Coronary calcium score as predictor of stenosis and events in pretransplant renal chronic failure

Miguel Abraão Rosário et al. José Jayme de Lima José R. Parga Luiz F. Ávila … Luis H. Gowdak Pedro A. Lemos Carlos E. Rochitte

BACKGROUND: Coronary artery disease (CAD) is the major cause of death among chronic renal failure (CRF) patients. Traditional, non-invasive exams to detect CAD and to predict events have shown insufficient results in this group. CT Scan evaluation of Coronary Calcium Score (CCS) has proven to be of prognostic value for the population reporting no renal condition. OBJECTIVE: To investigate CCS accuracy in detecting obstructive CAD and in predicting cardiovascular events in candidates to renal transplant as compared to quantitative invasive coronary angiography (ICA). METHODS: Ninety-seven (97) CRF patients aged > 35 were evaluated. Obstructive CAD was considered as >50% or >70% stenosis on ICA. Descriptive data, concordance, diagnostic tests, Kaplan-Meier, and multivariate analysis were used. RESULTS: Agatston mean score was 580.6 ± 1,102.2. Minimum and maximum values were 0 and 7,994, with median at 176. Only 14 patients had zero calcium score. No differences were reported in regard to ethnicity. Highest regional calcium was associated to the highest probability of coronary stenosis in the same segment. Agatston calcium score showed high accuracy for the diagnosis of >50% and >70% stenosis, with area under ROC curve (AUC) of 0.75 and 0.70, respectively. At the threshold of 400, calcium score identified a subgroup with a higher rate of cardiovascular events at an average follow-up time of 29±11.0 months. CONCLUSION: CCS proved to have good diagnostic and prognostic performance for cardiovascular events evaluation in CRF patients.

Left ventricular ejection fraction and volumes as measured by 3d echocardiography and ultrafast computed tomography

Marcelo Luiz Campos Vieira et al. César H. Nomura Bernardino Tranchesi Junior Wercules A. de Oliveira … Gustavo Naccarato Bruna S. Serpa Rodrigo B. D. Passos Marcelo B. G. Funari Cláudio H. Fischer Samira S. Morhy

BACKGROUND: Real-time three-dimensional echocargiography (RT-3D-Echo) and ultrafast computed tomography (CT) are two novel methods for the analysis of LV ejection fraction and volumes. OBJECTIVE: To compare LVEF and volume measurements as obtained using RT-3D-Echo and ultrafast CT. METHODS: Thirty nine consecutive patients (27 men, mean age of 57±12 years) were studied using RT-3D-Echo and 64-slice ultrafast CT. LVEF and LV volumes were analyzed. Statistical analysis: coefficient of correlation (r: Pearson), Bland-Altman analysis, linear regression analysis, 95% CI, p<0.05. RESULTS: RT-3D-Echo measurements: LVEF ranged from 56.1 to 78.6 (65.5±5.58)%; end-diastolic volume ranged from 49.6 to 178.2 (87±27.8) ml; end-systolic volume ranged from 11.4 to 78 (33.1±13.6) ml. CT scan measurements: LVEF ranged from 53 to 86% (67.8±7.78); end-diastolic volume ranged from 51 to 186 (106.5±30.3) ml; end-systolic volume ranged from 7 to 72 (35.5±13.4)ml. Correlations between RT-3D-Echo and CT were: LVEF (r: 0.7888, p<0.0001, 95% CI 0.6301 to 0.8843); end-diastolic volume (r: 0.7695, p<0.0001, 95% CI 0.5995 to 0.8730); end-systolic volume (r: 0.8119, p<0.0001, 95% CI 0.6673 to 0.8975). CONCLUSION: Good correlation between LVEF and ventricular volume parameters as measured by RT-3D-Echo and 64-slice ultrafast CT was found in the present case series.

Biological factors and overestimation of left ventricular ejection fraction by gated SPECT

Marco Antônio Condé de Oliveira et al. Paulo Schiavom Duarte Maria Margarita C. Gonzalez Valdir Ambrósio Moises … Gilberto Alonso Eduardo Vilaça Lima Paola Emanuela Smanio Luiz Roberto Fernandes Martins Carlos A. R. Oliveira Luiz Eduardo Mastrocolla

BACKGROUND: Some patients present an overestimated left ventricular ejection fraction (LVEF) on electrocardiogram-gated myocardial scintigraphy (gated SPECT). OBJECTIVE: To establish the relationship between biological factors and overestimated LVEF. METHODS: We selected 3838 patients who underwent gated SPECT between May 20, 2000 and September 16, 2005 with normal perfusion images and LVEF >50%. The following variables were analyzed: gender (29.4% females and 70.6% males), age (from 20 to 94 years – mean: 56 years), weight (from 33.5 to 150 kg – mean: 79.6 kg), height (from 138 to 220 cm – mean: 171 cm) and BMI (from 13.9 to 54 – mean: 27.2). In a subgroup of 1002 patients who underwent echocardiogram, the diastolic diameter (from 36 to 68 mm – mean 47.5 mm) and systolic diameter (from 22 to 41 mm – mean 29.8 mm) variables were included. The patients were divided into two groups: normal LVEF (80%). The odds ratio (OR) for presenting an overestimated LVEF was calculated for each variable using logistic regression. RESULTS: The following odds ratios were found (p 0.2). In the subgroup of 1002 patients, a statistically significant influence was found in overestimated LVEF values for the systolic diameter, gender and height variables. CONCLUSION: Although systolic diameter influences the overestimation of LVEF, the gender and height variables have an independent influence on LVEF overestimation by gated SPECT.

Gender differences in ventricular volumes and left ventricle ejection fraction estimated by myocardial perfusion imaging: comparison of Quantitative Gated SPECT (QGS) and Segami software programs

Alice Tatsuko Yamada et al. Guilherme de Carvalho Campos Neto José Soares Júnior Maria Clementina P. Giorgi … Fernando Araújo José Claudio Meneghetti Alfredo José Mansur

OBJECTIVES: To test for gender differences in the measurements obtained by Segami and Quantitative Gated SPECT (QGS) software programs. METHODS: 181 asymptomatic individuals without heart disease were submitted to myocardial perfusion imaging. End-diastolic volumes (EDV), end-systolic volumes (ESV) and left ventricular ejection fraction (LVEF) were measured by QGS and Segami software programs to evaluate the influence of gender, age, weight, height, heart rate, systolic blood pressure, diastolic blood pressure, body mass index and body surface area. RESULTS: The means in the QGS method were: EDV (women= 68 ml; men= 95 ml; p<0.001), LVEF (women= 66.24%; men= 58, 7%) and Segami: EDV (women= 137 ml; men= 174 ml), LVEF (women= 62.67%; men= 58, 52%). There were significant differences between men and women in the EDV (p<0.001), ESV (p<0.001) and LVEF (p=0.001) that persisted after adjusting for body surface area. CONCLUSION: Ventricular volumes were significantly lower and LVEF was significantly higher in women, estimated by QGS or Segami software programs.
