Coronary Artery Disease Associated with Coronary Anomaly and Situs Inversus Totalis in Man Submitted to Angioplasty

Alexandre Fuchs et al. Leandro Bonecker Lora João Renato Cardoso Mourão Fernando César da Costa Duarte … Igor André Telles da Cunha Claudia Regina de Oliveira Cantanheda

Introduction

Dextrocardia associated with situs inversus totalis is a rare condition with an incidence of 1:10,000 in the general population., It is characterized by a complete left-to-right reversal of the abdominal organs, including the abnormal position of the heart, resulting from changes in embryological development during the gastrulation stage (third week), characterized by the establishment of the craniocaudal, dorsoventral and right-left axes in the embryo.

This condition is transmitted by autosomal recessive genes, located in the long arm of chromosome 14, which affects the genetic cascade responsible for left-right differentiation. It should be noted that most affected individuals can have normal lives.

Case 4/2017 – Double-Chambered Right Ventricle with Dextrocardia and Hypoxemia Due to Atrial Shunt in a 4-Year-Old Girl

Edmar Atik et al. José Fernando Cavalini

Clinical data

A premature female twin (33-week gestation), weighing at birth 1935 g, remained hospitalized for one month due to the diagnosis of atrial septal defect (ASD) + ventricular septal defect (VSD) + persistence of ductus arteriosus (PDA). The patient gained less weight than the average children, but maintained full and similar activity, receiving furosemide and captopril, up to the age of 3 years, when her mother noticed cyanosis.

Dextroversion and Noncompaction

Josef Finsterer et al. Claudia Stöllberger

Dear Editor,

With interest we read the article by Gonçalves et al about a patient with dextrocardia and left-ventricular hypertrabeculation/noncompaction (LVHT). We have the following comments and concerns.

Dextrocardia in association with LVHT has been repeatedly reported.

Anatomy of a dextrocardia case with situs solitus

Fabíola Sawaguchi Faig-Leite et al. Horácio Faig-Leite

We present a rare case of dextrocardia in the corpse of a female child, aged approximately one year old, presenting situs solitus. The cases of dextrocardia reported in the literature normally describe multiple associated cardiac malformations. In the present study, it is noteworthy the fact that a complete specular image of the heart and the vessels of the base of the heart was not found. There was no atrioventricular discordance or other intra and extracardiac malformations, which are commonly seen in cases of dextrocardia. A large ductus arteriosus was found, as well as the anomalous position of the right portion of the heart.

Myocardial revascularization in a patient with situs inversus totalis

Paulo Manuel Pego-Fernandes et al. João Batista de Serro-Azul Fernando Matheus Bruno Shoiti Maehara

We report the case of a patient with dextrocardia and situs inversus totalis associated with obstructive coronariopathy in the anterior and posterior descending arteries, right coronary artery, first diagonal branch and left marginal branch. The patient underwent coronary artery bypass grafting surgery. This surgery has been rarely reported in literature and we found only one similar case in the national medical literature. The myocardial revascularization was carried out with the right mammary artery for the anterior descending artery. The saphenous vein anastomosed the aorta to the right coronary artery, left marginal branch, fist diagonal branch and posterior descending artery. The surgery was performed with extracorporeal circulation.
