Right Ventricle Involvement by Glycogen Storage Cardiomyopathy (PRKAG2): Standard and Advanced Echocardiography Analyses

José Luiz Barros Pena et al. Fabricio Junqueira de Melo Wander Costa Santos Isabel Cristina Gomes Moura … Gabriela Pansanato Nakashima Natalia Costa Freitas Eduardo Back Sternick

Abstract

Background

PRKAG2 syndrome is a rare, early-onset autosomal dominant inherited disease. We aimed to describe the right ventricle (RV) echocardiographic findings using two and three-dimensional (2D and 3D) modalities including myocardial deformation indices in this cardiomyopathy. We also aimed to demonstrate whether this technique could identify changes in RV function that could distinguish any particular findings.

Methods

Thirty patients with genetically proven PRKAG2 (R302Q and H401Q), 16 (53.3%) males, mean age 39.1 ± 15.4 years, underwent complete echocardiography examination. RV-focused, 4-chamber view was acquired for 2D and 3D measurements. Student’s t or Wilcoxon-Mann-Whitney tests were used to compare numerical variables between 2 groups, and p < 0.05 was considered significant.

Results

Twelve patients (40%) had a pacemaker implanted for 12.4 ± 9.9 years. RV free wall mean diastolic thickness was 7.9 ± 2.9 mm. RV 4-chamber longitudinal strain (RV4LS), including the free wall and interventricular septum, was –17.3% ± 6.7%, and RV free wall longitudinal strain (RVFWLS) was −19.1% ± 8.5%. The RVFWLS apical ratio measured 0.63 ± 0.15. Mean RV 3D ejection fraction (EF) was 42.6% ± 10.9% and below normal limits in 56.7% of patients. Positive correlation occurred between RV 3DEF, RV4LS, and RVFWLS, especially for patients without a pacemaker (p = 0.006).

Conclusion

RV involvement in PRKAG2 syndrome is frequent, occurring in different degrees. Echocardiography is a valuable tool in detecting RV myocardial abnormalities in this condition. The use of 2D RV4LS, RVFWLS, and 3DEF offers reliable indicators of RV systolic dysfunction in this rare, challenging cardiomyopathy.

Systemic Hypertension and Right Ventricle: Preliminary Echocardiographic Data

Anderson José Argemiro et al. Ana Cristina Camarozano Daniela de Castro Carmo Jerônimo Antonio Fortunato … Rubens Zenóbio Darwich Liz Andréa Villela Baroncini

Summary

Background

Systemic arterial hypertension (SAH) has not yet been directly correlated with right ventricular hypertrophy (RV).

Objective

To assess the correlation between the presence of systemic arterial hypertension and right ventricular thickness, dimensions and function.

Methods

Cross-sectional and observational study. A total of 65 individuals were selected. They underwent echocardiogram with right ventricular evaluation in five aspects: free wall thickness; proximal diameter (RPD), mid diameter (RMD), basal diameter (RBD); tricuspid annular plane systolic excursion; and tissue doppler S’ wave. The subjects were divided into two groups: Control Group, without systemic arterial hypertension, with 24 subjects (17 women) and Study Group, with systemic arterial hypertension, with 41 subjects (29 women).

Results

Study Group presented older individuals in relation to Control Group (67 years ± 12 years and 47 ± 20 years; p < 0.001). In the Control Group, men presented higher values compared to women: proximal diameter (24.1 mm ± 1.3 mm vs. 20.1 mm ± 2.9 mm; p = 0.002), basal diameter (32.9 mm ± 2.7 mm vs. 26.5 mm ± 4.2 mm; p = 0.001), mid diameter (27 mm ± 2.1 mm vs. 21.2m ; p = 0.005); left ventricle diastolic diameter (49.6 mm ± 2.1 mm vs. 45.5 ± 4.3 mm; p = 0.028); left ventricle systolic diameter (30.1 mm ± 3.2 mm vs. 27.1 mm ± 2.9 mm; p = 0.034); and S’ wave (14.9 cm/s ± 2.4 cm/s vs. 13 cm/s ± 1.7 cm/s; p = 0.04). However, in the Study Group there were no significant differences between men and women in the same variables. These findings suggest greater impact of systemic hypertension in women.

Conclusion

The presence of systemic hypertension may cause different structural alterations in geometric ventricular patterns in men and women, possibly determining greater effects on the female sex. However, further studies are needed to confirm these findings. (Arq Bras Cardiol: Imagem cardiovasc. 2018;31(4):241-249)

Effects of Copaiba Oil in Peripheral Markers of Oxidative Stress in a Model of Cor Pulmonale in Rats

Cristina Campos et al. Patrick Turck Angela Maria Vicente Tavares Giana Corssac … Denise Lacerda Alex Araujo Susana Llesuy Adriane Bello Klein

Abstract

Background

To date, copaiba oil’s systemic effects have never documented in Cor pulmonale induced by monocrotaline.

Objectives

To investigate copaiba oil’s effects in peripheral markers of oxidative stress in rats with Cor pulmonale.

Methods

Male Wistar rats (170±20g, n=7/group) were divided into four groups: control (CO), monocrotaline (MCT), copaiba oil (O), and monocrotaline+copaiba oil (MCT-O). MCT (60 mg/kg i.p.) was administered, and after one week, treatment with copaiba oil (400 mg/kg/day-gavage-14 days) was begun. Echocardiography was performed and, later, trunk blood collection was performed for oxidative stress evaluations. Statistical analysis: two-way ANOVA with Student-Newman-Keuls post-hoc test. P values<0.05 were considered significant.

Results

Copaiba oil reduced pulmonary vascular resistance and right ventricle (RV) hypertrophy (Fulton index (mg/mg): MCT-O=0.39±0.03; MCT=0.49±0.01), and improved RV systolic function (RV shortening fraction, %) in the MCT-O group (17.8±8.2) as compared to the MCT group (9.4±3.1; p<0.05). Moreover, in the MCT-O group, reactive oxygen species and carbonyl levels were reduced, and antioxidant parameters were increased in the peripheral blood (p<0.05). Conclusions: Our results suggest that copaiba oil has an interesting systemic antioxidant effect, which is reflected in the improvements in function and RV morphometry in this Cor pulmonale model. Cor pulmonale attenuation promoted by copaiba oil coincided with a reduction in systemic oxidative stress.
