Protocol For Weaning From Mechanical Ventilation in Patients Undergoing Coronary Artery Bypass Grafting: Cohort Study

André Luiz Lisboa Cordeiro et al. Wallace Fernando de Souza Lopes Tainara Conceição de Jesus Ana Julia Gomes de Oliveira … Ana Cecília de Oliveira Coutinho André Raimundo França Guimarães

Abstract

Background:

Patients with heart disease often require mechanical ventilation (MV). Although weaning protocols may reduce the duration of MV, the literature presents contradictory findings regarding their effectiveness, and there is a lack of studies specifically involving cardiac patients.

Objective:

To evaluate whether a weaning protocol reduces the duration of MV compared with weaning based on clinical judgment in cardiac patients.

Methods:

A retrospective cohort study was conducted with patients undergoing myocardial revascularization. They were randomly assigned to either the protocol-driven (intervention) group or the conventional weaning group. Weaning time, MV duration, and length of stay in the Intensive Care Unit (ICU) were assessed. Comparisons between groups were performed using the unpaired Student’s t-test. A p-value <0.05 was considered statistically significant.

Results:

A total of 214 patients were evaluated. The mean duration of MV was shorter in the intervention group (4 vs. 9 hours, p < 0.001). The mean ICU length of stay was 2 vs. 5 days in the intervention and control groups, respectively (p = 0.02).

Conclusion:

The findings suggest that protocol-guided weaning may be associated with reductions in weaning time, duration of MV, and ICU length of stay, raising the hypothesis of a potential clinical benefit that warrants further investigation in future studies.

Elements for Implementing Perioperative Optimization in Cardiac Surgery Based on Enhanced Recovery after Surgery

Marcelo Jamus Rodrigues et al. Andréa de Fátima Cristino Bastos Crespo Gabrielle Barbosa Borgomoni Fábio Antônio Serra de Lima Júnior … Paola Keese Montanhesi Luiz Fernando Falcão Valter Furlan Omar Asdrúbal Vilca Mejia

Introduction

The optimization of recovery after surgery, as proposed by the Enhanced Recovery After Surgery (ERAS) concept, involves perioperative care aimed at minimizing emotional and physiological stress while accelerating patients’ functional recovery.

ERAS utilizes intervention bundles that, collectively, decrease hospitalization time,– opioid use, postoperative complications,, and healthcare expenses.,

ABCDE Protocol in Stress Echocardiography

José Luis de Castro e Silva Pretto et al. Clarissa Borguezan Daros

A for Asynergy

This is based on the presumable myocardial contractility alteration induced by stress in the presence of flow-limiting coronary stenosis. The substrate supply-demand imbalance leads to a reduction in myocardial radial thickening, a parameter sometimes limited by its semi-quantitative and subjective nature. Operator dependency can be minimized (but not eliminated) by specialized training and the adoption of conservative reading measures, as well as the accreditation and quality control of advanced echocardiography laboratories.

The regional motility assessment indicates that the higher the number of affected segments (universally defined), the worse the prognosis. This parameter is viewed as stenosis centered and has its value limited outside the epicardial coronary disease context.

Evaluation of physicians' compliance with the hypertension protocol of the Municipal Health Department of the city of Curitiba

Ivan Maluf Jr et al. Mariana Ribas Zahdi Natasha Unterstell Eliane Mara Cesário Pereira Maluf … André Bonamigo de Sousa Felipe Dominici Loures

BACKGROUND: In Curitiba, systemic hypertension (SH) is the second leading cause of hospitalization and the leading cause of death from cardiovascular diseases. The protocols for the treatment of hypertension provide a systematic approach to patient management, aiming at improving the efficiency and quality of health services. OBJECTIVE: To evaluate medical professionals’ compliance with the protocol of hypertension of the Municipal Health Department (MHD) of the city of Curitiba. METHODS: This was a cross-sectional observational study. The data collection for the study was conducted in four health units in Curitiba. The sample consisted of 200 hypertensive patients enrolled in the hypertension program. The collected data refers to the first two consultations. The data source was the electronic records of the health units. The protocol used for comparative analysis was the protocol of the Municipal Health Department of Curitiba. RESULTS: The non-conformity percentage between clinical practice and the protocol in the first consultation was 56.8% on hypertension grade classification, 63.8% on cardiovascular risk evaluation, and 54% on treatment. In the second consultation, the non-conformity percentage was 67% on risk evaluation, and 51.3% on treatment. CONCLUSION: The non-conformity between clinical practice and the protocol of the MHD of Curitiba was evident on hypertension grade classification, cardiovascular risk evaluation, and treatment of hypertensive patients. This non-conformity may result in low efficiency of the health service, which hinders the efforts to reduce morbidity and mortality from cardiovascular disease in the population.

Methods to identify the anaerobic threshold for type-2 diabetic and non-diabetic subjects

Herbert G. Simões et al. Sérgio R. Moreira Robert J. Moffatt Carmen S. G. Campbell

BACKGROUND: In spite of Anaerobic Threshold (AT) to be widely used on exercise evaluation for different populations, there are few studies comparing methods to identify AT for individuals with type-2 diabetes. OBJECTIVE: To compare methods of AT determination on type-2 diabetics (T2D) and non-diabetic (ND) subjects and verify the acid-base balance as related to AT intensity. METHODS: T2D (n=10; 54.5±9.5 yr; 30.1±5.0 kg/m²) and younger ND (n=10; 36.6±12.8 yr; 23.9±5.0 kg/m²) performed an incremental test (IT) on a cycle ergometer. The over-proportional increase in VE/VO2 and blood lactate ([lac]) identified the ventilatory (VT) and lactate thresholds (LT) respectively. The workload corresponding to the lower blood glucose ([gluc]) during test identified the individual glucose threshold (IGT). The AT was also determined by polynomial adjustment of the VE/Workload and [lac]/Workload responses to identify exercise intensities above which an over-proportional increase in VE and [lac] did occur and were named VT VE/W and LT[lac]/W. RESULTS: The workload (Watts-W) corresponding to LT, VT, IGT, LT Lac/W and VT VE/W of diabetics (85.0±32.1; 88.0±31.7; 86.0±33.8; 82.0±20.9 and 90.2±22.2W) and non-diabetics (139.0±39.0; 133.0±42.7; 140.8±36.4; 122.7±44.3 and 133.0±39.1W) differed between groups (p<0.001), but not within groups. Thus it was evidenced an agreement among the studied methods. The pH and pCO2 were significantly decreased in parallel to the increase in [lac], pO2 and VE at supra AT intensities. CONCLUSION: The AT intensities, as determined by different methods both for diabetics and non-diabetic individuals, were in agreement to each other and identified exercise intensities above which the acid-basic balance is disrupted.
