Alternatives to Aerobic Exercise Prescription in Patients with Chronic Heart Failure

Mayron F Oliveira et al. Gabriela Zanussi Bianca Sprovieri Denise M. L. Lobo … Luiz E Mastrocolla Iracema I. K. Umeda Priscila A Sperandio

Background:

Exercise is essential for patients with heart failure as it leads to a reduction in morbidity and mortality as well as improved functional capacity and oxygen uptake (v̇O2). However, the need for an experienced physiologist and the cost of the exam may render the cardiopulmonary exercise test (CPET) unfeasible. Thus, the six-minute walk test (6MWT) and step test (ST) may be alternatives for exercise prescription.

Objective:

The aim was to correlate heart rate (HR) during the 6MWT and ST with HR at the anaerobic threshold (HRAT) and peak HR (HRP) obtained on the CPET.

Methods:

Eighty-three patients (58 ± 11 years) with heart failure (NYHA class II) were included and all subjects had optimized medication for at least 3 months. Evaluations involved CPET (v̇O2, HRAT, HRP), 6MWT (HR6MWT) and ST (HRST).

Results:

The participants exhibited severe ventricular dysfunction (ejection fraction: 31 ± 7%) and low peak v̇O2 (15.2 ± 3.1 mL.kg-1.min-1). HRP (113 ± 19 bpm) was higher than HRAT (92 ± 14 bpm; p < 0.05) and HR6MWT (94 ± 13 bpm; p < 0.05). No significant difference was found between HRP and HRST. Moreover, a strong correlation was found between HRAT and HR6MWT (r = 0.81; p < 0.0001), and between HRP and HRST (r = 0.89; p < 0.0001).

Conclusion:

These findings suggest that, in the absence of CPET, exercise prescription can be performed by use of 6MWT and ST, based on HR6MWT and HRST

Effects of the state and specificity of aerobic training on the %VO2max versus %HRmax ratio during cycling

Fabrizio Caputo et al. Camila Coelho Greco Benedito Sérgio Denadai

OBJECTIVE: To determine the effects of the status and specificity of exercise training in the ratio between maximum oxygen consumption (%VO2max) and the percentage of maximal heart rate (%HRmax) during incremental exercise on a cycle ergometer. METHODS: Seven runners, 9 cyclists, 11 triathletes, and 12 sedentary individuals, all male and apparently healthy, underwent exhaustive incremental exercise on cycle ergometers. Linear regressions between %VO2max x %HRmax were determined for each individual. Based on these regressions, %HRmax was assessed corresponding to a determined %VO2max (50, 60, 70, 80, and 90%) from each participant. RESULTS: Significant differences were not found between the groups in %HRmax for each of the %VO2max assessed. Analyzing the volunteers as a single group, the average of the corresponding %HRmax to 50, 60, 70, 80, and 90% %VO2max were 67, 73, 80, 87, and 93%, respectively. CONCLUSION: The ratio between %VO2max and %HRmax in the groups assessed during incremental exercise on the bicycle is not dependent on the status and specificity of aerobic exercise training.

Comparison between exercise intensity prescription based on a standard exercise test and cardiopulmonary exercise test

Maria Urbana Pinto Brandão Rondon et al. Cláudia Lúcia de Moraes Forjaz Newton Nunes Sandra Lia do Amaral … Antonio Carlos Pereira Barretto Carlos Eduardo Negrão

PURPOSE: To compare the lower (LL) and upper limits (UL) of exercise intensity prescription based on standard exercise test (60-70% of estimated VO2max or 70-85% of HRmax measured) with exercise intensity prescription based on cardiopulmonary exercise test [anaerobic threshold (AT) and respiratory compensation point (RCP)]. METHODS: Fourty seven men (30±5 years) who were submitted to a progressive cardiopulmonary exercise test until exhaustion were divided in subgroups according to treadmill speed during exercise test (4 or 5mph) and the physical capacity [lower (LPC) and moderate physical capacity (MPC)]. RESULTS: The LL of the indirect exercise intensity prescription showed VO2 and HR values significantly higher than VO2 and HR values measured at AT (4mph= 34.4±4.5 vs 19.6±4.6 and 5mph= 28.9±2 vs 18.9±5.4, and LPC= 32.0±4.1 vs 17.2±2.8 and MPC= 31.6±4.9 vs 21.1± 5.7mlO2.kg-1.min-1 ) and (4mph= 128.9±7.8 vs 113.1±15.6 and 5mph= 130.3±5.2 vs 114.1±18.9, and LPC= 127.6±7.2 vs 109.3±13.2 and MPC= 131.2±5.7 vs 117.4±19.2bpm) The UL of the indirect exercise intensity prescription in 4mph and LPC group showed VO2 values significantly higher than those measured at RCP (40.1±5.3 vs 32.2±4.3 and 37.4±4.8 vs 30.6±2.5mlO2.kg-1.min-1, respectively), but similar HR values to those obtained at RCP. CONCLUSION: The LL of prescription based on standard exercise test overestimate the AT, whereas the UL seem adequate only for subjects with moderate physical capacity.
