Hormone therapy and Hypertension in Postmenopausal Women: Results from the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil)

Luana Ferreira-Campos et al. Ligia Gabrielli Maria da Conceição Chagas Almeida Estela Maria Leão Aquino … Sheila Maria Alvim Matos Rosane Harter Griep Roque Aras

Abstract

Background

Hypertension is a major risk factor for cardiovascular morbidity and mortality in post-menopausal women. Although menopausal hormone therapy (MHT) is a very effective treatment for vasomotor symptoms during this period, the influence of this therapy on blood pressure is not yet clear.

Objective

To evaluate the relationship between the use of MHT and hypertension in participants of the ELSA-Brasil.

Methods

A cross-sectional study using the baseline ELSA-Brasil data in a cohort of 2,138 women who had experienced natural menopause. This study analyzed hypertension, defined as arterial pressure ≥140/90 mmHg or previous antihypertensive use, and use of MHT, with participants being classified into never, past, and current users. Associations were assessed using an adjusted logistic regression model, with statistical significance set at p<0.05.

Results

Overall, 1,492 women (69.8%) had never used MHT, 457 (21.4%) had used it in the past, and 189 (8.8%) were current users. The use of MHT was more common in women who had a body mass index (BMI) <25 kg/m2and triglyceride levels <150 mg/dl, and who were physically less inactive, non-smokers, and non-diabetics. Current MHT users were less likely to have hypertension (OR=0.59; 95% CI: 0.41-0.85) compared to those who had never used MHT. In most cases, MHT was started at or before 59 years of age, within 10 years of becoming menopausal, and its use lasted for up to five years.

Conclusion

Current MHT use was not related to hypertension, particularly in healthy women and in those under 60 years of age.

Carotid Atherosclerosis in Pre- and Post-Menopausal Women with a History of Pregnancy-Induced Hypertension: Case-Control Study

Rafael Alessandro Ferreira Gomes et al. Isly Maria Lucena de Barros Moacir de Novaes Lima Ferreira Laura Olinda Bregieiro Fernandes Costa

Abstract

Background:

Cardiovascular disease mortality among women remains high. Observational studies are controversial about the participation of a history of gestational hypertensive disorder in cardiovascular risk.

Objective:

To verify the association between carotid atherosclerosis in menopausal women who had pregnancy-induced hypertension.

Methods:

Case-control study, with cases consisting of women with carotid atherosclerosis, defined as carotid intima-media thickness > 1 mm and/or presence of carotid plaques; the controls did not have these alterations. The significance level was set at 95%.

Results:

A total of 504 women without previous cardiovascular disease were assessed, 126 cases and 378 controls. Of the total, 67% were hypertensive; 76% were dyslipidemic; and 16% were diabetic. Approximately 10% reported a history of hypertension during pregnancy. Women with carotid atherosclerosis had higher values of systolic blood pressure (134.18 mmHg vs. 128.59 mmHg, p = 0.008) and LDL-cholesterol (156.52 mg% vs. 139.97 mg%; p = 0.0005). No statistical difference was found regarding the presence of carotid atherosclerosis and history of hypertension during pregnancy (OR 1.672, 95% CI: 0.883-3.131).

Conclusion:

The history of hypertension during pregnancy was not associated with subclinical carotid atherosclerosis in menopausal women. However, an association was observed between carotid atherosclerosis and classic risk factors, such as elevated systolic blood pressure and LDL-cholesterol levels.

The Interface between Osteoporosis and Atherosclerosis in Postmenopausal Women

Neuza H. M. Lopes

In recent years, the association between osteoporosis and atherosclerotic disease has been described, regardless of patients’ age, and highlighted epidemiologic and pathophysiologic similarities between arterial wall calcification and osteogenesis., Cross-sectional, prospective studies have pointed out significant negative association between cardiovascular mortality and low bone mass, osteoporotic fractures, vascular calcification, extension of coronary artery disease and abdominal aortic injury.– Concomitant occurrence of both diseases seems to result from common pathophysiologic and molecular mechanisms between these conditions. However, it is still controversial whether a low bone mass is caused by arterial calcification or vice-versa, or whether these conditions only have the same pathophysiological mechanism.

Risk factors for osteoporosis and atherosclerotic disease include estrogen, parathyroid hormone, homocysteine and vitamin K deficiency, lipid oxidation products, inflammatory process, vitamin D excess, molecular pathways involved in both bone and vascular mineralization, and similar mechanisms of calcification involving vascular and bone structures. Arterial calcification is found in more than 90% of atherosclerotic lesions. The process starts with formation of vesicles in the endothelial matrix, followed by proliferation and mineralization of the arterial intima-media wall, similarly to the bone tissue. Many bone remodeling regulators have been described in calcified atherosclerotic lesions, including osteocalcin, hydroxyapatite crystals, osteopontin, bone morphogenetic protein 2 osteoprotegerin, sclerostin, dickkopf factor (DKK), leptin, oxidized lipids and calcium sensor-related factor.

Postmenopausal therapy reduces catalase activity and attenuates cardiovascular risk

Vera S. Castanho et al. Rui Tsutomu Nakamura Aarão M. Pinto-Neto Eliana Cotta De Faria

BACKGROUND: Menopause can lead to alterations in women’s health, with changes in the oxidative status of postmenopausal women in whom information regarding the influence of hormone therapy (HT) on antioxidant enzyme activities is limited. OBJECTIVE: To evaluate the influence of HT on catalase activity; concentrations of lipids and lipoprotein, cholesteryl ester transfer protein, thiobarbituric acid-reactive substances, nitrates, high-sensitivity C-reactive protein and carotid thickness in postmenopausal women. METHODS: Ninety-four consecutive women were allocated to one of four groups, without HT and with HT. The latter group was subdivided into women using estrogen and those using estrogen plus progestogen therapy. Plasma biochemical parameters and common carotid intima-media thickness measurements were performed. RESULTS: HT antagonized the decrease in catalase activity after menopause, but had no effect on the levels of cholesteryl ester transfer protein, thiobarbituric acid-reactive substances, lipid peroxide, nitrate, high-sensitivity C-reactive protein, or on the common carotid intima-media thickness. Multivariate analysis showed that estrogen-based HT attenuated the relationship between cardiovascular risk factors and the intima-media thickness of the common carotid. CONCLUSION: This study indicates that HT in postmenopausal women produces beneficial antioxidant and anti-atherosclerotic effects by ameliorating the plasma lipid and lipoprotein profiles, increasing plasma catalase activity and attenuating the association between cardiovascular risk factors and early atherosclerosis.

Transdermal estradiol and lipid profile: effects on a specific group of Brazilian postmenopausal women

Daniel R. Callejon et al. Danyelle Romana. A. Rios Sílvio A. Franceschini Maria Regina T. Toloi

BACKGROUND: In postmenopausal women, significant changes occur that can induce cardiovascular diseases, such as atherogenic lipid profile, due to an increase in total cholesterol and LDL levels, and a decrease in HDL cholesterol levels. The hormone replacement therapy (HRT) can prevent these changes in lipid profile. OBJECTIVE: Verify the effects of HRT consisting of transdermal estradiol gel associated with medroxyprogesterone acetate on the lipid profile and biochemical parameters in Brazilian postmenopausal women. METHODS: This study is an open prospective longitudinal study, in which thirty postmenopausal women received transdermal estradiol gel (1 mg/day) continuously combined with oral medroxyprogesterone acetate (MPA) (5 mg/day) for 12 days/month. The following parameters were determined: total cholesterol, triglycerides, high density lipoprotein (HDL), low density lipoprotein (LDL), very low density lipoprotein (VLDL), glucose, aspartate aminotransferase (AST), alanine aminotransferase (ALT), gama glutamyl transferase (GGT) and follicle-stimulating hormone (FSH). RESULTS: The parameters of the lipid profile did not show a significant decrease, while the levels of GGT and FSH had a statistically significant decrease. CONCLUSIONS: the treatment with transdermal estradiol gel did not have a significant impact on the lipid profile, thus not resulting in a beneficial effect on cardiovascular disease markers, suggesting that the dose, administration route and the time of treatment were important for these results. Moreover, the treatment using small dose and the transdermal administration route also had a significant hepatic effect in this population. Therefore, this treatment might provide interesting effects on the lipid profile in Brazilian postmenopausal women.
