Premature Atrial Contractions Originating from the Inferior Vena Cava: A Rare Arrhythmogenic Source Amenable to Ablation

Ningning Zheng et al. Yiyuan Chen Feng Xue Fangfang Zhang … Lin Ling Tingbo Jiang

Introduction

Premature atrial contractions (PACs) are common cardiac arrhythmias that are typically associated with trigger mechanisms. PACs originating from pulmonary veins (PVs) are considered important triggers for atrial fibrillation (AF), and pulmonary vein isolation (PVI) has been widely used as a treatment. However, the inferior vena cava (IVC) is generally regarded as an electrically quiescent region, and studies of ectopic discharges originating from the IVC are relatively rare. In this study, we present a case of PACs originating from the IVC, aiming to provide new insights into the pathophysiological mechanisms of these arrhythmias.

Aneurysm of the Suprahepatic Inferior Vena Cava: A Case Report

Gabriella Ghattas Mariano et al. Calina Araujo Thaines Gabrielle Silva Desani Letícia de Castro Gouvêa … Paulo Vítor Cabral Covilo Rogério de Paula Garcia Caravante

Introduction

Aneurysms of the inferior vena cava (IVC) are a rare occurrence in the cardiovascular system and can present with a variety of signs and symptoms. In many cases, patients remain asymptomatic, which makes diagnosis difficult. Venous aneurysms are defined as an abnormal and persistent dilation of a vein in a specific area, with a diameter at least twice that considered normal. An IVC aneurysm is a specific type of venous aneurysm. This case report is relevant because this condition is rare; by 2021, only around 70 cases had been described in literature.

Aneurysms may arise due to fragility in the vessel wall and can be triggered by various factors, including hypertension, trauma, infection, and genetic conditions. Smoking, atherosclerosis, and chronic obstructive pulmonary disease are also considered risk factors for aneurysm development. Although they can occur in different regions of the body, they most frequently affect the cerebral arteries, the aorta, and the peripheral arteries.

My Approach to Differentiating Pericardial Effusion with and without Hemodynamic Repercussions

Helder Moura Gomes et al. Halsted Alarcão Gomes Pereira da Silva

Abstract

Pericardial effusion is a common finding in cardiology practice. It is frequently identified in outpatient and inpatient follow-up examinations, especially during the postoperative period of heart surgery. In clinically stable patients, proper assessment can allow for early detection of signs of clinical deterioration. In patients in shock, careful analysis of pericardial effusion can confirm or rule out this condition as the main cause of hemodynamic instability. Precise identification of the location, anatomical characterization of severity, and analysis of hemodynamic repercussions by means of Doppler are essential elements to guide medical management. In addition to technical assessment, precautions when describing findings in imaging reports are equally important. This is because the hemodynamic repercussions observed on echocardiography do not always correspond to the patient’s clinical severity.

Inferior Vena Cava Ultrasound for Assessing Volume Status and Fluid Responsiveness in Critically ill Patients: A Systematic Review

Djalma Gomes Xavier Filho et al. Akeme Laissa Novais Coutinho Romero Henrique de Almeida Barbosa Matheus Rodrigues Lopes Adirlene Pontes de Oliveira Tenório

Abstract

Background:

The assessment of intravascular volume and fluid responsiveness is challenging in the management of critically ill patients. Diagnostic methods of hemodynamic monitoring must be safe, reproducible, and practical.

Objective:

To describe the applicability of ultrasound indices of the inferior vena cava in the assessment of intravascular volume and prediction of fluid responsiveness in critically ill patients.

Method:

A systematic review performed of the PubMed®, Latin American and Caribbean Health Sciences Literature, and Scientific Electronic Library Online databases for articles published in the previous five years. The descriptors used were “inferior vena cava,” “ultrasonography,” “fluid responsiveness,” and “volume status.”

Results:

The search identified 13 relevant articles. The collapsibility index of the inferior vena cava was 25%–50% as the cutoff point for defining hypovolemia and showed applicability in predicting fluid responsiveness in patients breathing spontaneously with cutoff points of 25%–57%. In mechanical ventilation scenarios, the distensibility index of the inferior vena cava was more effective at predicting fluid responsiveness than the other measurements with variation of 10.2%–20.5%. The inferior vena cava/aorta diameter index was especially useful in the pediatric population for defining intravascular volume; however, in adults, there were many divergences in its applicability.

Conclusion:

The assessment of intravascular volume and fluid responsiveness through ultrasound indices of the inferior vena cava is applicable and safe for diagnosing and monitoring hemodynamic instability. However, studies of value standardization are necessary due to divergences in the cutoff points used in each index.
