Transcatheter Bicaval Valve System in the Treatment of Torrential Tricuspid Regurgitation and Refractory Systemic Hypervolemia: 12-Month Outcome

Rômulo Francisco de Almeida Torres et al. Alessandro Kraemer Rafael de Almeida Torres Fernanda Proença Lepca Bozzi … Sarah Fagundes Grobe Eduardo Mendel Balbi Filho Guilherme Barreto Gameiro Silva

Abstract

Severe isolated tricuspid regurgitation (TR) is associated with limiting symptoms, negative impact on quality of life, and poor prognosis. The TricValve® device is a transcatheter self-expanding biological valve system designed to mitigate systemic volume overload, improve quality of life, and improve congestive symptoms. We report a 12-month follow-up of a 61-year-old woman with signs of refractory systemic hypervolemia and torrential TR who underwent the procedure. The treatment resulted in significant weight loss, improvement in signs of hypervolemia, and improvement in functional class (FC).

Heart Failure – Pathophysiology and Current Therapeutic Implications

Mariana Vieira de Oliveira Bello et al. Fernando Bacal

The concept of translational medicine involves promoting the rapid transfer of observations made in the areas of research and pathophysiological understanding to the medical practice. This approach is a reality in major Brazilian research centers which link academia and clinical practice.

Heart failure (HF) is a complex syndrome that can manifest itself as heart failure with preserved ejection fraction (HFpEF), acute heart failure, or chronic heart failure with reduced ejection fraction (HFrEF). Here, we will cover the type of HF whose pathophysiology is understood the best and thus has disease-modifying treatments: HFrEF.

Case 6/2017 – A 28-Year-Old Man with Anasarca And Restrictive Heart Disease

Desiderio Favarato et al. Luis Alberto Benvenuti

Clinical aspects

The patient was a 28-year-old male with anasarca. There were moderate left systolic dysfunction and marked enlargement of the atria and right ventricle, with mild increase in pulmonary arterial pressure and moderate regurgitation of the atrioventricular valves.

Apparently diastolic dysfunction and dilatation of the ventricles and systolic dysfunction of the right ventricle predominated, indicating a restrictive heart disease.
