The Route to Survival: Intravenous or Intraosseous Access in Cardiac Arrest

Imara Correia de Queiroz Barbosa et al. Larissa Neto Espíndola Ana Claudia Andrade Lucena Fátima Elizabeth Fonseca De Oliveira Negri Catia Sueli de Sousa Eufrazino Gondim

Out-of-hospital cardiac arrest (OHCA) continues to be one of the major challenges in emergency cardiovascular care. Despite advances in cardiopulmonary resuscitation (CPR) and defibrillation, overall survival rates remain low worldwide. Current international guidelines recommend the establishment of intravenous (IV) access as the first-line route for drug administration, while intraosseous (IO) access is suggested as an alternative when IV access cannot be promptly achieved.1,2 In this context, every minute counts, and establishing vascular access for drug delivery is often a race against time. However, a long-standing question still persists: does the route of administration – intravenous or intraosseous – truly impact outcomes?

In this issue of the International Journal of Cardiovascular Sciences, Cavalcante et al. present a comprehensive meta-analysis comparing IO and IV access in adults with OHCA. Pooling data from more than 9,000 patients across three major randomized clinical trials,3-5 the authors demonstrate that both routes yield similar results in terms of survival, with favorable neurological outcomes, the return of spontaneous circulation (ROSC), and survival to hospital discharge.6 Moderate heterogeneity was limited to the ROSC outcome, likely due to differences in its definition among studies. Beyond this, the evidence is consistent in showing that time to drug delivery, rather than the route of administration, determines patient outcomes.6

Evaluation of Catheter Ablation Using the PVAC Gold Catheter in Elderly Patients with Atrial Fibrillation

José Carlos Pachon-M et al. Tomas G. Santillana-P Enrique I. Pachon-M

Atrial fibrillation (AF) represents one of the most significant challenges in contemporary cardiological practice, especially in elderly patients, where the complexity of treatment is amplified by prevalent comorbidities and the fragility of this population. In this context, the study “Randomized Study Comparing Radiofrequency Ablation with the PVAC Gold System vs. Antiarrhythmic Drugs in Elderly Patients with Symptomatic Atrial Fibrillation” recently published in the ABC Cardiol by Martins et al., is remarkable. It offers crucial insights into therapeutic options, risks, and benefits for these patients. Additionally, this article is led by a team with remarkable international experience in AF ablation, navigating the challenges with astute clinical judgment having been a pioneer in several procedures.

This group of frequently polymedicated patients constantly inspires the search for therapies that reduce medication dependence and enhance quality of life. The prospect of eliminating AF through ablation and reducing multiple medications is highly appealing.,

Early Diagnosis and Treatment in Infective Endocarditis: Challenges for a Better Prognosis

Daniely Iadocico Sobreiro et al. Roney Orismar Sampaio Rinaldo Focaccia Siciliano Calila Vieira Andrade Brazil … Carlos Eduardo de Barros Branco Antônio Sergio de Santis Andrade Lopes Flávio Tarasoutchi Tânia Mara Varejão Strabelli

Infective endocarditis (IE), a microbial infection of the cardiac or adjacent vascular endothelium, remains a feared disease, although the modern diagnosis systematizations date back to 1885 by Osler. Although relatively uncommon, with approximately 3-10 cases per 100,000 individuals/year, the mortality remains high: more than one-third of patients die in the first year after the diagnosis., Only early diagnosis and treatment, whether exclusively clinical or associated with cardiac surgery, may interfere to reduce this high mortality rate.

IE used to be more frequent in young and middle-aged adults with rheumatic or congenital heart disease. However, recent studies have shown a significant reduction in the incidence of IE in these groups, especially in more developed countries.

Coronary Obstruction Following Transcatheter Aortic Valve Implantation

Henrique Barbosa Ribeiro et al. Rogério Sarmento-Leite Dimytri A. A. Siqueira Luiz Antônio Carvalho … José Armando Mangione Josep Rodés-Cabau Marco A. Perin Fábio Sandoli de Brito Jr.

Background:

Transcatheter aortic valve implantation (TAVI) was established as an important alternative for high-risk patients with severe aortic stenosis. However, there are few data in the literature regarding coronary obstruction, that although rare, is a potentially fatal complication.

Objective:

Evaluate this complication in Brazil.

Methods:

We evaluated all patients presenting coronary obstruction from the Brazilian Registry of TAVI. Main baseline and procedural characteristics, management of the complication, and clinical outcomes were collected from all patients.

Results:

From 418 consecutive TAVI procedures, coronary obstruction occurred in 3 cases (incidence of 0.72%). All patients were women, without prior coronary artery bypass grafting (CABG), and with mean age of 85 ± 3 years, logistic EuroSCORE of 15 ± 6% and STS-PROM score of 9 ± 4%. All of the cases were performed with balloon-expandable Sapien XT prosthesis. In one patient, with pre-procedural computed tomography data, coronary arteries presented a low height and a narrow sinus of Valsalva. All patients presented with clinically significant severe maintained hypotension, immediately after valve implantation, and even though coronary angioplasty with stent implantation was successfully performed in all cases, patients died during hospitalization, being two periprocedurally.

Conclusion:

Coronary obstruction following TAVI is a rare but potentially fatal complication, being more frequent in women and with the balloon-expandable prosthesis. Anatomical factors might be related with its increased occurrence, highlighting the importance of a good pre-procedural evaluation of the patients in order to avoid this severe complication.
