Value-based Health Care Impact on Cardiovascular and Metabolic Diseases

Ana Paula Beck da Silva Etges et al. Gabriela Boff Comiran Anna Luiza Reinehr Ferreira Carisi Anne Polanczyk

Final considerations

The development of a value-based health system that is centered around individual needs and scaled to deliver the best health outcomes within a budget constraint is possible and favored by structural changes to the system, such as payment methods and a technology base that enables real-life data-driven management. PHC centralization and the interconnection with data shared between the assisted population and specialized care systems seem to be a requirement for greater capacity for tracking, diagnosing, and managing cardiometabolic diseases in the health system. For Brazil’s current scenario, the challenges start with the precarious measurement culture and remuneration strategies that are centered on payment for the service delivery rather than the impact they have on population health.

As primary steps to establish a value-based health model for cardiometabolic diseases in Brazil, seeking inspiration from existing evidence, the following are recommended: start by disseminating mobile technologies that provide education and self-care and can be easily accessed by the population; advance towards implementing a measurement culture in PHC, sharing results with specialized care institutions and funding sources; and establish a better capacity for longitudinal monitoring of individuals in the care pathways, which can be driven by a change in the specific reimbursement strategy for each of care pathways.

The Arquivos Brasileiros de Cardiologia (ABC), the official scientific journal of the Brazilian Society of Cardiology (BSC), is currently the journal in Cardiology with the highest impact in Brazil and Latin America, with a 2018 JCR impact factor of 1.679. During its history, the ABC has played an important role of publishing scientific papers produced in Brazil in journals indexed in the most prominent English-language, international databases. It thus acts as a window to the world of the Brazilian scientific production.

The mission of the ABC is to disseminate the results of national and international studies on cardiovascular diseases, to foster scientific debate on the area by means of review articles, viewpoints, editorials, letters and others, and to publish BSC scientific guidelines and standards. The journal’s commitment is in line with the mission of the BSC to expand and spread the knowledge about cardiovascular science, to represent and promote the development of cardiologists and to take actions for the cardiovascular health in Brazil.

Long-term Results of a Cardiology Postgraduate Program

Edimar Alcides Bocchi et al. Danielle Pazzotti Borges Vagner Oliveira-Carvalho Rigaud

Introduction

Research and innovation help to drive economic growth and address socioeconomic challenges like poverty and health. Many developed and developing countries have introduced policies and systems to increase research and innovation.

In Brazil, a system was created on 1951 with the objectives of improving technology and innovation and strengthening scientific research., Similar to postgraduate doctorate programs in developed countries, postgraduate programs with broader and more in-depth scientific research objectives have been developed in Brazil. In fact, a linear relationship has been demonstrated between the number of students graduating from these programs and the number of scientific publications produced by them. Beyond scientific output, publishing a high-impact paper or in a journal with a high-impact factor seems to be an important requirement for innovation and technology growth. Considering that postgraduate students play an important role in scientific production in Brazil, a study including the characteristics of the scientific production of these students is justifiable.

Diseases of the circulatory system before and after the family health program, Londrina, Paraná

Brígida Gimenez Carvalho et al. Regina Kazue Tanno de Souza Darli Antonio Soares Mara Cristina Nishikawa Yagi

BACKGROUND: The reduction in morbidity and mortality rates from diseases of the circulatory system (DCS) is one of the greatest challenges in primary care, and the implementation of the Family Health Program provides access to the multisectoral and integrated measures required for dealing with these diseases. OBJECTIVE: To analyze DCS mortality and hospitalization rates before and after the implementation of the Family Health Program, in Londrina, Paraná. METHODS: This was an ecological study, comparing DCS, cerebrovascular disease (CbVD) and ischemic heart disease (IHD) mortality rates and hospitalization rates by the SUS, in residents of Londrina, in two 4-year periods: 1997 to 2000, and 2002 to 2005. The data were obtained from the Mortality Information System and the Hospital Information System of the SUS. The ratios between the rates were calculated for both periods at 95% confidence interval. RESULTS: The DCS was the first cause of death in the city, in both quadrennia. The CbVD and IHD accounted for more than 63% of DCS deaths. In comparing the two quadrennia, there was a reduction in CbVD mortality rates, which was significant only for subjects over 59 years, in both genders, and the hospitalization rates increased by 10%. For IHD, no significant change occurred in mortality rates, and there was a 40% increase in hospitalization rates. CONCLUSION: The significant reduction only in CbVD mortality in the elderly suggests the need for expanding health assistance coverage to different population groups and the development of preventive and health promotion measures.
