classification of cardiomyopathies by the American Heart Association. ,
