Is Spirituality a New or Renewed Tool?

Mario Borba

Editorial referring to the article: Religiosity and Spirituality: The Relationship Between Psychosocial Factors and Cardiovascular Health

The topic of spirituality and religiosity has become more prevalent in scientific publications and clinical practice. Researchers recognize that studying this area can lead to a better understanding of physiology and potential therapeutic interventions. There are even discussions about establishing a prognosis based on spirituality and religiosity, and we are finding evidence related to this topic.

Sergio Timerman et al. Helio Penna Guimarães Carlos Eduardo Rochitte Thatiane Facholi Polastri Marcelo Antônio Cartaxo Queiroga Lopes

Abstract

The term Chain of Survival provides a useful metaphor for the elements involved in the COVID-19 management. The 5 links of the COVID-19 Chain of Survival are:

A strong Chain of Survival can improve the chances of survival and recovery for victims of COVID-19.

Telemedicine and the Doctor/Patient Relationship

Protásio Lemos da Luz

Introduction

A.C., a friendly Italian man, received coronary stents 8 years ago and had been feeling well until 3 months ago. He then had progressive breathing difficulty with tightness in his chest. He had loss of appetite and lost 4 kg in that period, but continued to work with good disposition. He had been to two hospital visits, in which he was informed that his lung was normal, based on X-ray. They had not reached final diagnosis, so he came to the office. His general appearance was normal; no abnormality was found on visual inspection. He had normal vital signs; no pallor to suggest anemia. The lungs, heart, abdomen and lower limbs were normal on examination, and resting ECG revealed no abnormalities either. As I palpated his neck, I noticed a mass on the left side. Subsequent examinations proved that a tumor compressed the trachea, which explained his respiratory difficulty. This is a typical example of a case in which only complete clinical examination leads to diagnosis. Telemedicine (TM) would not allow this, as simple inspection did not give any clues, and the clinical history suggested several possibilities, such as heart disease, pulmonary disease, diabetic decompensation, uremia or anemia.

There is now a major debate, both at the health system and at the individual telemedicine level. Telecommunication technologies have long been used in teaching, in science, in the formation of groups for the study of diseases, in the transmission of diagnostic or therapeutic medical procedures, in imaging interpretation by specialists, in teleconference with undisputed efficiency, and will not be considered here. An interesting fact: many even said that, with the Internet, medical conferences would be emptied, because everything could be seen from a distance.

Competitive sports for children and adolescents: should an electrocardiogram be required in the pre-participation physical examination?

Cláudio Aparício Silva Baptista et al. Antonio Foronda Luciana de Pádua Silva Baptista

The growing number of children and adolescents, aged 7 to 17 years, that participate in competitive sports requires preventive medical care. The pre-participation physical examination (PPE) requires appropriate medical knowledge to insure safe medical clearance. Recent sudden death events related to sports practice have raised doubts concerning the need for a medical evaluation based on medical tests, which due to the delay in its implementation may result in demotivation and abandonment of the sports practice. This is a review study, including data collected during a period of 30 years at the Olympic Training and Research Center (COTP) of the Municipal Secretary of Sports of São Paulo, where future athletes are identified, socially included and trained; and the objective of the study was to evaluate the need for the involvement of medical organizations in the preparation of a EPP protocol for the cardiovascular assessment of this population, according to the Brazilian reality. We had no normative standard, and so we relied on data collected from protocols that were established by other countries, but we defined which conduct to be taken with each of our individuals.

Medical care and deaths due to coronary artery disease in Brazil, 1980-1999

Ines Lessa

OBJECTIVE: To estimate the frequency of medical care preceding deaths due to coronary artery diseases (CAD) in different Brazilian regions and capitals and to describe trends in medical care from 1980 to 1999. METHODS: Information on medical care preceding deaths due to coronary artery diseases/acute myocardial infarction in adults > 20 years from 1980 to 1999 was collected in the DATASUS, the databank of the Brazilian Health Ministry. Sex, states, and capitals selected for 1999 were analyzed in the study. Medical care was stratified as follows: with, without, and ignored medical care. The descriptive analysis comprised frequencies, ratios of frequency, test for proportions, and increments or reductions in frequencies. RESULTS: Acute myocardial infarction (AMI) represented 75 to 85% of the CAD in the period; the frequency of deaths with medical care ranged from 48.9 to 63%, and that of ignored medical care ranged from 27.2 to 41.5%. The frequency of other CAD with medical care ranged from 56 to 76%. The frequency of deaths preceded by medical care decreased by 17.8%, and that with ignored medical care increased by 36.5% (RF=2). The values for the other CAD were -20.2% and +64.6% (RF=44.4). Deaths preceded by medical care were more frequent in females at all ages and in all Brazilian regions. CONCLUSION: The results show a high frequency of sudden death and suggest errors in diagnosis or codification and overestimation of the statistics about mortality. Validation of the death certificate diagnosis and frequent surveillance are required.
