Syncope and Rhythm Disturbances as Common Clinical Manifestations in Amyloidosis

Nágela S. V. Nunes et al. Erivelton Nascimento Ronaldo Altenburg Gismondi Erito Marques de Souza Filho

Abstract

Amyloidosis results from deposition of insoluble fibrillar protein compounds in various organs, including the heart. Cardiac involvement occurs in both atria and ventricles, leading to restrictive cardiomyopathy, heart failure (HF), and arrhythmias. Five amyloid proteins affect cardiac tissue: light and heavy chain forms of immunoglobulin, transthyretin (TTR), apolipoprotein 1, and amyloid A. Approximately 95% of cardiac involvement stems from light chain of immunoglobulin and TTR deposition in heart. Chamber stiffness and rhythm disturbances occur in cardiac amyloidosis (CA), encompassing from HF to atrial and ventricular arrhythmias (VA), and both can compromise each other. It is known that conduction system disorders and VA increase sudden death risk, as amyloid infiltration, inflammation, and fibrosis alter myocardial electrophysiology. Beyond the heart, autonomous nerve systems suffer amyloid deposition, contributing to syncope manifestation and increasing the risk of death and poor prognosis. In this context, our review sheds light on the importance of recognizing syncope and rhythm disturbances as crucial markers in the early identification and management of CA, potentially offering avenues for prompt intervention and improved patient outcomes.

Abstract

Innovations in devices during the last decade contributed to enhanced diagnosis and treatment of patients with cardiac insufficiency. These tools progressively adapted to minimally invasive strategies with rapid, widespread use. The present article focuses on actual and future directions of device-related diagnosis and treatment of chronic heart failure.

Unscheduled Emergency Visits after Cardiac Devices Implantation: Comparison between Cardioverter Defibrillators and Cardiac Resynchronization Therapy Devices in less than one year of Follow Up

Stefan Warpechowski Neto et al. Laura Lessa Gaudie Ley Eduardo Dytz Almeida Marco Aurélio Lumertz Saffi … Luiza Zwan Dutra Antonio Lessa Gaudie Ley Roberto Tofani Sant'Anna Gustavo Glotz de Lima Renato Abdala Karam Kalil Tiago Luiz Luz Leiria

Abstract

Background:

The use of Cardiovascular Implantable Electronic Devices (CIED), such as the Implantable Cardioverter Defibrillator (ICD) and Cardiac Resynchronization Therapy (CRT), is increasing. The number of leads may vary according to the device. Lead placement in the left ventricle increases surgical time and may be associated with greater morbidity after hospital discharge, an event that is often confused with the underlying disease severity.

Objective:

To evaluate the rate of unscheduled emergency hospitalizations and death after implantable device surgery stratified by the type of device.

Methods:

Prospective cohort study of 199 patients submitted to cardiac device implantation. The groups were stratified according to the type of device: ICD group (n = 124) and CRT group (n = 75). Probability estimates were analyzed by the Kaplan-Meier method according to the outcome. A value of p < 0.05 was considered significant in the statistical analyses.

Results:

Most of the sample comprised male patients (71.9%), with a mean age of 61.1 ± 14.2. Left ventricular ejection fraction was similar between the groups (CRT 37.4 ± 18.1 vs. ICD 39.1 ± 17.0, p = 0.532). The rate of unscheduled visits to the emergency unit related to the device was 4.8% in the ICD group and 10.6% in the CRT group (p = 0.20). The probability of device-related survival of the variable “death” was different between the groups (p = 0.008).

Conclusions:

Patients after CRT implantation show a higher probability of mortality after surgery at a follow-up of less than 1 year. The rate of unscheduled hospital visits, related or not to the implant, does not differ between the groups.

Analysis of Cardiac Dyssynchrony – An Unsolved Issue! How to improve selection and response to Cardiac Resynchronization Therapy?

Eduardo Arrais Rocha

The authors Nascimento et al., published an important work demonstrating the value of myocardial perfusion scintigraphy (SPECT-Gated) to identify cardiac dyssynchrony (CD) and with posterior implantation of left ventricle (LV) electrode in the area with higher dyssynchrony, with favorable correlation with clinical results and cardiac function. The study rises important issues in the attempt to reduce non-respondent rate (30-40%). These high rates are due to implants in very advanced or irreversible phases of the disease or to positioning of the LV electrode in areas with fibrosis or due to absence of dyssynchrony.

All the current directives take the indication of cardiac resynchronization therapy (CRT) in consideration, based on electrical dyssynchrony criteria, in detriment of mechanical dyssynchrony criteria. The Echo CRT study confirmed the unfavorable results in patients submitted to CRT in absence of CD, minding it may be a deleterious therapy.

Surgical Epicardial CRT-D Implantation in a Patient with Complete Obstruction of the Superior Vena Cava

Gustavo Lima da Silva et al. Nuno Cortez-Dias João de Sousa Ângelo Nobre Fausto J. Pinto

Introduction

Current guidelines clearly define the subset of heart failure patients who benefit from device implantation. Although first-time trans-venous device implantation has a high success rate, some patients present complex and challenging technical problems.
