Sacubitril/Valsartan versus Enalapril or Losartan at Maximum Doses in Heart Failure with Reduced Ejection Fraction: Real-World Data

Abstract

Background:

Sacubitril/valsartan reduced mortality and hospitalizations in patients with heart failure with reduced ejection fraction (HFrEF) compared to enalapril in pivotal clinical trials. However, real-world data under optimized dosing remain limited.

Objective:

To compare the outcomes of HFrEF patients treated with sacubitril/valsartan versus either enalapril or losartan at guideline-recommended maximum dosages.

Methods:

We analyzed data from an observational heart failure cohort (n = 2,314) including patients receiving care between August 2010 and September 2024 at six public hospitals in a large metropolitan area. The primary analysis included 254 HFrEF patients receiving the guideline-recommended maximum dosages of the investigated drugs. A sensitivity analysis included 452 patients receiving any submaximal dosages. The primary outcome was combined hospitalization or death. Baseline differences were adjusted using propensity score matching. A p-value < 0.05 was considered significant.

Results:

Among 254 HFrEF patients (mean age 63.6 ± 12.3 years, 61.4% male), 115 received sacubitril/valsartan 97/103mg twice daily, 33 enalapril 20 mg twice daily, and 106 losartan 100 mg daily. Over the median follow-up of 23.9 months (interquartile range 11.9–41.1), there were 70 primary endpoint events (56 first hospitalizations, 32 deaths). Compared to equivalent dosages of enalapril or losartan, sacubitril/valsartan was not associated with significant reductions in combined hospitalization or death (odds ratio 0.650, 95% confidence interval 0.354–1.195, p = 0.165), but it was associated with functional status improvement (odds ratio 3.902, 95% confidence interval 1.745–8.726, p = 0.001).

Conclusion:

This real-world study did not demonstrate significant reductions in hospitalization or mortality for sacubitril/valsartan compared with enalapril or losartan at guideline-recommended maximum dosages; however, sacubitril/valsartan was associated with improved functional status.

Angiotensin-Converting Enzyme Inhibitors Evaluated by Office and Home Blood Pressure Measurements: TeleHBPM Study

Tariane Colombo et al. Priscila Valverde de Oliveria Vitorino Audes D. M. Feitosa Andréa Araujo Brandão … Eduardo C.D. Barbosa Roberto Dischinger Miranda Marco Mota Gomes Ana Luiza Lima Sousa Weimar Kunz Sebba Barroso

Abstract

Background

Angiotensin-converting enzyme inhibitors (ACEI) are a class of drugs fundamental to hypertension treatment; however, the individual medications within this class vary in their effectiveness at controlling Blood Pressure (BP).

Objectives

To analyze BP values obtained through casual measurements and home blood pressure monitoring (HBPM) in patients using ACEI as monotherapy or in dual combinations, focusing on BP control and goal achievement in a Brazilian population.

Methods

A cross-sectional study evaluating adult patients using ACEI between 2017 and 2020 included in the TeleHBPM platform. SBP and diastolic blood pressure (DBP) values were assessed by casual and home measurements. Those using three or more HBPMs were excluded. Paired t-tests, chi-square tests, and ANOVA were used, with a significance level of 5% (p < 0.05).

Results

3,466 patients who met the criteria were selected, 54% in a dual drug combination. The mean BP values obtained via casual measurement and HBPM, respectively, were 131.6 ± 18.7 mmHg and 125.5 ± 14.8 mmHg for SBP (p < 0.001), and 83.4 ± 11.3 mmHg and 79.1 ± 9.4 mmHg for DBP (p < 0.001). The combination of ACEI with calcium channel antagonists prevailed (29%). Enalapril had similar BP means to ACEI with longer half-lives. Captopril monotherapy and in combinations had higher SBP and DBP means.

Conclusion

Enalapril was the most commonly used ACEI, demonstrating a similar BP-lowering effect, both as monotherapy and in combination, when compared to other drugs in the same class with longer half-lives.

Combination of amlodipine and enalapril in hypertensive patients with coronary disease

Marcos Rienzo et al. José Francisco Kerr Saraiva Paulo Roberto Nogueira Everli Pinheiro de Souza Gonçalves Gomes … Miguel Antonio Moretti João Fernando Monteiro Ferreira Dikran Armagnajian Antonio de Pádua Mansur José Antonio Franchini Ramires Luiz Antonio Machado César

BACKGROUND: Patients (pts) with stable coronary artery disease (CAD) can benefit from a decrease in the blood pressure (BP), according to recent studies. OBJECTIVE: To evaluate the efficacy and tolerability of the fixed combination: amlodipine + enalapril, when compared to amlodipine in the normalization of the diastolic arterial pressure (DAP) (90 and <110 mmHg and CAD. Patients with left ventricular ejection fraction (LVEF) 110 mmHg during the four-week wash-out with atenolol treatment alone, were excluded. After the wash-out, pts were randomly distributed for the use of the combination (A) or amlodipine (B) and were followed every four weeks up to 98 days. The initial doses (in mg) were, respectively: A- 2.5/10 and B- 2.5; the doses were increased when DAP > 85mmHg, at the visits. Statistical analysis was carried out with χ2, Fischer and analysis of variance, for p< 0.05. RESULTS: Of the 110 selected pts, 72 (A= 32, B= 40) were randomized. The decreases in DAP and systolic arterial pressure (SAP) were significant (p< 0.01), but with no difference between the groups in mmHg: SAP, A (127.7 ± 13.4) and B (125.3 ± 12.6) (p= 0.45) and DAP, A (74.5 ± 6.7 mmHg) and B (75.5 ± 6.7 mmHg) (p= 0.32). Group A presented a lower incidence of lower-limb edema: (7.1% vs 30.6%, p=0.02) on the 98th day of follow-up. CONCLUSION: The fixed combination of enalapril and amlodipine, as well as isolated amlodipine, was effective in the normalization of BP in pts with CAD and SAH stages I and II, adding blockage of the renin-angiotensin system.

Enalapril alters the formation of the collagen matrix in spontaneously hypertensive rats

Alfredo de Souza Bomfim et al. Carlos Alberto Mandarim-de-Lacerda

OBJECTIVE: To assess the effect of the inhibition of the angiotensin-converting enzyme on the collagen matrix (CM) of the heart of newborn spontaneously hypertensive rats (SHR) during embryonic development. METHODS: The study comprised the 2 following groups of SHR (n=5 each): treated group – rats conceived from SHR females treated with enalapril maleate (15 mg. kg-1.day-1) during gestation; and nontreated group – offspring of nontreated females. The newborns were euthanized within the first 24 hours after birth and their hearts were removed and processed for histological study. Three fields per animal were considered for computer-assisted digital analysis and determination of the volume densities (Vv) of the nuclei and CM. The images were segmented with the aid of Image Pro Plus® 4.5.029 software (Media Cybernetics). RESULTS: No difference was observed between the treated and nontreated groups in regard to body mass, cardiac mass, and the relation between cardiac and body mass. A significant reduction in the Vv[matrix] and a concomitant increase in the Vv[nuclei] were observed in the treated group as compared with those in the nontreated group. CONCLUSION: The treatment with enalapril of hypertensive rats during pregnancy alters the collagen content and structure of the myocardium of newborns.
