Three-Dimensional Echocardiographic Assessment of the Right Ventricle: Why Should We Use It

Tiago R. Politi et al. Rodrigo B.M. Barretto João Cesar Nunes Sbano David Costa de Souza Le Bihan Wilson Mathias Jr.

Abstract

Right ventricular (RV) assessment using two-dimensional (2D) echocardiography has historically faced significant challenges due to the chamber’s complex and unique geometry and its thoracic orientation. In this context, three-dimensional (3D) echocardiography has emerged as a promising tool to overcome and illuminate these limitations, enabling accurate quantification of volumes and ejection fraction without relying on geometric assumptions. As a result, the routine incorporation of 3D echocardiography into RV evaluation may redefine diagnostic and prognostic paradigms, fostering a more precise and personalized approach in modern cardiology. To consolidate and highlight this technique, this review article explores the technical principles of 3D echocardiography for RV assessment, discusses its advantages over conventional 2D imaging, examines its validation against cardiac magnetic resonance (CMR), and reviews key clinical applications, including pulmonary hypertension, functional tricuspid regurgitation, congenital heart disease, and right-sided heart failure. Additionally, the article outlines current limitations of the technique, future perspectives, and practical recommendations based on contemporary literature.

Focusing on the Right Ventricle in PRKAG2 Syndrome

Luís Rocha Lopes

The PRKAG2 gene encodes the adenosine monophosphate-activated protein kinase (AMPK) gamma 2 regulatory subunit. AMPK has a central role in cellular energy homeostasis. Pathogenic variation in PRKAG2 causes an autosomal dominant syndrome comprising ventricular hypertrophy, supraventricular arrhythmias, electrocardiographic pre-excitation and conduction system abnormalities.– The genetic background of this syndromic presentation was discovered in 2001.. Histologically there is myocyte glycogen accumulation, and the typical pattern of hypertrophy is usually described as biventricular and concentric, similar to other metabolic phenocopies of hypertrophic cardiomyopathy (HCM), with systolic dysfunction as another possible “red-flag” feature. The more frequent pathogenic variants are p.Arg302Gln and p.Asn488Ile. Due to the condition’s rarity, most publications consist of small case series or case reports, with few exceptions.. None of the previous publications specifically report right ventricular imaging findings in this condition.

Expanding on their previous work, where 3D echocardiography and strain imaging findings of the left ventricle were described in a cohort of 30 patients with PRKAG2 syndrome, Pena et al. provide a short report focusing on the right ventricle (RV), using the same cohort, in the current edition of this Journal.

Role of Echocardiography in Assessment of Patients with Suspected Ph: Beyond the Peak Tricuspid Regurgitant Velocity

Viviane Tiemi Hotta

Pulmonary hypertension (PH) causes great clinical and functional limitations and significantly impacts quality of life and survival., Current pharmacological treatment options for PH include phosphodiesterase-5 inhibitors, endothelin receptor antagonists, and prostanoids. However, an early diagnosis and etiological definition are essential to improve prognosis.

Transthoracic echocardiography (TTE) is the first-line test for suspected PH, as it is widely available, noninvasive, relatively low cost, does not involve radiation exposure, and carries minimal risk to patients. However, TTE can only suggest the diagnosis of PH since the definitive diagnosis requires invasive hemodynamic measurements via right heart catheterization.

Echocardiographic Correlation Between Right Ventricular Diastolic Function and Age and Gender in Subjects With Preserved Systolic Function

Abstract

Introduction:

Right ventricular (RV) systolic and diastolic functions influence the outcomes of cardiovascular diseases. However, right chamber size and function have not been uniformly assessed by age and sex.

Objective:

To evaluate RV diastolic function by age and sex in an outpatient population with preserved left ventricular (LV) and RV systolic function using transthoracic echocardiography.

Method:

This observational cross-sectional study included a total of 97 patients (56 women) aged 61.5 ± 12.5 years. The assessed parameters included RV E/A ratio; peak E-wave and A-wave velocity, E-wave acceleration and deceleration time, and integral E-wave and A-wave velocity-time; and pulmonary artery systolic pressure (PASP), tricuspid reflux velocity, inferior vena cava diameter, and right atrial volume.

Results:

A reduced E-wave was observed with increasing age (Pearson’s correlation coefficient [PCC], −.30; p = 0.003). The same was observed for the E/A ratio (PCC, −0.21; p = 0.035). There was a significant positive association between age and PASP results (PCC, 0.40; p = 0.004) and tricuspid reflux velocity (PCC, 0.36; p = 0.008). There was no significant intersex difference in echocardiography variables.

Conclusion:

The present study showed the impact of age on RV diastolic function indices, which remained within the normal range, in subjects with preserved LV and RV function. The study did not evidence a significant difference between genders in relation to the diastolic function of the right ventricle.
