Precordial Pain and Infarction in the Elderly. It´S no so Elementary, My Dear Watson!

Ricardo Wang et al. José Carlos da Costa Zanon Fernando Carvalho Neuschwander

In Medical School, we learn the traditional way to conduct the investigative process in order to reach a diagnosis. This process is traditionally based on the collection of anamnesis and clinical exams, clinical reasoning, the generation of a hypothesis, and tests with complementary exams. In general, we follow the the Occam’s razor principle: “the simplest answer is usually the correct answer,” or as William Osler’s aphorism: “Where you hear hoofs, you don´t think of zebras”. However, in this investigative process, the clinical reasoning is challenged in an elderly population over 80 years of age, and not always is the simplest answer the most correct. In this age group, the presence of comorbidities is quite common and can interfere, changing the perception of cardiovascular symptoms (depression, dementia, medications that interfere in the central nervous system, diabetes, analgesics, etc.), or even modify the symptomatology. The presence of atypical symptoms, such as dyspnea, sudoresis, vomiting, diarrhea, epigastric pain, and mental confusion can all mask the cardiovascular pathology. ,

Many studies have shown the difficulty in diagnosing acute myocardial infarction (AMI) in this population, which clearly entails delays in or the lack of treatment. Even when the diagnosis and early treatment of AMI is established, the mortality rate remains high in this population. The under-treatment or lack of treatment contribute to the increase in mortality in this age group. – Understanding the factors that make the diagnosis difficult is of utmost importance in establishing protocols directed toward the older age elderly patient.

Challenging Diagnosis of Myocardial Infarction Due to Anomalous Left Circumflex Artery

Maksymilian P. Opolski et al. Kajetan Grodecki Mateusz Spiewak Mariusz Furmanek Ilona Michalowska

A 45-year-old male without past medical history presented with retrosternal chest pain and ST-segment elevation in inferolateral leads at ECG. Invasive coronary angiography, along with optical coherence tomography performed as part of the clinical study, showed normal coronaries, and myocardial infarction with non-obstructive coronary arteries (MINOCA) was diagnosed (-). Due to ongoing chest pain, triple-rule-out computed tomography angiography (CTA) was undertaken to exclude aortic dissection and pulmonary embolism. Incidentally, anomalous left circumflex artery (LCx) originating from the right sinus of Valsalva with a suspicion on severe stenosis was detected (-). Selective angiography of the LCx confirmed severe lesion in the distal vessel segment (), however given the resolution of patient’s symptoms, a decision on medical therapy with dual antiplatelet agents was undertaken. At discharge, cardiac magnetic resonance disclosed mildly reduced left ventricular ejection fraction (53%) with myocardial edema and transmural infarction of the basal-to-mid lateral wall (-).

Single Derivation Fragmented QRS Can Predict Poor Prognosis in Successfully Revascularized Acute STEMI Patients

Zulkif Tanriverdi et al. Huseyin Dursun Tugce Colluoglu Dayimi Kaya

Abstract

Background:

QRS fragmentation (fQRS) is classically defined as the presence of slurred QRS morphology in at least two contiguous leads, and its prognostic importance has been shown in ST elevation myocardial infarction (STEMI). However, no study has investigated the significance of single lead fQRS (sl-fQRS) in surface electrocardiography (ECG).

Objectives:

To evaluate whether sl-fQRS is as valuable as classical fQRS in patients with acute STEMI who had successful revascularization with primary percutaneous coronary intervention (pPCI).

Methods:

We included 330 patients with a first STEMI who had been successfully revascularized with pPCI. The patient’s electrocardiography was obtained in the first 48 hours, and the patients were divided into three groups according to the absence of fQRS (no-fQRS); fQRS presence in a single lead (sl-fQRS); and ≥2 leads with fQRS (classical fQRS).

Results:

In-hospital mortality was significantly higher both in patients with sl-fQRS and in patients with ≥ 2 leads with fQRS compared to patients with no-fQRS. In ROC curve analysis, ≥ 1 leads with fQRS yielded a sensitivity of 75% and specificity of 57.4% for the prediction of in-hospital mortality. Multivariate analysis showed that sl-fQRS is an independent predictor of in-hospital mortality (OR: 3.989, 95% CI: 1.237-12.869, p = 0.021).

Conclusions:

Although the concept of at least two derivations is mentioned for the classical definition of fQRS, our study showed that fQRS in only one lead is also associated with poor outcomes. Therefore, ≥1 leads with fQRS can be useful when describing the patients under high cardiac risk in acute STEMI.

Acute myocardial infarction diagnosis. The value of serum myoglobin levels, compared with creatine kinase and MB fraction

Alexandre Biasi Cavalcanti et al. Roberto Henrique Heinisch Evandro de Campos Albino João Nilson Zunino

PURPOSE: To compare the diagnostic performance of two traditional plasma markers of myocardial infarction (MI), creatine kinase (CK) and its MB fraction (CK-MB), with plasma myoglobin (Mgb) levels, for the diagnosis of MI. METHODS: From September of 94 to February of 95, in an observational, prospective, and non-controlled fashion, 64 patients admitted to a cardiology emergency room (ER), with non-traumatic chest pain were studied. Patients presenting with more than 6h after the onset of symptoms, muscular trauma, cardiopulmonary resuscitation and renal insufficiency were not included. Definitive MI diagnosis was established if the patient had at least two of the three classic MI findings: ischemic chest discomfort longer than 20min, electrocardiogram with necrosis (Q waves) and elevated CK and CK-MB levels. RESULTS: There were 18 patients with MI. The sensitivity for CK, CK-MB and Mgb were respectively 33%, 22% and 61%. The specificity was 85%, for CK ,95% for CK-MB and 97% for Mgb. The difference between Mgb and CK sensitivities was 28%, with a 95% confidence interval (CI) from -4% to 59%, the difference between Mgb and CK-MB sensitivities was 39%, (CI 9% to 69%). The difference between Mgb and CK especificities was 13%, (CI 12% to 14%) and the difference between Mgb and CK-MB specificities was 2%, (CI -5% to 9%). CONCLUSION: In the studied population, Mgb was more sensitive than CK-MB for the diagnosis of MI with similar specificity and Mgb was more specific than CK for this diagnosis.
