Accuracy of the Simplified Version of the Global Risk Score in Detecting Cardiovascular Risk in Women from Quilombola Communities in the State of Alagoas, Brazil

Andressa Lima Cavalcante et al. Haroldo da Silva Ferreira

Abstract

Background:

Cardiovascular risk (CVR) monitoring is important for defining preventive actions against cardiovascular disease; this condition prevails more intensely in scenarios with less infrastructure such as African descent communities. The Framingham Risk Score (FRS) and the Global Risk Score (GRS) have been used in Brazil for CVR monitoring based on scales of points for certain risk factors. Among these, hypercholesterolemia and low high-density lipoprotein cholesterol require tests not always available in primary care. An alternative would be the simplified GRS (sGRS), in which these tests are replaced by the body mass index (kg/m2).

Objective:

To determine the accuracy of the sGRS in estimating CVR in African descent women (quilombolas) from Alagoas.

Method:

This is a cross-sectional study with a representative sample (n=1015) of women from African descent communities in Alagoas. GRS, sGRS, and FRS consisted in the sum of points obtained according to their respective scales. Receiver operating characteristic curves were used to compare the accuracy of these instruments as CVR predictors, assuming the GRS as reference. Statistical significance was assumed when p<0.05.

Results:

The prevalence of high CVR assessed with the GRS or sGRS was similar (20.1% vs. 20.7%; p>0.05) and higher than that found with the FRS (4.5%; p<0.001). Considering the area under the curve (AUC), the sGRS had a higher discriminatory power (AUC=0.98; 95%CI: 0.98–0.99) than the FRS (AUC=0.91; 95%CI: 0.90–0.93).

Conclusion:

Among black women living in regions with less infrastructure, the sGRS produced similar results to the GRS, with greater operational simplicity.

Drug Profile and Therapeutic Adherence of African-Brazilians with Apparent Resistant Hypertension

Pedro Henrique Andrade Araújo Salvatore Barletta et al. Júlia Lasserre Moreira Vitor Fernandes de Almeida Mateus Andrade Bomfim Machado … Breno Lima de Almeida Tayla Samanta Silva dos Santos Yana Mendonça Nascimento Thaise Almeida Silva Roque Aras Cristiano Macedo

Abstract

Background

Resistant hypertension (RH) is manifested by the presence of blood pressure values resistant to antihypertensive therapy. RH is highly prevalent among black individuals, increasing cardiovascular risk in this population and requiring effective control of this comorbidity.

Objectives

To investigate the medication profile and therapeutic adherence in black people with apparent RH.

Methods

This is a cross-sectional study, with a convenience sample of individuals with apparent RH. Data were obtained from medical records. Therapeutic adherence was assessed using the Morisky Therapeutic Adherence Scale of 8 items (MMAS-8) and statistical analysis was performed using the SPSS, version 23. Significance was set at p <0.05.

Results

Of the 120 individuals, 90 (75%) were women and 72 (60%) were black. Mean SBP was 153.09 (SD 25.59) mm Hg and mean DBP, 90.82 (SD 16.91) mm Hg, with a statistical difference in relation to the target pressure for SBP. Regarding the medication profile, 79.2% of the individuals used the recommended regimen for RH (ACEI / ARB + Diuretic + CCB), with the fourth most used drug being beta-blockers. The average score in MMAS-8 was 6.62 (SD 1.38) points, with 19.2%, 50.0%, and 30.8% showing low, medium, and high adherence, respectively.

Conclusions

It was evidenced that two-thirds of the individuals did not have high therapeutic adherence and not all used the ideal regimen for the management of RH, nor full doses. Thus, most individuals were probably affected by pseudoresistance, which was initially diagnosed as apparent RH. (Int J Cardiovasc Sci. 2021; [online].ahead print, PP.0-0)

Prevalence of Systemic Arterial Hypertension in Quilombola Communities, State of Sergipe, Brazil

Deyse Mirelle Souza Santos et al. Beatriz Santana Prado Cristiane Costa da Cunha Oliveira Marcos Antonio Almeida-Santos

Abstract

Background:

The quilombolas are groups formed by black ancestry individuals, living in a context of social vulnerability due to low socioeconomic level, which influences health care and the development of chronic diseases.

Objective:

To assess the prevalence of systemic arterial hypertension and its association with cardiovascular risk factors in the quilombola population in the State of Sergipe, Brazil.

Methods:

Study design was cross sectional, involving the administration of a questionnaire to individuals aged ≥ 18 years, in 15 quilombola communities of the State of Sergipe, Brazil. A value of two-sided p < 0.05 was considered statistically significant.

Results:

sA total of 390 individuals were evaluated, 72.3% of whom were women, with a mean age of 44.7 years. The prevalence of hypertension was 26% (with a confidence interval of 95% [95% CI]: 22-30), with no significant sex-related differences. The age was associated with arterial hypertension (95% CI: 1.03-1.06), systolic (95% CI: 1.04-1.07) and diastolic (IC 95%: 1.01-1.04) arterial hypertension. The level of body mass index was associated with arterial hypertension (95% CI: 1.00-1.11) and diastolic arterial hypertension (95% CI: 1.03-1.17). Economic class was associated with diastolic arterial hypertension (95% CI: 1.22-5.03).

Conclusion:

The prevalence of arterial hypertension in the quilombola communities was high. Its association with cardiovascular risk factors indicates the need to improve access to healthcare services.
