Maternal-fetal outcome and prognosis of cardiac surgery during pregnancy

Walkiria Samuel Ávila et al. Ana Maria Milani Gouveia Pablo Pomerantzeff Maria Rita Lemos Bortolotto … Max Grinberg Noedir Stolf Marcelo Zugaib

BACKGROUND: Cardiac surgery improves the maternal prognosis in cases refractory to medical therapy. However, it is associated with risks to the fetus when performed during pregnancy. OBJECTIVE: To analyze maternal-fetal outcome and prognosis related to cardiac surgery performed during pregnancy and puerperium. METHODS: The outcome of 41 gestations of women undergoing cardiac surgery during pregnancy and puerperium was studied. Fetal cardiotocography was performed throughout the procedure in patients with gestational age above 20 weeks. RESULTS: Mean maternal age was 27.8 ± 7.6 years; there was a predominance of patients with rheumatic valve disease (87.8%), of whom 15 (41.6%) underwent reoperation due to prosthetic valve dysfunction. Mean extracorporeal circulation time was 87.4± 43.6min and hypothermia was used in 27 (67.5%) cases. Thirteen (31.7%) mothers experienced no events and gave birth to live healthy newborns. Postoperative outcome of the remaining 28 (68.3%) pregnancies showed: 17 (41.5%) maternal complications and three (7.3%) deaths; 12 (29.2%) fetal losses, and four (10%) cases of neurological malformation, two of which progressed to late death. One patient was lost to follow-up after surgery. Nine (21.9%) patients underwent emergency surgery, and this variable was correlated with maternal prognosis (p<0.001) CONCLUSION: Cardiac surgery during pregnancy allowed survival of 92.7% of the mothers, and 56.0% of the patients who presented cardiac complications refractory to medical therapy gave birth to healthy children. Worse maternal prognosis was correlated with emergency surgery.

Why do patients with chagasic cardiomyopathy have worse outcomes than those with non-chagasic cardiomyopathy?

BACKGROUND: Heart failure is a highly prevalent disease, the prognosis of which depends on different predictive factors. OBJECTIVE: Chagas disease is a predictor of poor prognosis in patients with chronic heart failure (HF). The purpose of this study is to investigate whether this condition also predicts poor outcome in acutely decompensated patients. METHODS: Four hundred and seventeen patients admitted for decompensated heart failure were studied. Mean age was 51.8 years, and 291 (69.8%) were male. They were divided into two groups: 133 (31.9%) patients with Chagas heart disease (CH) and 284 patients with heart failure of other etiologies. Cytokine and norepinephrine plasma levels were measured in a subgroup of 63 patients (15.1% with Chagas disease). RESULTS: At admission, 24.6% of the patients needed inotropic support, and one-year mortality was 54.7%. Mortality rates were higher in the CH group (69.2% vs. 47.9%, p < 0.001). When data were compared, patients with Chagas disease were younger (47.6 vs. 53.8 years, p < 0.001) and, on average, showed lower systolic blood pressure (96.7 vs. 111.2 mmHg, p < 0,001), ejection fraction (32.7 vs. 36.4%, p < 0.001), and serum Na (134.6 vs. 136.0, p = 0.026), in addition to higher TNF-α levels (33.3 vs. 14.8, p = 0.001). The presence of hypotension requiring inotropic support, left ventricular (LV) diastolic diameter, renal function findings, and interleukin-6 and norepinephrine plasma levels did not differ between both groups. CONCLUSION: Chagas disease patients admitted with decompensated heart failure had worse prognoses than patients with heart failure of other etiologies. This may be owing to a greater degree of cardiac impairment (lower ejection fraction) and hemodynamic instability (lower systolic blood pressure and heart rate), increased activation of the renin-angiotensin system (lower sodium), and increased cytokine levels (TNF-α).

BACKGROUND: Information on the clinical progression, in the long term, of patients submitted to mechanical reperfusion is scarce. OBJECTIVE: The objective of this study is to describe the long-term clinical progression of patients submitted to primary stenting. METHODS: Between January 1998 and December 2003 we studied a non-concurring cohort with a fixed population of 202 patients (mean age = 61.2 ± 7.7 years; 74.7% males and 25.3% females) submitted to primary stenting. All the patients were followed up clinically and we assessed the occurrence of deaths, acute myocardial infarction (AMI), cerebral vascular accident (CVA) and surgical or percutaneous myocardial revascularization (MR). Kaplan-Meier survival curves were built for the following events: death, deaths/AMI, deaths/AMI/CVA and major cardiovascular events (MCE). RESULTS: In 91.5% of the patients the procedure was successful. During hospital stay, mortality was 3.4%; reinfarction was 0.9%; CVA was 1.8%; and urgent MR was 1.4%. Clinical follow-up varied from 29 to 100 months (mean = 58.7 ± 19.7 months). Death-free survival was estimated at 93.6%; death/AMI-free survival at 89.6%; death-AMI/CVA-free survival at 87.1%; and MCE-free survival at 71.3% CONCLUSION: Primary stenting presented excellent results during hospital stay. Very late clinical follow-up demonstrated that these good initial results have held up.
