Progression of Left Valve Disease during Use of Dopaminergic Drugs

Lívia Santana Barbosa et al. Ana Clara Tude Rodrigues Shirlei Novillo Pereira Hugo Leonardo Medeiros Vieira … Kamila Fernanda Staszko José Lázaro Andrade

Abstract

Cabergoline and bromocriptine are dopaminergic drugs deriving from ergot and used for treating hyperprolactinemic idiopathic disorders or pituitary adenomas, whose mechanism of action results from the reduction of prolactin secretion. Some reports in the literature show that cabergoline can cause valvular heart disease after long-term administration. We report the case of a patient diagnosed with macroprolactinoma who did interleaved use of cabergoline and bromocriptine and developed valvular abnormalities that did not exist before.

Abstract

Background:

Right valve diseases are not benign, the tricuspid regurgitation has a significant impact on morbidity and mortality of patients.

Objectives:

This study aimed to report the short-term results of tricuspid annuloplasty using the De Vega technique modified by Manuel Antunes.

Methods:

A descriptive-analytical study was performed to evaluate the results of the tricuspid valvuloplasty performed at the Instituto de Medicina Integral Professor Fernando Figueira between 2012 and 2017. Data were collected by reviewing charts and databases of the Department of Cardiology and Cardiovascular Surgery of the institution. Those with rheumatic diseases or infective endocarditis with tricuspid valve involvement, or reoperation of the tricuspid valve were excluded. Student’s t-test and McNemar’s were used for statistical analysis. A p-value < 0.05 was considered statistically significant.

Results:

A total of 87 patients were studied, most of them were women (56.3%). The most associated heart valve diseases were mitral regurgitation (27.6%) and aortic regurgitation (20.7%). There was a significant decrease in the degree of tricuspid regurgitation in the postoperative period, with 83.3% of patients with none or mild regurgitation and only 1.1% with severe regurgitation (p = 0.0077).

Conclusions:

In the current study, tricuspid valve annuloplasty using the modified De Vega technique was shown to be effective in the short term. Further studies are needed to evaluate the long-term results.

Association between Aortic Valve Sclerosis and Adverse Cardiovascular Events

Cristiane Machado Lucena et al. Rodrigo Pires dos Santos

Dear Editor,

The presence of aortic valve sclerosis (AVS) is associated with adverse cardiovascular events even when pre-existing risk factors are considered. In the Cardiovascular Health Study, which included more than 5,000 patients who were ≥ 65 years of age and were periodically evaluated for more than five years, AVS was associated with a 40% increase in the risk of myocardial infarction; in patients who did not initially present with heart disease, there were a trend toward an increased risk of angina, heart failure, and stroke. It is still unclear whether the mechanism of association between AVS and adverse clinical cardiovascular events is caused by diffuse atherosclerosis, endothelial dysfunction, altered calcium metabolism, lipid accumulation, genetic polymorphism, or other as yet undefined factors.

AVS is defined as thickening and increased echogenicity in the aortic leaflets with no reduction in mobility on an echocardiograph. The early lesion of AVS is an active process with some similarities to atherosclerosis, including the histopathological features and an association with risk for coronary artery disease. Patients are at greater risk for coronary artery disease if they are older, male, hypertensive, smokers, diabetics, or if their HDL and LDL levels are high. A cross-sectional study by Marmelo et al. evaluated 2,494 individuals who presented with AVS diagnosed via transthoracic Doppler echocardiography. These individuals also had a greater prevalence of hypertension, previous myocardial infarction, diabetes, a history of smoking, left ventricular systolic dysfunction, and mitral valve sclerosis. Multivariate analysis allows for a more complete study of the association between each variable and its outcome.
