Combined therapy with cholestyramine and HMG-CoA reductase inihibtors in secondary prevention of coronary disease

José Paulo Novazzi et al. Leonor do Espírito Santo de Almeida Pinto Caio Auriemo Tania Leme da Rocha Martinez

PURPOSE: To evaluate lipid profile changes associated with cholestyramine addition in hypercholesterolemic patients with established coronary heart disease under treatment with HMG-CoA reductase inihibtors that had not achieved the ideal value of LDL-cholesterol. METHODS: Twenty patients with coronary heart disease, (12 submitted to coronary artery bypass grafts, 3 to coronary angioplasty and 5 maintened under clinical management) with mean age of 60.78 years old, who were under hypolipemic diet and were medicated with lovastatin 20mg/d or simvastatin 10mg/d, received cholestyramine, doses ranging from 8 to 16g/day during 8 weeks, aiming to reduce LDL-cholesterol to values less than 100mg/dL. RESULTS: There was a significant reduction of total cholesterol, from initial mean value of 239.52mg/dL to final mean value of 199.00mg/dL, with a mean reduction of 16.92%. The mean value of LDL-cholesterol was also reduced significantly from 172.73mg/dL to 118.26mg/dL, with a mean reduction of 31.53%. Mean triglyceridemia increased, still within the normal reference values, from 145.05mg/dL to 162.00mg/dL, and the mean difference was 11.69%. There was a significant increase of HDL-cholesterol fraction from an initial mean value of 38.00mg/dL to a final mean value of 48.21mg/dL, mean difference of 26.87%. Side effects were not frequent, and did not interfere in the duration of the study. CONCLUSION: The association of cholestyramine to HMG-CoA reductase inihibtors, both in low doses, in patients with primary hypercholesterolemia and high coronary risk is a good therapeutic option that can reach benefits on the lipid profile similar to those obtained when these drugs are used in association or separately in higher doses.

Sinus node modification by catheter using radiofrequency current in a patient with inappropriate sinus tachycardia. Evaluation of early and late results

Fernando E. S. Cruz Fº et al. Ivan G. Maia Marcio L. A. Fagundes Silvia Boghossian José Carlos Ribeiro

This article reports the early and late results of a patient in whom radiofrequency current was used to ablate an incessant inappropriate sinus tachycardia. During successful radiofrequency application there was a sudden change of rate and subsequent emergence of a stable rhythm with the same sinus node P wave characteristics. During follow-up, normal cardiac cycles were still present after six months of the procedure, although the patient still complained of palpitations, suggesting no correlation with the heart rate.

Comparison between exercise intensity prescription based on a standard exercise test and cardiopulmonary exercise test

Maria Urbana Pinto Brandão Rondon et al. Cláudia Lúcia de Moraes Forjaz Newton Nunes Sandra Lia do Amaral … Antonio Carlos Pereira Barretto Carlos Eduardo Negrão

PURPOSE: To compare the lower (LL) and upper limits (UL) of exercise intensity prescription based on standard exercise test (60-70% of estimated VO2max or 70-85% of HRmax measured) with exercise intensity prescription based on cardiopulmonary exercise test [anaerobic threshold (AT) and respiratory compensation point (RCP)]. METHODS: Fourty seven men (30±5 years) who were submitted to a progressive cardiopulmonary exercise test until exhaustion were divided in subgroups according to treadmill speed during exercise test (4 or 5mph) and the physical capacity [lower (LPC) and moderate physical capacity (MPC)]. RESULTS: The LL of the indirect exercise intensity prescription showed VO2 and HR values significantly higher than VO2 and HR values measured at AT (4mph= 34.4±4.5 vs 19.6±4.6 and 5mph= 28.9±2 vs 18.9±5.4, and LPC= 32.0±4.1 vs 17.2±2.8 and MPC= 31.6±4.9 vs 21.1± 5.7mlO2.kg-1.min-1 ) and (4mph= 128.9±7.8 vs 113.1±15.6 and 5mph= 130.3±5.2 vs 114.1±18.9, and LPC= 127.6±7.2 vs 109.3±13.2 and MPC= 131.2±5.7 vs 117.4±19.2bpm) The UL of the indirect exercise intensity prescription in 4mph and LPC group showed VO2 values significantly higher than those measured at RCP (40.1±5.3 vs 32.2±4.3 and 37.4±4.8 vs 30.6±2.5mlO2.kg-1.min-1, respectively), but similar HR values to those obtained at RCP. CONCLUSION: The LL of prescription based on standard exercise test overestimate the AT, whereas the UL seem adequate only for subjects with moderate physical capacity.

von Willebrand factor and pulmonary vascular endothelial cell dysfunction. Prognostic implications

Antonio Augusto B. Lopes et al. Nair Yukie Maeda Sérgio Paulo Bydlowski

PURPOSE: To analyze quantitative and structural changes in circulating von Willebrand factor (vWF) in 40 precapillary pulmonary hypertensive patients, as an attempt to identify possible correlations between endothelial cell dysfunction and patient short-term (one year) survival. METHODS: Plasma antigenic activity of vWF (vWF:Ag) was analyzed by immunoelectrophoresis. The relative concentration of vWF low molecular weight multimers (LMWM%) and the composition of vWF subunit were determined by densitometric analysis of Western blots. RESULTS: vWF:Ag was importantly increased in patients in comparison with normals (p<0.001). Patients also had increased LMWM% (p<0.001) and increased degradation of vWF main subunit (p<0.05). At the beginning of the study, nonsurvivors (N=11) had higher vWF:Ag (p<0.001) and LMWM% (p<0.005) values in comparison with survivors. LMWM% was selected by logistic regression analysis as a predictor of death during the first year of follow-up (p<0.05). CONCLUSION: Marked changes in circulating vWF likely reflect extensive pulmonary vascular endothelial cell dysfunction and are associated with poor short-term prognosis in pulmonary hypertension.

Initial experience in percutaneous occlusion of atrial septal defects with the Amplatzer device

Valmir F. Fontes et al. Carlos A. C. Pedra Simone R. F. Fontes Pedra César A. Esteves … Sérgio L. N. Braga Jorge E. Assef Sérgio C. Pontes Jr Maria Virgínia T. Santana Ziyad M. Hijazi J. Eduardo M. R. Sousa

PURPOSE: To evaluate our initial experience with percutaneous closure of secundum type atrial septal defects (ASD) with the Amplatzer septal occluder. METHODS: Seven patients underwent occlusion by anterograde approach, under general anesthesia and transesophageal echocardiography (TEE) guidance. One child had 2 ASD and a patent ductus arteriosus (PDA). The ASD size ranged from 8,7 to 20mm as measured by TEE. A transthoracic echocardiogram was performed in the morning after the procedure. RESULTS: Eight devices were successfully implanted in 7 patients and the PDA was occluded with a Gianturco coil at the same session. In this patient, there was an episode of supraventricular tachycardia during the occlusion of one ASD which was reverted with adenosin. All patients were discharged the day after, with complete occlusion of all defects. CONCLUSION: The procedure is safe, effective and versatile. It can be applied as an initial alternative to the treatment of selected patients with ASD.

Hospital outcome and short-term follow-up of predominatly elderly patients presenting to the emergency department with congestive heart failure

Humberto Villacorta et al. Nazareth Rocha Rosana Cardoso Sérgio Gaspar … Eduardo Reis Maia Therezil Bonates Daniel Kopiler Hans J. F. Dohmann Evandro Tinoco Mesquita

PURPOSE: To evaluate the in-hospital (IH) outcome and the short-term follow-up of predominantly elderly patients presenting to an emergency room (ER) with congestive heart failure (CHF). METHODS: In an 11 month period, 57 patients presenting to the ER with CHF were included. Mean age was 69+15 years (27 to 94) and 39 (68,4%) were male. CHF diagnosis was based on the Boston criteria. We evaluated IH outcome and prognosis in a mean follow-up of 5,7+2,7 months (1 to 12). In addition, some mortality predictors and mechanisms of death according on the ACME system were identified. RESULTS: Eight patients (14%) died in the IH period. Modes of death were circulatory failure (CF) in 7, and peri-operative (PO) in one (aortic valve replacement). During follow-up 9 deaths ocurred. Five were due to CF, 2 were sudden and 2 were PO (mitral valve replacement and ventriculectomy). Six-months and 1-year survival rates of the patients who were discharged were 82% and 66%, respectively. Sodium lower than 135mEq/l (p= 0.004) and female gender (p= 0.038) were independent predictors of mortality. CONCLUSION: Elderly patients with CHF admitted to the ER have high in-hospital and short-term follow up mortalities. The majority die from CF due to worsening heart failure.

Stenting a stent in saphenous vein graft to optimize the angiographic result

Antonio Esteves Fº et al. Celso Kiyochi Takimura Evandro Costa Lira Luiz Junya Kajita … Siguemituzo Arie Giovanni Bellotti Fulvio Pileggi

A 60 year-old woman with progressive angina who had been submitted to saphenous bypass-graft to right coronary artery and a left mammary artery graft to anterior descending artery eight years previously, underwent implantation of a Gianturco Roubin II stent in the proximal third of the saphenous vein graft. The result was suboptimal by persistence of a residual stenosis probably due to prolapse of atherosclerotic material through the coil spaces. Another stent (Palmaz-Schatz biliar stent) was implanted at the previously stented site with no residual stenosis. Another Palmaz-Schatz biliar stent was successfully implanted in the distal body of the graft to treat another lesion (passing through the previously stents without difficulty). Stenting a stent, in selected situations, is a useful tool to optimize the angiographic result of stent implantation.
