Incomplete Kawasaki Disease Diagnosed only with Prolonged Fever: Report of Two Cases and Review of the Literature

Hikmet Kiztanir et al. Ayse Sulu Pelin Kosger Tugcem Akin Birsen Ucar

Introduction

Kawasaki disease (KD) is an acute febrile vasculitis of unknown cause. Incomplete (atypical) KD has been defined for patients who do not meet the classical characteristic diagnostic criteria. The incidence of coronary artery aneurysm development is higher in patients with incomplete KD due to delays in diagnosis and treatment. In the literature, patients who do not meet the diagnostic criteria but are diagnosed solely with fever are very rare. Here, we present two cases of KD who were diagnosed with coronary artery aneurysm and supportive laboratory findings with no characteristic signs other than high fever. The first case was a 3-year-old girl who was diagnosed with KD after an echocardiographic examination on the 14th day of the fever revealed a coronary artery aneurysm.– The second case was diagnosed on the 8th day of the fever because of the involvement of the coronary arteries without other classic Kawasaki findings. Both patients had been given antibiotherapy with different diagnoses such as gastroenteritis and urinary tract infection before the diagnosis of KD. Both cases were diagnosed before the COVID-19 pandemic. Therefore, multisystem inflammatory disease in children (MIS-C) was not considered. We aimed to emphasize the importance of considering KD in children with unexplained fever and high acute phase reactants, even in the absence of characteristic diagnostic criteria, and the need for serial controls to evaluate suspected patients with echocardiography and other supportive laboratory findings

Kawasaki-Like Disease, a New Phenotype in Sars-CoV-2?

Beatriz Sordi Chara et al. Júlia Machado Rickli Caroline Figueiredo da Silva Gabriela Pomaleski … Rafael de March Ronsoni Marcus Vinícius Magno Gonçalves

Abstract

The coronavirus disease (COVID-19), that assumed pandemic proportions in March 2020, mainly affects the respiratory tract, causing severe interstitial pneumonia in adults. Worldwide data indicate that COVID-19 tends to be more benign in children, which is evidenced by a high incidence of asymptomatic or mild upper airways’ infection cases in this population. However, recent studies have been associating Kawasaki-like symptoms as a nonclassical presentation of coronavirus disease in pediatrics. It is suggested that the intense cytokine cascade, promoted by the SARS-CoV-2 infection, can trigger a multisystem inflammatory response as an atypical Kawasaki form in genetically predisposed individuals. In this context, patients may develop more severe clinical features with a greater predisposition to myocardial involvement, Macrophage Activation Syndrome, and Kawasaki Disease Shock Syndrome. Despite critical conditions, patients usually respond to conventional treatment of Kawasaki Disease with intravenous immunoglobulin. This article intends to provide an approach to the association between Kawasaki-Like Syndrome and COVID-19.

Coronary Artery Dilation in Children with Febrile Exanthematous Illness without Criteria for Kawasaki Disease

Jesus Reyna et al. Luz Marina Reyes Lorenzo Reyes Freya Helena Campos … Patricia Meza Alfredo Lagunas Carla Contreras Ana Elena Limón

Abstract

Background:

Coronary dilatation is the most important complication of Kawasaki disease (KD) and, in addition to some clinical characteristics, is common to KD and febrile exanthematous illnesses (FEIs).

Objective:

To assess whether children with FEI, who do not meet the criteria for KD, have changes in coronary arteries dimensions.

Methods:

Echocardiography was performed within the first two weeks of the disease in patients < 10 years with fever and exanthema without other KD criteria. To make a comparison with KD patients, we reviewed the echocardiograms and medical records of patients with a diagnosis of KD of the last five years. Coronary ectasia was assessed using Z scores of coronary arteries. The means of the dimensions of the coronary arteries were compared with a z test and a level of significance of 0.05 was adopted.

Results:

A total of 34 patients were included, 22 (64.7%) with FEI, and 12(35.2%) with a diagnosis of KD. Using the Z scores of coronary artery, a dilation of any of the coronary artery branches was observed in six (27.2%) patients with FEI.

Conclusions:

An important percentage of patients with FEI has coronary artery dilation.

Pregnancy in Woman with Kawasaki Disease and Multiple Coronary Artery Aneurysms

Walkiria Samuel Avila et al. Antônio Fernando Diniz Freire Alexandre Anderson de Sousa Soares Ana Neri Rodrigues Epitácio Pereira José Carlos Nicolau

Introduction

Kawasaki disease (KD), first described in 1967, is a systemic vasculitis of unknown cause. It is an important cause of cardiac diseases in children aged younger than five years. KD is an autoimmune disorder whose clinical features include high fever, exanthema, conjunctivitis, cervical linfadenopathy and peripheral edema. Laboratory tests are compatible with acute inflammatory condition.

KD predominantly affects the coronary arteries, which is the most important clinical manifestation of the disease, varying from dilation and stenosis to aneurysm (incidence of 5% in patients with adequate treatment, and 25% in untreated patients). Giant coronary artery aneurysms (CAAs), i.e., CAAs with diameters > 8 mm, are associated with increased risk of thrombosis, acute myocardial infarction (AMI) and sudden death.

Coronary artery bypass graft (CABG) in the presence of coronary aneurysms

Jarbas J. Dinkhuysen et al. Andrea de Souza Nogueira Jose Viera Zarate Domingos Spina Neto Teresa Lola Pena Soria

This is the case of a patient with coronary aneurysms, who underwent CABG for surgical exclusion of these aneurysms, followed by implant of the bypass grafts to the arteries affected, with satisfactory short-term and long-term results.
