Percutaneous Treatment of Secondary Mitral Regurgitation by MitraClip: Mitra-FR vs. COAPT

Sergio Barros-Gomes et al. Flávio Tarasoutchi Ana Clara Tude Rodrigues Lara Ferreira Nhola … Pedro Alves Lemos Samira Saady Morhy Claudio Henrique Fischer Marcelo Luiz Campos Vieira

Introduction

Secondary or functional mitral regurgitation (MR) is due to changes in the geometry of the left ventricle (LV) secondary to ventricular dysfunction. It occurs when an ischemic heart disease or a dilated cardiomyopathy of any etiology causes dilation of the LV, dilation of the mitral ring, and/or displacement of the papillary muscle, resulting in poor coaptation of the valve cusps and valve regurgitation. the American Heart Association indicates that 16,250 per million North Americans have secondary MR,, totaling more than 5 million cases in the United States of America alone, and this number is estimated to be even greater due to the continued growth and aging of the population. This is noteworthy, as secondary MR causes a poor prognosis and is an independent predictor of mortality.,

For many years, the mechanical intervention of secondary MR (surgical or percutaneous) has been restricted to cases refractory to conventional clinical treatment,, with evidence mainly supported by two important studies conducted by the Cardiothoracic Surgical Trials Network group., The first study randomized 301 patients with moderate ischemic MR and found no differences in ventricular geometry between patients who undergone surgical myocardial revascularization versus the combination of surgical revascularization and mitral valve repair. The second study surveyed 251 patients with severe MR and found no differences regarding mortality, in addition to the greater recurrence of mitral regurgitation and complication rates among patients treated with mitral valve repair versus valve replacement. Considering these two studies, the recommendations of the American Heart Association/American College of Cardiology and the Brazilian Guidelines for Valvular Heart Disease classified surgical or percutaneous mitral valve intervention as a Class IIb indication.

Nutritional interventions in Metabolic Syndrome: a systematic review

Leila Sicupira Carneiro de Souza Leão et al. Milena Miranda de Moraes Giulia Xavier de Carvalho Rosalina Jorge Koifman

There is no consensus on the most appropriate nutritional strategy for treating metabolic syndrome (MS), such that cardiovascular risk is reduced. This study was designed to assess the strength of evidence of the benefits of various nutritional interventions in MS remission. Performed in Medline, Cochrane Library and PubMed databases, the virtual search consisted of randomized clinical trials published between 1999 and 2009 in any language, studies involving individuals aged 18 or older and diagnosed with MS, regardless of the criterion. The Boolean operator and was used in the combination of the MeSH terms “Metabolic Syndrome”, “Metabolic x Syndrome” and “Metabolic Syndrome X”, the entry terms “Dysmetabolic Syndrome X”, Metabolic Cardiovascular Syndrome,” “Metabolic X Syndrome” and “Syndrome X, Metabolic”, plus the terms “diet”, “intervention and diet”, “treatment and diet” and “supplementation”. For each study included in the review, we estimated the prevalence of MS and the calculation of effectiveness after the follow-up period. Relative risk measures for each study were described by Forest Plot. We identified 131 articles, which, after eligibility criteria, resulted in 15 studies. These studies were divided into four groups: normocaloric diet associated with exercise; isolated normocaloric diet, low-calorie diet combined with exercises; and isolated low-calorie diet. Tests with low-calorie diet associated with exercising revealed higher efficiency values, helping to emphasize the global aspects of lifestyle change in the treatment of MS, in which healthy and low-calorie diet should be complemented with the practice of physical activity.

Accuracy of the Omron HEM-705 CP for blood pressure measurement in large epidemiologic studies

BACKGROUND: Accurate measurement of blood pressure is of utmost importance in hypertension research. In the context of epidemiologic and clinical studies, oscillometric devices offer important advantages to overcome some of the limitations of the auscultatory method. Even though their accuracy has been evaluated in multiple studies in the clinical setting, there is little evidence of their performance in large epidemiologic studies. OBJECTIVE: We evaluated the accuracy of the Omron HEM-705-CP, an automatic device for blood pressure (BP) measurement, as compared to the standard auscultatory method with a mercury sphygmomanometer in a large cohort study. METHODS: We made three auscultatory measurements, followed by two measurements with the Omron device in 1,084 subjects. Bias was estimated as the average of the two Omron minus the average of the last two auscultatory measurements, with its corresponding 95% limits of agreement (LA). RESULTS: The Omron overestimated systolic blood pressure (SBP) by 1.8 mmHg (LA:-10.1, 13.7) and underestimated diastolic blood pressure (DBP) by 1.6 mmHg (LA:-12.3, 9.2). Bias was significantly larger in men. Bias in SBP increased with age and decreased with BP level, while bias in DBP decreased with age and increased with BP level. The sensitivity and specificity of the Omron to detect hypertension were 88.2% and 98.6%, respectively. Minimum bias in the estimates of the effects of several factors resulted from the use of Omron measurements. CONCLUSION: Our results showed that the Omron HEM-705-CP could be used for measuring BP in large epidemiology studies without compromising study validity or precision.
