Abstract

Background:

The Venous Excess Ultrasound (VExUS) score has been proposed as an ultrasonographic method for assessing systemic venous congestion in patients with acute decompensated heart failure (ADHF). However, the relationship between VExUS and the biomarker N-terminal pro-B-type natriuretic peptide (NT-proBNP) remains unclear in literature.

Objectives:

To evaluate the correlation between the VExUS score and serum NT-proBNP levels in patients with ADHF.

Methods:

This retrospective observational study included 117 patients hospitalized with ADHF. Systemic venous congestion was assessed using the VExUS score, and serum NT-proBNP levels were obtained from laboratory records. Comparisons between groups were performed using the Kruskal-Wallis test, followed by Dunn’s test for multiple comparisons. The strength of association between variables was analyzed using Spearman’s correlation coefficient.

Results:

NT-proBNP levels increased progressively with increasing VExUS severity, with median values of 2,890 pg/mL (VExUS 0), 4,700 pg/mL (VExUS 1), 5,430 pg/mL (VExUS 2), and 13,200 pg/mL (VExUS 3). Statistical analysis demonstrated a significant difference between groups (Kruskal-Wallis: χ2 = 39.18; p < 0.0001). Dunn's test indicated that patients with VExUS 3 had significantly higher NT-proBNP levels compared with the other groups (p < 0.01). A moderate positive correlation was observed between the variables (Spearman's coefficient ρ = 0.567; p < 0.0001).

Conclusion:

The results indicate that the VExUS score is associated with NT-proBNP levels and may be integrated into clinical reasoning when assessing venous congestion in patients with ADHF.

Assessment of Interrater Reliability in Point-of-Care Ultrasound for Assessing Congestion in Cardiovascular Intensive Care

Marina Petersen Saadi et al. Guilherme Pinheiro Machado Gustavo Paes Silvano João Pedro da Rosa Barbato … Renato Ferraz Almeida Fernando Luis Scolari Guilherme Heiden Telo Anderson Donelli da Silveira

Abstract

Background:

The assessment of congestion is critical for managing patients with cardiovascular conditions, including heart failure (HF). Traditional methods often lack sensitivity, whereas point-of-care ultrasound (POCUS) provides an objective bedside alternative.

Objective:

To evaluate the interrater reliability (IRR) of key POCUS variables used to assess hemodynamic, pulmonary, and venous congestion.

Methods:

This single-center, prospective study was conducted from January to June 2023 in a cardiovascular intensive care unit (CICU) in Brazil. Adult patients underwent standardized POCUS examinations. Three trained investigators independently assessed lung ultrasound (LUS) (B-lines), left ventricular filling pressures (LVFP) (E/A and E/e′ ratios), inferior vena cava (IVC) measurements, hepatic and portal vein Doppler flow, and modified Venous Excess Ultrasound (mVExUS) Score. IRR was analyzed using intraclass correlation coefficient (ICC).

Results:

A total of 23 patients were included, with a median age of 65 years, each undergoing three independent POCUS examinations (69 total assessments). LUS and IVC measurements showed excellent IRR (ICC 0.903). Hepatic and portal vein flows demonstrated good IRR (ICCs 0.808 and 0.796, respectively). mVExUS grading achieved the highest IRR (ICC 0.957). The E wave showed excellent IRR (ICC 0.934), while the A wave and e′ velocity had lower IRRs (0.512 and 0.399, respectively). E/e′ and E/A ratios demonstrated moderate-to-good IRR (ICCs 0.662 and 0.852, respectively). The median exam duration was 10 minutes.

Conclusion:

POCUS variables demonstrated high reproducibility, particularly for LUS and mVExUS. The reproducibility of LUS and mVExUS was higher than that of LVFP parameters, suggesting they may be more reliable than traditional measures such as E/e′. These findings support the use of POCUS for standardized assessment of congestion. Further studies are needed to validate its prognostic value.

Lung Ultrasound in Outpatients with Heart Failure

Marco Stephan Lofrano-Alves

Over the last decade, we have witnessed the widespread use of lung ultrasound (LU) in the assessment of pulmonary congestion. In addition to the well-known technique of auscultation of adventitious lung sounds, LU makes an important contribution by sharpening our vision to the diagnosis of pulmonary interstitial edema by quantification of B-lines. LU has emerged as a relevant and updated method, since it is a safe, low-cost, rapid and available at bedside, being an alternative to imaging methods associated with ionizing radiation, such as chest tomography and X-ray.

LU is a standardized tool in diagnosis and treatment monitoring today, and has been studied in different clinical scenarios of pulmonary congestion of cardiogenic origin.– In outpatients with heart failure and reduced ejection fraction (HFrEF), B-line counting by LU showed an 89% accuracy with a cutoff ≥ 15 B-lines. In another study with patients hospitalized for HFrEF, the risk of an adverse in-hospital event increased with the rising number of B-lines at admission, and the risk of HF hospitalization and all-cause death was greater in patients with a higher number of B-lines at discharge.

Pharmacological stent deployment in the left anterior descending artery: late event indicators

Marcelo de Freitas Santos et al. Sergio Gustavo Tarbine Costantino Ortiz Costantini Maria do Rocio Peixoto Oliveira … Marcos Henrique Bubna Luiz Cesar Guarita-Souza Chiu Yun Yu Braga Francisco de Paula Stella Costantino Roberto Costantini Luiz Antonio Rivetti

BACKGROUND: The efficacy of pharmacological stents in decreasing the incidence of cardiac events is not homogeneous for all lesions or patient subgroups. OBJECTIVE: 1) To evaluate the late clinical evolution of patients submitted to pharmacological stent implantation in atherosclerotic lesions of the left anterior descending artery; 2) to identify, among the clinical, angiographic and intravascular ultrasonographic characteristics, the ones predictive of cardiac event risk. METHODS: From May 2002 to August 2005, 205 patients were treated with 236 pharmacological stent implants, guided by the intravascular US (IVUS). RESULTS: After a mean follow-up period of 711 days, the rate of stent thrombosis was 0.48%, the same observed for acute myocardial infarction or revascularization surgery. The revascularization rate of the treated lesion was 7.31% and the general event rate was 10.24%. The event indicators, according to the multivariate analysis were the implant of more than one stent in the same artery, concentric lesions and the minimal intra-stent area measured by IVUS 3.88 mm² obtained in the small reference-diameter segments is an independent indicator of event-free evolution.
