Inflammatory markers of atherosclerotic plaque stabilization after acute coronary event – temporal trends

Osana Maria Coelho Costa Mouco et al. José Carlos Nicolau Tatiana da Rocha e Souza Lilia Nigro Maia José Antônio Franchini Ramires

OBJECTIVE: To evaluate the length of time required for atherosclerotic plaque stabilization in acute coronary syndromes (ACS), using inflammatory markers. METHODS: In this prospective study, C-reactive protein (CRP), fibrinogen, factor VIIIc, interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha) levels were measured on admission, at discharge, and three and six months post-discharge in 40 patients with non-ST-segment elevation ACS (NSTE-ACS) and 40 healthy subjects. RESULTS: C-reactive protein levels were significantly higher on admission and at discharge, but not at three and six months post-discharge, compared with the control group. Fibrinogen levels remained unchanged, except at six months, when they were significantly lower than in the control group. Factor VIII-c did not differ from that of the control group on admission, but it was significantly higher at discharge, with no differences at three and six months. Interleukin-6 levels were significantly higher than in the control group in all time points. However, they declined significantly between discharge and three months. In no time point was TNF-alpha significantly different from that of the control group. Only IL-6 correlated significantly and independently with future cardiovascular events. CONCLUSION: With respect to CRP and factor VIIIc, plaque stabilization is suggested in up to three months; IL-6 analysis suggests stabilization as from the third month, although it remained higher than that of the control group for up to six months. Only IL-6 showed prognostic value for further events within a year.

Effects of acute exercise on postprandial lipemia in sedentary men

Mônica Teixeira et al. Nelson Kasinski Maria Cristina de Oliveira Izar Luiz Alberto Barbosa … José Paulo Novazzi Leonor Almeida Pinto Sérgio Tufik Turíbio Ferreira Leite Francisco Antonio Helfenstein Fonseca

OBJECTIVE: To examine the effects of a single session of isometric physical exercise on postprandial triglyceridemia in sedentary male individuals with fasting triglycerides values ” 150 mg/dl (TG ALT). METHODS: Twenty-seven individuals (10 NTG and 17 TG ALT), aged between 30-55 years were assessed in the study. Triglycerides were determined in the beginning, and at two, four and six hours after the oral ingestion of a solution with 50g/m² of fat in two occasions: at rest and after treadmill isometric exercise. RESULTS: Acute exercise did not affect the levels of postprandial triglycerides or the area under the curve (AUC) of triglycerides. However, the abnormal pattern of postprandial lipemia curve was associated with higher basal triglyceridemia with exercise (basal TG: 147 ± 90 vs. 238 ± 89 mg/dl, p = 0.02) and without exercise (basal TG: 168 ± 93 vs. 265 ± 140 mg/dl, p = 0.04). Analysis of the receiver operating characteristics (ROC) curves showed cut-off values for basal triglycerides with exercise of 166.5 mg/dl (sensitivity: 0.78; specificity: 0.72) and AUC of 0.772 [CI 95%: 0.588-0.955], and without exercise of 172 mg/dl (sensitivity: 0.78; specificity: 0.61) and AUC: 0.722 [CI 95%: 0.530-0.914]. CONCLUSION: Acute exercise did not affect postprandial triglyceridemia in sedentary male individuals, and basal triglyceride levels are predictors of an abnormal response of postprandial triglycerides.

Wesley Ferreira de Araújo et al. Luis Roberto Gerola Hyoung C. Kin Armindo Pereira Filho … Guilherme Flora Vargas Roberto Catani Enio Buffolo

OBJECTIVE: To present the immediate results and assess the clinical and hemodynamic performance of stentless bovine pericardial bioprostheses in aortic position. METHODS: Twenty patients were operated who were indicated for surgery for valve replacement with biological prostesis. Eleven patients were male; sixteen had aortic stenosis; four, insufficiency; the mean age was 66.3 ± 8.8 years. The surgical technique used was subcoronary implant. Associated procedures were performed in five patients. Postoperative morbidity, mortality and hemodynamic performance were assessed on transthoracic echocardiogram. RESULTS: Mean time of cardiopulmonary bypass was 136.5 ± 24.41 minutes and mean anoxic time was 105.2 ± 21.62 minutes. Hospital mortality was 5% (one patient). Mean time of intensive care unit stay was 3.65 ± 3.23 days. Mean postoperatively transvalvular gradient was was 25.39 ± 7.82 mmHg. Left ventricle ejection fraction was 67 ± 13.49% preoperatively and 63.24 ± 16.06% postoperatively (p = 0.45). Eleven patients did not present any degree of valve regurgitation, eight presented mild regurgitation and one ,mild to moderate regurgitation. CONCLUSION: Stentless prostheses can be used for the surgical tratment of aortic valve diseases, with in hospital mortality and morbidity similar to the mortality and morbidity described in the literature for similar procedures, with satisfactory hemodynamic performance.

Swim training attenuates myocardial remodeling and the pulmonary congestion in wistar rats with secondary heart failure to myocardial infarction

Leslie Andrews Portes et al. Paulo José Ferreira Tucci

OBJECTIVE: To evaluate the effects of swimming on pulmonary water content in animals with heart failure (HF) after myocardial infarction (MI). METHODS: After coronary occlusion, MI size 20%<MI 40% of the LV large. The animals swam for 60 min/day, 5 days/week for 8 weeks. The wet weight of lung, liver, atriums, LV and right ventricle (RV) as well as the dry weight of the liver and lung were determined. ANOVA and Tukey test were used for statistical analysis. RESULTS: An increase in the atrium/body weight ratio was noted in the sedentary animals with moderate (MImod-SED: n=8) and large (MIlg-SED: n=10) infarctions in comparison to the sedentary control (C-SED: n=14) and trained (C-TR: n=16) rats. An increase in the RV/body weight and LV/body weight ratios was noted in the MIlg-SED. The heart/body weight ratio was higher in MIlg-SED when compared to the other groups. The infarcted trained animals presented diminished hypertrophy. The pulmonary water content was higher in MIlg-SED animals (81±0.4%) than in C-SED animals (79±0.4%). No differences were found for the other comparisons (C-TR: 79±0.4%; MImod-SED: 80±0.3%; MImod-TR: 80±0.6%; MIlg-TR: 79±0.7%). CONCLUSION: The increase of cardiac mass and pulmonary water content presented by MIlg-SED was diminished in the trained animals. The results suggest that the practice of physical exercise can diminish HF and contribute to favorable cardiac remodeling.

Effects of greater-than-5% weight reduction on hemodynamic, metabolic and neuroendocrine profiles of grade I obese subjects

Kelly Biancardini Gomes Barbato et al. Rita de Cássia Vieira Martins Maria de Lourdes Guimarães Rodrigues José Ueleres Braga … Emílio Antonio Francischetti Virginia Genelhu

OBJECTIVE: To evaluate the effects of a greater-than-5% weight reduction in hemodynamic, metabolic, and neuroendocrine profiles of grade I obese subjects. METHODS: Observational study with 47 grade I obese subjects, with mean age of 33 years who received monthly orientation regarding diet, physical exercises, and eating behavior for four months. Blood pressure using the auscultatory method and pulse rate were assessed monthly, whereas the following variables (and respective methods) were measured at the beginning and at the end of the study: total cholesterol, triglycerides, HDL-cholesterol (enzymatic method), LDL-cholesterol (Friedewald formula), blood glucose (hexokinase method), leptin, adiponectin, renin, aldosterone, insulin (radioimmunoassay) and insulin-resistance index (HOMA). RESULTS: After adjustment for other variables, significant reductions of 6 mmHg in diastolic blood pressure, 7 pg/ml in renin, 13 mg/dl in total cholesterol and 12 mg/dl in LDL-cholesterol were observed in the greater-than-5% weight reduction group. Also, a tendency to a higher increase in adiponectin levels by the end of the study, as well as a three-fold higher reduction in blood glucose, insulin, and HOMA levels, and a six-fold higher reduction in leptin levels were observed in this group. CONCLUSION: Non-pharmacological measures that promote a greater-than-5% weight reduction produce hemodynamic, metabolic, and neuroendocrine effects that improve the cardiovascular risk of obese subjects.

Determinants of risk of cardiovascular diseases in schoolchildren

Estelamaris T. Monego et al. Paulo César Brandão Veiga Jardim

OBJECTIVE: To investigate the occurrence and association of arterial hypertension with several lifestyle variables. METHODS: Transversal population-based study with a random sample of students (7 to 14 years of age) of public and private schools. Variables investigated were nutritional status, blood pressure, and lifestyle (tobacco use, alcohol intake, physical activity and eating habits). RESULTS: Out of the 3,169 schoolchildren assessed, 5.0% had arterial hypertension and 6.2% had normal-high blood pressure. Classification by gender shows boys 6.4% and girls 6.0% with normal-high blood pressure, and boys 4.3% and girls 5.7% with arterial hypertension. Body mass index (BMI) measurements identified 16.0% excess weight students, 4.9% of whom were obese. A significant association (p = 0.01) between arterial hypertension and excess weight was observed. Among the students participating in the study, 11.6% did not attend physical education classes and 37.8% had sedentary leisure habits. Twenty students (0.6%) were smokers and 32.7% had already experimented with alcohol. None of these variables showed statistical significance as to blood pressure values and nutritional status. CONCLUSION: In light of the findings in this study which show schoolchildren with a higher than expected frequency of mean blood pressure and BMI values, associated with a lifestyle that tends to favour the development of cardiovascular diseases, we felt led to propose interventional measures focused on the school as an agent of change and capable of conveying information to family units. This possibility encourages us to propose that schools be partners in promoting health.

Arterial prehypertension and elevated pulse pressure in adolescents: prevalence and associated factors

Maria Luiza Garcia Rosa et al. Vania Matos Fonseca Gabriela Oigman Evandro Tinoco Mesquita

OBJECTIVE: To estimate the prevalence of prehypertension and elevated pulse pressure in adolescents and assess the association between those two conditions and sex, age, sexual development, obesity and physical activity. METHODS: Anthropometrical data and blood pressure were measured in and a questionnaire was applied to 456 adolescents (aged 12 to 17 years) recruited from public and private schools, in the Fonseca district, in the city of Niterói, state of Rio de Janeiro, Brazil, from 2003 to 2004. RESULTS: Thirty nine (8.6%) presented prehypertension (PH) and 13.4%, elevated pulse pressure (PP). At bivariate analysis, PH was significantly associated with sex, age and obesity, with more prevalent in boys aged between 15 and 17 years, and in the obese. Elevated PP was associated with gender only, as it was more prevalent in boys. Sexual maturation did not show an association with PH or elevated PP. Similar correlations were found at logistic regression. PH prevalence odds ratio was 7.7 for sex, 4.3 for age and 4.6 for obesity. Elevated PP prevalence odds ratio was 10.8 for sex. The correlation between PP and physical activity was positive and significant. The elevation of PP was attributable to systolic blood pressure. CONCLUSION: PH and the elevated PP were shown to be present in adolescents from a population with a low prevalence of hypertension, mostly in boys. Further prospective studies are necessary to assess the persistence and the impact of those conditions.
