Palliative Care in Patients with Advanced Heart Failure: An Integrative Review

Irving Gabriel Araújo Bispo et al. Clara Verrone da Silva Gabriela Tobias Gonçalves Pereira Victoria Paulino de Mello Lopez

Abstract

The term advanced heart failure (HF) defines a profile of patients with severe symptoms, recurrent decompensations, and cardiac dysfunction despite optimized therapy. The clinical course of advanced HF is difficult to predict, and identifying the appropriate moment for integrating palliative care (PC) remains a challenge. To conduct an integrative review on the incorporation of palliative care in patients with advanced HF, highlighting the main differences in quality-of-life outcomes based on the most recent clinical studies. The topic was defined and the guiding question formulated according to the PICO model. A systematic search was performed for studies in Portuguese, Spanish, and English in the PubMed, LILACS, and SciELO databases, published in the last five years. The descriptors used were “heart failure,” “palliative care,” and “elderly patient.” A total of 54 articles were identified; after applying the inclusion and exclusion criteria, 9 studies were selected. Patients with advanced HF present a high symptom burden (pain, dyspnea, fatigue, anxiety, and depression). Early introduction of PC, ideally at critical points in the disease trajectory, enables symptom relief, reduced hospitalizations, and improved communication between families and healthcare teams. Early integration of palliative care should be understood as an essential strategy in the management of advanced heart failure. We emphasize the substantial gap that persists in the early introduction of palliative care in advanced HF, which requires coordinated, multidisciplinary action. It is essential that healthcare teams and public policies recognize PC as a fundamental and humanized component of advanced HF treatment, ensuring maximum comfort, dignity, and well-being for patients.

Palliative Care in Decompensated Heart Failure Requiring Inotropic Therapy: Opportunities for Integration to Improve Outcomes

Daniel Battacini Dei Santi et al. Mucio Tavares de Oliveira Jr Ricardo Tavares de Carvalho

Abstract

Background

Advanced heart failure (HF) is associated with high morbidity and mortality rates, compromising the functionality and quality of life of patients and their families. Hospitalizations exacerbate disease severity, particularly when inotropic therapy is required. Palliative care (PC) supports the management of suffering caused by severe illnesses but is infrequently utilized in cardiology.

Objective

To evaluate the integration of PC in the management of decompensated HF, identifying opportunities to enhance patient care.

Method

This unicentric, retrospective, observational study was conducted between February 2015 and May 2018 with HF patients undergoing inotropic therapy. The study analyzed referrals for PC, the approach adopted by the PC-consultation team, and patient outcomes, including an analysis of 5-year survival rates. Statistical significance level: 5%.

Results

A total of 492 patients were included (66.9% male, median age 63 years, IQR 52-72). PC referral occurred in 23% of cases, with a median of 8.0 days (IQR 4.0-20) before death. Only 14% of intensive care patients were referred, and no transplant patients received PC evaluations. Patients assessed by the PC team were more involved in decision-making and received more opioid prescriptions for symptom management than those managed exclusively by cardiologists (p<0.01). In-hospital and 5-year mortality rates were 42% and 80%, respectively.

Conclusions

Patients experiencing decompensated HF demonstrate high mortality rates and are rarely referred to PC, often in the final days of life, limiting the potential benefits of this approach. Enhanced medical education in PC and the development of strategies to promote its integration may improve patient outcomes.

Can Palliative Care for Heart Failure be Indicated in the Emergency Department?

Louise Freire et al. Diane Xavier de Ávila Estêvão Lanna Figueiredo

Introduction

Heart failure (HF) is a progressive disease marked by a pattern of gradual decline interspersed with episodes of acute deterioration. This often culminates in either sudden death or hospitalization.– Discussing its trajectory, especially in emergency decompensations, can be challenging when physicians predict these situations using palliative care (PC) principles and practices.,

The World Health Organization defines PC as a patient and family-centered approach aimed to enhance the overall quality of life (QoL) by proactively addressing, preventing, and alleviating suffering. This prioritizes relief of pain and management of distressing symptoms, upholds life’s value, and recognizes the natural process of dying. PC also provides a support system, empowering patients to live as fully and actively as possible until the end of life (EoL).,

Ethical and Legal Aspects of Palliative Care in Heart Failure in Brazil

Daniel Battacini Dei Santi

Introduction

Terminal heart failure (HF) poses numerous difficulties to clinical practice, with ethical, moral, and legal dilemmas. Given that it is an advanced stage of incurable and progressive diseases, the few therapeutic possibilities that are available and tolerated aim to delay evolution, attempt to maintain the balance of organic functions, control symptoms, and offer comfort to patients. Generally, the clinical context is complex, involving interaction with other morbidities, low functionality, frailty, and high symptomatology.–

There is undoubtedly a high risk of death, in addition to a risk of suffering from the conditions caused by the disease or resulting from treatments. Decision making in terminal HF is a challenge, in the continuous search for adequate therapies that offer strategies that benefit the patient, without adding more risks or damages than are already inherent to their clinical condition.,

Implementation of Home Use of Continuous Intravenous Inotrope as Palliative Therapy for a Patient with Advanced Heart Failure within the Brazilian Unified Health System: a Case Report

Ana Paula Chedid Mendes et al. Rita Zambonato Laura C. T. Hastenteufel Letícia Orlandin … Nadine Clausell Livia Adams Goldraich

Introduction

Contemporary evidence-based treatment has modified the natural history of heart failure (HF). Nevertheless, with the current aging of the population with HF and the elevated number of comorbidities, there is a large proportion of individuals who are ineligible for advanced therapies and who could potentially benefit from palliative care. Accordingly, strategies for relieving symptoms and improving quality of life are being prioritized over interventions that aim for medium- and long-term outcomes. For a subgroup of patients with advanced HF receiving palliative care, home therapy with continuous inotropes may be considered and integrated into general palliative care.
