Comparison of Cardiac Structural Changes After Surgical and Transcatheter Atrial Septal Defect Closure With Color Doppler Echocardiography

Tuğçe Akın et al. Zeynep Bilge Yılmaz Dere Yılmaz Yozgat Halil Türkoğlu Murat Ugurlucan

ABSTRACT

Background:

Surgical and transcatheter techniques represent the two principal approaches for atrial septal defect (ASD) closure. Although both are widely used, comparative evidence regarding their mid-term effects on cardiac remodeling and right ventricular (RV) function remains limited.

Objectives:

To compare mid-term cardiac structural remodeling and right ventricular functional recovery after surgical versus transcatheter ASD closure in pediatric patients using serial color Doppler echocardiographic assessment. Additionally, to determine whether either technique leads to faster or greater improvement in right heart morphology and function.

Methods:

We retrospectively evaluated 69 pediatric patients who underwent ASD closure at a single center. A total of 39 patients underwent surgical repair (Group 1), and 30 patients underwent transcatheter closure (Group 2). Transthoracic color Doppler echocardiography was performed before the procedure and at 3 and 12 months after intervention. Measures of atrial and ventricular morphology and function were analyzed.

Results:

At 3 months, the surgical group showed significantly greater improvement in right atrium (RA) major axis, RA volume, interventricular septal thickness in diastole, interventricular septal thickness in systole, and RV end-diastolic diameter (RVEDd) compared with the transcatheter group (all p < 0.05). At 12 months, surgical repair remained superior regarding improvement in RA major axis, RA volume, and RVEDd (all p < 0.05). Residual shunt was identified in only one patient in each group at 12 months.

Conclusions:

urgical ASD closure was associated with earlier and more consistent recovery of right atrial and ventricular geometry and function compared with transcatheter closure. These findings indicate that surgical closure may offer advantages for selected patients, particularly in relation to right heart remodeling during the first postoperative year.

Challenges and Implications of Uncorrected Atrioventricular Septal Defect in Adults: A Complex Case Report

Saulo Rodrigo Cunha et al. Rafael Felipe Minotto Fernanda Pandolfo Fernando Colares Barros … Pedro Tregnago Barcellos Ana Carolina Martins Mazzuca Eduardo Gatti Pianca

Case report

Male patient, 18 years old, admitted to a tertiary care hospital presenting with fever, cough, and dyspnea for the past seven days. According to the patient’s mother, he sought medical attention on the second day of symptoms at an urgent care clinic, where he was prescribed ibuprofen and azithromycin to take at home. Due to lack of improvement, he sought hospital care.

Medical history includes congenital heart disease, specifically a surgically uncorrected AVSD, as he had lost follow-up at the reference hospital since childhood. On physical exam, he appeared in fair condition, with a productive cough, rapid breathing, and cyanosis around the lips and extremities. Hemodynamically stable, with blood pressure of 125 × 86 mmHg, heart rate of 105 bpm in atrial fibrillation rhythm, and oxygen saturation of 69% on room air.

Lutembacher Syndrome Associated With Pulmonary Hypertension: The Importance of Early Diagnosis for Enabling Surgical Treatment

Ana Luiza Caldeira Lopes et al. Maria Estefânia Bosco Otto Bianca Corrêa Rocha de Mello Michelle Bruna da Silva Sena Wagner Luis Gali

Introduction

Lutembacher syndrome (LS) is defined by the simultaneous presence of an atrial septal defect (ASD) and mitral stenosis (MS). Although it is a rare condition, its clinical significance lies in the considerable hemodynamic impact and the potential for early progression to irreversible pulmonary hypertension (PH).

The exact prevalence of LS remains unknown. However, for estimation purposes, MS is considered the most frequent manifestation of rheumatic fever (RF) in young women, and the prevalence of RF in developing countries ranges from 5 to 10 cases per 1,000 children. Furthermore, the incidence of congenital ASD in patients with MS is estimated at 0.6% to 0.7%.

Classifying the Size of an Atrial Septal Defect According to Echocardiographic Parameters and its Association with the Clinical Presentation in Pediatrics

Lívia de Castro Ribeiro et al. Rose Mary Ferreira Lisboa da Silva Henrique de Assis Fonseca Tonelli Adriana Furletti Machado Guimarães … Fátima Derlene da Rocha Araújo Zilda Maria Alves Meira Sandra Regina Tolentino Castilho Alan Alvarez Conde Lícia Campos Valadares

Abstract

Background

Few studies are using linear echocardiographic measurements for classifying the size of Atrial Septal Defect (ASD).

Objectives

Investigate the relationship between ASD hemodynamic repercussions and ASD diameter to the mitral annulus diameter (MI/ASD) ratio and ASD diameter to the interatrial septum diameter (ASD/septum) ratio, and describe cutoff points to classify the size of the defect.

Methods

An observational, prospective, cross-sectional study, including subjects aged 1 month and 18 years diagnosed with isolated ASD. Hemodynamic repercussion was quantified by clinical evaluation, tricuspid annulus (TA) Z-score, subjective estimation of the enlargement of the right chambers, and the relationship between systemic and pulmonary flows (QP/QS) on echocardiography. Its association with ASD/septum and MI/ASD measurements was also studied. A value of p < 0.05 was considered statistically significant.

Results

Thirty-five subjects, mean age 6.3 years, 69% female, mean ASD 13.5 mm. There was a correlation between MI/ASD and the clinical classification of ASD (Pearson: -0.61; p < 0.001), ASD/septum (Pearson: -0.80; p < 0.001), and QP/QS (-0.76; p < 0.001). There was a correlation between ASD/septum and the clinical classification of ASD (Pearson: 0.56; p < 0.001), QP/QS (0.63; p = 0.001), subjective assessment of the right chambers (0.62; p < 0.001), and a weak correlation with the TA Z-score (0.35; p = 0.04). According to the operating characteristic curve for the stable variable classification of ASD according to the subjective right chamber’s size, an area of 0.85 was obtained for ASD/septum (p = 0.001). The cutoff point of 0.27 for large ASD showed sensitivity of 85%, specificity of 86.7%, positive predictive value (PPV) of 86%, negative predictive value (NPV) of 85.2%, and positive likelihood ratio of 6.39.

Conclusions

ASD/septum was associated with the hemodynamic repercussion of ASD and was useful in detecting large ASD.

A Rare and Fatal Association With SLE: Lutembacher Syndrome With Huge Atrial Septal Defect

Muhammed Fatih Kaleli et al. Yakup Alsancak Mustafa Celik Mehmet Akif Düzenli

Introduction

Systemic lupus erythematosus (SLE) is an autoimmune disorder characterized by periods of relapse and remission, with the potential to damage multiple organs depending on disease activity and individual susceptibility. Valvular heart disease is one of the cardiac manifestations associated with SLE. Lutembacher syndrome (LS) is an exceedingly rare clinical syndrome defined by the presence of a combination of congenital or acquired ostium secundum atrial septal defect (ASD) and congenital or acquired mitral stenosis (MS). First reported by Lutembacher in 1916, this syndrome is more frequently observed in young adults, with a higher prevalence among females. In this case report, we present the case of a young pregnant woman with SLE who visited the cardiology clinic complaining of palpitations. Eventually, she was diagnosed with LS.
