Cerebrovascular Disease Mortality Trend in Brazil (1996 To 2015) and Association with Human Development Index and Social Vulnerability

Carlos Dornels Freire de Souza et al. Denilson José de Oliveira Leonardo Feitosa da Silva Camila Damasceno dos Santos … Monaliza Coelho Pereira João Paulo Silva de Paiva Thiago Cavalcanti Leal Renato de Souza Mariano Amanda Karine Barros Ferreira de Araújo Jussara Almeida de Oliveira Baggio

Abstract

Background

Cerebrovascular diseases (CBVD) are the second major cause of death in the world.

Objective

To analyze the mortality trend of CBVD in Brazil (1996 to 2015) and its association with Human Development Index (HDI) and the Social Vulnerability Index (SVI).

Methods

This is an ecological study. We analyzed the mortality rate standardized by CBVD. Death data were obtained from the Mortality Information System (SIM) and populational data from the Brazilian Institute of Geography and Statistics (IBGE). The model of regression by inflection points (Joinpoint regression) was used to perform the temporal analysis, calculating the Annual Percent Change (APC) and Average Annual Percent Change (AAPC), with 95% of confidence interval and a significance of 5%. Trends were classified as increasing, decreasing or stationary. A multivariate regression model was used to analyze the association between mortality by CBVD, HDI and SVI.

Results

During this period, 1,850,811 deaths by CBVD were recorded. We observed a reduction in the national mortality rate (APC -2.4; p = 0.001). Twenty federation units showed a significant trend, of which 13 showed reduction, including all states in the Midwest (n=4), Southeast (n=4) and South (n=3). The HDI was positively associated and the SVI was negatively associated with mortality (p = 0.046 and p = 0.026, respectively).

Conclusion

An unequal epidemiological course of mortality was observed between the regions, being higher in the Southeast and South states, with a significative tendency of reduction, and lower in the North and Northeast states, but with a significative tendency of increase. HDI and SVI showed an association with mortality. (Arq Bras Cardiol. 2021; 116(1):89-99)

Prevalence of Dyslipidemias in Three Regions in Venezuela: The VEMSOLS Study Results

Juan P. González-Rivas et al. Ramfis Nieto-Martínez Imperia Brajkovich Eunice Ugel Alejandro Rísquez

Abstract

Background:

The prevalence of dyslipidemia in multiple regions of Venezuela is unknown. The Venezuelan Metabolic Syndrome, Obesity and Lifestyle Study (VEMSOLS) was undertaken to evaluate cardiometabolic risk factors in Venezuela.

Objective:

To determine the prevalence of dyslipidemia in five populations from three regions of Venezuela.

Methods:

During the years 2006 to 2010, 1320 subjects aged 20 years or older were selected by multistage stratified random sampling from all households in five municipalities from 3 regions of Venezuela: Lara State (Western region), Merida State (Andean region), and Capital District (Capital region). Anthropometric measurements and biochemical analysis were obtained from each participant. Dyslipidemia was defined according to the NCEP/ATPIII definitions.

Results:

Mean age was 44.8 ± 0.39 years and 68.5% were females. The prevalence of lipids abnormalities related to the metabolic syndrome (low HDL-c [58.6%; 95% CI 54.9 – 62.1] and elevated triglycerides [39.7%; 36.1 – 43.2]) were the most prevalent lipid alterations, followed by atherogenic dyslipidemia (25.9%; 22.7 – 29.1), elevated LDL-c (23.3%; 20.2 – 26.4), hypercholesterolemia (22.2%; 19.2 – 25.2), and mix dyslipidemia (8.9%; 6.8 – 11.0). Dyslipidemia was more prevalent with increasing body mass index.

Conclusion:

Dyslipidemias are prevalent cardiometabolic risk factors in Venezuela. Among these, a higher prevalence of low HDL is a condition also consistently reported in Latin America.

Mortality Trend Due to Cerebrovascular Accident in the City of Maringá, Paraná between the Years of 2005 to 2015

Jéssica Pizatto de Araújo et al. João Vinícius Valério Darcis Adriana Cunha Vargas Tomas Willian Augusto de Mello

Abstract

Background:

Stroke is the second largest cause of death in the world, responsible for 6.7 million deaths in 2012. In Brazil, among the leading causes of death, cerebrovascular diseases occupy the first place.

Objective:

to analyze the trend of deaths from cerebrovascular accident (CVA) in the city of Maringá, Paraná, Brazil, between the years of 2005 and 2015.

Methods:

Cross-sectional study of time series performed at the Municipal Health Department of Maringá. Secondary data on death certificates for stroke in the city of Maringá between 2005 and 2015, selected according to the specific MC for CID-10 presented with few variations in the time interval studied. The coefficient for ICD-10 – I69.4 was higher in 2011. The lowest coefficient for the whole period was I61.9. The trend of total mortality coefficients reached peak in the years 2007 and 2008, small variations occurred after this period until 2015. () (ICD-10) from I60 to I69 were used. For trend verification, the polynomial regression model was used. Mortality Coefficients (MC) by main cause of death were calculated using the total number of people who died of stroke divided by the population exposed to the risk, multiplied by one hundred thousand. The trends were analyzed using the Polynomial Regression model, considered significant when p < 0.05. Population data were obtained from the database of the Brazilian Institute of Geography and Statistics.

Results:

Stroke was responsible for 1,843 deaths in the study period, with ICD- 10 – I64, I69.4 and I61.9 being the most frequent. The years with the highest number of deaths from the disease were 2007 and 2008. 52% of deaths occurred in males and 74% in individuals over 65 years.

Conclusions:

The trend of general mortality was constant, but there was an increasing trend of deaths among white married males.
