Contrast echocardiography in the diagnosis of intrapulmonary vascular dilations in patients eligible for liver transplantation

Paulo Roberto Pavarino et al. Hélio Augusto do Reis Corbucci Carlos Henrique de Marchi Paula Fernanda da Mata Moacir Fernandes de Godoy

OBJECTIVE: To determine the importance of contrast echocardiography in the diagnosis of intrapulmonary vascular dilations (PVD) in patients with severe hepatic diseases, eligible for transplantation. METHODS: Transthoracic ecocardiography (TTE) with second harmonic imaging was performed in 76 patients, among them 32 patients were consecutively undergone to a transesophageal study (ETE). Echocardiographic contrast was obtained from microbubbles derived from the injection of agitated saline solution, in venous peripheral access. Abnormal presence of contrast in the left cardiac chambers was considered positive, with a delay of 4 to 6 cardiac cycles , after initial opacicification of the right cardiac chambers. RESULTS: PVD diagnosis was performed in 53.9% of the patients (41/76). Sensibility, specificity, positive predictive value, negative predictive value, and accuracy of the TTE in relation to the ETE was 75%, 100%, 100%, 80% e 87.5%, respectively. Echocardiography was positive in 37 (55.2%) of 67 nonhypoxemic patients, and in 4 (44.4%) hypoxemic ones. No cardiologic hemodynamic repercussions from intrapulmonar shunt were observed. CONCLUSION: The contrast echocardiography is efficient, easy to be used, and safe in the search for and identification of intrapulmonary vascular alterations in patients eligible for hepatic transplantation

Bioprosthetic mitral valve thrombosis: importance of transesophageal echocardiography in the diagnosis and follow-up after treatment

Adelino Parro Jr et al. Marta Lancia Carramona Caio Augusto Ferreira Amaral José Luiz Balthazar Jacob José Carlos Nicolau

OBJECTIVE: To report the clinical and echocardiographic findings of bioprosthetic mitral valve thrombosis and the value of transesophageal echocardiography (TEE) in its diagnosis and monitoring of thrombolysis. METHODS: One hundred and eleven patients with mitral bioprostheses underwent TEE, and 4 out of 7 suspected of having a thrombus on these prostheses were included in the study (mean age = 60.2±10.2 years; 2 men). The diagnosis was confirmed with serial TEE and clinical evolution. The morphologic features of the prosthetic leaflets, as well as the presence and characteristics of attached echogenic masses were investigated. The mean gradient through the prosthesis and the valvular area were obtained. RESULTS: The diagnosis of bioprosthetic mitral valve thrombosis was established 48.7±55.2 months after surgery. Two patients had ischemic stroke in the early postoperative period. The mean overall gradient was high (11.4±3 mmHg) and the valvular area reduced (1.24±0.3 cm²). On TEE, echogenic masses on the left ventricular face of the mitral bioprosthesis suggestive of thrombus were evidenced in all patients. On serial TEE (136±233 days), in 2 patients the thrombus had disappeared and in 2 others it was smaller after treatment, the mean gradient dropped to 6.2±3 mmHg (P = 0.004; 95% CI), and the valvular area increased to 2.07±0.4 (P = NS). CONCLUSION: TEE proved to be useful for detecting bioprosthetic mitral valve thrombosis and was effective in monitoring the treatment in all patients.

Mortality decline after implementation of standard protocols for treating patients with acute myocardial infarction

Juliana G. Bordon et al. Sérgio A. R. Paiva Luiz S. Matsubara Roberto M. T. Inoue … Mirna Matsui Ana Lúcia Gut Ana Lúcia A. Ferreira Leonardo A. M. Zornoff

OBJECTIVE: To compare 30-day mortality in patients receiving different types of medication from 1992 to 1997, when no consensual treatment for acute myocardial infarction was available, versus 30-day mortality in patients being treated between 2000 and 2002 after standardization of that treatment was obtained in our service. METHODS: In the first and second study periods, 172 and 143 patients, respectively, admitted with the diagnosis of acute myocardial infarction were retrospectively assessed. Their diagnoses were confirmed, and the following statistical tests were performed: the chi-square test for comparing proportions and the Student t test and the Mann-Whitney test for comparing the means or medians. RESULTS: The analysis showed no difference in regard to white men with a mean age of 61 years in the 2 study periods. In regard to the traditional risk factors, a difference was observed only in the incidence of dyslipidemia (17 and 29%). In regard to the therapeutic strategy adopted, the following was observed: 1) a significant increase in the use of thrombolytic agents (39 and 61.5%), acetylsalicylic acid (70.9 and 96.5%), beta-blockers (34.8 and 67.8%), angiotensin-converting enzyme inhibitors (45.9 and 74.8%), and nitrates (61 and 85.3%); and 2) a significant reduction in the use of calcium channel blockers (16.8 and 5.3%), antiarrhythmics (29.1 and 9.7%), and diuretics (50.6 and 26.6%). The use of inotropic agents did not differ between the study periods (29.6 and 32.1%). The 30-day mortality showed a statistically significant reduction from 22.7 to 10.5%. CONCLUSION: The implementation of standard protocols for the treatment of acute myocardial infarction was accompanied by a significant reduction in the 30-day mortality rate.

Libman-Sacks endocarditis and oral anticoagulation

Fabiano Almeida Brito et al. Magali LMC Tófani Fábio Ávila Tófani Adriana Maria Kakehasi … Cristina Costa Duarte Lanna Marco Antonio Parreiras Carvalho

The patient is a 34-year-old female with systemic lupus erythematosus and secondary antiphospholipid antibody syndrome, who evolved with convulsive crises, partially controlled with an anticonvulsant, and auscultation of a cardiac murmur, whose investigation showed the presence of a mitral valve vegetation. Once the diagnosis of Libman-Sacks endocarditis was established, therapy with warfarin sodium was initiated, and, after 6 months of oral anticoagulation, the patient had total control of the convulsive crises and the valvular vegetation disappeared on echocardiography. This study discusses the occurrence of Libman-Sacks endocarditis in systemic lupus erythematosus, its association with antiphospholipid antibody syndrome, and the anticoagulant therapy. A literature review is also provided.

Isolation of the pulmonary veins in patients with permanent atrial fibrillation secondary to mitral valve disease

Gustavo G. Lima et al. Renato A.K. Kalil Tiago L.L. Leiria Gustavo F. Vanni … Marcelo H. Miglioransa Daniel L. Faria-Corrêa Domingos Hatem Rogério Abrahão João R. Sant'Anna Paulo Prates Ivo A. Nesralla

OBJECTIVE: To assess the efficacy of surgical isolation of the pulmonary veins for re-establishing sinus rhythm in patients with atrial fibrillation secondary to mitral valve disease. METHODS: Thirty-three (67% were women) patients with permanent atrial fibrillation and indication for surgical correction of the mitral valve underwent surgical isolation of the pulmonary veins. Their mean age was 56.3±10 years, preoperative NYHA functional class was 3.2±0.6, left atrial size was 5.5±0.9 cm, and ejection fraction was 61.3±13%. The surgical technique consisted of a circumferential incision surrounding the 4 pulmonary veins, excision of the left atrial appendage, and a perpendicular incision originating in the inferior margin of the circumferential incision isolating the pulmonary veins down to the mitral valve. Early arrhythmias were aggressively treated with cardioversion. RESULTS: The mean follow-up was 23.9±17 months, and 3 patients died in the postoperative period. Ten patients required electrical cardioversion in the postoperative period; 87% had sinus rhythm in the last medical visit, and 33% were using amiodarone. CONCLUSION: Isolation of the pulmonary veins associated with mitral valve surgery is an effective and safe technique for maintaining sinus rhythm in patients with permanent atrial fibrillation.

Isolated chylopericardium after cardiac surgery

Ranieli Pitol et al. José Renato Pederiva Fernando Pasin Domingos Vitola

Chylopericardium is a rare complication of cardiac surgery. It may be caused by a lesion in the thoracic duct or its tributaries or by thrombosis in the confluence of the jugular and left subclavian veins, obstructing the drainage of the thoracic duct. The treatment may be conservative or surgical, depending on the duration and on the volume of the effusion. We report the case of a 24-year-old female, who, in the late postoperative period of mitral valve replacement (bioprosthesis), was hospitalized with cardiac tamponade due to the presence of chylopericardium. The clinical findings and treatment performed are discussed.

Pericarditis: series of 84 consecutive cases

Marco Tulio Zanettini et al. João Otavio Zanettini Jacira Pisani Zanettini

OBJECTIVE: To identify differential clinical, laboratory, and echocardiographic characteristics in persons with diagnosed idiopathic and secondary pericarditis. METHODS: From January 1999 to December 2001, 84 patients with clinically and echocardiographically diagnosed pericarditis were identified in a heart clinic. These patients were analyzed according to age, sex, anthropometric measurements, body habitus, casual blood pressure (BP), signs and symptoms, morbid history, medicines and complications. The individuals were divided into 2 groups: group A comprised 61 patients with known causes of pericarditis and group B comprised 23 patients with idiopathic causes. The groups were compared with chi-square test. P<0.05 was considered statistically significant. RESULTS: The population of these 2 groups was similar in age, sex, anthropometric measures, body habitus, and casual BP. In group B (idiopathic), 23 (100%) cases were diagnosed between April and August versus 24 (39.4%) in the same period for group A (P<0.01). Twenty-three (100%) group B patients received anti-influenza vaccine versus none in group A. Breathlessness (P=0.02) and swelling (P=0.01) were more frequent in group A, but fatigue was more common in group B (P=0.01). For treatment, non-steroidal anti-inflammatory drugs (NSAID) were prescribed to 5 (8.2%) patients in group A and 19 (82.6%) in group B (P=0.01). CONCLUSION: In this series, patients labeled as having idiopathic pericarditis who had previously taken the influenza vaccine had seasonal distribution, a lower prevalence of previous disease, less exuberant signs and symptoms, and clinical regression with NSAID use.
