Heroin by Dreser x Eroica by Beethoven

Claudio Pinho

As a Classical Music lover, I go to the São Paulo Concert Hall frequently. On my way to get there it’s necessary to drive through an area with high density of addicts on streets, known as Crackland. In that moment I get to see closely the important negative social effects and damage of addiction in its users. Now I meet this issue in a different way, reading the paper: “Heroin and Electrocardiography- Effect of Heroin on Electrocardiography parameters” that adds an incremental knowledge in the medical literature about heroin cardiac effects and searches for signals of when this occurs.

The authors of “Heroin and Electrocardiography- Effect of Heroin on Electrocardiography parameters” make it clear that there is little scientific information published in this field. Research in the PubMed shows a 1980 reference called “ECG examinations 115 heroin addicts” with no access to the abstract or the full-text contents. Very little was found about this specific topic since then. How can this be explained considering addiction has since then increased in alarming scale? Is there lack of interest by researchers in this area? Are there little financial resources for researchers? Logistical difficulties for studies in this field? Lack of interest in this social group?

Acute Coronary Disease in very Young Patient: A Reality!

Erivelton Nascimento et al. Ana Luisa Freitas e Pena Mello Ludmila Souza da Cunha Marcelo Ferreira Villela

Introduction

Acute cardiovascular events such as acute myocardial infarction (AMI) are uncommon in young individuals. Additionally, it is known that different factors may be involved in the occurrence of AMI. Here, we report a case of acute myocardial infarction due to severe coronary lesion in a very young patient.

Case 1/2017 – 26-Year-old Male with Rapidly Progressive Heart Failure

Laís Costa Marques et al. Rogério Silva de Paula Ivna Lobo Camilo Vera Demarchi Aiello

Clinical aspects

The patient was a 26-year-old male, reporting prophylaxis for rheumatic fever from the age of 12 years to 17 years, who, at the age of 24 years, developed dyspnea on heavy exertion, which improved with medication. However, after two years, dyspnea worsening and edema occurred (April 19, 2013).

The clinical and laboratory assessments before the referral revealed cardiopathy with ventricular dilatation. The echocardiogram evidenced left ventricular systolic and diastolic diameters of 60 mm and 44 mm, respectively, and ejection fraction of 51%. His serology for Chagas disease was negative, and his coronary angiography, normal. On physical examination, a third cardiac sound and a systolic murmur (++/6+) over the mitral area were heard. The ECG showed atrial flutter, atrioventricular block with probable ASDB, and heart rate of 40 bpm.

Case 6 / 2016 – Heart Failure in a 23-Year-Old Male with a History of Illicit Drug Use

Fernando Faglioni Ribas et al. Paulo Sampaio Gutierrez

Table 1. Laboratory evolution

On October 15th, there was a need for a progressive increase of vasopressors. Dialysis was recommended for kidney insufficiency and hyperkalemia, but the patient did not tolerate it, and, after two hours, dialysis was stopped on the night of October 17th 2013, and he received an association of vasopressors as well as a correction of metabolic disorders. Despite high doses of vasoactive drugs, with noradrenaline 3.0 mcg/kg/min, vasopressin 0.04 UI/min, dobutamine 20 mcg/kg/min, intra-aortic balloon 1:1, the patient went into cardiorespiratory arrest in pulseless electrical activity, and died at 00:45 on October 18th, 2013.
