Abstract

Background

Cardiovascular disease (CVD) remains the leading cause of death among women in Latin America and the Caribbean (LAC), with ischemic heart disease as the primary contributor. Despite this burden, women’s health programs in the region have traditionally prioritized cancer prevention, often neglecting CVD.

Objective

To analyze the burden of CVD and the distribution of cardiovascular risk factors among women in LAC, highlighting sex-related differences and regional disparities.

Methods

This study analyzed epidemiological data on CVD mortality and cardiovascular risk factors across LAC countries, including behavioral, metabolic, and female-specific risk factors, and conducted comparisons with selected countries outside the region.

Results

Women in LAC showed a higher prevalence of sedentary lifestyle, dyslipidemia, elevated fasting blood glucose, and overweight/obesity, whereas smoking was more frequent among men. Female-specific risk factors – such as teenage pregnancy, preterm birth, gestational diabetes, and hypertensive disorders of pregnancy – were highly prevalent yet under-recognized, largely shaped by socioeconomic determinants. In 2021, COVID-19 temporarily surpassed ischemic heart disease and stroke as the leading cause of death in the Americas (176.7 deaths per 100,000 population), highlighting the vulnerability of women with underlying cardiometabolic risk. Premature CVD mortality varied widely, with the highest female probabilities of death between ages 30 and 70 observed in Haiti (21.9%) and Guyana (15.2%), associated with poorly controlled hypertension and elevated cholesterol, respectively. In contrast, Canada showed the lowest premature mortality (1.7%), reflecting stronger healthcare systems and lower prevalence of metabolic risk factors.

Conclusion

CVD remains the leading cause of mortality among women in LAC, marked by substantial regional disparities and a significant burden of both traditional and female-specific risk factors. These findings highlight the urgent need for sex-specific, context-sensitive strategies that integrate cardiovascular prevention into comprehensive women’s health policies, moving beyond cancer-centered approaches.

Cardiac Sarcoidosis: The Role of Multimodal Imaging

Lara Cristiane Terra Ferreira Carreira et al. Lívia Carreira Adriana Soares Xavier de Brito

ABSTRACT

Cardiac sarcoidosis (CS) is a potentially severe manifestation of systemic sarcoidosis, associated with advanced atrioventricular block, ventricular arrhythmias, heart failure, and sudden death. Diagnosis remains challenging due to phenotypic variability and the limitations of conventional diagnostic methods. Advances in imaging techniques, especially the combination of positron emission tomography/computed tomography (PET/CT) using 18F-FDG and cardiac magnetic resonance imaging (CMR), have revolutionized the diagnostic approach and therapeutic monitoring of CS. This article reviews current concepts of CS and its diagnosis, with a focus on the role of PET/CT, the importance of appropriate patient preparation, and integration with CMR.

Prosthetic Valve Endocarditis: A Multidisciplinary Challenge

Ana Beatriz Aisemann Goulart Paiva et al. Ana Carolina de Azevedo Pastori Gabriel Silva Vaz Martins Lucas Norambuena Aulicino Marcelo Goulart Paiva

Clinical case

We report the case of a 55-year-old male patient who was admitted to the hospital in July 2019 with sudden left hemiplegia, mitral systolic murmur 4+, and aortic systolic and diastolic murmurs 2+. He had a history of aortic valve replacement in 2015, with a mechanical prosthetic aortic valve (PAV) due to infective endocarditis (IE). Cranial computed tomography (CT) revealed a right frontoparietal hematoma (), and blood tests revealed hyperglycemia and elevated C-reactive protein. His electrocardiogram (ECG) showed sinus rhythm with repolarization abnormalities (), and a chest X-ray showed cardiomegaly +/4 (). Transthoracic echocardiography (TTE) showed preserved function, thickening of the mobile elements of the mechanical PAV, mild double aortic lesion, pseudoaneurysm of the mitral-aortic intervalvular fibrosa, and important eccentric mitral regurgitation (). Blood cultures were requested, and ceftriaxone 4 g/d and teicoplanin 800 mg/d were started. Transesophageal echocardiography (TEE) showed vegetations on the ventricular side of the mechanical PAV leaflets and periprosthetic abscess (). Group B Streptococcus agalactiae was identified, and surgical treatment was indicated after 4 weeks due to cardiovascular stability and hemorrhagic stroke.

On day 22 of hospitalization, the patient presented pulmonary congestion and respiratory failure. Repeated TTE revealed partial loosening of the mechanical PAV and severe periprosthetic leak. Emergency surgery was performed to clean and replace the mechanical PAV with a biological PAV, without mitral valve approach. Culture of the explanted prosthesis revealed no bacterial growth.

Multiple Intracardiac Metastases – A Chaplet Heart

Mariana Tinoco et al. Margarida Castro Hans Dabó Filipa Cordeiro … Pedro von Hafe António Lourenço

A 52-year-old male was undergoing first-line chemotherapy with pemetrexed due to a stage IV right hilar lung adenocarcinoma with likely pleural, pericardial, liver, and bone metastases. During follow-up, no evidence of disease progression was found. Four years later a follow-up chest CT scan documented a nodular heterogeneous enhancement of the left ventricular (LV) myocardium (), the remaining lesions were overlapping. He had no cardiovascular symptoms and clinical examination was unremarkable. Transthoracic echocardiography () and cardiac MRI (Figures 1C-E) revealed several well-rounded endo-myocardial masses, some of which multilobulated sometimes with almost transmural extension. The lesions are predominantly distributed along the LV inferoseptal wall, inferolateral wall, anterolateral wall, anterior wall, and right side of the interventricular septum. These lesions had heterogeneous signal characteristics, being predominantly hyperintense on T1-weighted images, hypointense on T2-weighted images with a peripheral halo of hyperintensity, showing early heterogeneous and late intense gadolinium enhancement; and abnormal uptake of fluorodeoxyglucose in a 18F-FDG PET/CT ().

What do Cardiologists Expect from Imaging For Ischemic Heart Disease in Women

Maria Cristina Costa de Almeida et al. Claudia Maria Vilas Freire Larissa Espíndola Neto Gláucia Maria Moraes de Oliveira

Abstract

Ischemic heart disease (IHD) is an important cause of morbidity and mortality in women and men. This paper emphasizes the differences of IHD in women, focusing on the diagnostic investigation in its stable phase.

The higher prevalence of atypical symptoms, anatomical differences, and adverse psychosocial aspects, in addition to the association of specific risk factors regarding obstetrical history, requires an accurate clinical assessment and a targeted diagnostic investigation that consider the limitations, as well as the positive and negative predictive values of the methods. Because of breast exposure, tests involving radiation should be reconsidered in selected cases.

It is worth emphasizing that non-cardiological tests often performed in women, such as mammography, provide an opportunity to assess CVR at early ages or even in patients with few traditional risk factors.

Complementary tests for microcirculation assessment should be used in cases of proven ischemia in the absence of obstructive coronary artery disease, which is known to be more frequent in women.

A Step-By-Step Fight for Life in a Young Woman with High-Risk Pulmonary Embolism and Bilateral Renal Artery Occlusion

Miruna Stefan et al. Roxana Cristina Rimbas Rozina Vornicu Ruxandra Daneţ … Vlad Damian Vintila Bogdan Dorobat Alexandra Carp Vinereanu Dragos

Introduction

Right heart thrombus-in-transit is rare in the context of acute pulmonary embolism (APE) and is associated with high mortality and morbidity. , Transthoracic echocardiography (TTE) should always be performed in patients presenting dyspnea, chest pain, and hemodynamic instability. This case illustrates the importance of multimodality imaging in managing difficult cases with APE and right heart thrombus in transit, as well as the complications of this condition. A therapeutic approach should be decided on by multidisciplinary teams.
