Risk Factors for Surgical Site Infection after Pediatric Cardiac Surgery

Rafael Quaresma Garrido et al. Cristiane da Cruz Lamas

Infections after pediatric cardiac surgery are important adverse events that can increase morbidity and mortality in these patients. Most studies describe the incidence of infection as between 0.5 and 8%, but some studies in low- to middle-income countries report rates of up to 48%.– The main microorganisms involved are Staphylococcus aureus, coagulase-negative staphylococci, and occasionally hospital-acquired Gram negatives. In high-income countries, there is a predominance of skin-colonizing Gram-positives, while in low- to middle-income countries, there is an increase in the frequency of hospital-acquired Gram-negatives. –

There are divergences between the risk factors for infection in pediatric cardiac surgery, as there are few studies dedicated to this population. In regard to pre-operative conditions or those inherent to the patient, we have as risk factors being a younger child, especially those under 12 months of age, the presence of immunodeficiencies, prolonged periods of pre-operative hospitalization and previous use of antimicrobials (the latter two conditions because they alter the colonizing microbiota) and malnutrition. Pre and post operative risk factors are the inadequacy of antibiotic prophylaxis, breaches of aseptic technique, prolonged cardiopulmonary bypass time (>105 minutes) and aortic clamping time (>85 minutes), excessive bleeding in the first 24 hours, blood transfusion, early chest reexploration due to operative bleeding, occurrence of nosocomial infections (pneumonia and bloodstream infections mainly) and the presence of invasive devices such as drains and pacemaker wires.–

Double Outlet Right Ventricle with Unrelated Ventricular Septal Defect and Pulmonary Stenosis, in Natural Evolution, in a 36-Year-Old Woman

Edmar Atik

Introduction

The congenital defect, characterized by the exit of both large arteries from the right ventricle (RV), completely or even when overlapping more than 50% of one of the arteries over the interventricular septum, presents variable relationships between these arteries and the ventricular septal defect (VSD), as well as association with other anatomical and functional variables.

In the most common associated defect, the isolated VSD, in a subaortic position or in that unrelated to the large arteries, the functional condition is expressed with a predominance of volume overload of the heart as a whole, increased by pressure overload, causing early heart failure.

Case 4/2016: 32-Year-Old Female, with Critical Pulmonary Valve Stenosis. Operated at 4 Months of Age, in Normal Healing Evolution

Edmar Atik

Supplementary Exams

Electrochardiogram showed junctional rhythm and discreet end disorder of conduction through the right branch with complex rSr in V1 with normal duration of QRS of 0.094 seconds. AP = -30º; AQRS = -10º; AT = +10º (). ECG, done before the surgery at 4 months old, highlighted the significant right ventricular and atrial overload with electrical signals of systolic pressure of the right ventricle greater than systemic pressure with depression of ST segment in the right precordials and QR waves from V1 to V5, largely negative T waves from V1 to V6 and peaked P wave with 5 to 6 mm amplitude from V2 to V4; AP = +50°, AQRS = + 170°, AT = -60°. ().

Case 5/2015 Late Outcome of Corrected Aortopulmonary Window in A 23-Year-Old Female Patient Who Underwent Surgery in Childhood

Edmar Atik

Laboratory tests

ECG () at 22 years of age showed normal sinus rhythm with no signs of overload, and normal ventricular repolarization. PA = +10º , QRSA = +50º , TA = +50º (lower tracing). ECG prior to operation, at 10 months of age (upper tracing), showed marked left ventricular overload, with QRSA = +70º , PA = +20º and TA = +80º ; RS morphology (amplitudes of 24 and 30 mm) from V1 to V3 and QRs (amplitudes of 10, 50 and 3 mm) from V4 to V6; Sokolow index of 90 mm; no ventricular repolarization abnormalities.

Chest radiograph showed normal cardiac silhouette (cardiothoracic ratio = 0.46) and pulmonary vasculature (). In the image prior to surgery, these parameters were markedly increased, with a cardiothoracic ratio of 0.85 ().
