Isolated Fungal Pulmonary Endocarditis: Role of Transthoracic Echocardiography from the Diagnosis to Follow-up

Eduardo Gatti Pianca et al. Marcelo Nicola Branchi Bernardo Mastella Álvaro Schmidt Albrecht … Orlando Carlos Belmonte Wender Murilo Foppa Angela Barreto Santiago Santos

Introduction

Pulmonary valve endocarditis, an uncommon entity, is associated with important complications such as pulmonary emboli, severe valvular regurgitation, and right-sided chamber dilatation and is usually concomitant with tricuspid valve endocarditis. Intravenous (IV) drug and cardiovascular implantable electronic device use are known risk factors for this condition. Staphylococcus aureus is the most prevalent causative bacteria, whereas fungi are rarely involved. Here, we present a case of Candida albicans–induced pulmonary valve endocarditis.

Relationship Between Neck Circumference and Epicardial Fat Thickness in a Healthy Male Population

Uğur Küçük et al. Hilal Olgun Küçük Ferhat Cüce Sevket Balta

Abstract

Background:

Epicardial fat is an upper body visceral fat depot that may play a significant role in the development of adverse metabolic and cardiovascular risk profiles. There is a significant direct relationship between the amount of epicardial fat and general body adiposity (body mass index, BMI), but data regarding subcutaneous adiposity is limited.

Objective:

We conducted a study to determine the association between neck circumference and epicardial fat thickness in healthy young male individuals, and assess their individual correlations with general body adiposity and cardiometabolic risk factors.

Methods:

One hundred consecutive male patients aged 18 years or older with no known major medical conditions were included in the study. All participants underwent detailed physical examination including measurement of blood pressure, weight, height, waist/hip ratio, and neck circumference. Blood was collected to determine fasting glucose and lipid parameters. A standard echocardiographic examination was performed with additional epicardial fat thickness determination.

Results:

Among 100 study participants, neck circumference correlated significantly with weight, waist circumference, BMI, blood glucose, serum total cholesterol, low-density (LDL)-cholesterol, and triglycerides levels. No significant correlation was found between neck circumference and high-density lipoprotein (HDL)-cholesterol levels. Neck circumference correlated moderately and positively with echocardiographic epicardial fat thickness.

Conclusion:

Among patients with low cardiometabolic risk, increased neck circumference was associated with increased epicardial fat thickness.

Echocardiographic Parameters and Survival in Chagas Heart Disease with Severe Systolic Dysfunction

Daniela do Carmo Rassi et al. Marcelo Luiz Campos Vieira Ana Lúcia Martins Arruda Viviane Tiemi Hotta … Rogério Gomes Furtado Danilo Teixeira Rassi Salvador Rassi

Background:

Echocardiography provides important information on the cardiac evaluation of patients with heart failure. The identification of echocardiographic parameters in severe Chagas heart disease would help implement treatment and assess prognosis.

Objective:

To correlate echocardiographic parameters with the endpoint cardiovascular mortality in patients with ejection fraction < 35%.

Methods:

Study with retrospective analysis of pre-specified echocardiographic parameters prospectively collected from 60 patients included in the Multicenter Randomized Trial of Cell Therapy in Patients with Heart Diseases (Estudo Multicêntrico Randomizado de Terapia Celular em Cardiopatias) – Chagas heart disease arm. The following parameters were collected: left ventricular systolic and diastolic diameters and volumes; ejection fraction; left atrial diameter; left atrial volume; indexed left atrial volume; systolic pulmonary artery pressure; integral of the aortic flow velocity; myocardial performance index; rate of increase of left ventricular pressure; isovolumic relaxation time; E, A, Em, Am and Sm wave velocities; E wave deceleration time; E/A and E/Em ratios; and mitral regurgitation.

Results:

In the mean 24.18-month follow-up, 27 patients died. The mean ejection fraction was 26.6 ± 5.34%. In the multivariate analysis, the parameters ejection fraction (HR = 1.114; p = 0.3704), indexed left atrial volume (HR = 1.033; p 70.71 mL/m2 were associated with a significant increase in mortality (log rank p < 0.0001).

Conclusion:

The indexed left atrial volume was the only independent predictor of mortality in this population of Chagasic patients with severe systolic dysfunction.

The echocardiography in the cardiovascular laboratory: a guide to research with animals

Maria Cristina Donadio Abduch et al. Renato Samy Assad Wilson Mathias Jr Vera Demarchi Aiello

The feasibility and potential for the morphological and hemodynamic investigation of the heart has been increasing the use of the echocardiography in the research setting. Additionally, the development of new technologies, like the real time 3D echocardiography and speckle tracking, demands validation throughout experimental studies before being instituted in the clinical setting.

This paper aims to provide information concerning the particularities of the echocardiographic examination in quadruped mammals, targeting the experimental research.
