Individual and Joint Association between Cardiovascular Disease Risk Factors and Inadequate Lifestyle Behaviors in a Sample from Brazil

Letícia Gonçalves et al. Suellem Zanlorenci Andreia Pelegrini Tiago Rodrigues de Lima Diego Augusto Santos Silva

Abstract

Background:

Cardiovascular diseases (CVD) are often influenced by modifiable factors, notably individuals’ lifestyle choices, which play a crucial role in modulating cardiovascular risk.

Objective:

To investigate the individual and simultaneous association between inadequate lifestyle behaviors and risk factors for CVD in adults and older adults.

Method:

A cross-sectional study with 1079 users of the Health Academy Program in Brazil. Information related to inadequate diet, excessive alcohol consumption, smoking, and physical inactivity were individually and collectively investigated (0, 1, or ≥ 2 factors) in association with CVD risk factors (hypertension, hypercholesterolemia, diabetes mellitus, and obesity), considering the following two outcomes: presence of CVD risk factors and number of CVD risk factors present in the same individual (0, 1, 2, or ≥ 3 risk factors). Logistic and multinomial logistic regression analyses were used. The statistical significance adopted was 5%.

Results:

A higher number of inadequate lifestyle behavior was associated with greater odds of simultaneous presence of 1, 2, or ≥ 3 CVD risk factors. The simultaneous adoption of 1 and ≥ 2 inadequate lifestyle behaviors was associated with greater odds of hypercholesterolemia. Simultaneous adherence to ≥ 2 inadequate lifestyle behaviors was associated with lower odds of hypertension.

Conclusion:

A greater number of inadequate lifestyle behaviors was associated with higher odds of simultaneous presence of multiple CVD risk factors.

The Uncommon Orthostatic Hypotension in Brazil: Are We Underestimating the Problem?

Humberto Graner Moreira

Blood pressure (BP) homeostasis depends on complex physiological mechanisms that involve continuous interactions of the cardiovascular, renal, neural, and endocrine systems. These mechanisms must also guarantee the maintenance of adequate cardiac output, even in situations of rapid circulatory variations. One of these situations is related to the dynamic posture changes, from lying to standing, when the rapid reduction in venous return can affect preload, stroke volume, and mean BP. Orthostatic hypotension (OH) is a manifestation of autonomic dysfunction and occurs when cardiovascular adaptive mechanisms fail to compensate for those changes when assuming the standing position.

Diagnosing OH requires the demonstration of significant persistent BP decrease during orthostasis, either by the bedside active-standing test or a tilting test. National and international guidelines have endorsed the definition of OH as a ≥20 mmHg drop in systolic blood pressure (SBP) or a ≥10 mmHg drop in diastolic blood pressure (DBP) within 3 minutes after standing, regardless of the presence of symptoms. – This definition was first established by a consensus in 1996 and was based on several small physiology studies as well as on pragmatic considerations. Upon this definition, growing evidence has been shown that OH predicts all-cause mortality , and incidence of cardiovascular disease, , being even more relevant than the ambulatory BP monitoring-derived nighttime reverse dipping for predicting cardiovascular events . A recent meta-analysis involving 121,913 individuals and a median follow-up of 6 years reported that OH was associated with a 50, 41, and 64% greater risks of all-cause death, coronary heart disease, and stroke, respectively.

Co-occurrence of Smoking and Unhealthy Diet in the Brazilian Adult Population

Priscila Maria Stolses Bergamo Francisco et al. Daniela de Assumpção Deborah Carvalho Malta

Abstract

Background:

Smoking and an inadequate diet are behavioral risk factors that contribute to the majority of deaths and disabilities caused by noncommunicable diseases.

Objectives:

To estimate the prevalence of the co-occurrence of smoking and inadequate diet and identify associated factors in adults.

Methods:

A cross-sectional population-based study was conducted with a sample of 28,950 Brazilian adults (18 to 59 years old). Data were obtained from Sistema de Vigilância por Inquérito Telefônico (Vigitel [Brazilian Health Surveillance Telephone Survey]) in 2014. Independent associations were investigated using Poisson hierarchical regression analysis with 5% significance level.

Results:

The prevalence of the co-occurrence of smoking and unhealthy eating was 8.6% (95% CI: 7.9-9.3) and was higher among individuals residing in the southern region of the country than in those living in the central western region (PR = 1.50; 95% CI: 1.18-1.89), those with no private health insurance (PR = 1.14; 95% CI: 1.03-1.25), those who drank alcohol abusively (binge drinkers) (PR = 3.22; 95% CI: 2.70-3.85) and those who self-rated their health as fair (PR = 1.65; 95% CI: 1.36-1.99) or poor/very poor (PR = 1.70; 95% CI: 1.18-2.44). The prevalence of both factors was lower among individuals residing in the northeastern region of the country, women, individuals with brown skin color, those with a spouse, the more educated ones and those with overweight or obesity.

Conclusion:

The more vulnerable segments to the co-occurrence of the risk factors studied were men residing in the southern region of the country, individuals with a lower socioeconomic status and those who reported binge drinking. Interventions addressing multiple behavioral risk factors adapted to specific contexts could have a greater impact on the Brazilian population.
