Impact of COVID-19’s on Cardiovascular Rehabilitation Programs in Brazil: An Online Survey-Based Cross-Sectional Study

Iara de Sousa Cezário Jardim et al. Mauricio Milani Isac Castro Dominique Hansen … Marlus Karsten Lawrence Patrick Cahalin Graziella França Bernardelli Cipriano Gerson Cipriano Jr

Abstract

Background

The COVID-19 pandemic had an impact on cardiovascular rehabilitation (CR) programs in Brazil.

Objectives

To describe the characteristics of CR programs in Brazil, the impacts of the first epidemiological wave of COVID-19 (first 60 days) on the programs and present the initiatives used to overcome the impacts.

Method

This cross-sectional and retrospective study utilized a specific online survey. Participants were coordinators of CR programs. Variables were presented by Brazilian geographic region and as the following categories: demographic, clinical and operational characteristics. The significance level for statistical analysis was set at 5%.

Results

Fifty-nine CR programs were responsible for 5,349 patients, of which only 1,817 were post-acute cardiovascular events, which corresponded to 1.99% of hospitalized patients in the month prior to the survey (n=91,231). The greatest impact was the suspension of on-site activities, which occurred similarly in areas with the highest and the lowest rates of COVID-19 in the period. Forty-five programs (75%) were shortly interrupted, while three (5%) were ended. All 42 programs that already used remote rehabilitation strategies noticed a substantial increase in activities, based primarily on the use of media and video calling. Only three (5%) considered safe to see patients during the first 60 days.

Conclusions

There was a reduction in the number of CR programs with the COVID-19 pandemic. Telerehabilitation activities increased during the first two months of the COVID-19 pandemic, but this was not enough to overcome the reduction in CR program activities across Brazil.

An Unusual Manifestation of Rejection

Carlos Xavier Correia de Resende et al. Pedro Grilo Diogo Sandra Amorim Gonçalo Pestana … José Pinheiro Torres Filipe Macedo

Introduction

Orthotopic heart transplantation is the current treatment of choice for selected patients with end-stage heart failure. With the improvement of surgical techniques and the efficiency of new immunosuppressive treatments, short-term survival has markedly improved throughout the years. However, these patients still suffer from important comorbidities caused by chronic transplant complications such as rejection, coronary allograft vasculopathy (CAV) and malignancy. Rhythm disorders are common in heart transplant patients and in some circumstances, they can be the first clinical manifestation of rejection. Coronary vasospasm has been recently associated with acute rejection and CAV, but the mechanisms underlying this phenomenon are still speculative.

We present a clinical case of acute heart transplant rejection, manifested by coronary vasospasm and advanced rhythm disorder.

Fúlvio Pileggi: An Icon of Brazilian Cardiology

Protásio L. da Luz et al. Charles Mady Roberto Kalil Filho Carlos Eduardo Rochitte

Carlos Eduardo Rochitte

In this special editorial, we would like to pay tribute to an icon of Brazilian Cardiology who was directly responsible for developing one of the greatest cardiology institutions in the world, The Heart Institute (InCor), at the Hospital das Clínicas of the Faculty of Medicine of the University of São Paulo (USP). In the words of 3 people who were very close to him (Professors Kalil, Mady, and Protásio da Luz), we shall remember the career and the personality traits that characterized him as an undisputed leader for a generation of cardiologists. As a resident of InCor, I had the honor of meeting and interacting with Professor Pileggi, and he played a fundamental role in my academic career. First, there was apparently an obstacle to getting into a postgraduate program, “To enroll in a postgraduate program, you must have completed an internship abroad.” This, in association with the fact that nearly all scholarships to study abroad required, as a pre-requisite, that the applicant be enrolled in a postgraduate program, seemed to be an insurmountable barrier, which only served to stimulate me to look even harder for alternative ways to surmount it. And, in fact, he was the one who opened all the doors to make it possible for me to complete an internship at Johns Hopkins University. After that, the rest was history. I am eternally indebted to him for his teachings.

From Evidence-Based Medicine to Precision Health: Using Data to Personalize Care

Marcio Sommer Bittencourt

The historical practice of medicine has evolved over centuries based on empirical knowledge derived from experience and observation rather than rigorous scientific data. Over the second half of the twentieth century, this form of medical knowledge progress was largely supplanted by rigorous scientific data collection, particularly in the realm of cardiovascular diseases, where virtually every new drug discovery has been thoroughly evaluated in randomized clinical trials (RCT). The impressive improvement in the quality of the information provided by such study design has led to the development of an entirely new field in the medical knowledge which became known as Evidence-Based Medicine (EBM).

EBM soughs to move away from empirical information by providing a structured grading of the epistemological strength of evidence available. It further requires the highest levels of evidence to give strong recommendations for or against the use of any particular therapy. By this approach, RCTs are considered among the highest quality study designs which support those stronger recommendations. Still, despite their capability to avoid confounding and other biases, RCTs conclusions can only be interpreted as an overall averaged benefit for the collective population included in the study. Although such information may suffice to document the effect of any given therapy at the population level, this does not necessarily apply to any individual patient. While some individuals may benefit considerably more than the average population included in the study, other might benefit significantly less, whereas no benefit or even significant harm may occur in some individuals.

Network Meta-analysis to Synthesize Evidence for Decision Making in Cardiovascular Research

Leonardo Roever et al. Giuseppe Biondi-Zoccai

Abstract

Clinical decision-making requires synthesis of evidence from literature reviews focused on a specific theme. Evidence synthesis is performed with qualitative assessments and systematic reviews of randomized clinical trials, typically covering statistical pooling with pairwise meta-analyses. These methods include adjusted indirect comparison meta-analysis, network meta-analysis, and mixed-treatment comparison. These tools allow synthesis of evidence and comparison of effectiveness in cardiovascular research.
