Prevention strategies of cardioembolic ischemic stroke in Chagas' disease

Andréa Silvestre de Sousa et al. Sérgio Salles Xavier Gabriel Rodriguez de Freitas Alejandro Hasslocher-Moreno

BACKGROUND: The cardioembolic (CE) ischemic stroke is an important clinical manifestation of chronic chagasic cardiopathy; however, its incidence and the risk factors associated to this event have yet to be defined. OBJECTIVE: To determine prevention strategies for a common and devastating complication of Chagas’ disease, the cardioembolic (CE) ischemic stroke. METHODS: 1,043 patients with Chagas’ disease were prospectively evaluated from 03/1990 to 03/2002 and followed up to 03/2003. Cox regression was performed to create the CE risk score that was related with the annual incidence of this event: 4-5 points – >4%; 3 points – 2-4%; 2 points – 1-2%; 0-1 points – 48 years) and an CE risk score was developed, which was associated with the annual incidence of this event. In group (2) there were no bleeding complications and the annual incidence of CE was 3.2%, all of them in patients with 4-5 points. CONCLUSION: Based on the risk-benefit analysis, warfarin prophylaxis for cardioembolic stroke in Chagas’ disease is recommended for patients with a score of 4-5 points, in whom the risk of CE overweighs the risk of a major bleeding. With a 3-point score, the risks of bleeding and CE are the same, so the medical decision of using either warfarin or ASA has to be an individual one. In patients with a low risk of CE (2-point score) either ASA or no therapy can be chosen. The prophylaxis is not necessary in patients with 0-1 point scores, in whom the stroke incidence is near zero.

Risk factors for stroke after coronary artery bypass grafting

Dinaldo Cavalcanti de Oliveira et al. Carlos Romerio Ferro João Bosco de Oliveira Marcelo Menezes Malta … Plínio Barros Neto Silvia J. F. Cano Stevan Krieker Martins Luis Carlos B. Souza Adib Domingos Jatene Leopoldo Soares Piegas

BACKGROUND: Stroke is a feared complication after coronary artery bypass grafting surgery (CABG), with an incidence between 1.3 and 4.3%. OBJECTIVE: To identify predictive factors for stroke after CABG in the modern era of cardiac surgery. METHODS: This is a case-control study of 65 pairs of patients, paired by sex, age (+ 3 years) and date of CABG (+ 3 months). The cases were patients submitted to elective CABG with extracorporeal circulation (ECC) that presented stroke (defined as clinical neurological deficit up to 24 hours post-operatively and confirmed by imaging assessment) and the controls were those individuals submitted to elective CABG with ECC, but without stroke. RESULTS: The univariate analysis demonstrated that the number of revascularized vessels was associated with the occurrence of stroke after the CABG (3 ± 0.8 vs. 2.76 ± 0.8, p = 0.01). The multivariate analysis by conditional logistic regression showed that systemic arterial hypertension (SAH) [OR: 6.1 (1.5 – 24), p = 0.009] and diabete mellitus (DM) [OR: 3.1 (1.09 – 11), p= 0.03] were the determinants of the highest chance of stroke after CABG, whereas acute myocardial infarction (AMI) > 1 month, was the determinant of the lowest chance of stroke [OR: 0.1 (0.03 – 0.36), p = 0.003]. CONCLUSION: Hypertension and diabete mellitus were identified as independent predictors of stroke within the first 24 postoperative hours after CABG. In patients with such risk factors, it is possible that the knowledge of the causal mechanisms of brain injury represents a strategy capable of decreasing the incidence of stroke after CABG.

Patent foramen ovale and ischemic stroke in young people: statistical association or causal relation?

Edson Marcio Negrão et al. Ivar Viana Brandi Simone Vilela Nunes Daniel Gurgel Fernandes Távora … Mauro Nakayama Paulo Sergio Siebra Beraldo

OBJECTIVES: To determine if there are evidences of a causal relation between patent foramen ovale (PFO) x cryptogenic ischemic stroke (IS) in the young population and to analyze this relation in terms of causal criteria. METHODS: A total of 168 young patients with IS was retrospectively evaluated and divided into two groups: cryptogenic and with a defined cause. As a routine procedure, the patients underwent investigation of the PFO by means of transesophageal echocardiogram and/or transcranial Doppler sonography, both of them associated with the bubble test. Multivariate analysis was performed after demonstration of univariate statistical association between PFO x IS. RESULTS: After multivariate analysis, the association between PFO x cryptogenic IS was still statistically significant with odds ratio (adjusted OR = 3.3; 95% CI: 1.5-7.4). The total number of cerebral lesions also presented a significant association with cryptogenic IS (adjusted OR = 0.4; 95% CI: 0.2-0.9). The association between PFO and cryptogenic IS met all the causality criteria. CONCLUSION: The causal relation between PFO and cryptogenic IS in the young population is highly probable. This fact should be considered in the therapeutic decision.

Cardiovascular complications in a child with chronic renal failure

Gesmar Volga Haddad Herdy et al. Vânia Gloria Silami Lopes Maria Cecília Olivaes Isabele Coelho Mota Marcio Moacyr Vasconcelos

We describe a case 11 year-old boy, in which a chronic renal disease and secondary hyperparathyroidism was treated by dyalisis, calcitriol, and calcium supplementation. He developed dyslipidemia, calcified lesions with thrombus formation in several organs and vessels. Necropsy findings showed ischemic cerebral necrosis, calcification in arteries including coronaries, and myocardial infarction.
