Epidemiological Profile of Arterial Hypertension in the Municipality of Rio de Janeiro, Brazil, 2011 and 2019

Catia Martins Cristina Oliveira et al. Anna Carolina Pinho da Silva Bianca Ramos Marins Silva Ana Clara Campos

Abstract

Background:

Cardiovascular diseases are currently the leading global cause of morbidity and premature mortality. High blood pressure is one of the precursors to the disease, which, when undetected, can lead to other serious complications such as thrombosis, acute myocardial infarction, and stroke.

Objective:

To analyze the profile of hypertension and associated factors in the municipality of Rio de Janeiro in the years 2011 and 2019.

Method:

This is a population-based cross-sectional study that used data from the Vigitel telephone survey implemented by the Ministry of Health. The eligible population was that of the municipality of Rio de Janeiro, aged eighteen or older, in 2011 and 2019. To analyze the association between the outcome and the independent variables, multivariate logistic regression was used, estimating the Odds Ratio and its respective 95% confidence intervals.

Results:

The predominant sociodemographic profile remained similar in 2011 and 2019: a majority of women, 18-39 years, and with 9-12 years of schooling. Education level in 2011 appeared as a protective factor among individuals with 12 or more years of schooling (OR: 0.64, 95% CI: 0.43; 0.97). Obese individuals had a 2.81- and 2.96-times greater chance, in 2011 and 2019, respectively, of reporting hypertension, compared to non-obese individuals.

Conclusion:

The study highlights the need to expand the healthcare network to implement more effective strategies that consider the determinants of hypertension in order to achieve a greater impact on the management and control of the disease.

The Era of Personalized Medicine in Cardiomyopathies

Diane Xavier de Avila et al. Evandro Tinoco Mesquita

Learning about cardiomyopathies has become more in-depth in recent years. Thus, several views of specialists from Brazil are present in this edition of ABC Heart Failure & Cardiomyopathy, of the ABC Family of Arquivos Brasileiros de Cardiologia (ABC Cardiol), from the most common etiologies to the rarest and ultra-rare ones, with their systemic repercussions, including toxic, genetic, peripartum, stress, diabetes, collagenosis, cardiogenetic interaction, as well as a vision of the future with artificial intelligence and 3D printing in cardiomyopathies.

Advances in imaging such as coronary computed tomography angiography in the non-invasive diagnosis of coronary artery disease, magnetic resonance imaging and its various specific evaluations of cardiomyopathies, nuclear medicine in microvascular disease and non-invasive diagnosis of transthyretin amyloidosis, and genetic tests have facilitated the recognition of different phenotypes. Moreover, based on the genotype of a given case, an entire family can also be identified related to preexisting clinical syndromes. Interaction between specialties and heart teams have improved care, disease management, and assistance of affected families. Accordingly, follow-up with a geneticist and genetic counseling are imperative in the era of personalized medicine.

The Evolving Landscape of the Geriatric Cardiology Field in Brazil: New Challenges for a New World

Caio de Assis Moura Tavares et al. André Feitosa Wanderley Cavalcanti Wilson Jacob Filho

The essence of geriatric cardiology

Because it is a recent and not widespread specialty, Geriatric Cardiology is often confused with Geriatrics that is practiced by a cardiologist or simply with Cardiology that is applied to elderly patients. Although this is part of the discipline’s core, it does not represent the whole picture. It would be more adequate to define it as integrated and age-adequate cardiovascular care, centered on the patients and their functionalities – a concept previously presented by our colleagues,– that has been evolving and taking shape, as a result of the use of specific and predetermined tools.

Objectively, one could consider it as cardiology practice integrated with the Geriatric’s 5 Ms: medication (focusing on prescribing the absolutely necessary, targeting at reducing polypharmacy, minimizing interactions and adverse reactions; following Beers criteria to select appropriate medication for the elderly), mentation (vigilance, prevention and treatment of cognitive disturbances), mobility (valuing and implementing strategies that seek to maintain the patient’s mechanical functionality), multimorbidity (approaching the patient not only by looking up the cardiovascular system, but also by considering the occurrence of multiple comorbidities to be the rule, not the exception, in these individuals), and last, but literally not least is matters most (always consider the patient’s opinion regarding the benefits and burdens of the treatment, taking into account the biography and personal values, bringing the patient into the center of decision making). We would also add a sixth and last M, multidisciplinary, for the care of the elderly, remembering it must be coordinated in a horizontal fashion by a professional, but never concentrated in only one person, giving due importance to the participation of other specialists and healthcare professionals. At our institute, in the Geriatric Cardiology Unit, we perform in all patients a Comprehensive Geriatric Assessment with the aid of the 10-TaGA (10-Minute Targeted Geriatric Assessment) tool.,

Positions, Guidelines and Standardizations. Vehicles of Assistance to Medical Practice

Antônio Carlos Sobral Sousa et al. Claudio Pereira da Cunha Lucélia Batista Neves Cunha Magalhães Sergio Emanuel Kaiser José Francisco Kerr Saraiva

Positioning document

These documents aim to address a particular topic (diagnostic, therapeutic or laboratory) of recognized clinical interest, for which there are no (or unlikely to exist) evidence of substantial quality or, especially, those from randomized clinical trials. These documents are complementary to the guidelines and are prepared by a team of professionals with established experience in the subject.

As an example, we could cite the use of direct anticoagulants in pregnant patients. In general, the guidelines contained in these documents are anchored in the best evidence available; however, often incorporate the personal opinion of specialists.
