Six-Minute Walk Test in Patients with Cardiac Amyloidosis: Does it mean anything?

Isabel Carvajal et al. Luis Caballero Hortiales Valeria Rojo Galicia Denka Durán Aguilar Erick Alexanderson

Cardiac amyloidosis (CA) is a restrictive cardiomyopathy characterized by the extracellular deposition of proteins in the myocardium. Transthyretin (TTR) protein is predominantly synthesized in the liver and normally transports thyroxine and retinol. When TTR is misfolded, it can form amyloid fibrils deposited in different organs, such as the heart.

TTR amyloidosis (ATTR) can be inherited as an autosomal dominant trait caused by variants in the TTR gene (TTRv), or by the deposition of wild-type TTR (TTRwt).

Alternatives to Aerobic Exercise Prescription in Patients with Chronic Heart Failure

Mayron F Oliveira et al. Gabriela Zanussi Bianca Sprovieri Denise M. L. Lobo … Luiz E Mastrocolla Iracema I. K. Umeda Priscila A Sperandio

Background:

Exercise is essential for patients with heart failure as it leads to a reduction in morbidity and mortality as well as improved functional capacity and oxygen uptake (v̇O2). However, the need for an experienced physiologist and the cost of the exam may render the cardiopulmonary exercise test (CPET) unfeasible. Thus, the six-minute walk test (6MWT) and step test (ST) may be alternatives for exercise prescription.

Objective:

The aim was to correlate heart rate (HR) during the 6MWT and ST with HR at the anaerobic threshold (HRAT) and peak HR (HRP) obtained on the CPET.

Methods:

Eighty-three patients (58 ± 11 years) with heart failure (NYHA class II) were included and all subjects had optimized medication for at least 3 months. Evaluations involved CPET (v̇O2, HRAT, HRP), 6MWT (HR6MWT) and ST (HRST).

Results:

The participants exhibited severe ventricular dysfunction (ejection fraction: 31 ± 7%) and low peak v̇O2 (15.2 ± 3.1 mL.kg-1.min-1). HRP (113 ± 19 bpm) was higher than HRAT (92 ± 14 bpm; p < 0.05) and HR6MWT (94 ± 13 bpm; p < 0.05). No significant difference was found between HRP and HRST. Moreover, a strong correlation was found between HRAT and HR6MWT (r = 0.81; p < 0.0001), and between HRP and HRST (r = 0.89; p < 0.0001).

Conclusion:

These findings suggest that, in the absence of CPET, exercise prescription can be performed by use of 6MWT and ST, based on HR6MWT and HRST

Prognostic value of the six-minute walk test in heart failure

Valéria Siqueira Martins Rubim et al. Cantídio Drumond Neto José Luiz Martins Romeo Marcelo Westerlund Montera

OBJECTIVE: This study aimed to assess the true usefulness of the Six-Minute Walk Test as a prognostic indicator and its contribution to clinical practice with heart failure patients. METHODS: In order to investigate the actual value of the Six-Minute Walk Test as an objective measure of mortality probability in patients with heart failure, the test was applied to 179 stable patients (120 men and 59 women, mean age 58.32 ± 12.7 years, with NYHA class II and III heart failure and an ejection fraction (LVEF) of 34.91 ± 12.4%). Patients were instructed to walk for 6 minutes and then, four hours later, underwent a conventional exercise stress test (as per Naughton Protocol). Patients were followed for an average of eighteen months. RESULTS: The average distance walked was 521.11 ± 76.1 meters. During the follow-up period, 66 patients (36.9%) died. There was a significant correlation between the distance walked during the test and mortality (p < 0.0001). The logistic regression model identified the distance walked during the test as the most important independent predictor of mortality (p = 0.0001). A distance shorter than 520 meters identified the patients with an increased probability of death. There was a significant correlation between the number of metabolic equivalents (METs) measured during the conventional exercise stress testing and mortality rate (p = 0.0001). CONCLUSION: The Six-Minute Walk test is a simple, safe and powerful method to assess the prognosis of patients with NYHA class II and III heart failure. It is an objective examination that may replace the conventional ergometric test for the prognostic evaluation of these patients.

Diuretics improve functional capacity in patients with congestive heart failure

Francesca Tadeu Eterno et al. Múcio Tavares de Oliveira Jr Antonio Carlos Pereira Barretto

PURPOSE: The 6-9 minute walking test was used in this study to evaluate the impact of these drugs on functional capacity of patients admitted to the Hospital because of Heart Failure (CHF). METHODS: Ten patients (5 males and 5 females) with mean age of 47 years and NYHA CHF functional class III or IV underwent a 6-9 minute walking test at admission and on the day of discharge from the Hospital. The following parameters were evaluated both at admission and discharge: body weight, echocardiography-derived LV dimensions and function, plasmatic levels of sodium, potassium, BUN, creatinine, hemoglobin and hematocrit. Treatment consisted of increasing outpatient dose of furosemide (IV and/or PO) plus the association of thiazide if necessary. The previous dose regimen of digitalis, ACE inhibitors or the association nitrate and hydralazine was kept unchanged. RESULTS: Time to compensation of CHF varied from 4 to 30 days (mean 8.7±7.8 days). LV end diastolic dimension varied from 47 to 81mm. LV EF spanned from 0.26 to 0.74. The 6min walking distance improved from 193.4± 71.5m to 341.8±67.7m (p<0.00002) and the 9min walking distance from 268.1±119.6m to 518.0±114.8m (p<0.00005). Hemoglobin, hematocrit, BUN, creatinine and sodium levels were unchanged from admission to discharge, whereas plasma potassium level increased from 4.0±0.91mEq/l to 4.69±1.00mEq/l (p=0.01), and body weight was reduced from 58.9±6.42kg to 52.9±5.31kg (p<0.0006). CONCLUSION: Compensating CHF with diuretics leads to a significant improvement in physical capacity. This benefit is already evident in the in-hospital phase.
