Hospital readmissions and death from Heart Failure: rates still alarming

Antonio Carlos Pereira Barretto et al. Carlos Henrique Del Carlo Juliano Novaes Cardoso Paulo Cesar Morgado … Robson Tadeu Munhoz Marcelo Ochiai Eid Mucio Tavares Oliveira Jr Airton Roberto Scipioni Jose Antonio Franchini Ramires

BACKGROUND: Patients who require hospitalization because of decompensated HF represent a group of the most seriously ill individuals who evolve with high mortality and hospital readmission rates. OBJECTIVES: We sought to evaluate the current natural course of HF by analyzing mortality and readmission rates in this new era of neurohormonal blockage. METHODS: We followed the progress of 263 patients with a mean EF of 27.1%, admitted for decompensated HF between January 2005 and October 2006. Patients readmitted were only those whose health status precluded discharge after assessment and drug treatment in the Emergency Department. Patients were classified as HF-FC III/IV, mean age was 59.9±15.2 years, most were men, and 63.1% required inotropic drugs for cardiac compensation in the acute phase. RESULTS: Average hospital stay was 25.1±16.7 days. During hospitalization, 23 (8.8%) patients died. After discharge, over an average follow-up period of 370 days, of the 240 patients who were discharged 123 (51.2%) returned to the Emergency Department 1 to 12 times (total number of visits: 350); 76 of them were readmitted, and the average length of readmission stay was 23.5±18.0 days. Over the first year of follow-up, 62 (25.8%) patients died. CONCLUSIONS: HF remains a condition associated with high mortality and high hospital readmission rates. At the end of the first year, 44.5% of these patients had not needed to visit the ER or had died, which indicates that we should provide HF patients with the best possible care in an attempt to change the natural course of this increasingly frequent syndrome.

Reduction of arterial pressure and double product at rest after resistance exercise training in elderly hypertensive women

Denize Faria Terra et al. Márcio Rabelo Mota Heloísa Thomaz Rabelo Lídia M. Aguiar Bezerra … Ricardo Moreno Lima André Garcia Ribeiro Pedro Henrique Vinhal Raphael M. Ritti Dias Francisco Martins da Silva

BACKGROUND: Due to the existing controversies in literature about the potential benefits of resistance exercise training (RT) on arterial blood pressure (BP) at rest, and the lack of studies conducted with elderly hypertensive individuals, RT is seldom recommended as a non-pharmacological treatment for arterial hypertension. OBJECTIVE: To verify the effect of progressive RT on BP, HR, and RPP in elderly women with controlled hypertension. METHODS: 20 elderly women (66.8 ± 5.6 years of age), with a sedentary lifestyle, monitored with anti-hypertensive medication, participated in a 12-week RT program (resistance training group – RTG). Twenty-six elderly women (65.3 ± 3.4 years of age) with controlled hypertension did not engage in physical exercise during the study period, and composed the control group. RESULTS: After RT, there was a significant reduction in SBP, MBP, and DP values at rest. No significant drops in DBP and HR values at rest were observed after RT in both groups. The reduction in the RTG was 10.5 mmHg, 6.2 mmHg, and 2218.6 mmHg x bpm for SBP, MBP, and RPP, respectively. CONCLUSION: Progressive RT reduced SBP, MBP, and RPP values at rest of hypertensive elderly women who were on anti-hypertensive treatment.

Hemodynamic performance and inflammatory response during the use of VAD-InCor as a bridge to transplant

João Galantier et al. Luiz Felipe P. Moreira Anderson Benício Adolfo A. Leirner … Idágene Cestari Edmar A. Bocchi Fernando Bacal Noedir A. G. Stolf

BACKGROUND: Cardiac transplantation faces the serious problem of lack of donors and it is estimated that 20 to 40% of the patients die while waiting for heart transplantation. For these patients, the use of mechanical circulatory assist devices is the only choice of survival while waiting for a donor. In Brazil, the experience with mechanical circulatory support is limited and there is no regular program regarding the use of these devices as a bridge to heart transplantation. OBJECTIVE: To evaluate the hemodynamic performance and the systemic inflammatory response during the clinical use of the InCor-type ventricular assist device (VAD-InCor) as a bridge to heart transplantation. METHODS: Between October 2003 and April 2006, 11 patients in the waiting list for heart transplantation presented hemodynamic deterioration due to refractory cardiogenic shock. Seven of these patients were submitted to VAD-InCor implantation for left ventricular assistance. The etiologic diagnosis was Chagas’ disease in 5 patients and idiopathic dilated cardiomyopathy in 2. RESULTS: The duration of left ventricular assistance ranged from 14 to 42 days (mean 26.2 days). During this period, the hemodynamic performance of the DAV-InCor was adequate to support a normal hemodynamic state. There was normalization of central venous oxygen saturation and serum lactate. Two patients were submitted to heart transplantation, while the other 5 patients died under assistance due to infection and multiple organ failure. CONCLUSION: The performance of the VAD-InCor, in the hemodynamic behavior of the studied patients, was adequate for the maintenance of a satisfactory circulatory state during the studied period. There was improvement in the tissue perfusion parameters and maintenance of systemic inflammatory response signs. There was a high incidence of complications; however, complications related to the device, which could compromise the safety of its use, were not demonstrated.

Echocardiographic assessment of myocardial infarction evolution in young and adult rats

Francisco Cesar Pabis et al. Nelson Itiro Miyague Julio César Francisco Vinícius Woitowicz … Katherine Athayde Teixeira de Carvalho José Rocha Faria-Neto Valdir Ambrósio Moisés Luiz César Guarita-Souza

BACKGROUND: The regeneration of cardiomyocytes after a myocardial infarction (MI) is more evident in young animals; however, it is not known whether it is associated with functional improvement. OBJECTIVE: To perform the functional analysis by echocardiography (echo) of young adult rats submitted to MI. METHODS: Seventy-two animals were included in the study: 35 young rats (group Y) that were 28 days old and 37 adult rats (group A) that were 153 days old. The rats were subdivided in two subgroups: infarcted (YI and AI) and control (YC and AC). The animals were assessed by echocardiogram on the 7thand 30th postoperative days for the analysis of the ejection fraction (EF) and the final systolic (FSV) and diastolic volume (FDV) of the left ventricle. Only animals with EF < 40% were included in the study. RESULTS: The comparison of the FDV and FSV between infarcted and control animals showed that there was a significant increase in infarcted adult animals at the two analyzed phases. Among young animals only the FSV was significantly higher on the 7th day. The intragroup evolution analysis showed an increase in FDV and FSV in the two young subgroups, which was proportional to growth and only increase in FDV in the infarcted adult group. There was an improvement in EF in young rats, whereas EF remained decreased in adult rats when compared to controls. CONCLUSION: The infarcted young rats presented improvement in the systolic function and ventricular volumes 30 days after the infarction, whereas the adult rats presented increased FDV with no improvement in systolic function.

Practical use of a risk assessment model for complications after cardiac surgery

Tânia Mara Varejão Strabelli et al. Noedir Antonio Groppo Stolf David Everson Uip

BACKGROUND: The identification of risk factors for postoperative complications in cardiac patients with surgical indication may influence the therapeutic decision. OBJECTIVE: To describe the experience of a Cardiology hospital in the validation and practical use of a preoperative risk score. METHODS:To validate TUMAN’s score, chosen by considering morbidity and mortality, 300 adult patients were prospectively evaluated before elective cardiac surgery with the use of extracorporeal circulation (ECC). Patients with a score of zero to five were considered as being low risk; from six to nine, as moderate risk and a score higher than 10 as high risk for cardiac, infectious, neurological, pulmonary and renal complications, as well as death. RESULTS: The TUMAN classification showed a statistically significant association with the occurrence of infectious complications (p = 0.010), with the other postoperative complications (p = 0.034) and death (p <0.001). Pulmonary infection was the most frequent infectious complication (15.3%) and Infected patients had a longer ICU stay duration (p = 0.001) and more prolonged hospitalization (p = 0001). After routine use, a new review of 154 patients operated in 2005, confirmed the validity of this score in the identification of those with the highest risk of postoperative infections. CONCLUSION: TUMAN's score was chosen as it uses variables that can be promptly obtained, classifies in the same system the most frequently performed surgeries and predicts risk of postoperative complications, in addition to mortality. Its continued use in this hospital has been able to identify the group of patients with increased risk of complications, especially infectious ones, although it was not useful in the prediction of individual risk.

Comparison of maximal heart rate using the prediction equations proposed by Karvonen and Tanaka

Sérgio Ricardo de Abreu Camarda et al. Antonio Sérgio Tebexreni Cristmi Niero Páfaro Fábio Bueno Sasai … Vera Lúcia Tambeiro Yara Juliano Turíbio Leite de Barros Neto

BACKGROUND: Equations for predicting maximal heart rate (HRmax) are widely used in exercise testing and for training prescription, but their efficacy remains controversial in the literature. OBJECTIVE: To compare maximal heart rate during cardiopulmonary exercise testing (CPET) using the prediction equations developed by Karvonen and Tanaka. METHODS: Of the 24,120 maximal treadmill graded exercise tests stored in the CEMAFE database from 1994 to 2006, 2047 HRmax values were analyzed, 1091 of which were from male and 956 from female sedentary subjects. These data were used as a gold standard to compare Karvonen’s and Tanaka’s prediction formulas. RESULTS: Mean measured maximal heart rates were 181.0 ± 14.0; 180.6 ± 13.0, and 180.8 ± 13.8 for men, women, and both genders combined, respectively. Likewise, mean values from Karvonen’s equation were 182.0 ± 11.4; 183.7 ± 11.5, and 183.9 ± 11.7; and from Tanaka’s, 182.0 ± 8.0; 182.6 ± 8.0, and 182.7 ± 8.2. Karvonen’s and Tanaka’s equations yielded the same correlation coefficients, as compared with measured maximal heart rate (r = 0.72). CONCLUSION: Karvonen’s and Tanaka’s equations are similar in predicting maximal heart rate and show good correlation with measured maximal heart rate.

The importance of pre-clinical animal testing in interventional cardiology

Yoriyasu Suzuki et al. Alan C. Yeung Fumiaki Ikeno

The treatment of cardiovascular disease has changed dramatically over the past 2 decades, allowing patients to live longer and better quality lives. The introduction of new therapies has contributed much to this success. Nowhere has this been more evident than in interventional cardiology, where percutaneous cardiovascular intervention has evolved in the past 2 decades from a quirky experimental procedure to a therapeutic cornerstone for patients with symptomatic cardiovascular disease. The development of these technologies from the earliest stages requires preclinical experiments using animal models. Once introduced into the clinical arena, an understanding of therapeutic mechanisms of these devices can be ascertained through comparisons of animal model research findings with clinical pathological specimens. This review provides an overview of the emerging role, results of preclinical studies and development, and evaluation of animal models for percutaneous cardiovascular intervention technologies for patients with symptomatic cardiovascular disease.

Contrast echocardiography for the evaluation of tumors and thrombi

Eliza Kaori Uenishi et al. Márcia A. Caldas Ally N. R. Saroute Jeane Mike Tsutsui … Gustavo H. M. Piotto Sandra N. R. S. Falcão Wilson Mathias Jr.

Contrast echocardiography is based on intravenous injection of microbubbles that act as blood flow tracers and increase ultra-sound signal. Contrast agents have shown to improve cardiac cavities opacification and endocardial border delineation in addition to helping perfusion evaluation. Contrast echocardiography has recently been used to evaluate cardiac masses. In this report we will describe three cases evaluated by contrast echocardiography: a left atrial myxoma (benign tumor), a lung adenocarcinoma metastasis (malign tumor), and one thrombus. Contrast echocardiography showed to be valuable in the diagnosis of the different types of cardiac masses.

Prevention strategies of cardioembolic ischemic stroke in Chagas' disease

Andréa Silvestre de Sousa et al. Sérgio Salles Xavier Gabriel Rodriguez de Freitas Alejandro Hasslocher-Moreno

BACKGROUND: The cardioembolic (CE) ischemic stroke is an important clinical manifestation of chronic chagasic cardiopathy; however, its incidence and the risk factors associated to this event have yet to be defined. OBJECTIVE: To determine prevention strategies for a common and devastating complication of Chagas’ disease, the cardioembolic (CE) ischemic stroke. METHODS: 1,043 patients with Chagas’ disease were prospectively evaluated from 03/1990 to 03/2002 and followed up to 03/2003. Cox regression was performed to create the CE risk score that was related with the annual incidence of this event: 4-5 points – >4%; 3 points – 2-4%; 2 points – 1-2%; 0-1 points – 48 years) and an CE risk score was developed, which was associated with the annual incidence of this event. In group (2) there were no bleeding complications and the annual incidence of CE was 3.2%, all of them in patients with 4-5 points. CONCLUSION: Based on the risk-benefit analysis, warfarin prophylaxis for cardioembolic stroke in Chagas’ disease is recommended for patients with a score of 4-5 points, in whom the risk of CE overweighs the risk of a major bleeding. With a 3-point score, the risks of bleeding and CE are the same, so the medical decision of using either warfarin or ASA has to be an individual one. In patients with a low risk of CE (2-point score) either ASA or no therapy can be chosen. The prophylaxis is not necessary in patients with 0-1 point scores, in whom the stroke incidence is near zero.

Interleukin-1 receptor antagonist gene VNTR polymorphism is associated with coronary artery disease

Ahmet Arman et al. Ozer Soylu Ahmet Yildirim Andrzej Furman … Nesrin Ercelen Hakki Aydogan Ajda Coker Tuna Tezel

BACKGROUND: Coronary Artery Disease (CAD) is the atherosclerosis of coronary arteries that carry blood to the heart muscle. Atherosclerosis is an inflammatory disease. Cytokine gene variations such as those associated with the IL1 family are involved in the pathogenesis of atherosclerosis. OBJECTIVE: The purpose of this study was to determine the relationship between IL1 family polymorphisms (IL1RN VNTR, IL1B positions -511 and +3953) and CAD in Turkish population. METHODS: 427 individuals were submitted to coronary angiography and were grouped as 170 control subjects and 257 CAD patients. The CAD subjects were divided into two subgroups: 91 Single Vessel Disease (SVD) and 166 Multiple Vessel Disease (MVD) subjects. The genotypes of IL1RN and of IL1B (-511, +3953) were determined by polymerase chain reaction (PCR) followed by restriction digestion analysis. RESULTS: No significant difference was found in IL1RN and IL1B (-511 and +3953) genotype distributions between CAD and control subjects or MVD and control subjects. However, significant association was seen in IL1RN 2/2 genotype between SVD and control subjects (P= 0.016, x2: 10.289, OR: 2.94, 95% CI: 1.183-7.229). Similarly, no statistically significant difference was found in IL1RN and IL1B (-511 and +3953) allele frequencies between CAD and control subjects, MVD and control subjects or SVD and control subjects. CONCLUSION: No association was found in either allele frequency or genotype distribution of IL1RN and IL1B polymorphisms between CAD and the control groups. However; IL1RN 2/2 genotype may be a risk factor for SVD in the Turkish population.

Doctorate in cardiology at FM-USP heart institute from 1994 through 2004: defense and publication

Julio Flávio Meirelles Marchini et al. Bruno Caramelli

BACKGROUND: A study evaluated the relationship between the defense of dissertations and their publication within the realm of the entire Medical School of the University of São Paulo (FM-USP). The existence of differences among different areas of knowledge and the time between defense of dissertations and their publication is questioned. OBJECTIVE: To characterize publications related to dissertations defended in graduate courses of the Heart Institute (InCor) of the University of São Paulo Medical School (FM-USP). METHODS: A retrospective survey was carried out featuring graduate work at FM/USP for the 1994 – 2004 period. Initially, data were collected on students who defended dissertations during this period, on their advisors, and on the dissertations defended in the course of the period. Then, by crossing these data with data from Medline® and Web of Science®, publications by these authors and data were located that referred to the respective publications. RESULTS: During that period, 268 dissertations were defended, resulting in 195 publications within a period of up to 10 years after their defense. The median time for publication after defense was one year and nine months, with a median impact factor of 2.1, and a median of 4 citations per paper. There was no statistically significant correlation among any of the data studied. CONCLUSION: A significant percentage of the dissertations were published, and in the group studied, publication took place within increasingly short times after their defense. This fact may be related to the success of the graduate commissions’ policies and to the importance currently lent to debates on this theme, all of which contribute to improvement in the quality of graduate studies.
