Abstract

Background:

Identifying risk factors in cardiovascular surgery assists in predictability, resulting in optimization of outcomes and cost reduction.

Objective:

This study aimed to identify preoperative and intraoperative risk predictors for prolonged hospitalization after coronary artery bypass grafting (CABG) surgery in the state of São Paulo, Brazil.

Methods:

A cross-sectional analysis using data from the REPLICCAR II database, a prospective, consecutive, multicenter registry that included CABG surgeries performed between August 2017 and July 2019. The primary outcome was a prolonged hospital stay (PHS), defined as a postoperative period exceeding 14 days. Univariate and multivariate logistic regression analyses were performed to identify the predictors with significance set at p <0.05.

Results:

The median age was 63 (57-70) years and 26.55% of patients were female. Among the 3703 patients analyzed, 228 (6.16%) had a PHS after CABG, with a median hospital stay of 17 (16-20) days. Predictors of PHS after CABG included age >60 years (OR 2.05; 95% CI 1.43-2.87; p<0.001); renal failure (OR 1.73; 95% CI 1.29-2.32; p <0.001) and intraoperative red blood cell transfusion (OR 1.32; 95% CI 1.07-2.06; p=0.01).

Conclusion:

Age >60 years, renal failure, and intraoperative red blood cell transfusion were independent predictors of PHS after CABG. The identification of these variables can help in multiprofessional strategic planning aimed to enhance results and resource utilization in the state of São Paulo.

Prognostic value of stress hyperglycemia for in-hospital outcome in acute coronary artery disease

Carlos Passos Pinheiro et al. Marcos Danillo Peixoto Oliveira Gustavo Baptista de Almeida Faro Esdras Correa Silva … Eduardo Augusto Andrade da Rocha José Augusto Soares Barreto-Filho Joselina Luzia Menezes Oliveira Antônio Carlos Sobral Sousa

BACKGROUND: In acute coronary syndrome (ACS), admission hyperglycemia is associated with adverse cardiovascular events in diabetic and nondiabetic patients. OBJECTIVE: To assess the prognostic value of stress hyperglycemia for the in-hospital outcome of patients admitted due to ACS. METHODS: This study included 152 patients admitted to the chest pain unit of a tertiary hospital diagnosed with ACS, who had admission blood glucose data, from September 2005 to February 2010. Group I comprised patients with stress hyperglycemia, defined as admission blood glucose concentration > 126 mg/dL for nondiabetic individuals and admission blood glucose concentration > 200 mg/dL for diabetic individuals. Group II was formed by patients with admission blood glucose concentration lower than those established. The association of hyperglycemia and in-hospital outcome was assessed. RESULTS: Stress hyperglycemia associated with in-hospital complications, age increase and female sex. On multivariate analysis, only female sex (OR = 2.04; 95% CI: 1.03 – 4.06; p = 0.007) and in-hospital complications (OR = 3.65; 95% CI: 1.62 – 8.19; p = 0.002) associated independently with admission hyperglycemia. CONCLUSIONS: Stress hyperglycemia is an independent predictive factor for in-hospital complications after ACS in diabetic and nondiabetic patients. The results highlight the need to assess admission blood glucose concentration in all patients admitted due to ACS, including nondiabetic ones, aiming at identifying those at higher risk for complications.

Metabolic syndrome in patients undergoing coronary artery bypass graft: prevalence and a marker of morbidity/mortality during hospitalization and 30 days after hospital discharge

Eduardo Pimenta et al. Oswaldo Passarelli Junior Flávio Borelli Márcio G. Sousa … Carlos Gun Vivian Amato Celso Amodeo Leopoldo Soares Piegas

OBJECTIVES: To compare the morbidity and mortality of patients with and without metabolic syndrome (MS) (according to the First Brazilian Guidelines for Diagnosis and Treatment of MS) undergoing coronary artery bypass graft (CABG) during hospitalization and after 30 days. METHODS: A total of 107 patients who underwent CABG with no associated procedures were assessed between August and October 2005; 74 (69.2%) of them had MS. Criteria of in-hospital outcome were: acute atrial fibrillation (AF), prolonged mechanical ventilation (MV), need for transfusion of blood products, number of hours spent in the ICU, length of hospital stay (days), respiratory and surgical wound infection, and death. For 30 days, the combination of events such as the need for readmission to hospital, surgical wound infection and death was assessed. RESULTS: The mean age was 60.6±9.7 years, and the group with MS presented a higher body mass index and body surface, and a higher prevalence of diabetes. No statistically significant difference was observed in the occurrence of in-hospital events such as AF (15.2% vs. 16.2%), MV (3.0% vs. 2.7%), number of units of blood products used (3.2±2.7 vs. 2.6±2.9), length of ICU stay (53.7±27.3 vs. 58.9±56.5), length of hospital stay (9.2±8.7 x 8.5±8.5), respiratory infection (6.1% vs. 2.7%), surgical wound infection (3.0% vs. 5.4%), and mortality rate (3.0% vs. 0.0%). The occurrence of combined events in 30 days was similar in both groups (12.2% vs. 20.3%, p = NS). CONCLUSION: Patients with MS did not have increased morbidity and mortality when undergoing CABG, however they had a higher trend of occurrence of surgical wound infection.
