Determinants of the Six-Minute Walk Distance in Subjects with Heart Failure-Chronic Obstructive Pulmonary Disease Overlap

Maria Luísa Dadalt et al. Isadora Farraco Correa Fernanda Lais Loro Marlus Karsten … Mariane Borba Monteiro Pedro Dal Lago

Abstract

Background

Heart failure (HF) and chronic obstructive pulmonary disease (COPD) frequently coexist in clinical practice, with both conditions negatively affecting individuals’ functional capacity, often measured using the 6-minute walk test (6MWT). Cardiovascular, pulmonary, and metabolic impairments may influence the 6-minute walk distance (6MWD).

Objective

This study aims to identify and analyze the critical determinants of the 6MWD among subjects with HF-COPD overlap.

Methods

A cross-sectional study was conducted. Individuals underwent the 6MWT. Respiratory muscle strength was assessed by measuring maximum inspiratory pressure and maximum expiratory pressure. Pulmonary function was evaluated using forced expiratory volume in the first second (FEV1) and forced vital capacity (FVC). Physical frailty was assessed using the Cardiovascular Health Study frailty index. Quality of life was assessed with the Minnesota Living with Heart Failure Questionnaire, and cognitive function was assessed using the Montreal Cognitive Assessment.

Results

Thirty-two individuals were evaluated, with a mean age of 67.6 ± 8.5 years; 65.6% were women. The mean left ventricular ejection fraction was 54.7 ± 15.9%, and the mean 6MWD was 281 ± 102 meters. The 6MWD was significantly correlated with FVC (r = 0.61, p < 0.01); percentage of predicted FEV1 (r = 0.58, p < 0.01); percentage of predicted FVC (r = 0.56, p < 0.01); Cardiovascular Health Study frailty index (r = 0.53, p < 0.01); and Minnesota Living with Heart Failure Questionnaire score (r = −0.47, p < 0.01). Gait speed (β = −0.54, p < 0.001); FEV1 (β = −0.44, p < 0.001); and New York Heart Association functional class (β = −0.24, p < 0.001) were identified as significant predictors of 6MWD.

Conclusions

These findings suggest that an integral approach, incorporating both physical and psychosocial assessments, may be essential for effectively managing functional limitations in patients with HF-COPD. Identifying these predictors can aid clinicians in tailoring treatment strategies, optimizing rehabilitation programs, and improving this population’s overall quality of care.

Echocardiographic Parameters for Evaluating the Right Ventricle in Patients with Chronic Obstructive Pulmonary Disease: Literature Review

Irving Gabriel Araújo Bispo et al. Vitor Morais Brambila Gabriela Câmara Chagas Maria Luisa Ribeiro Gonçalves Pereira Maria Angélica Miqueri de Melo

Abstract

Chronic obstructive pulmonary disease (COPD) is a lung disease that often has an indolent course and systemic repercussions. Its main cause is constant and prolonged exposure to cigarette smoke or other gases. Such particles lead to reduced airflow and consequent lung hyperinflation with varying degrees of airway obstruction, also causing skeletal muscle dysfunction, respiratory failure, and decreased peripheral blood flow. COPD patients with concomitant cardiovascular disease (CVD) have a worse prognosis than simply the prognosis of each disease alone. However, it is difficult to establish what the risk is for a given group of patients due to the complex network of etiological and pathophysiological interactions underlying both diseases. Quantitative and qualitative echocardiographic assessment of right ventricular function is becoming of increasing interest in cardiac diseases that affect the right ventricle (RV), such as congenital heart disease and pulmonary hypertension (PH). However, it is still challenging due to the complex anatomy of the RV. The objective is to revisit the echocardiographic parameters that functionally assess the RV in patients with COPD, even if they do not present clinical signs of ventricular dysfunction.

Comparable Ventilatory Inefficiency at Maximal and Submaximal Performance in COPD vs. CHF subjects: An Innovative Approach

Abstract

Background:

Currently, excess ventilation has been grounded under the relationship between minute-ventilation/carbon dioxide output (). Alternatively, a new approach for ventilatory efficiency () has been published.

Objective:

Our main hypothesis is that comparatively low levels of between chronic heart failure (CHF) and chronic obstructive pulmonary disease (COPD) are attainable for a similar level of maximum and submaximal aerobic performance, conversely to long-established methods ( slope and intercept).

Methods:

Both groups performed lung function tests, echocardiography, and cardiopulmonary exercise testing. The significance level adopted in the statistical analysis was 5%. Thus, nineteen COPD and nineteen CHF-eligible subjects completed the study. With the aim of contrasting full values of and for the exercise period (100%), correlations were made with smaller fractions, such as 90% and 75% of the maximum values.

Results:

The two groups attained matched characteristics for age (62±6 vs. 59±9 yrs, p>.05), sex (10/9 vs. 14/5, p>0.05), BMI (26±4 vs. 27±3 Kg m2, p>0.05), and peak (72±19 vs. 74±20 %pred, p>0.05), respectively. The slope and intercept were significantly different for COPD and CHF (27.2±1.4 vs. 33.1±5.7 and 5.3±1.9 vs. 1.7±3.6, p0.850 for both).

Conclusion:

The is a valuable method for comparison between cardiopulmonary diseases, with so far distinct physiopathological mechanisms, including ventilatory constraints in COPD.

Assessment of the ventricular function of patients with advanced chronic obstructive pulmonary disease by using magnetic resonance imaging

Nazareth de Novaes Rocha et al. Rafael Stelmach Alberto Cukier José Rodrigues Parga … Luiz Francisco Ávila Márcia Caldas Paula Buck Charles Mady

OBJECTIVE: To assess the mechanisms that may be involved in the evolution of right and left ventricular dysfunction in patients with chronic obstructive pulmonary disease (COPD). METHODS: Magnetic resonance imaging was used in 11 control patients (group C) and 27 patients with COPD, who were divided into 2 groups, COPDc and COPDs, according to the presence or absence of right ventricular dysfunction, respectively. Doppler echocardiography was used for assessing the degree of pulmonary hypertension. RESULTS: The right ventricular diameter was similar in the 3 groups, COPDs, COPDc and C (29±8 mm; 31±7 mm; and 30±6 mm; respectively, P=NS). Right ventricular hypertrophy was observed only in the COPD groups (8±2 mm and 9±3 mm vs 5±1 mm; P<0.01). The percentage of systolic right ventricular lateral wall thickening (%RVLWT) in the 3 groups were as follows: 86±82% vs 41±35% vs 86±89%; P=NS). Different left ventricular ejection fractions were observed in the groups as follows: 69±9% vs 55±16% vs 76±6%; P < 0.01. A positive and significant linear correlation was observed between the left ventricular (LV) diastolic diameter and the LV systolic volume (r = 0.72; P < 0.01). No correlation was observed between the pulmonary volumes, arterial blood gas analysis, and ventricular function. CONCLUSION: No correlation was observed between the severity of pulmonary function and the degree of ventricular function impairment. Whether a preserved %RVLWT means the possibility of reversibility of right ventricular function remains to be elucidated. However, the presence of the phenomenon of ventricular interdependence was confirmed.
