Evaluation of Antibiotic Prophylaxis Prescriptions for Infectious Endocarditis Before Oral Procedures Between Cardiologists and Dentists

Mariana Mattos Brandão et al. Luisa Latado Lívia Brito Oliveira Viviane Almeida Sarmento Adriana Lopes Latado

Abstract

Background

Infective endocarditis (IE) is a serious disease with a high mortality rate. Antibiotic prophylaxis for bacterial endocarditis before invasive procedures has been recommended in patients with predisposing cardiac conditions since 1960, but contemporary guidelines worldwide have proposed changes.

Objective

To evaluate the knowledge and pattern of prescription by cardiologists and dentists regarding antibiotic prophylaxis for bacterial endocarditis before risky oral procedures.

Methods

This is an observational and cross-sectional study. Data were obtained from an online questionnaire, sent to cardiologists and dentists linked to specialty societies, in the first semester of 2021. Data analysis was performed using descriptive statistics, and comparisons between variables were done in an exploratory approach. The significance level adopted was 5%.

Results

From 613 responders, 82.5% of cardiologists and 79.5% of dentists reported prescribing antibiotic prophylaxis for patients at high and moderate risk for IE. Of dental procedures capable of generating bacteremia, all were correctly identified by more than 50.0% of the sample. As for the habits of daily living, flossing and toothbrushing had almost 50.0% of correct answers, chewing had only 17.3%, and 40.9% reported that none of the actions presented a risk of bacteremia. When comparing variables, the correct prescription of amoxicillin (2 g, 30–60 minutes before the procedure) was more prevalent among cardiologists and in responders with less than 20 years of graduation (p<0.01).

Conclusion

In the present study, the prescription of antibiotic prophylaxis for IE were frequent for high- and moderate-risk patients, before oral/dental procedures. Partial knowledge was found about endocarditis, which highlights the need for continuous medical/dental education.

When Everything goes Wrong

Helder Santos et al. Hugo Miranda Mariana Santos Inês Almeida … Joaquim Peixoto Joana Chin Lurdes Almeida

Introduction

The Bentall procedure was described for the first time 50 years ago and has undergone several improvements through the years. This technique is considered to be a longstanding and safe procedure. However, as any surgery, it can have several complications such as anastomotic pseudoaneurism, myocardial infarction and endocarditis.

In developed countries with differentiated access to health care and prophylaxis, endocarditis is an uncommon pathology, associated to frequent complications and high mortality rates. Antibiotic therapy aims to eradicate the responsible microorganism. However, some of the drugs used cause several side effects, like the DRESS (Drug Reaction with Eosinophilia and Systemic Symptoms) syndrome.

Oral Antibiotic Therapy for Infective Endocarditis Due to Enterococcus Spp. After Hemorrhagic Stroke and Heart Surgery

Guilherme Augusto Reissig Pereira et al. Graziela Regina Kist Tiago Santini Machado Christiano da Silveira de Barcellos

Introduction

Infectious endocarditis (IE) is a severe infectious disease with an annual incidence of 3 – 7 cases per 100 000 people, and is considered the 3rd most life-threatening infection after pneumonia and intra-abdominal abscess. Intra-hospital mortality ranges from 15 to 30% and patients affected today are older and have more comorbidities than in the past.

In IE the treatment is traditionally performed in hospitalized patients, due to the need for intravenous antibiotics. Patients with IE on the left side of the heart are typically treated for 2 – 6 weeks, according to the American and European Guidelines. – Enterococcus spp. is the third cause of IE, representing 10% of cases in non-users of intravenous drugs, and is considered to be a bacterium that is difficult to eradicate, requiring 6 weeks and the combination of antibiotics.

Acute Pericarditis in Crohn's Disease under Pharmacological Immunosuppression: A Diagnostic and Therapeutic Dilemma

Maria Trêpa et al. Isabel Neves Marta Salgado Graziela Carvalheiras Vasco Dias

Introduction

Acute pericarditis in young patients usually has a benign course. However, in those with autoimmune diseases it is mandatory to consider the risk of opportunistic infections, and of association with the underlying disease or immunomodulatory drugs. This case report aims to show possible strategies for diagnosis and treatment of this particularly complex group of patients.
