Change of Strategy in Coronary Artery Bypass Graft Surgery Waiting List during the COVID-19 Pandemic: One-Year Follow-Up

Franc Jorge Sampaio Santos Pereira et al. Marília Prudente Menezes Gabriela Carolina Santamaria Naranjo José Henrique Herrmann Delamain … José Ribamar Costa Mario Issa Vivian Lerner Amato Fausto Feres Pedro Silvio Farsky

Introduction

The COVID-19 pandemic led to a significant occupation of intensive care beds, causing the suspension of elective cases of coronary artery bypass grafting (CABG).

At the same time, the evolution of techniques for the percutaneous treatment of coronary artery disease (CAD) has expanded the possibilities for this modality, and published studies have not shown a reduction in outcomes in stable patients with moderate/high ischemic burden under optimized clinical treatment (OCT).

Waiting for Cardiac Procedure in Congenital Heart Disease: Portrait of an a Hospital in the Amazonian Region

Valeria Santos de Jesus et al. Aline Marques Nascimento Rogério dos Anjos Miranda Joel Silva Lima … Milene de Andrade Gouvea Tyll Adriana de Oliveira Lameira Veríssimo

Abstract

Background:

Congenital heart disease is an important cause of morbidity and mortality in childhood, and in 50% of cases, surgery is required in the first year of life. A high deficit of surgical procedures is estimated in Northern Brazil.

Objective:

To analyze the waiting time for elective surgical treatment and/ or intervention in children with congenital heart disease in a Cardiology referral center, and to make considerations about heart diseases and forms of treatment in that institution.

Methods:

A cross-sectional study of all patients aged less than 14 years, with a diagnosis of congenital heart disease that were waiting for elective surgical or percutaneous cardiac treatment.

Results:

Among the 407 children with congenital heart defects, the most prevalent age group was > 2 to 6 years (34.0%). The average waiting time was 23.1 ± 18.3 months, with a median of 19. The most frequent heart disease was ventricular septal defect (28.98%), patent ductus arteriosus (18.42%) and atrial septal defect (11.05%). Most children (63.4%) were not from the metropolitan area. The percutaneous interventions represented only 27.84% of the catheterization procedures and 14,85% of all heart treatments. Approximately 60% of the pediatric surgeries occurred in children who were not previously registered due to urgency cases.

Conclusion:

Most of the children waiting for a cardiac procedure were not from the metropolitan area and had malformations potentially treatable by catheterization. It is necessary to increase the capacity of the single referral center in the state of Pará, as well as decentralize the high-complexity cardiological care in the metropolitan region.

Profile of patients in the Unified Waiting List for heart transplantation in state of Ceará

Francisca Elisângela Teixeira Lima et al. Adriana Kelly Almeida Ferreira Kelly de Araújo Fontenele Eliana Régia Barbosa de Almeida

BACKGROUND: Organ transplants have increased considerably in recent years because of technological developments and society’s awareness for organ donation. OBJECTIVE: To describe the characteristics of the single list of patients waiting for heart transplantation; to identify the main heart diseases; and determine the average time a patient stays in the list until the surgery. METHODS: This is a descriptive, documentary and retrospective study with a quantitative approach. It was developed at the Transplant Center of the State of Ceará, with 156 patients included in Unified Waiting List from 1999 to 2006. Data were organized into figures. RESULTS: There were: 81% males; 22.4% of young adults (20 to 40 years old) and 56.4% middle-aged adults (40 to 64 years old), averaging 36 years old; 79% from Fortaleza-CE; 91% with cardiomyopathy as a cause of heart transplantation. Among these, 102 patients (69%) were transplanted; 37 (25%) died before transplantation; and 8 (6%) were excluded due to improvement or worsening of clinical status. CONCLUSION: The patients from the Unified Waiting List for Heart Transplantation in the state of Ceará, from 1999 to 2006, were male (80%), with ages ranging from 1 to 71 years, with a predominance of dilated cardiomyopathy (53.4%), and the average waiting time was 136 days until the day of cardiac transplantation.
